        Diagnosis and treatment of chronic myelomonocytic leukemia

        Chronic myelomonocytic leukemia (CMML) is a clonal disorder of hematopoietic cells and is a complex of heterogeneous conditions with both myeloproliferative and myelodysplastic features. The diagnosis of CMML is made using morphologic criteria including monocyte-dominant leukocytosis, dysplastic changes, and increased blasts in the bone marrow. Recently, the identification of monocyte subtypes in peripheral blood using multiparameter flow cytometry has been actively studied. Chromosomal abnormalities are the basis of CMML risk stratification, and mutations in several genes including ASXL1 are known to be important not only for the diagnosis and treatment of this disease but also for predicting its prognosis. The standard treatment principles for CMML have not yet been clearly defined; however, hypomethylating agents are mainly considered the frontline therapy in most cases. Although allogeneic hematopoietic stem cell transplantation has limited applications owing to its toxicity, it still plays an important role as the only curative treatment option. Researchers are continuing to develop new drugs for CMML treatment and to prove their clinical usefulness. This review summarizes what is known to date on the diagnosis, treatment, and prognostic factors of CMML and presents future directions by analyzing recent research trends.

        Long-term Survival after Repeated Local Therapy and Salvage Chemotherapy for Recurrent Metastases from Gastric Cancer: a Case Report and Literature Review

        We report a rare case of long-term survival in a patient who received local therapy and salvage chemotherapy for recurrent metastases, along with a literature review. A 65-year-old male patient underwent subtotal gastrectomy for advanced gastric adenocarcinoma. Six months after gastrectomy, 2 metastatic intra-abdominal lymph node enlargements were detected, which were treated with radiotherapy. At 55 months after gastrectomy, an abdominal wall mass was detected, which was treated by surgical resection. The patient received 5-fluorouracil/leucovorin/irinotecan chemotherapy for 27 months before and after radiotherapy and docetaxel chemotherapy for 6 months after surgical resection of the abdominal wall metastasis. At the last visit, 7.8 years since the initial resection of the primary gastric cancer and 6.2 years since detection of the first metastases, the patient was disease-free and required no further chemotherapy. This case suggests that repeated local therapy offers potential for long-term survival in a carefully selected subset of patients with recurrent metastases.

        능동적 신노년 시청자는 어떤 방송 서비스를 필요로 하는가?

        본 연구는 디지털 기기 사용에 능숙하고, 영상 제작 능력이 있는 능동적 성향의 신노년 시청자 12명을 인터뷰 결과를 바탕으로 디지털 시대 노인 시청자들이 필요로 하는 방송 콘텐츠와 방송 서비스를 파악했다. 조사 결과를 요약하면 다음과 같다. 첫째 능동적인 신노년 시청자는 웹에서 원하는 교육을 찾아서 유튜브를 통해 수업을 시청하고, 취미 생활을 누리는 등 바쁜 일상을 보냈다. 둘째, 생활 주기에 따라 선호하는 방송 시간대가 다르기에 노인을 대상으로 하는 방송 프로그램 편성에 대한 불만이 적었다. 셋째, 노인 시청자를 무시하는 TV에 의존하는 충성도 높은 소극적 노인 수용자로 남기보다는 자신이 좋아하는 콘텐츠를 방송하는 인터넷 플랫폼(유튜브)을 찾아서 이동하는 비판적 능동적 이용자로 진화했다. 넷째, 다수의 영상 콘텐츠 제작 경험을 토대로 노인 대상 프로그램 제작할 때 유용한 영상 편집 방법을 제시했다. 다섯째, 신조어 중심의 방송 내용을 이해하지 못해서 소외감을 느끼고, 노인에 대한 부정적인 이미지를 고착하는 방송 내용을 보면서 자존감 하락을 경험한 노인들은 노인을 위한 방송 콘텐츠 제작을 요구했다. 여섯째, 세대 갈등을 줄이기 위해서 젊은 층과 소통할 방법을 알려주는 콘텐츠 제작을 요구했다. 마지막으로 디지털 취약 계층인 노인이 쉽게 이해할 수 있게 디지털 미디어 교육 영상을 제작해서 QR 코드나 ‘자막’을 활용해서 반복적으로 시청할 수 있는 시스템을 갖추기를 요구했다. 이 연구의 함의는 첫째, 능동적 신노년 시청자는 더 이상 TV 시청에 의존하는 고립되고 소외된 수동적 수용자 위치로 남기보다는 적극적이고 비판적인 디지털 미디어 이용자로 진화함을 발견한 것이다. 둘째, 영상 제작 경험, 디지털 미디어 이용 경험, 또는 교육자로서 시행착오를 바탕으로 노인을 위한 방송 서비스를 개발하고 디지털 격차 해소를 위한 실천적 방안을 제시했다는 것이다. The goal of this study is to conduct in-depth interviews with 12 active new-aged viewers (who are skilled in digital devices and have experience in video production), and then propose broadcast content and broadcast services that elderly viewers in the digital age need. The summary of the research results is as follows. First, active new-age viewers had a busy daily life, finding necessary education on the web, watching YouTube classes, and enjoying hobbies. Second, there were few complaints about the program programming time for the elderly because the preferred broadcast time slot differed depending on the life cycle. Third, they have evolved into critical active users who move to their preferred content platform instead of remaining loyal, dependent, passive and marginalized TV audiences that ignore elderly viewers. Fourth, based on the experience of producing video contents, useful video editing methods were presented when producing programs for the elderly. Fifth, they did not understand the content of the broadcast centered on neologisms and demanded the production of broadcast content for the elderly because their self-esteem declined while watching the content of the broadcast that emphasized the negative image of the elderly. Sixth, in order to reduce generational conflict, it was requested to produce content that shows how to communicate with the younger generation. Lastly, it was requested to have a broadcasting system for the development of digital educational content for the digitally underprivileged elderly.

        인터넷 팬 커뮤니티의 규율권력이 어떻게 행사되는가?

        본 연구는 푸코의 ‘권력’이론을 토대로 인터넷 팬 커뮤니티에도 규율권력이 작용하고 있음을 밝히고 있다. 디시인사이드 ‘주군의 태양’ 갤러리를 참여관찰하고 팬 담론을 분석하여, 팬 커뮤니티에서 규율권력이 행사되는 과정과 규율권력을 지지하는 ‘지식’과 권력관계에서 구성된 ‘진리’가 무엇인지를 찾고자 했다. 분석결과, 인터넷 팬 커뮤니티에서는 ‘고나리’라는 장치를 활용해서 규율권력을 행사하고 있었다. 팬 커뮤니티의 질서와 규칙을 체득하고 있는 오래된 팬들은 감시와 처벌을 통해서 신입 팬을 팬 공간이 요구하는 주체가 될 수 있도록 교육했다. 신입 팬들은 훈육과정을 통해서 적극적이고 열정적인 팬으로 성장했다. 팬 커뮤니티에서는 구체적으로 ‘친목’, ‘셀털’, ‘찻내’ 행위를 규제했다. 규율권력은 “팬 커뮤니티의 성장과 발전을 위해서”라는 지식에 의해 정당화되었다. 팬 커뮤니티는 ‘규율 권력’과 그 ‘저항’이 투쟁하는 역동적 공간이었으며, 다양한 지점에서 얽혀있는 권력관계와 규율권력의 지식은 “팬 공간을 유지하고 발전시켜야 한다”는 담론적 진리를 구성했다. This study analyzes how the disciplinary power in the internet fan community is exercised on the basis of Foucault’s theory of power. The diverse discourses from fan activities collected through participant observation in the internet fan community (DCinside “Master’s Sun” gallery) are analyzed by discourse analysis method. This study aims to trace the trajectories in which the disciplinary powers are exercised, to discover the ‘knowledge’ which bolsters disciplinary powers, and to identify the ‘truth’ formed in the web of power relations. The research findings show that the internet fan community utilizes a disciplinary power apparatus called ‘Gonari to manage its own community. Long-term fans who are well aware of the hierarchical order and the rules of the fan community take charge of educating new fans to become desirable subjects of the fan community by means of surveillance and regulation. New fans have eventually become active fans under the supervising gaze and through penal interventions. In this process the knowledge of disciplinary power enacts and refines the disciplinary power itself. The disciplinary powers appear in the form of regulations which prohibit ‘Chinmok (forming friendly groups)’, ‘Channae (sweet-talking)’, and Selftell (talking about personal stuff)’. This kind of disciplining power is justified by the knowledge that it is necessary “for the growth and development of the fan community.” Internet fan community is a dynamic space in which ‘disciplining powers’ and their resisting counterparts collide. Knowledge of disciplinary power and power relations entangled at various points construct the discursive truth that “fan community should be maintained and flourished at any rate.”

      • Bifacial CdS/CdTe thin-film solar cells using a transparent silver nanowire/indium tin oxide back contact

        <P>A hybrid silver nanowires (AgNWs)/indium tin oxide (ITO) contact was used as a transparent back-electrode to fabricate a bifacial CdS/CdTe thin-film solar cell. The photovoltaic properties of the bifacial CdS/CdTe thin-film solar cell were investigated under front and back illumination conditions. The hybrid AgNWs/ITO back contact changed the average conversion efficiency from 0.4% (front) and 3.5% (rear) to 8.1% (front) and 0.9% (rear), respectively, improving the sum efficiency from 3.9% (ITO-only) to 9.0%. Our research demonstrates the use of a nanowire network as a transparent electrode in CdS/CdTe thin-film solar cells for bifacial or tandem applications. (C) 2017 Optical Society of America</P>

        Variability of Transrectal Shear Wave Elastography in a Phantom Model

        Purpose This study aimed to assess the variability of transrectal shear wave elastography (SWE) using a designed phantom. Materials and Methods In a phantom, the SWE values were examined by two radiologists using agarose and emulsion silicone of different sizes (1, 2, and 3 cm) and shapes (round, cubic) at three depths (1, 2, and 3 cm), two region of interest (ROI) and locations (central, peripheral) using two ultrasound machines (A, B from different vendors). Variability was evaluated using the coefficient of variation (CV). Results The CVs decreased with increasing phantom size. Significant changes in SWE values included; agarose phantom at 3 cm depth (p < 0.001; machine A), 1 cm depth (p = 0.01; machine B), emulsion silicone at 2 cm depth (p = 0.047, p = 0.020; both machines). The CVs increased with increasing depth. Significant changes in SWE values included; 1 cm agarose (p = 0.037, p = 0.021; both machines) and 2 cm agarose phantom (p = 0.047; machine A). Significant differences in SWE values were observed between the shapes for emulsion silicone phantom (p = 0.032; machines A) and between ROI locations on machine B (p ≤ 0.001). The SWE values differed significantly between the two machines (p < 0.05). The intra-/inter-operator agreements were excellent (intraclass correlation coefficient > 0.9). Conclusion The phantom size, depth, and different machines affected the variability of transrectal SWE.
