        Effect of angiotensin Ⅱ inhibition on the epithelial to mesenchymal transition in developing rat kidney

        Purpose : To investigate the effects of angiotensin II inhibition on the epithelial to mesenchymal transition (EMT) in the developing kidney, we tested the expression of EMT markers and nestin in angiotensin converting enzyme (ACE) inhibitor-treated kidneys. Methods : Newborn rat pups were treated with enalapril (30 mg/kg/d) or a vehicle for 7 days. Immunohistochemistry for the expression of α-smooth muscle actin (SMA), E-cadherin, vimentin, and nestin were performed. The number of positively-stained cells was determined under 100 magnification in 10 random fields. Results : In the enalapril-treated group, αSMA-positive cells were strongly expressed in the dilated tubular epithelial cells. The number of αSMA-positive cells in the enalapril-treated group increased in both the renal cortex and medulla, compared to the control group (P<0.05). The expression of E-cadherin-positive cells was dramatically reduced in the cortical and medullary tubular epithelial cells in the enalapril-treated group (P<0.05). The number of vimentin- and nestin-positive cells in the cortex was not different in comparisons between the two groups; however, their expression increased in the medullary tubulointerstitial cells in the enalapril-treated group (P<0.05). Conclusion : Our results show that ACE inhibition in the developing kidney increases the renal EMT by up-regulating αSMA and down-regulating E-cadherin. Enalapril treatment was associated with increased expression of vimentin and nestin in the renal medulla, suggesting that renal medullary changes during the EMT might be more prominent, and ACE inhibition might differentially modulate the expression of EMT markers in the developing rat kidney.         Intestinal lymphangiectasia, one of the protein-losing gastroenteropathies, is an uncommon disease characterized by dilated intestinal lymphatics, enteric protein loss, edema, hypoalbuminemia, and lympocytopenia. Small bowel biopsy and CT have been used to confirm the diagnosis of intestinal lymphangiectasia. Small bowel biopsy shows collections of abnormal dilated lacteals in submucosa with distortion of villi and CT findings have been described as diffuse nodular thickening of the small bowel and as linear hypodense streaking densities in the small bowel caused by dilated lymphatic channels. Demonstration of increased enteric protein loss using $^{51}Cr-$, $^{131}I-$ or $^{99m}Tc-labeled$ albumin, timed measurement of fecal excretion of radioactivity or by measuring fecal clearance of alpha 1-antitrypsin can also help the diagnosis. We experienced a rare case of intestinal lymphangiectasia in an eight year old boy who presented with facial edema, abdominal distension and intermittent diarrhea. We report a patient with intestinal lymphangiectasia, in whom abdominal CT, $^{99m}Tc-labeled$ albumin scintitigraphy, and stool alpha 1-antitrypsin measurement played key roles in determining the diagnosis. A brief review of literature was made. 저자들은 얼굴 부종, 복부 팽만 및 저알부민혈증을 주소로 내원한 8세 환아에서 원발성 장림프관 확장증으로 진단된 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.

        Membranous Nephropathy in a 13-Year-Old Boy with Common Variable Immunodeficiency

        Various forms of hypogammaglobulinemia can occur in patients with autoimmune diseases and vice versa. We report a 13-yr-old boy with membranous nephropathy and common variable immunodeficiency. He presented with the nephrotic syndrome, pneumonia with bronchiectasis, and profound hypogammaglobulinemia. Renal biopsy showed diffusely thickened glomerular capillary walls with ‘spikes’ suggesting a membranous nephropathy. Secondary causes were ruled out by laboratory studies; however, heavy proteinuria persisted with steroid therapy. Cyclosporine and intravenous immunoglobulin were added,and the patient was discharged with decreased proteinuria. Hypogammaglobulinemia may have a deleterious impact on the immune dysregulation in some patients with membranous nephropathy.

        목 적 : Epithelial to mesenchymal transition (EMT)은 태생기에 있어 필수 불가결한 발달과정일 뿐 아니라, 신 섬유화에 있어서도 중요한 역할을 하며, nestin은 고전적인 줄기세포 표지자로 신 세뇨관 간질 손상에 있어 새로운 표지자로 밝혀지고 있다. 신생 백서 신장에서 Angiotensin (Ang) II가 EMT에 미치는 영향을 알아보고자, 안지오텐신 전환 효소 억제제를 투여한 신생 백서의 신장에서 EMT 표지자 및 nestin의 발현 양상을 조사하였다. 방 법 : 7일 동안 신생 백서에게 enalapril (30 mg/kg/d) 또는 vehicle을 투여하였으며, ${\alpha}-smooth$ muscle actin (SMA), E-cadherin, vimentin 및 nestin에 대한 면역 조직 화학 염색을 시행하였다. 결 과 : enalapril 투여군에서 대조군에 비해 신 피질 및 수질 모두에서 ${\alpha}-SMA$ 발현이 증가하였으며, 이는 확장된 세뇨관 상피 세포에서 뚜렷하였다(P<0.05). E-cadherin 발현은 enalapril 투여군의 신 피질 및 수질의 세뇨관 상피 세포에서 확연히 감소하였다(P<0.05). vimentin 및 nestin 발현은 신 피질에서는 양군간의 차이가 없었으나, 신 수질에서는 enalapril 투여군에서 세뇨관 간질 세포에서 발현이 의미있게 증가하였다(P<0.05). 결 론 : 신생 백서 신장에서 Ang II 억제는 ${\alpha}-SMA$ 발현을 증가시키고, E-cadherin 발현을 감소시킴으로써 발달하는 신장의 EMT를 증가시켰다. Enalapril 투여는 또한 신 수질에서 vimentin과 nestin의 발현을 증가시켰으며, 이는 신생 백서 신장에서의 Ang II 억제로 인한 EMT 과정 중 신 수질의 변화가 더욱 뚜렷한 것을 시사하며, Ang II 억제가 EMT 표지자들의 발현을 다르게 변화시키는 것으로 사료된다.         Osteopontin and Developing Kidney

        Osteopontin (OPN) is a glycosylated phosphoprotein which mediates cell adhesion and migration, and is produced by bone, macrophages, endothelial cells, and epithelial cells. The many regulatory functions of OPN include bone remodeling, tumor invasion, wound repair, and promotion of cell survival. It is produced by renal tubular epithelial cells, and expression is upregulated in glomerulonephritis, hypertension, ischemic acute renal failure, renal ablation, and UUO. In this review, we discuss about osteopontin in general aspect, expression, role on the development and pathologic condition of neonatal kidney.

        Impaired angiogenesis in the enalapril-treated neonatal rat kidney

        Purpose: Nephrogenesis is normally accompanied by a tightly regulated and efficient vascularization. We investigated the effect of angiotensin II inhibition on angiogenesis in the developing rat kidney. Methods: Newborn rat pups were treated with enalapril (30 mg/kg/day) or vehicle (control) for 7 days after birth. Renal histological changes were checked using Hematoxylin & Eosin staining. We also investigated the intrarenal expression of vascular endothelial growth factor (VEGF)-A, VEGF receptor 1 (VEGFR1), VEGFR2, platelet-derived growth factor (PDGF)-B, and PDGF receptor-β with Western blotting and immunohistochemical staining at postnatal day 8. Expression of the endothelial cell marker CD31 was examined to determine glomerular and peritubular capillary density. Results: Enalapril-treated rat kidneys showed disrupted tubules and vessels when compared with the control rat kidneys. In the enalapril-treated group, intrarenal VEGF-A protein expression was significantly higher, whereas VEGFR1 protein expression was lower than that in the control group (P<0.05). The expression of VEGFR2, PDGF-B, and PDGF receptor-β was not different between the 2 groups. The increased capillary CD31 expression on the western blots of enalapril-treated rat kidneys indicated that the total endothelial cell protein level was increased, while the cortical capillary density, assessed using CD31 immunohistochemical staining, was decreased. Conclusion: Impaired VEGF-VEGFR signaling and altered capillary repair may play a role in the deterioration of the kidney vasculature after blocking of angiotensin II during renal development.

        The objective of this study is analyzing of requirements for improvement of the work-life balance of construction managers working in Korean construction companies. A survey has been conducted to analyze the requirements for improvement of construction managers. The survey was conducted on construction managers working in the field of architectural construction. 59 field managers working in 18 Korean construction companies responded to the survey. Upon analysis of the survey, the working hours were excessive and recreational time was very poor for the construction managers. There were differences in the answers regarding the areas that require improvement in the working conditions based on the age groups of the construction managers. A majority of young technicians selected that the recreational time and content of work require improvement, and technicians with a lot of experience selected that the stability of occupation and wage require improvement.
