A twin pregnancy consisting of a complete hydatidiform mole and live fetus is an unusual event, occurring with an incidence of 1:22,000 to 1:100,000 of all pregnancies. Due to advances in cytogenetics and ultrasonography, now permit the diagnosis of this pregnancy antenatally. And clinicians will be confronted with councelling between immediate intervention or expectant management, particularly in the case of a highly desired pregnancy. But there exists no predictors of which patients will developing invasive growth or malignant change. Close monitoring of the patients would be necessary during the pregnancy due to possibilities of the developing the persistent gestational trophoblastic disease. And it may be acceptible for a patient showing no symptoms and no metastasis, continuing of the pregnancy to term. We report one case of a complete mole with coexisting fetus , healthy, normal fetus and be carried to 29 weeks with a good fetal outcome.
Fractional Photothermolytic Effect of Sellas Laser Device on Guinea Pig Skin

This study was designed to evaluate photothermolytic effects of a fractional laser device and to evaluate healing process after fractional laser treatment on guinea pig skin. Eleven guinea pigs received a single treatment with a prototype diode laser (Sellas<SUP>?</SUP>) emitting a wavelength of 1,550 ㎚, delivering 10 mJ and Excisional biopsies were done immediately after (0 day), 1, 3 and 7 days and analyzed by two blind pathologists using hematoxylin and eosin and Van Gieson stain. Definitive MTZs were observed in both 10 and 20 mJ energy with sparing normal tissue between MTZs. Average depths and diameters of microbeam produced were 308.46, 105.61 ㎛ in 10 mJ and 414.15, 130.57 ㎛ in 20 mJ, respectively. Degeneration of dermal collagen in MTZ is evident, revealing loss of collagen birefringence under polarizing microscope and lack of red staining on Van Gieson stain. One day after fractional laser treatment regeneration of epidermis was completed without disruption of stratum corneum. Microscopic epidermal necrotic debris (MENDs) were noted 1 day on the laser treated epidermis and disappear within 3 days after the treatment. Areas of each microscopic treatment zones were rapidly reduced by replacing newly formed collagen. Repair rate reached 30% at 1 day after laser treatment and at day 7 a wound healing process was almost completed. A single treatment with fractional laser device reveals fractional photothermolytic effects and wound healing responses in both epidermis and dermis.

<P><B>Objective.</B></P><P>Guanosine triphosphate cyclohydrolase 1 (GCH1) is the rate-limiting enzyme in the synthesis of tetrahydrobiopterin, which is an essential cofactor in nitric oxide (NO) production. Polymorphisms in the <I>GCH1</I> gene have been implicated in protection against pain sensitivity. The aim of our study was to determine whether single-nucleotide polymorphisms (SNP) in the <I>GCH1</I> gene affect susceptibility and/or pain sensitivity in fibromyalgia syndrome (FM).</P><P><B>Methods.</B></P><P>A total of 409 patients with FM and 422 controls were enrolled. The alleles and genotypes at 4 positions [rs3783641(T>A), rs841(C>T), rs752688(C>T), and rs4411417(T>C)] in the <I>GCH1</I> gene were analyzed. The associations of the <I>GCH1</I> SNP with susceptibility and clinical measures in patients with FM were assessed.</P><P><B>Results.</B></P><P>The frequencies of alleles and genotypes of the 4 SNP did not differ between patients with FM and healthy controls. Among 13 constructed haplotypes, we further examined 4 (CCTT, TTCT, TTCA, and CCTA) with > 1% frequency in both FM and controls. No associations of <I>GCH1</I> polymorphisms with FM-related activity or severity indexes were found, although the number and total score of tender points in patients with FM differed among the 4 haplotypes (p = 0.03 and p = 0.01, respectively). The CCTA haplotype of <I>GCH1</I> was associated with significantly lower pain sensitivity and occurred less frequently than the CCTT haplotype in patients with FM (p = 0.04, OR 0.45, 95% CI 0.21–0.96).</P><P><B>Conclusion.</B></P><P>Our study provides evidence that certain <I>GCH1</I> haplotypes may be protective against susceptibility and pain sensitivity in FM. Our data suggest that NO is responsible for pain sensitivity in the pathogenesis of FM.</P>

Determination of Cyanogenic Compounds in Edible Plants by Ion Chromatography

Cyanogenic glycosides are HCN-producing phytotoxins; HCN is a powerful and a rapidly acting poison. It is not difficult to find plants containing these compounds in the food supply and/or in medicinal herb collections. The objective of this study was to investigate the distribution of total cyanide in nine genera (Dolichos, Ginkgo, Hordeum, Linum, Phaseolus, Prunus, Phyllostachys, Phytolacca, and Portulaca) of edible plants and the effect of the processing on cyanide concentration. Total cyanide content was measured by ion chromatography following acid hydrolysis and distillation. Kernels of Prunus genus are used medicinally, but they possess the highest level of total cyanide of up to 2259.81 CN?/g dry weight. Trace amounts of cyanogenic compounds were detected in foodstuffs such as mungbeans and bamboo shoots. Currently, except for the WHO guideline for cassava, there is no global standard for the allowed amount of cyanogenic compounds in foodstuffs. However, our data emphasize the need for the guidelines if plants containing cyanogenic glycosidesare to be developed as dietary supplements.

Hyperemesis gravidarum is a complication defined as nausea and vomiting during early pregnancy. Wernickes encephalopathy is an illness of acute onset characterized by mental disturbance, paralysis of eye movements, and ataxia of gait cause by thiamine deficiency. We recently experienced a case of Wernickes encephalopalhy due to hyperemesis gravidarum, which is treated by administration of thiamine even though outcome of serious healthy problem to both mother and fetus, and reported it with a brief review of the literature.
