Background/Aims: The aim of this study was to investigate long-term post-discontinuation outcomes in patients with rheumatoid arthritis (RA) who had been treated with tumor necrosis factor-α inhibitors (TNF-αi) which was then discontinued. Methods: Sixty Korean patients with RA who participated in a 5-year GO-BEFORE and GO-FORWARD extension trials were included in this retrospective study. Golimumab was deliberately discontinued after the extension study (baseline). Patients were then followed by their rheumatologists. We reviewed their medical records for 2 years (max 28 months) following golimumab discontinuation. Patients were divided into a maintained benefit (MB) group and a loss-of-benefit (LB) group based on treatment pattern after golimumab discontinuation. The LB group included patients whose conventional disease-modifying antirheumatic drug(s) were stepped-up or added/switched (SC) and those who restarted biologic therapy (RB). Results: The mean age of patients at baseline was 56.5 years and 55 (91.7%) were females. At the end of follow-up, 23 (38.3%) patients remained in the MB group. In the LB group, 75.7% and 24.3% were assigned into SC and RB subgroups, respectively. Fifty percent of patients lost MB after 23.3 months. Demographics and clinical variables at baseline were comparable between MB and LB groups except for age, C-reactive protein level, and corticosteroid use. Restarting biologic therapy was associated with swollen joint count (adjusted hazard ratio [HR], 1.90; 95% confidence interval [CI], 1.01 to 3.55) and disease duration (adjusted HR, 1.12; 95% CI, 1.02 to 1.23) at baseline. Conclusions: Treatment strategies after discontinuing TNF-αi are needed to better maintain disease control and quality of life of patients with RA.

<P>The purpose of this study is to examine the patient-reported outcomes (PRO) after discontinuing nonsteroidal anti-inflammatory drugs (NSAIDs) and clinical factors associated with a favorable outcome in patients with rheumatoid arthritis (RA) in remission or with low-disease activity (LDA). A 16-week prospective open-label trial was conducted at eight rheumatology clinics in Korea. RA patients with 28-joint disease activity score based on erythrocyte sedimentation rate (DAS28-ESR) < 3.2 who were on NSAIDs for more than a month were enrolled, and NSAIDs were discontinued. Acetaminophen (AAP) was used as the rescue medication, and NSAIDs were restarted when joint pain was intolerable with AAP. The endpoint was to analyze the group of patients who continued to withdraw NSAIDs. Among 109 enrolled patients, 105 completed the 16-week follow-up. Eighty-nine (84.8%) patients remained without restarting NSAIDs. In these patients, there was a slight increase in their pain levels compared with baseline (median 14.0 versus 19.0 using the pain-visual analog scale, p = 0.010). However, changes in DAS28-ESR (p = 0.638) and routine assessment of patient index data 3 (RAPID-3) (p = 0.128) were insignificant. Moreover, 66 (62.9%) patients showed sustained effectiveness on PRO without restarting NSAIDs. In the multivariate regression models, joint swelling was the detrimental factor in NSAID withdrawal (odds ratio [OR] 0.149, 95% confidence interval [CI] 0.033-0.680, p = 0.014) and sustained effectiveness (OR 0.284, 95% CI 0.091-0.883, p = 0.030). Joint pain in RA patients in remission or with LDA can be well managed without NSAIDs, especially in those without swollen joints at the time of cessation.</P>

An experimental technique for 2-dimensional quantitative measurements of NO concentrations is presented by planar laser-induced fluorescence(PLIF). Various interferences such as Rayleigh scattering, O₂ and C atom fluorescences are minimized by filtering and careful choosing of the excitation-detection scheme. By point-by-point background correction, broadband O2 and pressure-broadened NO fluorescences are precisely eliminated even at high pressure, so that quantitative NO concentrations are successfully calibrated in laminar premixed C₃H_8/air burner flames and the constant-volume combustion chamber. In both cases, quick increasing of NO concentration is observed adjacent to the flame front.<br/>

The coronavirus disease 2019 (COVID-19) pandemic has caused more than 100 million infections and 2 million deaths worldwide. In up to 20% of cases, COVID-19 infection can take a severe, life-threatening course. Therefore, preventive measures such as mask-wearing, hand hygiene, and social distancing are important. COVID-19 vaccines that use novel vaccine technology can prevent up to 95% of infections. However, the uncertainty regarding the efficacy and safety of vaccination in patients with autoimmune inflammatory rheumatic disease (AIIRD), who are immunocompromised due to underlying immune dysfunction and concomitant immunosuppressive treatment, warrants clear guidance. A task force of the Korean College of Rheumatology formulated a set of vaccination guidance based on the currently available data and expert consensus. The currently available COVID-19 vaccines are considered to be safe and effective. Every patient with AIIRD should receive one of the available COVID-19 vaccines unless contraindicated for medical reasons such as prior allergy/anaphylaxis to the COVID-19 vaccine or its components. Patients should continue immunosuppressive treatment for their underlying AIIRD, including biological and targeted synthetic disease-modifying anti-rheumatic drugs (b/tsDMARDs). Corticosteroids should be reduced to the lowest dose possible without aggravating the AIIRD. To improve the vaccine response, methotrexate can be withheld for 1–2 weeks after each vaccination, and the timing of rituximab and abatacept infusion should be adjusted if clinically acceptable. Rheumatologists should play a leading role in educating and vaccinating patients with AIIRD.

Polysaccharides prepared from marine algae sometimes contain contaminants such as polyphenols and endotoxins that may mislead their bona fide biological activities. In this study, we examined bioactive contaminants in commercially available fucoindan from Fucus vesiculosus, along with ascophyllan and fucoidan from Ascophyllum nodosum. F. vesiculosus fucoidan inhibited the growth of Vibrio alginolyticus in a concentration-dependent manner ($0-1,000{\mu}g\;mL^{-1}$). However, the antibacterial activity of the fucoidan significantly reduced after methanol-extraction, and the methanol-extract showed a potent antibacterial activity. The extract also showed cytotoxicity to RAW264.7 and U937 cells, and induced apoptotic nuclear morphological changes in U937 cells. These results suggest that the antibacterial activity of the fucoidan is partly due to the methanol-extractable contaminants that can also contribute to the cytotoxicity on RAW264.7 and U937 cells. On the other hand, the activities to induce secretion of nitric oxide and tumor necrosis factor-${\alpha}$ from RAW264.7 cells were observed in the fucoidan even after methanol extraction, and the extract had no such activities. Our observations suggest that commercially available fucoidan should be purified prior to biochemical use.

<P>The basic electrical insulation design of 2.5 MJ conduction cooling high temperature superconducting magnetic energy storage (HTS SMES) have been studied in this paper. The high voltage is generated both ends of magnet of HTS SMES by quench or energy discharge. Therefore, the insulation design of the high voltage needs for commercialization, stability, reliability and so on. In this study, we analyzed the insulation composition of a HTS SMES, and investigated about the insulation characteristics of the materials such as Polyimide, Al<SUB>2</SUB>O<SUB>3</SUB> , GFRP and vacuum at cryogenic temperature. Base on these results, the basic insulation design for 2.5 MJ conduction cooling HTS SMES was performed.</P>

한반도 및 근해의 지진?지진해일 감시를 위해 지진관측망이 구축되어 있다. 이러한 지진관측망는 향후 조기경보를 위해 보다 조밀하게 설치될 예정이다. 지진계측기의 정보통신망은 대부분 행정망 및 전용회선 단일망으로 운영되고 있으며 지진계측기의 전력체계는 상용전원의 단일 전력체계로 운영되고 있다. 국내 지진관측망 운영상의 문제점으로 단일 정보통신망 운영으로 인한 통신 단절 발생 시 지속적인 자료 수집 불능과 단일 전력체계인 상용전원 단절시 통신장비에 전원공급 단절로 인한 실시간 자료 전송 장애 발생이 있다. 또한 지진관측기의 정보통신망과 전력체계에 대한 원격감시 및 관리 기능이 부재하여 유지 보수 및 안정적 자료수집에 취약점을 가지고 있다. 이러한 취약점을 극복하기 위해서는 각종 재해에 대한 내재해성과 유지보수성을 향상시키는 것이 시급하며 정보통신망과 전력체계의 다중화/이중화가 좋은 대안이 될 수 있다.
Stability of 2G HTS Double Pancake Coil With Metal Insulation and Two Parallel Resistors

<P>The enhanced stability of 2G HTS double pancake (DP) coil with metal insulation and parallel resistor was observed in contrast with the case of only metal insulation. Here, we used stainless steel tape and additional indium metal sheet as metal insulator and parallel resistor. The total length of the DP coil was 66 m and the critical current (I<SUB>c</SUB>) of the DP coil was about 78.1 A in liquid nitrogen in the criterion of 0.1 μV/cm. Minimum quench energies (MQEs) were 3.154 J at 60 A of the coil current in liquid nitrogen. MQE was 1.295 J at 60 A of the coil current and 77 K in conduction cool. New DP coil was fabricated by using the indium sheet resistor. MQE was 10.935 J in liquid nitrogen and 1.306 J in conduction cool. In this case, 2G DP MI coil attached with parallel resistors was robust under the condition of the quench induced by the heater.</P>

<P>The turn-to-turn contact resistance of second-gene-ration (2G) HTS coil with metal insulation is a very important parameter in the viewpoint of the stability and current charging speed of the HTS coil. Controllability of the contact surface resistance of the 2G HTS coil with advanced metal insulation was observed in contrast with the case of the stainless steel (SS) metal insulation (MI). Three types of model coils—a no insulation coil, a metal insulation coil cowound with SS tape, and an MI coil cowound with copper (Cu) electroplated SS tape—were prepared and evaluated. The contact surface resistances between two Cu-plated 2G HTS tapes, between a Cu-plated 2G HTS tape and an SS tape, and between a Cu-plated 2G HTS tape and a Cu-plated SS tape are 6.3, 362.4, and 7.9 <I>μ</I> Ω·cm<SUP>2</SUP>, respectively. Dramatic reduction of the turn-to-turn contact surface resistance in model coils was achieved by using MI coil cowound with the Cu-plated SS tape. Also, the possibility of changing such contact surface resistance will be useful for designing the MI magnet.</P>
