심근경색 모델 흰쥐에서 스트레스가 B-type Natriuretic Peptide 발현 및 심박변이율 변화에 미치는 영향

심박변이율의 감소는 심근 경색 환자에서 예후의 악화와 관련되어 있다. 심박변이율 중 very low frequency가 심부전환자의 예후를 판단할 수 있는 독립적 인자로서 사용이 가능하다고 보고된 바 있으며, 심실의 압력 증가에 반응해서 생성되는 B-type natriuretic peptide가 심근 경색의 예후 인자로서 사용할 수 있음 역시 보고된 바 있다. 본 연구에서는 심근경색 모델을 제작하여 일정의 회복 기간을 거친 다음 다시 일정 기간의 스트레스를 겪게 한 후 심박변이율의 변화 및 심실 조직에서의 BNP 발현 정도를 비교하여 스트레스에 의한 영향을 관찰하고자 하였다. Sprague-Dawley계 수컷 흰쥐 15마리를 대상으로 하였다. 실험동물은 인위적 스트레스 및 수술을 받지 않은 대조군(CON, n=5), 심근경색 수술을 받은 후 restraint stress를 받지 않은 군(MI+No Stress, n=5), 심근경색 수술을 받은 후 1주일 동안 restraint stress를 받은 군(MI+Stress, n=5)으로 분류하였다. 심근경색 모델 제작 직후 15분 이상 심전도의 변화를 기록하였으며, 2개월 정도 후 심근경색 수술을 받은 동물을 두 군으로 나누어 그 중 한 군에는 1주일 동안 restraint stress를 가한 후 restraint stress를 가하지 않은 동물과 심박변이율을 비교 분석하였다. 심실 조직은 좌심실 전벽 부위의 위축을 관찰할 수 있었고, 대식세포에 의하여 응고, 괴사된 심근세포의 탐식과 혈관이 풍부한 육아조직 및 섬유 변화를 볼 수 있었다. 심박변이율은 심근경색 수술을 받은 직후 심박수는 유의한 증가를 보였고, standard deviation of the normal to normal intervals (SDNN), very low frequency (VLF) 및 low frequency (LF)의 유의한 감소를 보였다. 심근경색 수술을 받았던 동물에서 두 달 정도의 회복 기간을 거친 후 스트레스를 받지 않은 군은 심박수 및 기타 심박변이율 분석에서 SDNN 값을 제외하고는 정상군과 유의한 차이를 관찰할 수 없었던 반면 1주일 동안 스트레스를 받은 군에서는 심박수가 다시 유의하게 증가했을 뿐만 아니라 SDNN, VLF 및 LF 역시 정상군과 유의한 차이를 보였다. 이상의 결과를 종합하면 심근경색 동물에 대해 1주일 동안 스트레스를 가한 결과 심박변이율이 감소하고 심실에서의 BNP 발현은 더욱 증가하여 악화함을 보여주고 있다. Diminished heart rate variability (HRV) is associated with less favorable prognosis after myocardial infarction (MI). It has been reported that very low frequency (VLF) power in HRV analysis is an independent risk predictor in patients with congestive heart failure and B-type natriuretic peptide (BNP) can be used as a prognostic factor of MI. The purpose of this study was to investigate whether stress has an effect to the changes of BNP expression and/or heart rate variability in MI model in rats. Male Sprague-Dawley rats were divided into 3 groups: control group (CON), MI group (MI+No Stress), and MI followed by stress group (MI+Stress). MI+Stress group rats were raised for a two month recovery period after the operation, followed by being exposed to restraint stress for 2 hours per day for 1 week. Electrocardiogram was recorded after the operation and the last day after 1 week of stress. The frequency components of HRV were calculated in the frequency domain such as VLF, low frequency (LF), high frequency (HF) and so on. In HRV analysis, standard deviation of the normal to normal intervals (SDNN) was significantly reduced in both groups compared to the control group. VLF and LF also were significantly reduced in MI+Stress group compared to the control groups. In addition, BNP expression in western blotting was shown the strongest bands in MI+ Stress group among experimental groups. These results suggest that BNP and HRV were aggravated by stress in MI rat model. (Korean J Str Res 2010;18:275∼285)

Background: Although a direct association has been established between oral health management and maintaining military combat readiness and fulfilling defense duties, unmet dental-care needs have been consistently reported, and there has been little research on the role of mobile healthcare services in addressing this issue. This study explored the association between unmet dental-care needs and the intention to use mobile healthcare services among military personnel. Methods: This study was conducted on military personnel who visited a military hospital in Gyeonggi-do, Korea, from August 19 to August 31, 2024. A total of 150 self-administered questionnaires were distributed and 135 valid responses were analyzed. The analysis included general characteristics, dental care utilization, and intention to use mobile healthcare services. Statistical analyses, such as t-tests, ANOVA, and multiple regression, were employed to determine the factors influencing the intention to use mobile healthcare services. Results: This study found that military personnel with unmet dental-care needs had significantly higher intentions to use mobile healthcare services than those without such needs. The analysis revealed that the key factors influencing this intention were perceived health status and previous dental-care utilization. Specifically, participants who rated their overall health as better and those who had used dental care services in the past year showed a greater tendency to express interest in mobile healthcare services than those who did not. Furthermore, unmet dental-care needs were more prevalent among soldiers who reported limited access to dental facilities owing to time constraints or a lack of prioritization of oral health, which increased their inclination toward utilizing mobile healthcare solutions as an alternative. Conclusion: Mobile healthcare solutions can offer personalized and timely care as viable alternatives for improving oral health management in the military. Moreover, integrating mobile healthcare services into military health systems could significantly reduce unmet dental-care needs and enhance overall combat readiness by promoting better health outcomes.

Background: The purpose of this study was to confirm the major motivation for and self-efficacy of career decisions among dental hygiene students and to analyze differences in career decision self-efficacy according to the major motivation.Methods: An online survey was conducted among 194 dental hygiene students. To investigate career decision self-efficacy, 18 questions were posed, including 5 questions on future planning, 4 questions on self-evaluation, 4 questions on job information collection, 3 questions on goal setting, and 2 questions on problem solving. The survey was conducted using a 5-point scale. Results: The motives for choosing a major were a high employment rate (68.6%), a desire to become a professional (36.1%), according to aptitude (27.8%), recommendations by others (26.3%), and according to grades (12.9%). The career decision self-efficacy of the study participants was in the order of self-evaluation factors (3.75 points), problem solving factors a (3.62 points), job information collection factors (3.59 points), future planning factors (3.46 points), goal setting factors (3.44 points) score was high. The career decision self-efficacy scores were higher when the major motivation was “because I want to become a professional” and “because I think I have an aptitude for it.” Conclusion: It is necessary to give students who wish to major in dental hygiene an opportunity to consider whether they have an aptitude for it by providing information on not only major subjects but also the duties and roles of dental hygienists. In addition, it is necessary to develop a career education program to help dental hygiene students set their career goals.
Organizational Member Attitudes According to Workplace Type of Dental Hygienist

Background: This study aimed to investigate whether there is a difference in organizational member attitudes by workplace environment and workplace types in dental hygienists working at dental health centers.Methods: A face-to-face survey or online survey was administered to dental hygienists working at dental health centers, and a total of 95 subjects were included. The survey includes 13 items to ask factors affecting employee’s job satisfaction. Also, some questions were included to assess perception of organizational member attitude: five about organizational citizenship behavior, two about innovative behavior, and four about organizational commitment. Results: Dental hygienists working at dental clinics were more satisfied with their incomes and numbers of working days, while those from dental university hospitals and general hospitals were more satisfied with education support. In addition, hygienists working at dental hospitals were more satisfied with job autonomy, individual work capability, safe working environment, personal relation, potential for personal development, and positive labor-management relations, compared to those working at general and university hospitals (p<0.05). Among the items about perception of organizational member attitudes, the scores of items about organizational citizenship behaviors were higher, whereas the scores of items about innovative behaviors were relatively lower. Individuals working at dental hospitals than those working at general and university hospitals, chiefs and team and department leaders than team members, and those with increased satisfaction with current workplace had more positive perception of organizational member attitudes. Conclusion: For dental hygienists to have positive attitudes as organizational members, working environment should be improved, and executives of dental healthcare centers should pay attention to improving job satisfaction of organizational members. Moreover, since dental hygienists need to improve their perception of innovative behaviors and citizenship behaviors to strengthen specialty of dental hygienists in a changing dental healthcare, relevant training should be addressed in refresher courses or school programs.
Anticancer Activity of Asparagus cochinchinensis Extract and Fractions in HepG2 Cells

Asparagus cochinchinensis Merrill (Liliaceae) has been traditionally used for the treatment of cancer in Korea and China, but its anticancer activity and underlying mechanism remain to be defined. Anticancer activities were investigated on fractions obtained from A. cochinchinensis 70% ethanol extract (ACE-EtOH) in human hepatocellular carcinoma HepG2 cells. Ethylacetate fraction from A. cochinchinensis extract (ACE-EA), more effective than other fractions, induced apoptosis of HepG2 (IC50=72.33±0.34 μg/mL), as revealed by apoptotic feature observation, increased capase-3 activity and Poly ADP ribose polymerase cleavage, and decreased expression of X-linked inhibitor of apoptosis protein in a dose-dependent manner. Protein levels of autophagy-related molecules, microtubule-associated protein 1 light chain 3 α and beclin 1, appeared to be induced by ACE-EA, suggesting ACE-EA exhibits anti-cancer activity with induction via both apoptosis and autophagy signaling pathways in HepG2 cells.

Background: This study aims to investigate the perceptions of dentists and dental hygienists regarding quality dental job conditions and to identify differences in perceptions based on job type. Methods: As a result of conducting face-to-face and online surveys, data from a total of 132 people were analyzed. In order to investigate the perception of quality job conditions, the importance of a total of 13 items was investigated, and the work policies and job satisfaction of the current workplace were examined. Since the data did not follow a normal distribution, a non-parametric test, the Mann–Whitney U test, was performed. Results: Both dentists and dental hygienists perceived income and working hours to be of priority importance for quality job conditions. Dental hygienists valued holiday support and welfare, human relations, and personal development potential more than dentists (p<0.05). Looking at differences by job type, dental hygienists rated all conditions as more important than dentists except income, indicating a statistically significant difference (p<0.05). In terms of the work policies, 96.2% of the practices in the study were required to have the four types of social security contributions, but fewer had flexible working hours (19.7%), healthcare support (23.5%), and incentives (25.0%). Of the participants, 60.6% had parental leave available at their workplace, and dental hygienists had statistically significantly higher job satisfaction when parental leave was available (2.57 points) than when it was not (p<0.05). Conclusion: Quality dental jobs are an important factor in keeping workers happy and maintaining an efficient practice. Dental practice owners need to pay attention to the quality of jobs required by the dental workforce, provide flexible working hours and welfare programs such as parental leave, and create a workplace atmosphere and human resource management system that supports the use of these programs.

Background: The objective of the present study was to specifically divide the various work performed by dental hygienists in clinical practice for legal amendments regarding problems associated with conflict between job roles and illegal delegation to establish key basic data for legislation and policy utilization for realization of legal scope of dental hygienists. Methods: The study used work reports drafted based on research methods in the “Second Job Analysis Report on Dental Hygienists” researched by the Korean Health Personnel Licensing Examination Institute in 2012 and “Opinions of Dentists on Actual and Legal Work of Dental Hygienists,” a report published by the Korean Dental Hygienists Association. Of these, the study focused on conservation dentistry, pediatric dentistry, prosthodontics, oral and maxillofacial surgery, and dental implant treatment, which make up some of the fields covered by dental hygiene practice, to investigate and analyzed work performed by clinical experience. Results: Analysis of work actually performed in dental practice showed that for work related to 33 items presented in the study methods, the participants responded that they are currently performing such work or are likely to perform such work in the future, although there were differences by year. Investigation by type of workplace showed that dental hygienists working in university hospitals could perform the work presented if they had ≥5 years of dental hygienist experience, whereas dental hygienists working in dental clinics or hospitals could perform simple duties in their first year and performed more diverse duties with greater degree of difficulty after their second to fourth year. Conclusion: The reality that medical assistance during surgical operations and various procedures that is still being performed is no longer legally protected directly contradicts the needs in dental practice, and thus, there is the need to amend laws that are realistic by clearly recognizing the work of dental hygienists.
Comparative Study of Extraction Solvents on the Anti-inflammatory Effects of Scutellaria baicalensis

Objectives: This study was performed to evaluate the influence of different extracting solvents (water, methanol, ethanol, or n-hexane) on the anti-inflammatory efficacy of Scutellaria baicalensis (Lamiaceae), which has been used widely as a traditional herbal medicine for its anti-inflammatory properties. Methods: The ability of each extract to inhibit the production of pro-inflammatory mediators such as NO, TNF-$\alpha$, and $PGE_2$ by lipopolysaccharide (LPS)-stimulated mouse macrophage RAW 264.7 cells was measured. Results: The results showed that extraction solvents (except n-hexane) for S. baicalensis showed significant inhibitory effects on NO, TNF-$\alpha$ and $PGE_2$ production. Especially, methanol was the solvent with the greatest activity against NO and $PGE_2$ production. However, there was no difference between the extracts for inhibitory activity of TNF-$\alpha$. Conclusion: The present study suggests that methanol is a superior extraction solvent than water, ethanol, or n-hexane for maintaining the anti-inflammatory effects of S. baicalensis.
