        Safety of long-term subcutaneous free flap skin banking after skin-sparing mastectomy

        Background A persistent problem in autologous breast reconstruction in skin-sparing mastectomies is skin restoration after skin necrosis or secondary oncological resection. As a solution to facilitate reconstruction, skin banking of free-flap skin has been proposed in cases where the overlying skin envelope must be resected, as this technique spares the patient an additional donor site. Herein, we present the largest series to date in which this method was used. We investigated its safety and the possibility of skin banking for prolonged periods of time. Methods All skin-sparing mastectomies and immediate autologous breast reconstructions from December 2009 until June 2013 at our institution were analysed. Results We identified 31 patients who underwent 33 free flap reconstructions in which skin banking was performed. Our median skin banking period was 7 days, with a maximum duration of 171 days. In 22.5% of cases, the banked skin was used to reconstruct overlying skin defects, and in 9.6% of cases to reconstruct the nipple-areolar complex. Microbiological and histological investigations of the banked skin revealed neither clinical infections nor malignancies. Conclusions In situ skin banking, even for prolonged periods of time, is a safe and cost-effective method to ensure that skin defects due to necrosis or secondary oncological resection can be easily reconstructed.

        Aesthetic outcomes after surgical repair of pectus excavatum in females: Differences between patients and professional evaluators

        Background Pectus excavatum is less common in females than in males, and it often aggravates a coexistent breast asymmetry. We conducted a study comparing female patients' versus medical professionals' evaluation of pectus excavatum repair to assess differences in aesthetic outcome ratings. Moreover, we evaluated the influence of surgical correction on patients' self-perception. Methods Of 30 female patients who were initially screened, 18 patients (mean age, 20 years) who underwent bar removal after surgical correction of pectus excavatum deformity participated in the survey (60%). They completed a questionnaire rating their appearance before and after surgery and responded to a psychological questionnaire about the changes that they had experienced. The mean interval between pectus bar removal and evaluation was 28 months. Standardized preoperative and postoperative patient photographs were evaluated using the same questionnaire by a panel of medical professionals and students (n=24) and the results were compared. Results Patients rated their preoperative deformity as more severe than the other evaluators, revealing the significant impact of the deformity on patients' self-perception. Postoperatively, patient and professional evaluations were much better than before and were very similar. The psychological evaluation showed a clear improvement in well-being. The ratings of the medical professionals were not influenced by their degree of medical education. Conclusions Surgical correction of pectus excavatum in female patients positively influences body perception and psychological well-being. It should therefore not be considered as a merely aesthetic correction, but as an important procedure to restore a patient's self-perception.

        확산포집기로 공기중 ppb 농도수준의 휘발성유기물질 포집시 확산길이와 기류변화가 시료포집속도에 미치는 영향

        Passive samplers have been used for personal, indoor, and outdoor air monitoring of VOCs at ppb concentrations in community and office environments. The path length of modified passive sampler was shortened, so it was intended to increase an uptake rate. The performance of the modified 3M 3500 organic vapor monitor(OVM) as a tool for assessing exposures to toxic air pollutants in nonoccupational community environments was evaluated using combined controlled test atmospheres of six selected target volatile organic compounds(VOCs): benzene, methyl tert-butyl ether(MTBE), chloroform, 1,4-dichloro-benzene, tetrachloroethylene, and toluene. The experiments were conducted by exposing the dosimeters to concen-trations of 50∼100 ㎍/㎥ on six velocity(0.00, 0.02, 0.06, 0.12, 0.20. 0.30 m/sec) for 24 hours. IF the uptake rate was increased, that means that we could use the passive sampler more effectively. The uptake rates were increased linearly according to reduce the path length. Although the diffusion path length was shortened, the change of uptake rate was with ±25% of theoretical value, in-dicating that the modified passive sampler(TM) can be effectively used over the range of concentrations and enviro-nmental conditions tested with a 24-h sampling period if the face velocities were over 0.12㎧ for 6 components of VOCs. But when the face velocities were less than 0.12㎧, uptake rates were reduced more than expected values. So, the passive sampler with the shortened path length should be used at indoor or outdoor environment where the face velocity should be over about 0.10 ㎧. If the path length was shortened more, the uptake rate was more effected by starvation.

        Source Proximity and Meteorological Effects on Residential Ambient Concentrations of PM<sub>2.5</sub>, Organic Carbon, Elemental Carbon, and p-PAHs in Houston and Los Angeles, USA

        Concentrations of fine particulate matter (PM<sub>2.5</sub>) and several of its particle constituents measured outside homes in Houston, Texas, and Los Angeles, California, were characterized using multiple regression analysis with proximity to point and mobile sources and meteorological factors as the independent variables. PM<sub>2.5</sub> mass and the concentrations of organic carbon (OC), elemental carbon (EC), benzo-[a]-pyrene (BaP), perylene (Per), benzo-[g,h,i]-perylene (BghiP), and coronene (Cor) were examined. Negative associations of wind speed with concentrations demonstrated the effect of dilution by high wind speed. Atmospheric stability increase was associated with concentration increase. Petrochemical source proximity was included in the EC model in Houston. Area source proximity was not selected for any of the PM<sub>2.5</sub> constituents` regression models. When the median values of the meteorological factors were used and the proximity to sources varied, the air concentrations calculated using the models for the eleven PM<sub>2.5</sub> constituents outside the homes closest to influential highways were 1.5-15.8 fold higher than those outside homes furthest from the highway emission sources. When the median distance to the sources was used in the models, the concentrations of the PM<sub>2.5</sub> constituents varied 2 to 82 fold, as the meteorological conditions varied over the observed range. We found different relationships between the two urban areas, illustrating the unique nature of urban sources and suggesting that localized sources need to be evaluated carefully to understand their potential contributions to PM<sub>2.5</sub> mass and its particle constituents concentrations near residences, which influence baseline indoor air concentrations and personal exposures. The results of this study could assist in the appropriate design of monitoring networks for community-level sampling and help improve the accuracy of exposure models linking emission sources with estimated pollutant concentrations at the residential level.

        Infant Percutaneous Endoscopic Gastrostomy: Risks or Benefits?

        Background/Aims: To present a single center’s experience with percutaneous endoscopic gastrostomy (PEG) tube placement ininfants. Methods: Clinical records of infants who underwent PEG tube placement between January 2010 and December 2015 were reviewed. All patients underwent an upper gastrointestinal contrast study and an abdominal ultrasonography before the procedure. PEGs wereperformed with a 6-mm endoscope using the standard pull-through technique. Data regarding gestational age, birth weight, age andweight, days to feeding start, days to full diet, and complications were reviewed. Results: Twenty-three patients were included. The most common indication was dysphagia related to hypoxic-ischemic encephalopathy. Median gestational age was 37 weeks (range, 24–41) and median birth weight was 2,605 grams (560–4,460). Patients underwent PEGprocedures at a median age of 114 days (48–350); mean weight was 5.1 kg (3.2–8.8). In all patients but one, a 12-Fr tube was positioned. Median feeding start was 3 days (1–5) and on average full diet was achieved 5 days after the procedure (2–11). Six minor complications wererecorded and effectively treated in the outpatient clinic; no major complications were recorded. Conclusions: PEG is safe and feasible in infants when performed by highly experienced physicians.

        Effect of a professional soccer match in skin temperature of the lower limbs : a case study

        The purpose of this study is to analyze the relationship between lower limbs skin temperature (Tsk) recorded by infrared thermography and creatine kinase (CK) response following an official soccer game. A pro-fessional soccer player, from the first division Brazilian League, defender, 27 years and 183 cm of height was analyzed. Measures of Tsk with a thermal camera and blood CK concentrations were obtained 24 hr be-fore (24H-PRE), 24 (24H-POST) and 48 hr (48H-POST) after an official match. The results showed that CK values were 193 U/L 24 hr before, ris-ing to 1,083 U/L 24 hr after the game and 414 U/L 48 hr after the game. Anterior thigh Tsk was 31.5ºC, 33.8ºC, and 31.8ºC in the moments 24H-PRE, 24H-POST, and 48H-POST, respectively. Similarly, anterior leg presented temperature of 31.2ºC, 33.3ºC, and 31.5ºC at the same moments. Qualita-tive analysis of the thermograms showed that whole lower limbs are much warmer 24 hr after the match and certain areas as the right anteri-or thigh, the left anterior leg, both anterior ankles, and both posterior thighs have not fully recovered their initial Tsk 48 hr after the match. The results of this study indicate that participation in a professional soccer match can lead to significant increases in Tsk values measured by IRT 24 hr after the match. Considering this variable as an indicator of muscu-lar damage, it could help in the process of training control, being part of an injury prevention program in professional soccer clubs.

        Notch ligand Jagged1 promotes mesenchymal stromal cell-based cartilage repair

        Placenta-derived mesenchymal stromal cells (PMSCs) provide a promising cell source for tissue regeneration. However, rapid induction of PMSC chondrogenic differentiation during therapeutic transplantation remains extremely challenging. Here we undertook a study to determine if Notch inhibition by soluble Jagged1 (JAG1) peptides could be utilized to accelerate PMSC-induced cartilage regeneration in a mouse post-traumatic osteoarthritis (PTOA) model. Our results showed that treatment of PMSCs with soluble JAG1 significantly enhanced chondrogenesis in culture as shown by increased alcian blue staining and decreased Notch target Hes1 expression when compared to those in lgG-treated control cells. Importantly, significantly enhanced cartilage formation and decreased joint inflammation were observed when JAG1-treated PMSCs were injected into mouse PTOA knee joints. Finally, in vivo cell tracing showed that more JAG1-treated PMSCs remained in knee joint tissues and that JAG1-treated PMSCs exhibited greater PMSC chondrogenic differentiation than lgG-treated control PMSCs at 4 weeks after injection. These data indicate that transient Notch inhibition by soluble JAG1 could be used to enhance PMSC survival and chondrogenic differentiation, thereby increasing the therapeutic potential of PMSCs for cartilage regeneration.
