        Macromolecule diffusion in cells and tissues is important for cell signaling, metabolism and locomotion. Biophysical methods, including non-invasive or minimally invasive in-vivo photobleaching techniques and single quantum-dot tracking, have been used to measure the rates of macromolecule diffusion in living cells and tissues, including central nervous system and tumors. Mathematical modeling and statistical analysis of experimental data revealed various modes of diffusion, which are strongly coupled with spatiotemporal changes in nanoscale structures and material properties.

        간호사의 내부지향적 정책, 사회적지지 및 직무배태성이 이직의도에 미치는 영향

        본 연구는 간호사의 내부지향적 정책, 사회적지지 및 직무배태성이 이직의도에 미치는 영향에 대해 알아보기 위한 서술적 조사연구이다. 연구대상자는 연구 참여에 동의한 D광역시에 위치한 2개 대학병원 간호사들이며, 자료 수집은 2018년 10월 2일부터 2019년 3월 20일 까지 실시되었다. 자료 분석은 SPSS 22.0 프로그램을 이용하였으며, 빈도와 백분율, 평균과 표준편차, 상관분석, t-test, ANOVA, Pearson’s correlation, Multiple regression을 이용하였다. 연구결과 내부지향적 정책은 3.04점, 사회적지지는 상사 3.08점, 동료 3.81점, 가족 3.67점이었고, 직무배태성은 2.98점, 이직의도는 3.55점이었으며, 내부지향적 정책, 사회적지지 및 직무배태성이 높을수록 이직의도는 감소하는 것으로 나타났다. 일반적 특성에서 이직의도에 미치는 요인을 파악한 결과 임상경력과 근무부서가 유의미한 결과를 보였다. 임상경력은 높을수록 이직의도가 높았고, 근무부서는 외과병동, 내과병동, ICU, OR순으로 이직의도가 높았다. 이상의 결과를 바탕으로 간호사의 이직의도를 낮추기 위한 중재방안으로 병원 내부지향적 정책, 사회적지지 및 간호사의 직무배태성을 높이기 위한 프로그램 개발 및 교육이 적극적 이루어져야 할 것이다. This study is a descriptive research to investigate the effects of nurses" internal orientation, social support and job orientation on turnover intention. The subjects were two university hospital nurses in D metropolitan city who agreed to participate in the study. Data collection was conducted from October 2, 2018 to March 20, 2019. The data were analyzed using SPSS 22.0 program, frequency and percentage, mean and standard deviation, correlation analysis, t-test, ANOVA, Pearson"s correlation, and multiple regression. The study results were 3.04 ± .62 points for internal orientation policy, 3.08 ± .55 points for social support, 3.81 ± .59 points for peers, 3.67 ± .60 points for family members, and 2.98 ± 0.43 points for job placement and intention to turnover. The degree of turnover decreased with higher internal orientation, social support and job orientation. An examination of the general characteristics of the factors affecting turnover intention showed significant results with clinical experience and working department. The higher the clinical experience, the higher the turnover intention, and the department of work showed the higher turnover intention in the order of surgical ward, medical ward, ICU, and OR. These results show the need to actively develop and educate programs to improve nurses" intention to turnover, as well as internal hospital-oriented policies, social support, and job placement.

        피부 치료를 위한 공압식 무침 주사 시스템의 주요 인자에 관한 실험적 연구

        A needle-free injector is one of the new non-invasive players in impregnating the biological barriers. It is considered as the next phase in drug delivery and therapeutic applications. One of the major fields of application is in skin remodeling procedures. Although many studies were carried out in understanding the principle in the needle-free injection procedure, fewer studies were done with the aim of therapeutic applications. In the present study, we tried to identify key parameters that affect the jet divergence and peak stagnation pressure on the skin surface in a conventional needle-free injector for skin treatment. A summary of the working principle and effect of the key parameters are presented.

        증례 : 신장 ; 지속성 외래 복막 투석 환자의 반코마이신 내성 장구균 복막염 치료 1예

        본 증례를 통하여 VRE에 의한 복막투석 복막염에 대해 quinupristin-dalfopristin를 사용하여 치료한 효과는 확인하였다. 하지만 치료 방법에 대한 명확한 기준은 아직 미흡한 실정이다. VRE 감염이 확인된 후 2주간의 quinupristin-dalfopristin를 사용하여 치료에 성공한 경우로 다른 항생제의 투여가 병용된 점은 quinupristin-dalfopristin만의 단독 효과로 생각하기에는 제한점을 가진다. 하지만 균주가 동정된 후 항생제내성 결과에 따라 투여된 약제가 효과를 나타냄에 따라VRE 감염증을 도관제거 없이 quinupristin-dalfopristin를 정맥 투여하여 감염증을 치료하였다. 도관을 제거하지 않고 지속적인 관류함으로 감염된 상처를 열어 놓으면서 균주의 박멸을 시도하는 것도 한 가지 방법이 될 수 있음을 이번 증례를 통해 경험하여 보고하는 바이다[17]. 최근 반코마이신 내성균주의 증가추세로 볼 때 향후 본 증례와 같은 VRE 복막염의 예가 증가할 것으로 예측되며 먼저 이를 줄이기 위해VRE의 전파 방지를 위한 노력이 필요하다. 또한, VRE로 인한 복막염이 진단된 경우에는 quinupristin-dalfopristin의 사용이 효과적일 것으로 생각되나 향후 지속적 연구를 통해 적절한 용량 및 투여기간에 대한 연구 및 보고가 있어야 할 것이다. Peritonitis is one of the major complications of continuous ambulatory peritoneal dialysis (CAPD). Multidrug-resistant organisms, including vancomycin-resistant enterococci (VRE), have been reported as pathogens of CAPD-associated peritonitis. The incidence of hospital-associated infections caused by VRE has recently increased. Some drugs, such as linezolid and quinupristin/dalfopristin, have been introduced as treatments of VRE infection. However, there is limited information about the effects of VRE-associated CAPD peritonitis. We present a case of successful treatment of CAPD peritonitis caused by VRE with quinupristin/dalfopristin and include a review of the literature. (Korean J Med 2011;80:S245-S248)

        Collapsing glomeruopathy (CG) is a clinicopathologic variant of focal segmental glomerulosclerosis (FSGS) and is characterized by severe nephrotic syndrome, rapid progression to end stage renal disease, and features of visceral epithelial cell injury and glomerular capillary collapse. Such characteristics closely resemble those of HIV associated nephropathy. The frequency of CG has increased over the last decade. The cause of CG is unknown. The lesion has rarely been described in renal allografts with features similar to CG in native kidney. We recently identified allograft CG in a 44 year-old male patient who underwent biopsy for graft dysfunction after autodermic graft. The biopsy showed typical characteristics of CG. Serologically, the patient had no evidence of HIV infection. The renal function was not restored to normal in spite of methylprednisolone pulsing therapy. Now he is on conservative treatment with a functioning graft.

        직사각형 단면을 갖는 미세채널에서 완전 발달된 다층유동에 관한 해석

        An analytical solution for a vertically stratified viscous flow in a microchannel with a rectangular cross-section is constructed, assuming fully developed laminar flow where the interfaces between the fluid layers are flat. Although the solution is for n-layer flow, restricted results to symmetrical three-layer flow are presented to investigate the effects of the viscosity and thickness ratios of the fluid layers and the aspect ratio of the microchannel on the flow field. Relations between the flow rate and thickness ratios of the fluid layers with varying viscosity distributions are found, considering the cross -sectional velocity profiles which vary noticeably with the three parameters and differ significantly from the velocity profiles of the flow between infinite parallel plates. Interfacial instability induced by the viscosity stratification in the microchannel is discussed referring to previous studies on the instability analysis for plane multilayer flow. Exact solution derived in the present study can be used for examining a diffusion process and three -dimensional stability analysis. More works are needed to formulate the equations including the effects of interfacial' tension between immiscible liquids and surface wettability which are important in microscale transport phenomena.

        Acute fulminant invasive fungal sinusitis in an immunocompromised host and bacterial rhinosinusitis with intracranial or orbital extension is challenging to manage. And it sometimes constitutes true otolaryngologic emergencies. In the absence of rapid diagnosis and treatment, these diseases can be fatal. A 57-year-old female was admitted for chills and headache, who received a deceased donor renal transplantation 3 months ago. Paranasal sinus CT showed enhanced soft tissue density and MRI showed low-signal with hyperintense signal of around paranasal sinus cavity. The histological investigation revealed invasive aspergillosis of paranasal sinuses. Clinical improvement occurred after endoscopic sinus surgery and post-operative systemic antifungal therapy with amphotericin B and voriconazole.

        회전하는 나선형 미니채널 카트리지를 이용한 미세입자 정렬

        Separation of particles based on different sizes, detection of pathogenic bacteria and isolation of leukocytes from whole blood are typical applications of spiral or helical microchannels. The present study focuses on developing a CD4+ T-cell counting device for monitoring HIV/AIDS patients with the aid of a helical minichannel used for a sample cartridge. For the experiment, 10 μm sized microbeads were used for visualization with a fluorescence imaging system. Alignment of microbeads was investigated in a stationary and spinning sample cartridge filled with glycerol-water mixtures of different densities. The helical minichannel was spun using a DC motor controlled by an Arduino board with a Bluetooth shield. It was found that when the sample cartridge was made stationary, no bead alignment was achieved for a medium with density (0% and 20% glycerol) lower than that of the beads, but when it was spun at 2000-3000 rpm for 1-4 min, an alignment was obtained at the top of the channel facilitating optical detection and enumeration of those microbeads. Since an alignment of microbeads was achieved for a medium with density as that of blood plasma, the same approach can be applied for aligning and counting CD4+ T-lymphocytes in whole blood samples collected from patients.

        형광 영상 기반 CD4 신속 검사법 개발을 위한 항원-항체 반응 조건 정량화

        CD4+ T-cell count determines the effectiveness for antiretroviral therapy (ART) in patients with human immunodeficiency virus (HIV). Although ART slows the progression of HIV to AIDS, rapid counting of CD4+ T lymphocytes with a drop of patient’s blood sample is urgently needed to ensure timely ART treatment in rural areas. Recently point-of-care CD4 testing devices have been developed by using non-flow based imaging cytometer incorporated with a sample cartridge where CD4+ T cells are reacted with fluorescently tagged specific antibodies. Here we conducted an experimental study using a conventional fluorescence microscope-based imaging system to quantitate the interaction of CD4 antibodies with CD4+ T cells at different reaction conditions. We demonstrated that a fast and affordable point-of-care CD4 test is feasible with a far less amount of antibodies and a shorter incubation time compared with a conventional sample preparation protocol for flow cytometry. We also proposed a general method to evaluate and compare the detection limit across different CD4 counting platforms by using fluorescently labelled microbeads for intensity calibration.
