        Autosomal dominant polycystic kidney disease(ADPKD) is one of most common hereditary disorder. It is potentially fatal and the major mobilities are hypertension and renal failure. To elucidate clinical characteristics and outcomes of ADPKD, 67 ADPKD patients were analyzed retrospectively. The male to female ratio was 48 years for males and 42 years for females. The factors leading to the dignosis of ADPKD were flank or abdominal pain (46%), palpable abdominal mass (24%), incidental findings (18%), gross hematuria and headache. Urinalysis result showed hematuria (31%), proteinuria (19%), pyuria (18%). Among renal complications hypertension (67%) was the most common. Other complications were urolithiasis (16%) upper urinary tract infection (9%), cyst infection (7%). Azotemia was observed in 24% of cases. Echocardiography showed left ventricular hypertrophy in 24% and valvular regurgitation in 18% of cases, but there was no mital valve prolapse. Cerebrovascular events occurred in 6 cases (2 cases of intervertebral hemorrhage and 4 cases of cerebral infarction). Hipatic cyst were observed in 64% of cases. Eighteen cases culminated in end stage renal failure (ESRF) at an of 47. According to this study, the most common complications of ADPKD was hypertension (67%), ESRF (27%), and cerebrovascular event (9%). Hypertension was more frequently observed in increased creatinine group than normal.

        Heat stroke is a life-threatening illness characterized by an elevated core body temperature that rises above 40% and central nervous system dysfunctions that results in delirium, convulsions, or coma. Heat stroke is subdivided into two forms, classic and exertional. Since strenuous exercise is one of the major exacerbating and precipitating factors, the incidence of exertional heat stroke is high among young adult, especially military personnel undergoing military training. Exertional heat stroke commonly causes rhabdomyolysis, lactic acidosis, acute renal failure, shock and pulmonary edema. We report a case of heat stroke with rhabdomyolysis, acute renal failure, disseminated intravascular coagulopathy(DIC) after strenuous exercise. A 18 year old man was admitted to the hospital because of loss of consciousness. He was a wrestler and he had tried to lose 6kg in weight by jogging and strenuous exercise on that summer day with hot temperature. The development of this illness was favoured by much clothes inappropriate for that environmental condition with a purpose to enhance sweating, and by restricted intake of water and food. On admission, the blood pressure was 80/50mmHg, and body temperature was 39.4℃ with stuporous mentality. Laboratory finding were as follows : in complete blood cell count, hemoglobin was 16.7g/dL and platelet count was 158,000/ mm³. In blood chemistry, AST 1061U/L, ALT 201U/L, BUN 30mg/dL, Cr 2.2mg/dL, LDH 941IU/L, CK 696IU/L were resulted. In arterial blood gas analysis, pH 7.39, pCO₂ 30mmHg pO₂ 86mmHg, HCO₃ 18.4mEq/L, O₂ saturation 96.7% were resulted. In urinalysis, specific gravity 1.025, protein 3+, erythrocyte 2+ were resulted. In brain imaging study, brain CT finding was normal. After appropriate treatments with rapid cooling, hydration and supportive care, he improved with alert mental status. On 2nd hospital day, in laboratory result, thrombocytopenia and coagulopathy were appeared and renal insufficiency was aggravated. So he was treated with platelet concentrate and fresh frosen plasma for DIC, and enough hydration for appropriate urine volume preservation. After all, On 22nd hospital day, he fully recovered and discharged. Nowadays, with followed for some duration, he is wellbeing with no complications.

      • 도약ㆍ단거리 선수들의 하퇴 삼두근에 대한 등속성 상대근력에 관한 연구

        The purpose of this research was to measure and evaluate relative strength of left and right triceps surae muscle of excellent jumpers and sprinters using Cybex-Ⅱ System. The subjects of this research were 5 men and women of 100-m sprinter, 5 men and women of 400-m sprinter, 5 men and women of high jumper, 5 men and women of triple jumper, and 5 men and women of long distanceer, respectively. The method of the experiment was exercised centering around peantar flexion and dorsiflexion of triceps surae muscle using Cybex-Ⅱ System. the procedures of the experiment were as follows: After falling on one's face in an upright position on the upper-body exercise and testion table, the parts of gluteus maximus and femur(thigh bone) were fastened with fixed belt. After sticking and fastenning available foot with fixed belt to the sole of a foot on a springboard equipped with the anklebone and dynamoneter, the light exercise was tried without discomforts in exerting strength. The guage was fixed at 0˚ the ankle joint so that the strength can be produced correctly, torque guage and cybex data reduction computer were controlled. After setting at 30˚/sec, the strength was exerted maximally. This velocity was the velocity that can measure maximal strength of triceps surae muscle centering around the ankle joint. After exerting the strength of 3 to 5 times, the best peak torque of those was selected and produced.

        大邱地域 地殼 γ-線의 照射線量率 測定

        본 연구는 대구지역의 지각 r-선에 의한 조사선량율을 측정하고 그에 따른 성분별 공간선량율 및 지역적 분포 등의 환경방사선 특성을 조사하였다. r-선 스펙트럼의 측정에는 4"ø ×4"Nal(Tl) 계측기와 휴대용 다중파고분석장치를 이용하여 이 지역내의 28개 지점에서 in-situ spectrometry를 수행하였으며, 측정된 스펙트럼으로부터 조사선량율을 환산하여 지각 r-선의 총선량율과 일반적으로 자연방사능의 주종이 되는 ?K, ?U계열 및 ?Th계열의 성분별 조사선량율을 구하였다. 조사결과 대구지역의 지각 r-선에 의한 평군조사선량율은 9.4 μR/h였으며 지점별 분포는 7.6∼11.0 μR/h범위로 다소 차이를 보였다. 측정시에 수반되는 기후나 우주선 등에 따른 일변화폭을 고려한다면 지점간의 차이는 주로 표토층의 자연방사성핵종인 ?K, ?U계열 및 ?Th계열의 핵종에 기인함을 알 수 있었으며, 성분별 선량율은 ?K2.9∼4.6 μR/h, ?U계열1.2∼3.1 μR/h, ?Th계열2.5∼5.0 μR/h의 분포를 나타냈다. This study concerns about the measurement and the investigation of environmental radiation characteristics which the components and the distribution of exposure rates by terrestrial r-rays in Taegu area. A 4"ø ×4"Nal(Tl) scintillation detector with a multichannel analyzer was used in the measurement of r-rays as a part of in-situ spectrometry at twenty eight different locations in this area. The conversion into the exposure rate from the measured r-rays spectrum has been carried out leading to a net exposure rate and component ones by ?K, ?U series and ?Th series products which are known by the major parts in the terrestrial r-rays generally. As a result, the average exposure rate by the terrestrial r-rays in Taegu area is 9.4 μR/h and the distribution of individual exposure rates shows more or less differences between these locations even after the consideration of diurnal and rearly variations which are always involved in these measurements. The component parts of exposure rates are distributed ?k2.92.9∼4.6 μR/h, ?U series1.2∼3.1 μR/h and ?Th series 2.5∼5.0 μR/h over the measured locations.

        Effect of Poly(Lactide-Co-Glycolide) Nanoparticles on Local Retention of Fluorescent Material: An Experimental Study in Mice

        <P><B>Objective</B></P><P>Poly(lactide-co-glycolide) (PLGA) nanoparticles are promising materials for the development of new drug-releasing systems. The purpose of this study was to evaluate the <I>in vivo</I> retention time of materials loaded in nanoparticles as compared with that of the material alone by <I>in vivo</I> imaging in nude mice.</P><P><B>Materials and Methods</B></P><P>Mice (n = 20) were injected with 0.1 mL fluorescent material 1,1′-dioctadecyl-3,3,3′,3′ tetramethylindotricarbocyanine iodide (DiR)-loaded PLGA nanoparticles (200 nm) into the right paraspinal muscle, and the same volume of pure DiR solution was injected into the left paraspinal muscle. Fluorescence images were obtained using an <I>in vivo</I> optical imaging system. Fluorescent images were taken 1 day after the injection, and seven more images were taken at 1-week intervals. Image analysis was done with ImageJ program, and one region of interest was chosen manually, which corresponded to the highest signal-intensity area of fluorescence signal intensity.</P><P><B>Results</B></P><P>After 7 weeks, 12 mice showed a right-sided dominant signal, representing the DiR loaded PLGA nanoparticles; 5 mice showed a left-side dominant signal, representing the free DiR solution; and 3 mice showed no signal at all beginning 1 day after the injection. During the 7-week period, the mean signal intensities of the free DiR solution and DiR-loaded PLGA nanoparticles diverged gradually. On day 1, the mean signal intensity of free DiR solution was significantly higher than that of DiR-loaded PLGA (<I>p</I> < 0.001). Finally, by week 7, DiR-loaded PLGA express significantly high signal intensity compared with free DiR solution (<I>p</I> = 0.031).</P><P><B>Conclusion</B></P><P>The results of the current study suggested that therapeutic agents bound to PLGA nanoparticles may exhibit prolonged retention times.</P>

        A study on the barrier effect with respect to the condition of solid insulation materials in GN<sub>2</sub>

        High voltage superconducting apparatuses have been developed presently around the world under AC and DC sources. In order to improve electrical reliability of superconducting apparatuses with AC and DC networks, a study on the DC as well as the AC electrical breakdown characteristics of cryogenic insulations should be conducted for developing a high voltage superconducting apparatus. Recently, a sub-cooled liquid nitrogen cooling system is known to be promising method for developing a high voltage superconducting apparatus. A sub-cooled liquid nitrogen cooling system uses gaseous nitrogen to control the pressure and enhance the dielectric characteristics. However, the dielectric characteristics of gaseous nitrogen are not enough to satisfy the grade of insulation for a high voltage superconducting apparatus. In this case, the application of solid insulating barriers is regarded as an effective method to reinforce the dielectric characteristics of a high voltage superconducting apparatus. In this paper, it is dealt with a barrier effect on the DC and AC dielectric characteristics of gaseous nitrogen with respect to the position and number of solid insulating barriers. As results, the DC and AC electrical breakdown characteristics by various barrier effects is verified.

        High aspect ratio microdisplay and thin optical component for glass-like AR devices

        Organic light-emitting diode (OLED) microdisplays have attracted much attention as displays for small form factor augmented reality (AR) devices. To realize glass-like thin and wide field of view (FoV) AR devices, we designed a display module with a high aspect ratio microdisplay and a thin optical component. For the high aspect ratio microdisplay, we developed the color OLED microdisplay with a 32:9 aspect ratio and a 0.8-inch diagonal ∼2,490-ppi CMOS backplane. To express color and reduce optical crosstalk,wefabricated the color filter (C/F) patterning directly on the white OLED. We also developed a pin mirror lens with 11 pin mirrors to improve the optical efficiency and quality with a thin lens. By combining the microdisplay with LetinAR’s pin mirror lens, we successfully demonstrated an AR device with a wide horizontal FoV of 46° but with a small form factor 4mmlens.
