        Investigation on deck-stay interaction of cable-stayed bridges with appropriate initial shapes

        This paper provides a variety of viewpoints to illustrate the mechanism of the deck-stay interaction with the appropriate initial shapes of cable-stayed bridges. Based on the smooth and convergent bridge shapes obtained by the initial shape analysis, the one-element cable system (OECS) and multi-element cable system (MECS) models of the Kao Ping Hsi Bridge in Taiwan are developed to verify the applicability of the analytical model and numerical formulation from the field observations in the authors' previous work. For this purpose, the modal analysis of the two finite element models are conducted to calculate the natural frequency and normalized mode shape of the individual modes of the bridge. The modal coupling assessment is also performed to obtain the generalized mass ratios among the structural components for each mode of the bridge. The findings indicate that the coupled modes are attributed to the frequency loci veering and mode localization when the "pure" deck-tower frequency and the "pure" stay cable frequency approach one another, implying that the mode shapes of such coupled modes are simply different from those of the deck-tower system or stay cables alone. The distribution of the generalized mass ratios between the deck-tower system and stay cables are useful indices for quantitatively assessing the degree of coupling for each mode. These results are demonstrated to fully understand the mechanism of the deck-stay interaction with the appropriate initial shapes of cable-stayed bridges.

        Transarterial Chemoembolization Using Gelatin Sponges or Microspheres Plus Lipiodol-Doxorubicin versus Doxorubicin-Loaded Beads for the Treatment of Hepatocellular Carcinoma

        To retrospectively compare treatment of hepatocellular carcinoma (HCC) with transarterial chemoembolization (TACE) using gelatin sponges or microspheres plus lipiodol-doxorubicin vs. doxorubicin-loaded drug-eluting beads (DEB). A total of 158 patients with HCC received TACE from November 2010 to November 2011 were enrolled in this study, including 64 (40.5%) received TACE with lipiodol-doxorubicin and gelatin sponges (group A), 41 (25.9%) received TACE with lipiodol-doxorubicin and microspheres (group B), and 53 (33.5%) received TACE with doxorubicin-loaded DEB (group C). Tumor response and adverse events (AEs) were evaluated. No significant difference was found at baseline among the three groups. The doxorubicin dosage in group C wassignificantly (p < 0.001) higher compared to the dose used in groups A or B (median, 50 mg vs. 31 mg or 25 mg). Significantly (p < 0.001) more patients in group C achieved complete response compared to those in groups A or B (32.1% vs. 6.3% or 2.4%). Significantly (p < 0.001) less patients in group C had progressive disease compared to those in groups A or B (34.0% vs. 57.8% or 68.3%). Minor AEs were more common in groups A and B compared to group C, with rates of 54.7%, 34.1%, and 5.7%, respectively. In patients with HCC, TACE with DEB offers better safety and efficacy profiles compared to either TACE with gelatin sponges or TACE with microspheres.

        Analytical and experimental research on wind-induced vibration in high-rise buildings with tuned liquid column dampers

        In recent years, high-strength, light-weight materials have been widely used in the construction of high-rise buildings. Such structures generally have flexible, low-damping characteristics. Consequently, wind-induced oscillation greatly affects the structural safety and the comfort of the building's occupants. In this research, wind tunnel experiments were carried out to study the wind-induced vibration of a building with a tuned liquid column damper (TLCD). Then, a model for predicting the aerodynamic response in the across-wind direction was generated. Finally, a computing procedure was developed for the analytical modeling of the structural oscillation in a building with a TLCD under the wind load. The model agrees substantially with the experimental results. Therefore, it may be used to accurately calculate the structural response. Results from this investigation show that the TLCD is more advantageous for reducing the across-wind vibration than the along-wind oscillation. When the across-wind aerodynamic effects are considered, the TLCD more effectively controls the aerodynamic response. Moreover, it is also more useful in suppressing the acceleration than the displacement in biaxial directions. As s result, TLCDs are effective devices for reducing the wind-induced vibration in buildings. Parametric studies have also been conducted to evaluate the effectiveness of the TLCD in suppressing the structural oscillation. This study may help engineers to more correctly predict the aerodynamic response of high-rise buildings as well as select the most appropriate TLCDs for reducing the structural vibration under the wind load. It may also improve the understanding of wind-structure interactions and wind resistant designs for high-rise buildings.

        Impact of Esophageal Motility on Microbiome Alterations in Symptomatic Gastroesophageal Reflux Disease Patients With Negative Endoscopy: Exploring the Role of Ineffective Esophageal Motility and Contraction Reserve

        Background/AimsIneffective esophageal motility (IEM) is common in patients with gastroesophageal reflux disease (GERD) and can be associated with poor esophageal contraction reserve on multiple rapid swallows. Alterations in the esophageal microbiome have been reported in GERD, but the relationship to presence or absence of contraction reserve in IEM patients has not been evaluated. We aim to investigate whether contraction reserve influences esophageal microbiome alterations in patients with GERD and IEM. MethodsWe prospectively enrolled GERD patients with normal endoscopy and evaluated esophageal motility and contraction reserve with multiple rapid swallows during high-resolution manometry. The esophageal mucosa was biopsied for DNA extraction and 16S ribosomal RNA gene V3-V4 (Illumina)/full-length (Pacbio) amplicon sequencing analysis. ResultsAmong the 56 recruited patients, 20 had normal motility (NM), 19 had IEM with contraction reserve (IEM-R), and 17 had IEM without contraction reserve (IEM-NR). Esophageal microbiome analysis showed a significant decrease in microbial richness in patients with IEM-NR when compared to NM. The beta diversity revealed different microbiome profiles between patients with NM or IEM-R and IEM-NR (P = 0.037). Several esophageal bacterial taxa were characteristic in patients with IEM-NR, including reduced Prevotella spp. and Veillonella dispar, and enriched Fusobacterium nucleatum. In a microbiome-based random forest model for predicting IEM-NR, an area under the receiver operating characteristic curve of 0.81 was yielded. ConclusionsIn symptomatic GERD patients with normal endoscopic findings, the esophageal microbiome differs based on contraction reserve among IEM. Absent contraction reserve appears to alter the physiology and microbiota of the esophagus.

      • Low Expression of the FoxO4 Gene may Contribute to the Phenomenon of EMT in Non-small Cell Lung Cancer

        Because of its importance in tumor invasion and metastasis, the epithelial-mesenchymal transition (EMT) has become a research focus in the field of cancer. Recently, evidence has been presented that FoxO4 might be involved in EMT. Our study aimed to detect the expression of FoxO4, E-cadherin and vimentin in non-small cell lung cancers (NSCLCs). We also investigated clinical features and their correlations with the markers. In our study, FoxO4, E-cadherin and vimentin were assessed by immunohistochemistry in a tissue microarray (TMA) containing 150 cases of NSCLC. In addition, the expression level of FoxO4 protein was determined by Western blotting. The percentages of FoxO4, E-cadherin and vimentin positive expression in NSCLCs were 42.7%, 38.7% and 55.3%, respectively. Immunoreactivity of FoxO4 was low in NSCLC when compared with paired normal lung tissues. There were significant correlations between FoxO4 and TNM stage (P<0.001), histological differentiation (P=0.004) and lymph node metastasis (P<0.001), but no significant links with age (P=0.323), gender (P=0.410), tumor size (P=0.084), smoking status (P=0.721) and histological type (P=0.281). Our study showed that low expression of FoxO4 correlated with decreased expression of E-cadherin and elevated expression of vimentin. Cox regression analysis indicated FoxO4 to be an independent prognostic factor in NSCLC (P=0.046). These data suggested that FoxO4 might inhibit the process of EMT in NSCLC, and might therefore be a target for therapy.

      • Survival Analysis of Oral Squamous Cell Carcinoma in a Subgroup of Young Patients

        Oral squamous cell carcinoma (OSCC) is predominantly a disease of middle-aged men with long-term exposure to tobacco and alcohol. An increasing trend has been reported at a younger age worldwide. Clinical records of 100 patients under the age of 45 years treated specifically for oral cavity SCC in our hospital during a 10-year period were retrospectively analyzed to calculate the survival rates. An obvious male predominance coincided with smoking trend among Chinese young individuals and female patients were more likely to have no traditional risk factors such as smoking or drinking. The 5-year overall survival rate and disease-free survival rate were 61.0% and 75.5%, respectively, consistent with other published series over the decade showing a relatively better survival among the young. No significant differences clearly correlated with outcome when comparing non-smokers non-drinkers to ever-smokers and ever drinkers (P>0.05). Overall survival rate and disease free survival rate was found to be significantly higher in patients with early-stage disease than with advanced stage disease (P=0.001, P=0.009 respectively). The strong influence of clinical stage on prognosis emphasizes the importance of early diagnosis and treatment of oral malignancies for this unique clinical subgroup.

        The tyrosine kinase inhibitor nintedanib activates SHP-1 and induces apoptosis in triple-negative breast cancer cells

        Triple-negative breast cancer (TNBC) remains difficult to treat and urgently needs new therapeutic options. Nintedanib, a multikinase inhibitor, has exhibited efficacy in early clinical trials for HER2-negative breast cancer. In this study, we examined a new molecular mechanism of nintedanib in TNBC. The results demonstrated that nintedanib enhanced TNBC cell apoptosis, which was accompanied by a reduction of p-STAT3 and its downstream proteins. STAT3 overexpression suppressed nintedanib-mediated apoptosis and further increased the activity of purified SHP-1 protein. Moreover, treatment with either a specific inhibitor of SHP-1 or SHP-1-targeted siRNA reduced the apoptotic effects of nintedanib, which validates the role of SHP-1 in nintedanib-mediated apoptosis. Furthermore, nintedanib-induced apoptosis was attenuated in TNBC cells expressing SHP-1 mutants with constantly open conformations, suggesting that the autoinhibitory mechanism of SHP-1 attenuated the effects of nintedanib. Importantly, nintedanib significantly inhibited tumor growth via the SHP-1/p-STAT3 pathway. Clinically, SHP-1 levels were downregulated, whereas p-STAT3 was upregulated in tumor tissues, and SHP-1 transcripts were associated with improved disease-free survival in TNBC patients. Our findings revealed that nintedanib induces TNBC apoptosis by acting as a SHP-1 agonist, suggesting that targeting STAT3 by enhancing SHP-1 expression could be a viable therapeutic strategy against TNBC.

        Antioxidative and Hepatoprotective Effects of Magnolol on Acetaminophen-induced Liver Damage in Rats

        Acute liver failure (ALF), an often fatal condition characterized by massive hepatocyte necrosis, is frequently caused by drug poisoning, particularly with acetaminophen (N-acetyl-p-aminophenol/APAP). Hepatocyte necrosis is consecutive to glutathione (GSH) depletion and mitochondrial damage caused by reactive oxygen species (ROS) overproduction. Magnolol, one major phenolic constituent of Magnolia officinalis, have been known to exhibit potent antioxidative activity. In this study, the anti-hepatotoxic activity of magnolol on APAP-induced toxicity in the Sprague-Dawley rat liver was examined. After evaluating the changes of several biochemical parameters in serum, the levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), and lactate dehydrogenase (LDH) were elevated by APAP (500 mg/kg) intraperitoneal administration (8 and 24 h) and reduced by treatment with magnolol (0.5 h after APAP administration; 0.01, 0.1, and 1 μg/kg). Histological changes around the hepatic central vein, lipid peroxidation (thiobarbituric acid-reactive substance/TBARS), and GSH depletion in liver tissue induced by APAP were also recovered by magnolol treatment. The data show that oxidative stress followed by lipid peroxidation may play a very important role in the pathogenesis of APAP-induced hepatic injury; treatment with lipid-soluble antioxidant, magnolol, exerts anti-hepatotoxic activity. Our study points out the potential interest of magnolol in the treatment of toxic ALF.

        중국 윈난성 뎬츠(滇池)보호에서의 공중참여에 관한 연구 : 자원동원이론의 시각으로

        This research takes the private environment organization--Green Kunming as an example, to explore the growth of resources by mobilization. The study includes a public participation activity--Protecting Lake Denz, the largest lake in Kunming, Yunnan Province. To ensure this organization survive and thrive, we must have sufficient resources and are able to mobilize them. This study focuses on the development of Green Kunming ancestry conservation activities based on institutional resources, financial resources, human resources and social resources. These resources are organically integrated with each other and are characterized by collective actions. The survey results show that Green Kunming is limited by institutional resources, but this is also a prerequisite for obtaining legitimacy and obtaining new resources. Financial resources are mainly funded by private fund meetings. and these funds are a major participatory survival method including projects. In mobilizing human resources, the members are mainly volunteers. Doing this will ensure the sustainability of group development, promote the formation of social capital, and fully mobilize new media as a social resource. This study aims to determine the growth of citizens in our society, and hope to use it as an indicator to measure the degree of the political structure openness to the Chinese public participation and the degree of democratization in the public sphere. 본 연구는 중국 윈난성 쿤밍시에서 가장 큰 호수인 뎬츠(滇池)에 대한 보호를 비롯한 공중의 참여활동 중에서 민간 환경단체-녹색곤명을 사례로, 자원동원론의 시각으로 참여과정에 자원을 어떻게 동원하면서 성장하였는가를 살펴본다. 단체가 생존하고 성장하려면 다양한 자원을 소유함과 아울러 자원을 충분히 동원하여야 한다. 이런 자원들은 또 서로 유기적으로 결합되면서 단체활동 전개의 특성을 가진다. 본고에서는 자원을 동원하기 위한 전제조건, 재정적 자원, 인적 자원, 사회적 자원을 토대로 뎬츠보호의 주요한 행위주체‒녹색곤명의 뎬츠보호 활동의 전개에 대한 분석이 중심을 이룬다. 연구결과 녹색곤명은 합법적인 지위를 취득하여 새로운 자원을 얻을 수 있는 정치적 기회구조를 확충하고, 재정적 자원은 주로 민간 기금회의 자금 지원을 통해 얻는데 이러한 자금조달은 프로젝트를 비롯한 참여방식이 된다. 인적 자원의 동원을 볼때 구성원은 주로 자원봉사자들인데 이러한 특성은 인력동원이 용이하고 자발적으로 활동할 수 있으며 단체 발전의 지속성을 보장하며, 사회자본의 형성을촉진할 수 있다. 사회적 자원으로는 주로 뉴미디어를 활용했는데 전파속도가빠르고 전파범위가 넓어서 공론장의 형성을 촉진하였다. 이러한 현상을 통해중국에서 공중참여에 대한 제도적 보장은 이미 갖춰져 있고, 정치적 기회 구조는 질적으로 변화하고 있으며, 정부의 집권이념의 변화가 시민사회의 성장을추진하고 있다는 결론을 얻을 수 있었다.
