Achalasia Previously Diagnosed as Gastroesophageal Reflux Disease by Relying on Esophageal Impedance-pH Monitoring: Use of High-Resolution Esophageal Manometry in Children

Gastroesophageal reflux disorder (GERD) is the most common esophageal disorder in children. Achalasia occurs less commonly but has similar symptoms to GERD. A nine-year old boy presented with vomiting, heartburn, and nocturnal cough. The esophageal impedance-pH monitor revealed nonacidic GERD (all-refluxate clearance percent time of 20.9%). His symptoms persisted despite medical treatment for GERD, and he was lost to follow up. Four years later, he presented with heartburn, solid-food dysphagia, daily post-prandial vomiting, and failure to thrive. Endoscopy showed a severely dilated esophagus with candidiasis. High-resolution manometry was performed, and he was diagnosed with classic achalasia (also known as type I). His symptoms resolved after two pneumatic dilatation procedures, and his weight and height began to catch up to his peers. Clinicians might consider using high-resolution manometry in children with atypical GERD even after evaluation with an impedance-pH monitor.

오프닝타이틀시퀀스에 나타난 애니메이션 캐릭터디자인에 관한 소고 - 스티븐 스필버그의 [Catch Me If You Can, 2002]를 중심으로 -

`오프닝타이틀시퀀스`는 영화 전체 시퀀스들의 함축적인 상징성을 표상화 하여 보여줌과 동시에, 관객이 영화 전체의 맥락을 한눈에 읽어 볼 수 있도록 돕는 역할을 담당한다. 영화에서의 전개과정은 영화의 중심사건이 축을 이루어 방향성을 가지게 되며, 그 중심사건은 여러 캐릭터들이 만들어내는 독특한 유형들에 의하여 다시 축을 구성, 각각의 사건들을 존재케 한다. 따라서 일련의 사건들은 캐릭터를 중심으로 주제단락을 이루게 되며, 캐릭터 유형 중심으로 이루어진 시퀀스들의 유기적 구성은 영화전체의 이미지를 전달한다. 이를 단적으로 보여주도록 디자인 되어진 대표성 있는 시퀀스를 `오프닝타이틀시퀀스`라 할 수 있다. 따라서 `오프닝타이틀시퀀스`를 단지 영화 타이틀 소개의 수단이라고 생각하는 개념에서 벗어나 영화의 본 내용과 구분되는 중요한 단락으로 인식할 필요가 있다. 영화가 관객과 처음 만난다는 시점에서 오프닝타이틀시퀀스는 중요한 의미를 지니고 있으며, 영화의 전반적인 내용 이해와 주제 전달의 측면에서 수용자에게 많은 효과를 주고 있다. 이에 많은 영화들이 오프닝타이틀시퀀스의 중요성을 인식하여, 기존의 제작방식에서 벗어난 새로운 장르의 형식을 도입시켜 영화의 본 내용과는 다른 독특한 표현방식을 보여주고 있다. 본 논문에서는 `오프닝타이틀시퀀스`의 제작에 있어 감각적인 아이디어와 애니메이션 기법의 디자인으로 찬사를 받은 스티븐 스필버그(Steven Spielberg) 감독의 2002년도 작품 [캐치 미 이프 유 캔Catch Me If You Can]의 `오프닝타이틀시퀀스`를 분석하여 그 중요성을 소고해보는데 의의를 갖고자 한다. Opening title sequence`shows the implicit symbolization of sequence of the whole movie through the image, and plays the role of making the audience grasp the whole context of movie at the same time. The movie comes to possess the direction because the central incidents in the movie serve as the axis to unfold stories of movie. Those central incidents form the axis again through unique patterns created by many characters, and in this way, each incident comes into existence. Therefore, a series of incidents structure the theme paragraph mainly around the character, and the organic structure of sequence which is formed mainly around the patter of character transmits the image of whole movie. `Opening title sequence`can be said to be the representable sequence that is designed to show this clearly. Therefore, it is necessary to understand the `Opening title sequence`as a paragraph distinct from the main stories of movie, getting away from the concept which considers it as a simple tool to introduce the tile of movie. The `Opening title sequence` is significant at the point of the first encounter between the movie and the audience, and has much effect on the audience as to the understanding of the general stories of movie and the transmission of the theme of movie. In response to that, many movies understand the importance of `Opening title sequence` and are presenting unique way of expression by introducing new type of genre, which is different from the main stories of movie. This dissertation aims to analyze the `Opening title sequence`of `Catch Me If You Can`, which was produced by Steven Spielberg and released in 2002, drawing lot of praises for his sensible ideas and animation techniques that he applied to the `Opening title sequence`, in order to consider the importance of it.

교육용 영상미디어의 외적체제에 관한 연구 - 황금분할 분석을 중심으로 -

영상미디어는 시청각적 영상 및 음향효과들이 응집되어있는 콘텐츠이다. 특히 앞으로도 많은 수요와 발전가능성이 예상되는 교육용 영상미디어는 정보전달력이 무엇보다 뛰어나야하기 때문에 화면의 구성, 즉 레이아웃이 매우 중요하다. 화면의 구성적 측면에서 안정감과 균형감을 내제하면서 방향성과 운동감을 표현하는데 효율적인 화면비율로는 황금비율을 꼽을 수 있다. 황금비는 동서고금, 과거 현재를 초월하여 자연의 내재적 질서로 예술디자인 작품의 1) 형태 발상의 기본 구조, 2) 조형의 구성 원리, 3) 보편적 아름다움으로 중요한 역할을 담당한다(윤민희, 2009). 또한 황금분할은 이상적인 비율로서 질서체계 및 형태미를 갖고 있으며 인간의 마음을 편안하게 만들어 역사적으로 수많은 디자인에 활용되어 왔다. 본 연구에서는 효율적인 영상미디어 디자인 외적체제 개발을 위해 황금분할을 기반으로 한 레이아웃 포맷을 제시하였다. 이를 위해 16:10의 비율에 해당하는 디스플레이 발전과정에 대한 소개와 함께 `황금사각형`과 `황금나선`으로 교육용 영상미디어 레이아웃 포맷을 제안하였으며, 개발된 포맷을 사용하여 <변화하는 사회(중등학교 교사용)> 영상미디어 콘텐츠를 제작하였다. 제작한 영상장면에 대한 황금분할을 통해 황금분할을 기반으로 한 레이아웃이 콘텐츠의 명시성 및 정보전달력에 도움을 줄 수 있다는 것을 알 수 있다. 황금분할을 기반으로 한 외적체제 연구를 통해 다양한 황금비율의 활용가능성을 제시하였으며, 이러한 외적체제가 계속적으로 연구된다면 향후 교육용 영상미디어의 학습효과 제고를 위해 보다 향상된 미디어 전략을 창출할 수 있을 것으로 기대한다. Video media is a content where visual images and sound effects are concentrated. Expected to greatly develop and be consumed in the future, educational video media puts its stress on screen organization, layout, in other words, because its power of information delivery should be more excellent than any other media. In the respect of screen organization, the golden section is a ratio that is easy to express directions and movements while it feels stability and balance. The golden section, transcending all ages, countries and times, is an immanent order of nature; it takes an important role in artcraft design as 1) a basic structure of form generation, 2) constituting principle of figures, and 3) general beauty. The golden section also has the order and physical beauty as an ideal proportion, making human minds comfortable; therefore, it has been used for so many designs in history. This study presented a layout format based on the golden section, to develop an effective outward system for video media design. For that, the formula and development of 16:10 proportion display were introduced and applied for the layout format of educational video media, in the forms of `the golden square` and `the golden spiral.` The developed format was used in making video media content for <The Changing Society (for secondary school teachers). Through the golden section on the video scenes, the layout based on the golden section proved that it could help to clarify the content and deliver information. Through the study on outward system based on the golden section, this study presented various possibilities for using it. Consistent studies on such outward system are expected to create rather improved media strategy where educational video media would raise the learning effect in the future.

Eosinophilic gastroenteritis in an 18-year-old male with prolonged nephrotic syndrome

Eosinophilic gastroenteritis is a rare disease characterized by prominent eosinophilic tissue infiltration of the gastrointestinal tract. Here, we report a case of eosinophilic gastroenteritis in an 18-year-old patient with prolonged nephrotic syndrome who presented with abdominal pain and peripheral hypereosinophilia. During the previous 2 years, he had visited local Emergency Department several times because of epigastric pain and nausea. He had been treated with steroid-dependent nephrotic syndrome since 3 years of age. Tests ruled out allergic and parasitic disease etiologies. Gastroduodenoscopy with biopsy revealed marked eosinophilic infiltration in the duodenum. Renal biopsy findings indicated minimal change disease spectrum without eosinophilic infiltration. The oral deflazacort dosage was increased, and the patient was discharged after abdominal pain resolved. To our knowledge, this is the first report of eosinophilic gastroenteritis in a patient with minimal change disease.

정체성이미지에 관한 미장센 디자인 연구 - [센과 치히로의 행방불명]을 중심으로 -

본 논문은 미야자키하아오의 2001년도 작품 [센과 치히로의 행방불명] 전개상 편의를 위하여 이하 [센과 치히로]라 칭하겠다. 을 중심으로 감독이 말하고자 하는 주제 표현을 위하여 화면구성에 있어 미장센디자인의 중요성을 연구한 것이다. 영화에서 감독의 메시지는 서술되기보다는 서사되어지기 때문에 담론 안에서 이미지기호들로 상징화되어진다. 이러한 감독의 생각을 읽고 이해하기 위하여 본 연구에서 미야자키 하야오 영화에 나타난 표상이미지가 주제 전달에 있어 미장센이 어떻게 디자인 되어 졌는지 살펴보는데 목적을 두었다. 미야자키하야오의 [센과 치히로의 행방불명]에 나타난 정체성이미지에 대한 미장센디자인 고찰을 크게 다섯 가지로 요약해 보면 다음과 같다. 첫째 서구문화와 일본문화를 구성하는 문화기호에 대한 열린정체성이며, 둘째 인물유형이 지니는 자아에 대한 실존정체성, 셋째 기성세대와 신세대에 대한 소통정체성, 넷째 노동의 가치에 대한 공동분배를 원칙으로 하는 반자본주의에 대한 이데올로기정체성, 다섯째 서구문화를 대변하는 자본주의의 물질만능에 대한 순수정체성을 중심으로 미장센디자인을 살펴보았다. 화면틀frame 본 연구에서는 `화면틀`의 개념을 정지된 이미지보다는 움직이는 이미지로 말하려고 한다. 질 들뢰즈, 유진상 역, [시네마Ⅰ: 운동-이미지], (시각과 언어, 2002), p.29 안에 감독이 담으려는 세계는 그만의 순수한 창작 공간이며, 작품에서 말하려는 메시지는 미장센mise-en-scene이라는 디자인 구성으로 작가의 서술행위를 증명하는 중요한 요소임을 본고에서 밝히는데 의의를 두었다. This study is about the importance of Mise-en-scene design on frame work to present what a director wants to talk, focusing in the movie: [The Spiriting Away of Sen and Chihiro](2001, Miyazaki Hayao). The message of the director is not described but narrated in movies. Thus it is symbolized as image symbol. As one method of understanding the director`s idea, we will see how the image is designed as mise-en-scene to present the theme with Miyazaki Hayao`s movie. Mise-en-scene analysis about identity image in [The Spiriting Away of Sen and Chihiro] has five major categories ; the first, open identity of cultural symbol in forming western and japanese culture, the second, characters` existential identity of ego, the third, communication identity between elder and new generation, the fourth, ideologic identity - anti-capitalism - with joint distribution, and the fifth, pure identity against mammonism representing western cultures. This study will demonstrate that the world the director wants to show in frames is his own pure and creative space and the mise-en-scene as a design structure is the important element to translate the message in works to viewers.
