S-532 An unusual case of accessory renal artery occlusion presenting with hypertensive emergency

Renal infarction is a condition resulting from an obstruction or a decrease in renal arterial blood flow, which triggers the release of renin, aldosterone and a secondary elevation in blood pressure. Here, we report a case of renal infarction due to accessory renal artery occlusion presenting as hypertensive emergency. A 18-year old female admitted to the hospital complaining of pulsatile headache, blurred vision and central scotoma for a month. Her blood pressure was 156/78mmHg on admission, and occasional surge in blood pressure was checked during hospitalization. Fundus examination showed papilledema and cerebrospinal fluid analysis showed increased intracranial pressure (25.5 cmH2O). Hypokalemia and elevated LDH level were noted from the laboratory findings. Further hormonal studies showed high plasma renin level, and abdominal computed tomography angiography showed partial infarction of right (rt.), resulted from focal occlusion due to dissecting aneurysm of renal accessory artery. Selective angiography showed dissecting aneurysm of small accessory renal artery arising form the rt. renal artery root supplying upper pole of rt.kidney. Renal vein sampling revealed high renin level at rt. superior renal vein. Selective embolization of rt. accessory renal artery was performed. Following embolization, her blood pressure normalized over several weeks. In the literature, renovascular hypertension from stenosis or occlusion of accessory artery is rare. In this case, our patient was successfully treated with embolization of renal accessory artery.

Clonal hematopoiesis of indeterminate potential (CHIP), a common aging-related process that predisposes individuals to various inflammatory responses, has been reported to be associated with COVID-19 severity. However, the immunological signature and the exact gene expression program by which the presence of CHIP exerts its clinical impact on COVID-19 remain to be elucidated. In this study, we generated a single-cell transcriptome landscape of severe COVID-19 according to the presence of CHIP using peripheral blood mononuclear cells. Patients with CHIP exhibited a potent IFN-γ response in exacerbating inflammation, particularly in classical monocytes, compared to patients without CHIP. To dissect the regulatory mechanism of CHIP (+)-specific IFN-γ response gene expression in severe COVID-19, we identified DNMT3A CHIP mutation-dependent differentially methylated regions (DMRs) and annotated their putative target genes based on long-range chromatin interactions. We revealed that CHIP mutant-driven hypo-DMRs at poised cis-regulatory elements appear to facilitate the CHIP (+)-specific IFN-γ-mediated inflammatory immune response. Our results highlight that the presence of CHIP may increase the susceptibility to hyperinflammation through the reorganization of chromatin architecture, establishing a novel subgroup of severe COVID-19 patients.

1990년대와 2000년대 초반 미국 경제가 소위 고용 없는 성장(jobless growth)을 경험한 바 있는데 최근 몇 년간 우리나라도 이와 유사한 상황에 직면하고 있다. 고용 없는 성장의 주된 원인으로는 기술혁신이 지목되고 있는데 기술혁신에 따라 생산성은 향상되고 산출은 확대되는 반면 고용은 오히려 악화된다는 것이다. 본 연구에서는 이러한 최근의 논의에 바탕을 두고 우리나라의 경우 기술혁신이 고용에 어떠한 영향을 미치는지 제조업부문과 서비스업부문으로 구분하여 살펴보았다. 특히 기술혁신의 형태와 관련하여 기술혁신이 숙련 노동자의 고용 및 임금에는 긍정적인 영향을 미치는 반면 비숙련 노동자에게는 부정적인 영향을 미치는, 소위 숙련편향적(skill biased) 성격이 있는지도 분석하였다. 분석결과, 우리나라에서는 기술혁신이 고용 및 산출을 증가시키고 노동생산성을 향상시키는 것으로 나타났는데 그 효과는 1980년대(1980~92)보다는 1990년대(1993~2004)에 더욱 뚜렷한 것으로 추정되었다. 또한 기술혁신이 제조업의 고용을 확대시키는 반면 서비스업의 고용에는 큰 영향을 주지 않으며 숙련노동자와 비숙련노동자간 고용 및 임금 격차를 확대시키는 숙련편향적 성격을 갖는 것으로 분석되었다. 기술혁신의 고용증대효과가 주로 제조업부문을 통해 나타나는 것은 1990년대 이후 개방화의 진전과 IT산업의 급속한 발전 영향 등으로 가격의 신축성이 제조업부문에서 상대적으로 크게 나타났기 때문인 것으로 풀이된다. 이에 따라 기술혁신의 고용증대효과를 높이기 위해서는 제조업의 지속적인 기술혁신을 유도하는 한편 서비스업 부문도 대외개방의 확대와 경쟁의 확산 등을 통해 가격경직성을 완화해 나가야 할 것이다. 그리고 기술혁신이 비숙련노동자의 일자리 감소 및 숙련․비숙련노동자간 소득격차를 확대할 가능성이 있으므로 기술교육 확대 등을 통해 비숙련노동자의 숙련도를 높이는 대책이 마련되어야 할 것이다. This paper focuses on the following five issues; (1) What effect does technological innovation have on aggregate employment? (2) Has the effect changed over the past 20 years? (3) Are the empirical results with aggregate data consistent with those with industrial data? (4) Is there any interaction between idiosyncratic industrial technological shocks and employment in the industries affected? (5) Is the effect skill-biased? The estimation results indicate that technological shocks have had temporary and permanent effects on the level of aggregate employment over the past 10 years rather than in the preceding 20 years. That is to say that, since a structural break that occurred around 1993, technological shocks are found to have been favorable. These results differed widely across industries. Also we found that labor movement between industries as well as technology spillover effects have been caused by industrial level technological innovations. Finally, even though technological progress is favorable for aggregate employment, this effect was found to be skill-biased.
Smart Damper Using Sliding Friction of Aramid Brake Lining and Self-centering of Rubber Springs

<P>The purpose of this study is to develop a smart damper with flag-shaped behavior by using the sliding friction of aramid brake lining and the restoring capacity of pre-compressed rubber springs. For this purpose, the friction force of aramid brake lining pressed by bolts was used along with polyurethane rubbers, each with a length of 80 mm, a diameter of 95 mm, and a circular hole of 37 mm. In the experiments, loading frequency and torque force were considered. The loading frequency varied from 0.1 to 2.0 Hz, and frictional force was controlled by variable bolt torque force. The tests were conducted to demonstrate that the clamping force by the bolts could provide normal force to frictional material. The friction force by the aramid brake lining sliding was tested, followed by the pre-compressed rubber springs' behavior. Afterward, a damper combining the two components was tested to verify flag-shaped behavior by using a dynamic actuator, and the damping ratios were evaluated from the hysteretic curves. The behavior of the damper closely matched flag-shaped behavior, resulting in self-centering and energy dissipation capacity.</P>

SOA Architecture 를 기반으로 한 SOiVA 의 Interactive Video 형식을 사용하여 개인정보를 유출하지 않고 전자 상거래를 가능하게 하는 Application 을 설계하고, 휴대폰, PDA, PMP 등 다양한 휴대 단말장치에서 공통적으로 기존의 웹사이트 시스템으로 최적화 시키지 못하는 문제점을 개선하여 다양한 사이즈의 디스플레이에서, 보다 효율적이고, 최적화된 서비스를 제공 가능케 하는 설계를 하고, SOiVA 서비스를 적용한 응용프로그램을 구현하였으며, 앞으로 발전 가능성을 제안하며, 향후 과제를 제시한다.
