This case report describes a patient who developed positional vertigo after surgery for chronic otitis media on the right side. Canal wall up mastoidectomy was performed, and the stapes was moderately mobilized during removal of the inflammatory granulation tissues that were attached to it. Immediately after the surgery, positional vertigo developed. The patient showed weakly left-beating spontaneous nystagmus in a seated position. Examination of positional nystagmus revealed geotropic direction-changing positional nystagmus with a prolonged duration and weak intensity in a supine head-roll test, which may be caused by a change in inner ear fluids due to a disruption of inner ear membrane around the oval window or penetration of toxic materials into the labyrinth during surgery.

In this research, from the perspective of marginal village in a county of Chungnam province, It is suggested by types by investigating and analysing rural village and check the standards of marginal village. So this research will show that if they are applicable in Chungnam province or not, and that what the features of marginal village are. Although it is judged that the village of type IV and type II where hollowing out of people and land is done has characteristics of marginal village, verification of 50 criteria of population scale is further necessary, It is judged that the type village which constitutes the majority of the current rural village may change to the marginal village in the long term. In Japanese studies, marginal villages are judged on the basis of the population size and the aging rate, but in the case of South Korea, since the aging rate is generally high overall in rural areas, In the case of prescribing to the marginal village, many villages can belong to the marginal village, and it is not compliant with the criteria of the marginal village. Considering that the hollowing out of the land is affected by the aging rate instead, it is judged that it is desirable to set the hollowing out of the land as the limit of the marginal village.

베체트병의 환자에서 뇌동맥류는 흔치 않은 중추신경계침범 임상상이며, 특히 lenticulostriate 동맥류가 발생하는 경우는 매우 드물다. 저자들은 활동성 베체트병에의한 lenticulostriate 동맥류 파열로 발생한 급성 좌측 편마비환자에서 고용량 글루코코르티코이드제제 투여로 호전된 국내첫 번째 증례를 경험하였기에 문헌고찰과 함께 보고한다. Behcet`s disease (BD) is characterized by recurrent oro-genital ulcers, skin lesions, and intraocular inflammation, but can also affect various internal organs. Vascular BD usually presents with luminal stenosis, thrombosis, or aneurysm formation in aorta and peripheral arteries. However, intracranial artery involvement has been uncommonly reported in patients with BD and BD cases with lenticulostriate artery aneurysm have been rarely described in the English-language literature. We hereby reported the first case of a Korean BD patient presenting with a ruptured lenticulostriate artery aneurysm, who received medical treatment, and reviewed the literature on reported cases of BD with intracranial aneurysms. 808 nm 파장의 고출력 근적외선 레이저 조사 시 돼지 조직의 천공 특성 연구

소화기 내시경에 장착된 808 nm 파장의 고출력 근적외선 레이저를 장벽에 조사하여 병변을 제거할 수 있는 치료용 의료기기의 개발을 목적으로 레이저-조직 상호작용에 관한 기초 실험을 진행했다. 레이저 출력 3~12 W, 조사 시간 5~20 s 범위에서 각 변수의 증가에 따라 천공 깊이가 1~4 mm 범위에서 선형적으로 증가했다. 돼지에서 적출한 각 장기에 대한 레이저 조사 시 천공 깊이가 조직 특성에 따라 달라짐을 확인했다. 온도 측정 결과로부터 열에너지가 레이저 조사 지점에 집중되고 심부로 전달되어 주변 조직의 열손상은 방지됨을 알 수 있었다. 본 연구 결과는 위, 대장 등의 소화기 조직에서 일정한 절개 깊이를 얻기 위해 필요한 근적외선 레이저의 구동 조건을 결정하는데 활용될 수 있다. A fundamental study on laser-tissue interaction was conducted with the aim of developing a therapeutic medical device that can remove lesions on the intestinal wall by irradiating a high-power 808-nm infrared laser light incorporated in an endoscopic system. The perforation depth was linearly increased in the range of 1~4 mm in proportional to laser output (3~12 W) and irradiation time (5~20 s). We demonstrated that the perforation depth during laser irradiation was varied according to the tissue property of each extracted porcine organ. The measurement of the temperature distribution suggests that the energy is localized in the irradiation spot and transferred to deep tissue, which protects the surrounding tissue from thermal injury. These results can be used to set the driving parameters for a laser incision technique as an alternative to conventional surgical interventions.
