        Is Bladder Training by Clamping Before Removal Necessary for Short-Term Indwelling Urinary Catheter Inpatient? A Systematic Review and Meta-analysis

        Purpose: Urinary catheterization is a common technique in clinical practice. There is, however, no consensus on management prior to removal of the indwelling catheter for short-term patients. This systematic review examined the necessity of clamping before removal of an indwelling urinary catheter in short-term patients. Methods: A systematic literature review was conducted using eight databases and predetermined keywords-guided searches. Some 2,515 studies were evaluated. Ten studies that met the inclusion criteria were selected. Results: The quality of the studies was assessed using the Jadad scoring system. Only 40.0% of studies were rated as high quality. This review found that catheter clamping prior to removal was not necessary for the short-term patient. When made a comparison with the unclamping group, there was no significant difference in recatheterization risk, risk of urine retention, patients’ subjective perceptions and rate of urinary tract infection. Conclusions: This review indicated that bladder training by clamping prior to removal of urinary catheters is not necessary in short-term catheter patients. In addition, clamping carries the risk of complications such as prolonging urinary catheter retention and urinary tract injury. Further investigation requires higher quality methodologies and more diverse study designs.

        Saikosaponin a and Saikosaponin d Inhibit Proliferation and Migratory Activity of Rat HSC-T6 Cells

        The proliferation and migration of hepatic stellate cells (HSCs) profoundly impact the pathogenesis of liver inflammation and fibrogenesis. As a perennial herb native to China, Bupleurum falcatum is administered for its anti-inflammatory,antipyretic, and antihepatotoxic effects. Saikosaponin a (SSa) and Saikosaponin d (SSd) are the major active components of triterpene saponins in Bupleurum falcatum. This study analyzes how SSa and SSd affect rat HSC-T6 cell line proliferation and migration. Experimental results indicate that, in addition to suppressing HSC-T6 proliferation, wound healing activity and cell migration in a time- and dose-dependent manner, SSa and SSd significantly induce apoptosis. Additionally, SSa and SSd decreased the expressions of extracellular matrix-regulated kinase 1/2 (ERK1/2), platelet-derived growth factor receptor 1 (PDGFR1), and subsequently transforming growth factor-b1 receptor (TGF-b1R), a-smooth muscle actin, TGF-b1 and connective tissue growth factor. They also decreased phosphorylation of p38 (p-p38) and ERK1/2 (p-ERK1/2) of HSC-T6. Furthermore, both SSa and SSd can block PDGF-BB and TGF-b1-induced cell proliferation and migration of HSC-T6. These results suggest that SSa and SSd may inhibit proliferation and activation of HSC-T6, and the modulated mechanisms warrant further study.

        Acupuncture for ventilator-dependent patients at a hospital-based respiratory care center: A randomized controlled trial

        Background: In intensive care units, mechanical ventilation is an important therapy to help patients with dysp- nea. However, long-term ventilator dependence would consume huge medical resources and increase the risk of morbidity and mortality. The aim of the study was to examine the efficacy of the acupuncture combined with western medical care on ventilator parameters in ventilator-dependent patients. Methods: In this clinical trial, 80 ventilator-dependent patients aged 20 to 80 years old were randomly assigned to acupuncture group and control group in the respiratory care center (RCC) of Changhua Christian Hospital. Besides regular medical care and therapy, participants in the acupuncture group received acupuncture therapy at the same 17 acu-points for 20 minutes once a day, a total of 12 sessions. The ventilator parameters were recorded to evaluate the respiratory efficiency for all participants. The primary outcome was rapid shallow breathing index (RSBI), and secondary outcomes were respiratory rate (RR), tidal volume (TV) and ventilation per minute (MV). Results: Though there was no significant difference in the parameter between the acupuncture group and the control group, we found the trend of decreasing RSBI in the acupuncture group. In subgroup analyses, the mean of RSBI significantly decreased 16.02 (with the SD in 60.84) in acupuncture group, while it increased 17.84 (with the SD in 39.38) in control group (p = 0.036) after 12 sessions. Conclusion: Acupuncture treatment can improve breathing ability of patients with respirator dependence in respiratory care center.

        Risk factor analysis of coexisting endometrial carcinoma in patients with endometrial hyperplasia: a retrospective observational study of Taiwanese Gynecologic Oncology Group

        Objective: To evaluate the clinical outcome and parameters related to coexisting endometrial carcinoma in women with tissuediagnosed endometrial hyperplasia. Methods: Between January 1991 and December 2009, three hundred and eighty-six patients with the presumptive diagnosis of endometrial hyperplasia were retrieved. Among these, one hundred and twenty-five patients were identified as having coexisting endometrial carcinoma in hysterectomy specimens. The three hundred and eighty-six patients were divided into two groups: the hyperplasia-benign group (261 cases) and the hyperplasia-malignant group (125 cases). Several clinical parameters including age, menopausal status, history of abnormal uterine bleeding, obstetrical history, medical history of diabetes and hypertension, BMI, and preoperative pathologic results were investigated. Results: Age ≥53 (odds ratio [OR], 2.40; 95% confidence interval [CI], 1.26 to 4.57), menopausal status (OR, 2.07; 95% CI, 1.14to 3.76), diabetes history (OR, 7.33; 95% CI, 2.79 to 19.26), abnormal uterine bleeding (OR, 3.99; 95% CI, 1.22 to 13.02), atypical endometrial hyperplasia (OR, 7.38; 95% CI, 4.03 to 13.49), and body mass index ≥27 (OR, 3.24; 95% CI, 1.76 to 5.97) were independent risk factors for prediction of endometrial hyperplasia coexisting with endometrial carcinoma. The diagnostic efficacy of atypical endometrial hyperplasia to predict the endometrial hyperplasia coexisting with endometrial carcinoma was better than or similar to those of other independent factors and combinations of these factors. Conclusion: Coexisting malignancy should be considered when examining endometrial hyperplasia patients with the related risk factors, especially atypical endometrial hyperplasia.

      • Predictors of Positive Bone Metastasis in Newly Diagnosed Prostate Cancer Patients

        Background: The prevalence of prostate cancer (PCa) has been increasing in recent years. Treatment strategies are largely based on the results of bone scan screening. Therefore, our aim was to investigate predictors of positive bone metastasis in newly diagnosed PCa patients. Materials and Methods: After extensive review, 336 consecutive patients newly diagnosed with PCa between April 2010 and November 2013 at our institution were enlisted in the study. Patients were divided into two groups according to bone scan results. Univariate analyses (Chi-square test for discrete variables and independent t-test for continuous variables) were applied to determine the potentially significant risk factors associated with distant bone metastasis. Binary logistic regression analyses were used to further investigate the influence of these factors on bone metastasis. Results: The patient mean age was $71.9{\pm}8.6years$ (range: 48 to 94 years). The mean prostate specific antigen (PSA) level and biopsy Gleason score were $260.2{\pm}1107.8ng/mL$ and $7.4{\pm}1.5$, respectively. The body mass index (BMI) for the series was $24.5{\pm}3.4kg/m^2$. Sixty-four patients (19.0%) had a positive bone scan result. Patients with positive bone scan results had a significantly lower BMI ($23.3{\pm}3.5$ vs. $24.8{\pm}3.3$; p=0.003), a higher Gleason score ($8.5{\pm}1.1$ vs. $7.1{\pm}1.5$; p < 0.001), and a higher PSA level ($1071.3{\pm}2337.1$ vs. $69.4{\pm}235.5$; p < 0.001) than those without bone metastasis. Multivariate logistic regression analysis employing the above independent predictors demonstrated that a Gleason score of ${\geq}7$, clinical stage ${\geq}T3$, $BMI{\leq}22kg/m^2$, and an initial PSA level of ${\geq}20ng/mL$ were all independent predictors of bone metastasis. Conclusions: A bone scan might be necessary in newly diagnosed PCa patients with any of the following criteria: clinical stage T3 or higher, a Gleason score of 7 or higher, BMI equal to or less than 22, and a PSA level of 20 or higher.

      • Motion planning with obstacle avoidance of an UR3 robot using charge system search

        For a cyber-physical system (CPS) of a future intelligent factory, a robotic manipulator is requested to co-work with human efficiently and safely in an environment with flexible arrangements. Therefore, an autonomous path planning of robotic manipulator is the most necessary issue to be resolved for the factory automation. For the robotic manipulator, optimizations and artificial intelligence (AI) methods are widely used to investigate the autonomous dynamic path-planning tasks with obstacle avoidance. Among these methods, the Rapidly Exploring Random Tree (RRT) algorithm has been widely used in path planning for a complex environment, because the RRT algorithm has the advantages of perfect expansion, probability completeness, and fast exploring speed. However, for some practical cases, the existing RRT algorithm may obtain a discontinuous solution of the angular trajectory. To solve the above problem, we studied a particle swarm optimization with the charge search system (CSS) to find the optimal path planning with obstacle avoidance. The steps of the proposed method are mentioned as follows: (1) establish the configuration space with the obstacle regions, (2) formulate the motion planning with obstacle using the CSS method and (3) use the PSO method to solve the path planning problem. Finally, the simulation of the path-planning task with obstacle avoidance is visually illustrated using the software RoboDK and the proposed method is implemented by the real-time experiments of the UR3 robot.

      • Influence of Current Measurement Error to Split Capacitor Voltage in Single-Phase Half-Bridge Grid Connected Inverter

        For a single-phase half-bridge grid connected inverter with a split capacitor configuration, the adoption of the Proportional-Integral (PI) will generally suffice the need of current control action in normal inverter operation. However, the presence of measurement error in the current feedback signal can cause distortion in the grid current. Since the grid and the midpoint of split capacitors are connected through a neutral line, the unwanted dc component will flow to both capacitors and drift their voltages. This article aims to formulate mathematical equations to elucidate this phenomenon, and to recommend suitable gain ratio of the controller to maintain the safety of the inverter and to collect the voltage deviation profile before specific additional control action can be incorporated. Computer simulations under MATLAB/SIMULINK environment of 1 kVA inverter rating were performed to corroborate the theoretical analysis and the proposed method.

      • Research on the Positioning of Work Machines in Greenhouse Using Bluetooth Low Energy Indoor Detection Technology

        Outdoor is using a mobile device combined with global positioning system (GPS) for positioning, the most popular application today; and indoor is based on the signal strength of Bluetooth, Wi-Fi, ZigBee and other wireless communication technology as a positioning basis. This research uses a fruit harvester for positioning in a greenhouse. However, the positioning accuracy of GPS in a greenhouse is easily affected and unable to be precise due to obstacles, communication channels, and multiple paths. To solve this problem, this study used iBeacon Bluetooth 4.0 module as a communication device for indoor positioning. In the experiment, ten iBeacons were used as the sending end, placed at various points in the room to serve as a positioning basis, and then installed an iBeacon combined with the Arduino on a fruit harvester as the receiving end. Then use the intensity of the sending and receiving signal as the basis, with the least square method, to calculate the corresponding distance between the signal receiving point and the sending point, and finally estimate the position of the fruit harvester by triangulation. At present, the preliminary experiment results show that, with the classroom as the test space, the indoor positioning success rate can reach 77% within 3 meters of the error value. The same method will be applied to find out the best positioning method for the harvester in the greenhouse.
