This study was compared pre-operative MRI finding with operative finding on 36 cases with the failed back surgery syndrome (FBSS) retrospectively. The purpose of this study was to determine the accuracy of the MRI on the FBSS including early complications such as hematoma of infection. Of the 51 patients with FBSS, we analyzed pre-operative MRI finding compared with operative finding on 36 cases excluding nonunion, instability, metal failure and pseudoarthrosis who underwent an operation for the FBSS from December 1994 to June 1997. There were 25 men (69.4%) and 11 women (30.6%), aged from 16 to 68 years (average 43.6 years). These were divided into 5 sub-groups and calculated sensitivity, specificity and positive predictability. MRI accuracy in recurred disc was 84%, scar adhesion, recurred or developed stenosis, infection and hematoma 100% each and all, overall accuracy of the MRI 93%, respectively. Average interval of re-operation in FBSS WAS 4.3 years. In the 18 cases (50%), symptoms persist without pain-free interval after first operation. Early complications including hematoma and infection are easily detected with MRI. Especially if patient complains of severe leg pain or neurologic deficit, it is a good diagnostic procedure to check the MRI. The MRI is a useful method for evaluation of most cases of the FBSS, but it has limitations to evaluate recurred disc or scar adhesion only in T1, T2 weighted image. Therefore Gd-DTPA enhancement is necessary for the accurate diagnosis.

In this paper, the fracture simulation of ceramic dome under shock pressure is performed by the finite element method with nonlinear code LS-Dyna. The material properties of simulation are applied with experimental result of tensile test for MACOR Glass-filled ceramic. The simulation is carried out with 3 type dome models under various impact pulse shape. The fracture strength and pressure are compared about each case. The results of the simulation are useful to evaluate the behavior of ceramic dome.

목적: 고관절 주위 골절이 있었던 노인 환자에서 대퇴골두의 관절 연골의 변성 정도를 육안적, 조직학적, 면역 조직 화학적 소견을 통해 평가해 보고자 하였다. 대상 및 방법: 고관절 주위 골절로 양극성 반치환술을 시행 받은 65세 이상 환자 중 수상 전에 고관절부 동통없이 보행이 가능하고 술 전 단순 방사선 사진상 퇴행성 변화가 없었던 30예를 대상으로 하였고, 평균 나이는 79.7세 (65-94)였다. 육안적 소견은 술 중 적출된 대퇴골두의 체중 부하부위 3구역(전상, 중상, 후상방) 및 비체중 부하 부위 1구역(하방)에서 시행한 후, 총 4구역에서 연골하골을 포함한 관절 연골을 채취하여 Hematoxylin & Eosin 염색 및 Safranin O 염색을 시행하여 조직학적 변성정도를 분류하였고, Ⅱ형 collagen 항체를 이용하여 면역 조직 화학염색을 시행하였다. 결과: Outerbridge 분류에 의한 육안적 소견은 grade Ⅰ 이하가 82.5%였고, Mankin 등급에 의한 조직학적 소견과는 유의한 상관 관계를 보였다. Mankin 등급에 의한 조직학적 소견은 24예(80%)에서 경도의 변성을, 나머지 6예에서 중등도의 변성을 보였으며, 각 구역별 변성 정도는 모두 평균 5점 이하로 경도의 변성을 보였다. 체중 부하부위와 비체중 부하부위 간의 변성 정도에는 유의한 차이가 없었다. Hollander 등의 분류에 의한 면역 조직 화학염색상 결괴는 grade Ⅰ 이하가 28예(93.3%)로 대부분 정상 또는 경미한 변성을 보였다. 결론: 고관절 주위 골절이 있었던 노인 환자에서 방사선적으로 고관절부의 퇴행성 변화 소견이 없는 경우 대퇴 골두 연골의 변성 정도는 대부분 경도의 변성을 보였으며, 부위와 상관없이 전반적인 변화를 보였다. Purpose: To evaluate the incidence, grade and extent of the degenerative changes in the femoral head articular cartilage in the elderly with hip fracture through gross, histological and immuno-histochemical studies. Materials and Methods: The study examined 30 femoral heads obtained from patients over 65 years with no arthrosis by radiograph, who had undertaken bipolar hemiarthroplasty due to hip fracture. The mean age of patients was 79.7 years and the study examined three weight bearing zones (anterior, middle and posterior) and one non-weight bearing zone (inferior) of each femoral head. Results: The gross findings showed significant correlation with the Mankin grade for each zone (p<0.05). Twenty four cases (80%) showed mild degeneration according to the Mankin grade and the others moderate. The mean Mankin score at each zone of all femoral heads was 5 points, which indicated mild degeneration. No significant difference was found between the weight bearing zone and the non-weight bearing zone (p=0.75). The distribution of type Ⅱ collagen antibody stainabilities mainly showed normal or mild degeneration (93.3%). Conclusion: Degeneration of the articular cartilage of the femoral heads in the elderly with hip fracture and no arthrosis by radiograph was mostly mild, and showed generalized changes regardless of the zone.

<P><B>Aims</B></P><P>At present no proven standard treatment for drug-eluting stent (DES) restenosis is available, and the efficacy and safety of everolimus-eluting stent (EES) and zotarolimus-eluting stent (ZES) for DES restenosis are limited. The purpose of this prospective, randomized 9-month intracoronary ultrasound (IVUS) and 3-year clinical follow-up study was to compare the effects of EESs and ZESs on neointima volume and major adverse cardiovascular events (MACEs) such as death, myocardial infarction (MI), target lesion revascularization (TLR) and stent thrombosis in DES restenosis patients.</P><P><B>Methods and results</B></P><P>Patients were eligible for this study if they were between 40 and 75 years old with in-stent restenosis >50% by quantitative coronary angiographic analysis in DES or within 5 mm of the stent edges with signs of ischaemia. Eligible patients (<I>n</I> = 304, 146 women and 158 men) were randomly assigned to receive either EES (158 patients) or ZES (146 patients). The primary endpoint of the study was to compare neointima volume between the EES and ZES groups at the 9-month follow-up IVUS. MACEs, including death, non-fatal MI, stent thrombosis and the need for repeated TLR within 3 years, were noted. The 9-month angiographic and IVUS follow-up showed no significant differences in late lumen loss (0.40 ± 0.56 vs. 0.45 ± 0.61 mm, <I>P</I> = 0.57, respectively) and neointima volume (0.51 ± 0.48 vs. 0.56 ± 0.54 mm<SUP>3</SUP>/1 mm, <I>P</I> = 0.47, respectively) in the EES and the ZES groups. Composite MACEs such as death, MI, stent thrombosis and TLR during 3-year follow-up were comparable between the two groups [15.8% (<I>n</I> = 25) in the EES group and 22.6% (<I>n</I> = 33) in the ZES group, <I>P</I> = 0.276], independent of <I>de novo</I> DES type, sex, age, body mass index, presence of diabetes, hypertension and dyslipidaemia.</P><P><B>Conclusions</B></P><P>Patients with first- and second-generation DES restenosis, both EES and ZES implantation were effective and safe in reducing neointima volume and late loss with a comparable rate of MACEs independent of cardiovascular risk factors. </P>

Objective: The purpose of this investigation was to establish the prenatal diagnosis for identifying the risk for epidermolytic palmoplantar keratoderma(EPPK) of a fetus by sequence analysis of fetal genomic DNA from chorionic villi. Methods: Chorionic villus sampling under transvaginal sonography at 12 weeks of gestation from a woman at risk for a child in a EPPK-affected family was perfomed. Polymerase chain reaction amplification of specific allele (PASA) assay was carried out for the detection of mutation(R162W in keratin 9 [K9] gene) previously identified in this family. Direct DNA sequencing analysis of K9 gene was accomplished to confirm the mutation. Results: We had found the point mutation, R162W of K9 gene, in affected family members and confirmed by PASA assay. Affected family members were shown to have PCR products reactive with both the mutant and wildtype specific primers. Because we could not find any expected products after PASA assay with the primers la(+)/KSmt(-) of the fetal DNA, we predicted that the fetus did not inherited the mutant allele and that the fetus could be unaffected. After PASA assay, we analyzed DNA sequences of two family members to confirm the mutation. A C-to-T substitution at bp 545 was detected in the father, instead the fetus did not have any mutant band at that base pair. Conclusion: The PASA assay and direct DNA sequencing analysis of K9 gene through chorionic villi sampling and extraction of genomic DNA had validity to early prenatal diagnosis whether fetus was affected in EPPK or not.
