Background: Diabetes mellitus (DM), as a strong risk factor of atherosclerosis, is associated with coronary artery disease (CAD) and ischemic stroke. Common carotid artery intima-media thickness (CCA-IMT) is considered as a surrogate marker of atherosclerosis and is used for a primary prevention of CAD. But CCA-IMT is used as a secondary prevention in ischemic stroke restrictively. This cross sectional study assessed CCA-IMT cut-off point and associating factors for predicting cerebrovascular stenosis in asymptomatic patients with type 2 diabetes. Methods: Type 2 diabetic patients without past stroke history were enrolled between January 2011 and February 2013. We measured CCA-IMT and other risk factors of atherosclerosis. Cerebrovascular stenosis was evaluated by magnetic resonance angiography (MRA) for patients with carotid artery thickness. Carotid artery thickness was defined as CCA-IMT =0.9 mm or the presence of plaque (i.e. focal CCA-IMT =1.5 mm). Results: Among 65 subjects, 47(72%) showed cerebrovascular stenosis on MRA. Maximun CCA-IMT(max-IMT) was 1.8 ± 0.5 mm in subjects with no cerebrovascular stenosis, 2.5 ± 0.8 mm in subjects with cerebrovascular stenosis(p< 0.0001). The apolipoprotein B/A-I (Apo B/A-I) ratio was 0.5 ± 0.1 in subjects with no cerebrovascular stenosis, 0.7 ± 0.0 mm in subjects with stenosis after adjusting atherosclerosis risk factors like age, sex, hypertension and smoking(p < 0.024). The area under the curves (AUC) for max-IMT was 0.782(95%CI 0.661-0.875; p<0.0001) and for Apo B/A-I ratio was 0.712(0.578-0.823; p=0027) by ROC curve analysis. The cutoff value for the greatest sensitivity and specificity for max-IMT was 2.12 mm (sensitivity 65.2, speci- ficity 83.3). Conclusions: We suggest the carotid artery plaque and Apo B/A-I ratio might be independent predictors of cerebrovascular stenosis in asymptomatic patients with type 2 diabetes.

Purpose: At present, a human epidermal growth factor receptor 2 (HER2)-based concept of tumor biology has been established, and trastuzumab ($Herceptin^{(R)}$; Genentech/Roche, San Francisco, CA, USA), a monoclonal humanized antibody directed against HER2, is a pivotal agent for the management of HER2 positive (HER2+) metastatic breast cancer. It is also known that HER2 has a predictive value in gastric cancer; however, its association with the prognosis of this disease remains uncertain. The purpose of this study was to evaluate both the relationship between HER2 overexpression in the tumors of gastric cancer patients, and the prognosis of these patients who have had curative resection. Materials and Methods: A total of 139 consecutive patients with gastric cancer who underwent surgery at the Kosin University Gospel Hospital between October 2011 and March 2012 were included in this retrospective study. All tumor samples were examined for HER2 expression by immunohistochemistry. A retrospective review of the medical records was conducted to determine the correlation between the presence of HER2 overexpression and clinicopathological factors. Results: The HER2+ rate was 15.1%. HER2 overexpression was associated with histological grade (P=0.044) and Lauren classification (P=0.036). There was no significant difference in the 2-year overall survival between HER2+ and HER2- patients (P=0.396). Multivariate analysis showed that HER2 was not an independent prognostic factor. Conclusions: HER2 overexpression in tumors was associated with histological grade and Lauren classification in gastric cancer patients with curative resection. However, HER2 was not an independent prognostic factor for gastric cancer in our study.
Perfluorooctanoic acid alters T lymphocyte phenotypes and cytokine expression in mice

<P>Perfluorooctanoic acid (PFOA) has been used in commercial applications and detected in environmental matrices. This study focuses on whether PFOA affects the function of immune organs (spleen and thymus). Male ICR mice were exposed to 0, 2, 10, 50, and 250 ppm of PFOA in drinking water for 21 days. PFOA differently altered T lymphocyte populations. In the spleen, all doses of PFOA decreased CD8<SUP>+</SUP> lymphocytes; CD4<SUP>+</SUP> lymphocytes were increased by 50 and 250 ppm of PFOA. Exposure to 250 ppm of PFOA increased CD8<SUP>+</SUP> lymphocytes in the thymus. In the histopathological evaluation, the spleen of 250 ppm PFOA-treated groups revealed the increase of lymphoid hyperplasia of white pulp without significant alteration of red pulp. The thymus of 250 ppm PFOA-treated group showed decreased thickness of the cortex and medulla, but lymphoid cells were more densely arranged. PFOA elevated the expression of proinflammatory cytokines (tumor necrosis factor α, interleukin-1β, and interleukin-6) in the spleen, and proto-oncogene, c-myc, in the spleen and thymus. In conclusion, our data demonstrated that PFOA has an immunomodulatory effect by altering T lymphocyte phenotypes and gene expression of proinflammatory cytokines. © 2008 Wiley Periodicals, Inc. A Study on Trouble Management and Necessity for Preventive Check in PACS

PACS has been run at the Kyung Hee University Medical Center since 2001, and the installation and operation of PACS have contributed to automation and quantification of center's medical environment. In order to classify tile annual number of trouble cases processed by PACS, the authorshave made a classification code system which enabled detailed statistical processes for each section. Such process method has not only shown the management efficiency to trouble management of PACS, but also raised the interests in frequently occurring troubles, and enabled the prediction of troubles that may occur hereafter. Predictable troubles lead to preventive check, and this has direct effects on medical treatment and the hospital administration. The authors intend to arouse the necessity of preventive check of PACS by analyzing trouble management processes for the last 1 year.
