        Effects of Dexmedetomidine Infusion During Sevoflurane Anesthesia on Otoacoustic Emissions

        Background and Objectives: Knowing the ototoxic potential of the agents used in medical treatments is important for the protection of hearing. Although we have knowledge regarding some effects of dexmedetomidine, which is an anesthetic-sparing drug, its influence over the hearing system has never been studied and is obscure yet. The aim of this study is to determine the effects of intravenous dexmedetomidine application during sevoflurane anesthesia on otoacoustic emissions (OAEs). Subjects and Methods: This prospective randomized study was performed on 60 patients (34 male, 26 female, mean age: 30.6±9.2 years) who were scheduled for an elective surgery under general anesthesia and the patients were enrolled and randomly divided into 2 groups. They received dexmedetomidine (Group D) or Saline (Group S) infusion during a standardized Sevoflurane anesthesia. Transient and distortion product OAEs were measured preoperatively and postoperatively (24th hour). OAE results were compared within and between groups. Results: In group D postoperative OAEs were lower than preoperative OAEs and postoperative levels of group S, especially at low frequencies (p<0.05). Conclusions: Dexmedetomidine infusion affects the micromechanical function of cochlea especially in the low-frequency region. Dexmedetomidine should be carefully used during general anesthesia to avoid its probable harmful effects on cochlear micromechanics.

        Premedication Methods in Nasal Endoscopy: A Prospective, Randomized, Double-Blind Study

        Objectives. To identify the optimal pharmacological method of preparing patients for nasal endoscopy. Methods. Twenty healthy volunteers were enrolled in this prospective, randomized, double-blind study. Four types of medications were applied in their nostrils with binary combinations of spray bottles on four different days in a random order: placebo (normal saline [NS]+NS), decongestant (NS+oxymetazoline), anesthetic (NS+lidocaine), and decongestant plus anesthetic (oxymetazoline+lidocaine). Rigid nasal endoscopy was performed 10 minutes after spray application. The volunteers evaluated the discomfort caused by each spray application, and nasal pain scores due to the passage of the endoscope. The physicians quantified nasal decongestion using a visual analogue scale. Endoscopy duration as well as pulse and mean blood pressure (MBP) before spray application, 10 minutes after the application, and immediately after endoscopic examination were also recorded. Results. The discomfort caused by lidocaine was significantly higher than that caused by the other sprays (P<0.001). The lowest pain score related to endoscopy was obtained for oxymetazoline+lidocaine (P<0.001). Nasal decongestion was best achieved with NS+oxymetazoline (P<0.001). Endoscopy duration was the shortest for oxymetazoline+lidocaine (P<0.05). Statistically significant MBP changes were only seen with the application of NS+oxymetazoline (P<0.05). However, neither MBP nor pulse rate change was significant clinically. Conclusion. Application of decongestant and anesthetic sprays together seems to be the best method of pharmacological preparation of patients for nasal endoscopy.

        Zinc Picolinate in the Prevention of Leiomyoma in Japanese Quail

        Epidemiologic studies suggest that zinc deficiency may be associated with increased risk of cancer. We investigated the effects of zinc picolinate supplementation on the development of leiomyomas, malondialdehyde (MDA), 8-isoprostane, 4-hydroxyalkenal (HAE), and 8-hydroxy-2′-deoxyguanosine (8-OHdG) levels, and heat shock protein 70 (Hsp70) expression in Japanese quails. One hundred fifty quails (6 months old) were assigned to three treatment groups consisting of 50 birds in each group. Birds were fed either a basal diet or the basal diet supplemented with 30mg or 60mg of zinc/kg of diet. The animals were sacrificed after 350 days, and the tumors were identified. Zinc picolinate supplementation did not affect the number of leiomyomas compared to control birds (P>.05). However, the tumors in zinc-fed birds were smaller than those found in control birds (P=.01) Serum MDA, 8-isoprostane, and HAE levels were lower in the treatment groups than in the control group: MDA, 1.95 versus 0.93μmol/L; 8-isoprostane, 108 versus 85pg/mL; HAE, 1.55 versus 0.96μmol/L (P=.01 for all three parameters). The concentrations of serum 8-OHdG, which is a marker of oxidative damage, in the groups were 28.5, 23.6, and 20.1ng/mL, respectively (P=.01). Hsp70 expression was significantly decreased in zinc-treated birds (P<.01). The results indicate that dietary zinc picolinate supplementation reduces the growth of spontaneously occurring leiomyomas of the oviduct in the Japanese quail. Clinical trials should be conducted to investigate the efficacy of zinc supplementation in the prevention and treatment of uterine leiomyoma in humans.

        Lycopene Protects Against Spontaneous Ovarian Cancer Formation in Laying Hens

        Background: Dietary intake of lycopene has been associated with a reduced risk of ovarian cancer, suggesting its chemopreventive potential against ovarian carcinogenesis. Lycopene’s molecular mechanisms of action in ovarian cancer have not been fully understood. Therefore, in the present study, we investigated the effects of lycopene on the ovarian cancer formation using the laying hen model, a biologically relevant animal model of spontaneous ovarian carcinogenesis due to high incidence rates similar to humans. Methods: In this study, a total of 150 laying hens at age of 102 weeks were randomized into groups of 50: a control group (0 mg of lycopene per kg of diet) and two treatment groups (200 mg or 400 mg of lycopene per kg of diet, or ~26 and 52 mg/d/hen, respectively). At the end of 12 months, blood, ovarian tissues and tumors were collected. Results: We observed that lycopene supplementation significantly reduced the overall ovarian tumor incidence (P < 0.01) as well as the number and the size of the tumors (P < 0.004 and P < 0.005, respectively). Lycopene also significantly decreased the rate of adenocarcinoma, including serous and mucinous subtypes (P < 0.006). Moreover, we also found that the serum level of oxidative stress marker malondialdehyde was significantly lower in lycopene-fed hens compared to control birds (P < 0.001). Molecular analysis of the ovarian tumors revealed that lycopene reduced the expression of NF-B while increasing the expression of nuclear factor erythroid 2 and its major target protein, heme oxygenase 1. In addition, lycopene supplementation decreased the expression of STAT3 by inducing the protein inhibitor of activated STAT3 expression in the ovarian tissues. Conclusions: Taken together, our findings strongly support the potential of lycopene in the chemoprevention of ovarian cancer through antioxidant and anti-inflammatory mechanisms.         Protective Effects of the Nuclear Factor Kappa B Inhibitor Pyrrolidine Dithiocarbamate on Experimental Testicular Torsion and Detorsion Injury

        Testicular torsion results with the damage of the testis and it is a surgical emergency. Pyrrolidinedithiocarbamate (PDTC) is a low-molecular-weight antioxidant and potent inhibitor of nuclear factorkappa B (NF-κB) activation. In this study, we aimed to investigate the effects of PDTC to testiculartorsion-detorsion (T/D) injury. Forty adult male Sprague-Dawley rats were separated into four groups. A sham operation was performed in group I. In group II, torsion is performed 2 hours by 720 degreeextravaginally testis. In group III, 4 h reperfusion of the testis was performed after 2 h of testiculartorsion. In group IV, after performing the same surgical procedures as in group III, PDTC (100 mg/kg,intravenous’s) was administered before 30 min of detorsion. The testes tissue malondialdehyde (MDA),superoxide dismutase (SOD) catalase (CAT) level was evaluated. Histological evaluations were performedafter hematoxylin and eosin staining. Testicular tissue MDA levels were the highest in theT/D groups compared with treatment group. Administration of PDTC prevented a further increase inMDA levels. Significant decrease occurred in CAT and SOD levels in treatment group compared withthe control group. The rats in the treatment group had normal testicular architecture. The resultssuggest that PDTC can be a potential protective agent for preventing the biochemical and histologicalchanges related to oxidative stress in testicular injury caused by testis torsion.

        Translation, Cross-cultural Adaptation, and Psychometric Properties of the Turkish Version of the Self-care Ability Scale for the Elderly

        Purpose: The Self-care Ability Scale for the Elderly (SASE) is a frequently used scale that was created in Sweden according to Orem’s self-care deficit theory and later modified according to Pörn’s theory of environment and purpose. This study translated and cross-culturally adapted the Turkish version of the SASE (T-SASE) and demonstrated its psychometric properties, including its reliability and construct validity. Methods: This study recruited a total of 122 participants and applied international guidelines for the translation and adaptation of the scale. The test-retest reliability of the SASE was calculated at 1-week intervals. Internal consistency was analyzed using Cronbach’s alpha. For construct validity, we compared the T-SASE to the Nottingham Extended Activities of Daily Living Scale (NEADLS) and the Exercise of Self-Care Agency Scale (ESCAS). Results: The mean age of the participants was 68.6±5.7 years. The test-retest reliability of the T-SASE was excellent (intraclass correlation coefficient=0.914; 95% confidence interval, 0.81–0.95). The internal consistency of the total T-SASE score was also excellent (Cronbach’s α=0.912), as were the Cronbach’s alpha scores for the individual items, ranging from 0.901 to 0.915. The items were also highly consistent with each other (Cronbach’s α>0.80). The construct validity of the T-SASE was acceptable (0.35<r<0.50) in a convergent manner. The T-SASE was also strongly correlated with the NEADLS and ESCAS (r1=0.405, r2=0.437, p<0.01). Conclusions: The T-SASE was a reliable and valid tool for assessing the self-care ability of community-dwelling older adults. Owing to its multidimensional structure, the T-SASE is an essential tool for evaluating self-care in older adults.

        Assessment of nonlinear static and incremental dynamic analyses for RC structures

        In this study, seismic behaviour of reinforced concrete buildings using the pushover and incremental dynamic analysis method was investigated. A numerical study was performed for a reinforced concrete frame building. Pushover analysis according to uniform and triangular load shapes and incremental dynamic analyses were performed for selected building. For the nonlinear analysis, three ground motion records were selected to ensure compatibility with the design spectrum defined in the Turkish Seismic Code. The maximum response, dynamic pushover curve, capacity curves, interstorey drifts and moment rotation curves for various element ends of the selected building were obtained. Results were compared each other and good correlation was obtained between the dynamic analyses envelope with static pushover curves for the building.

        Investigation of flexural strength and cytotoxicity of acrylic resin copolymers by using different polymerization methods

        PURPOSE. The aim of this study was to appraise the some mechanical properties of polymethyl methacrylate based denture base resin polymerized by copolymerization mechanism, and to investigate the cytotoxic effect of these copolymer resins. MATERIALS AND METHODS. 2-hydroxyethyl methacrylate (HEMA) and isobutyl methacrylate (IBMA) were added to monomers of conventional heat polymerized and injection-molded poly methyl methacrylate (PMMA) resin contents of 2%, 3%, and 5% by volume and polymerization was carried out. Three-point bending test was performed to detect flexural strength and the elasticity modulus of the resins. To determine the statistical differences between the study groups, the Kruskall-Wallis test was performed. Then pairwise comparisons were performed between significant groups by Mann-Whitney U test. Agar-overlay test was performed to determine cytotoxic effect of copolymer resins. Chemical analysis was determined by FTIR spectrum. RESULTS. Synthesis of the copolymer was approved by FTIR spectroscopy. Within the conventional heat-polymerized group maximum transverse strength had been seen in the HEMA 2% concentration; however, when the concentration ratio increased, the strength decreased. In the injection-molded group, maximum transverse strength had been seen in the IBMA 2% concentration; also as the concentration ratio increased, the strength decreased. Only IBMA showed no cytotoxic effect at low concentrations when both two polymerization methods applied while HEMA showed cytotoxic effect in the injection-molded resins. CONCLUSION. Within the limitations of this study, it may be concluded that IBMA and HEMA may be used in low concentration and at high temperature to obtain non-cytotoxic and durable copolymer structure.

        Case Report : Quadriceps Tendon Rupture due to Postepileptic Convulsion

        We present a case of quadriceps tendon (QT) rupture. QT ruptures can occur in all ages. The cause is mostly traumatic in origin. Spontaneous ruptures that are thought to result from predisposing conditions are rare. Post-convulsion QT ruptures lacking traumas in their history can be overlooked in clinical examinations. This should be born in mind by the attending physician, as early diagnosis and treatment of the condition can lead to satisfactory outcomes.
