Purpose: To assess the prevalence and progression of a stage 0 macular hole in the fellow eye of patients with an idiopathicfull-thickness macular hole. Methods: The fellow eyes of 189 patients who underwent idiopathic full-thickness macular hole surgery were examinedby biomicroscopy and spectral domain-optical coherence tomography (SD-OCT). A subset of 21 fellow eyes with a stage 0macular hole was observed. Changes in the macular hole were evaluated by biomicroscopy and SD-OCT for an average of 29months. Results: Among the 21 eyes, 15 showed no change in perifoveal vitreous detachment (71.4%). Two eyes (9.5%) developedcomplete vitreofoveal separation, and one of the two developed a separation after progression to stage 1A. Among 21 eyes,5 (23.8%) developed above stage 1A, and one of the five progressed to stage 1B after five years, which was successfullytreated with vitrectomy and gas tamponade. Conclusions: Perifoveal vitreous detachment in the fellow eye on SD-OCT, defined as a stage 0 macular hole, occurred at anearlier phase than stage 1A macular holes and may progress to an advanced stage. Therefore, patients who undergo macularhole surgery and have a stage 0 macular hole or perifoveal vitreous detachment in the fellow eye should be followed closely.
Elevated serum immunoglobulin E level as a marker for progression of immunoglobulin A nephropathy

Background: Immunoglobulin E (IgE) has traditionally been associated with anaphylaxis and atopic disease. Previous studies reported that serum IgE levels are elevated in nephrotic syndrome and suggested IgE levels as a prognostic indicator in glomerular diseases. The aim of this study was to explore the association between serum IgE levels and renal outcome in patients with immunoglobulin A nephropathy (IgAN). Methods: We included 117 patients with biopsy-proven IgAN. Renal progression was defined if a patient meets one of these criteria: (1) a negative value of delta estimated glomerular filtration rate (mL/min/1.73 m2/mo) or (2) a rise in serum creatinine to an absolute level of 1.3 mg/dL (male) or 1.2 mg/dL (female). We defined delta changes in serum creatinine, estimated glomerular filtration rate, and proteinuria as a difference of values during the follow-up period. Results: A total of 117 patients with IgAN were included. The serum IgE level was significantly high in the renal progressive group compared with the nonprogressive group. Sex and history of gross hematuria were significantly different between the high-IgE group and the low-IgE group. Regression analysis showed that a male sex, initial proteinuria, and change of proteinuria were significantly associated with serum IgE levels. Conclusion: The serum IgE level is potentially associated with disease progression and pathogenesis of IgAN.

Background: The objective of this study was to compare the impact of citrate dialysate (CD) and standard acetate dialysate (AD) in hemodialysis by central delivery system (CDS) on heparin demand, and clinical parameters. Methods: We retrospectively evaluated 75 patients on maintenance hemodialysis with CDS. Patients underwent hemodialysis with AD over a six-month period (AD period), followed by another six-month period using CD (CD period). Various parameters including mean heparin dosage, high sensitivity C-reactive protein (hsCRP), calcium-phosphate product (CaxP), intact parathyroid hormone (iPTH), and urea reduction ratio (URR) were collated at the end of each period. Results: Patients were 60.5 ± 14.7 years old, of whom 62.7% were male. Patients required less heparin when receiving CD (AD period: 1,129 ± 1,033 IU/session vs. CD period: 787 ± 755 IU/session, P < 0.001). After the CD period (Δ<sub>CD</sub>), pre-dialysis total CO<sub>2</sub> increased to 1.21 ± 2.80 mmol/L, compared to -2.44 ± 2.96 mmol/L (P < 0.001) after the AD period (Δ<sub>AD</sub>). After the CD period, concentrations of iPTH (Δ<sub>AD</sub>: 73.04 ± 216.34 pg/mL vs. Δ<sub>CD</sub>: -106.66 ± 251.79 pg/mL, P < 0.001) and CaxP (Δ<sub>AD</sub>: 4.32 ± 16.63 mg<sup>2</sup>/dL<sup>2</sup> vs. Δ<sub>CD</sub>: -4.67 ± 15.27 mg<sup>2</sup>/dL<sup>2</sup>, P = 0.015) decreased. While hsCRP levels decreased after the CD period (Δ<sub>AD</sub>: 0.07 ± 4.09 mg/L vs. Δ<sub>CD</sub>: -0.75 ± 4.56 mg/L, P = 0.705), the change was statistically insignificant. URR remained above clinical guideline of 65% after both periods (Δ<sub>AD</sub>: 72.33 ± 6.92% vs. Δ<sub>CD</sub> period: 69.20 ± 4.49%, P = 0.046). Conclusion: Our study confirmed that the use of CD in CDS required lower heparin doses compared to the use of AD. The use of CD also provided a more stable acid-base status.

<P>To develop an appropriate drug carrier for drug delivery systems, we prepared random poly(lactide-<I>co</I>-glycolide-<I>co</I>-ε-caprolactone) (PLGC) copolymers in comparison to commercial poly(lactic acid-<I>co</I>-glycolic acid) (PLGA) grades. The molecular weights of PLGC copolymers varied from 20k to 90k g mol<SUP>−1</SUP> in the total polyester segments, when poly-l-lactic acid (PLLA), polyglycolic acid (PGA), and polycaprolactone (PCL) compositions were kept constant. The lengths of PLGC copolymers varied from 10 : 10 : 80 to 40 : 40 : 20 in the PLLA : PGA : PCL segments, when the molecular weights of the total polyester segments were kept constant. The crystalline properties of the PLGA copolymers can be changed to amorphous by the incorporation of PCL segments. <I>In vitro</I> and <I>in vivo</I> degradation behavior can be easily tuned from a few days to a few weeks by changing the chemical composition of the PLGC copolymers. The <I>in vivo</I> inflammation associated with the PLGC implants was less pronounced than that associated with PLGA. In this study, as drug delivery carriers for locally implantable paclitaxel (Ptx) dosages, Ptx-loaded PLGC and PLGA films showed <I>in vitro</I> and <I>in vivo</I> Ptx release for 35 days. The orders of Ptx release showed profiles similar to those of <I>in vitro</I> and <I>in vivo</I> degradation of PLGC. Using near-infrared (NIR) fluorescence imaging, we confirmed the sustained release of NIR over an extended period from IR-780-loaded PLGC and PLGA implanted in live animals. In conclusion, we confirmed that compared to PLGA, PLGC effectively acts as a drug carrier for drug delivery systems.</P>

Liquefied natural gas has the unique distinction of being the only flammable or hazardous material. Inspection effectiveness may be introduced into the probability of failure calculation by using Bayesian analysis or more directly by modifying the model for the independent variables, the distribution function and parameters, such as mean and coefficient of variation, changed based on the NDE method and coverage used during an inspection. Available failure data from the LNG industry are often statistically unreliable. Therefore, we need to improve more accurate information data. NDE reliability matrix is to apply optimization method of the NDE method. Reliability matrix is composed with combinations of environment reliability and inspection reliability. Environment parameters consist of inspector qualification and experience, test condition and inspection reliability parameters are composed of the inspection method and process method, such as UT, ET, RT and s/n ratio, signal processing method, etc. Finally, this paper purposes that the Reliability matrix is a powerful tool for risk assessment of not only the LNG industry but also in other industry.

The construction market of high value-added LNG plants is spotlighted world widely. The project has been expanded and improved the LNG plant of processing facilities of the past , however the study on risk assesment on developed countries is attracting public attention to performance improvement components of LNG plant system of risk based inspection until recently. Thus, we have to study the risk based inspection for the defending oneself against to developed countries. In this paper, non-destructive evaluation can be used for enhancement of LNG plant as core parts that are designed for probability of detection in the facilities. The modular parts are related to a system with artificial optimized algorithms as well as diverse databases of facility inspection and diagnosis fields.
