Primary malignant melanoma of the esophagus is an extremely rare disease which counts for approximately 0.1-0.2% of all esophageal malignancies. It is also a very aggressive disease with a poor prognosis, with its 5yr survival rate ranging from 2.2% - 37.5%. Here we report a case of primary malignant melanoma of the esophagus in a patient whose diagnosis was made relatively early and thus total resection by an Ivor Lewis procedure was possible. A 51 year old Asian woman visited our hospital with epigastric discomfort. The patient had no noticeable medical history. She complained of mild nausea and dyspepsia, but there was no vomiting or weight loss. Physical examination showed no palpable lymph nodes in the head and neck region. There was no tenderness or rebound tenderness on the abdomen. For evaluation of her upper gastrointestinal symptoms, she had the upper endoscopy done. Endoscopy identifi ed a few dark pigmented, polypoid lesions scattered from 22cm to 30cm from the incisor tooth. The lesions were various in size, ranging from 0.2cm to 4-5cm and were fl at, slightly elevated with a irregular base. Biopsy done from two spots showed a well demarcated, slightly elevated and black pigmented tumor. For stage work-up, additional examinations were performed. The chest CT showed no abnormal thickening of the esophagus or any enlarged lymph nodes in the mediastinum. The PET-CT showed a mild focal abnormal uptake in the upper thoracic esophagus. The patient underwent an Ivor-Lewis procedure in June, 2013. Pathologic examination revealed spindle shaped malignant melanocytes with hyperchromated nucleis and black pigmentation. The lesion had invaded the submucosal layer. Thus the patient was diagnosed as esophageal malignant melanoma, stage T2N0M0. She is now under close observation at the out-patient department and has been disease free for 13 months.e

<P><B>Abstract</B></P> <P>The endangered Black-faced Spoonbill <I>Platalea minor</I> has experienced drastic reductions in population size, geographic distribution, and habitat availability throughout East Asia. In the present study, we examined population genetic structure and genetic diversity of Black-faced Spoonbills inhabiting five sites off the west coast of South Korea encompassing a few of its major breeding sites. Ten microsatellite loci and the mitochondrial sequence were used to assess patterns of genetic variation based on 63 individuals. Three ND2 haplotypes were found among 61 individuals; the remaining two were identified as Eurasian Spoonbills, revealing an unexpected hybridization between these two species having different ecological niches in South Korea—the Eurasian Spoonbill overwinters in inland areas, whereas the Black-faced Spoonbill inhabits coastal areas during the summer. Analyses of microsatellite variation revealed no discrete population structure among the five breeding sites but very weak genetic differentiation among geographically distant regions. Assignment tests identified several possible migrants among sites. Our findings suggested that Black-faced Spoonbills from the five breeding sites could be managed as a single population and highlighted the importance of conserving the populations from Maedo, Suhaam, and Namdong reservoir, which are geographically close and have retained high levels of genetic diversity and large populations.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Ten polymorphic microsatellite loci and the mitochondrial DNA sequence were used to assess genetic diversity and structure. </LI> <LI> No discrete population structure among the five breeding sites in South Korea but very weak genetic differentiation among geographically distant regions. </LI> <LI> Black-faced Spoonbill individuals from the five breeding sites could be managed as a single population in South Korea. </LI> <LI> Unexpected hybridization between the Eurasian Spoonbill and the Black-faced Spoonbill was observed in our genetic studies. </LI> </UL> </P>

The purpose of this study was to examine the associations of men`s ‘work-family conflict’ and ‘family-work conflict’ with their parental satisfaction, by their families’ developmental stage. This study used data collected for the Men’s Work, Family and Quality of Life(Seo & Lee, 2002). The analysis was based on the subsample of 214 fathers who worked at companies. The data were analyzed with the descriptive statistics, stepwise multiple regression, and Peason`s correlations. The major results of this study were as follows: First, the fathers reported an average level of ‘work-family conflict’ and a lower level of‘family-work conflict’. The variables that explained men`s ‘work-family conflict’ were night-work frequency, daily work hours and flexibility. The variables which explained ‘family-work conflict’ were a compressed workweek, number of children and father’s age. Second the fathers reported a high level of parental satisfaction. Third, ‘work-family conflict’ was not related to men`s parental satisfaction regardless of the first child`s age. Forth, only for men whose first child entered university, men`s ‘family-work conflict’ was negatively related to men`s parental satisfaction.

Aims: Bioartificial liver (BAL) support offers a potential means of improving survival of acute liver failure (ALF) patients by providing partial liver function until a suitable donor liver is found or the native liver undergoes regeneration. Previous studies have suggested that ALF patients treated using BAL maintain a more stable medical condition, which may positively influence outcome after liver transplant (LT). This clinical trial was conducted to evaluate the safety and efficacy of the LifeLiver BAL system in ALF patients. Methods: Patients with evidence of acute liver failure exhibiting hepatic encephalopathy grade 2 or worse who were listed for deceased donor liver transplant were eligible for enrollment. Hepatocytes were harvested from 3~4 weeks old male pigs weighing 4 to 10 kg, raised in a sterile environment. The isolate hepatocytes were cultured to form spheroids and were then mixed with 1.5% alginate solution and placed in a high content/speed immobilization apparatus and dropped into 100mM calcium solution. The Ca-alginate-immobilized hepatocyte spheroid beads were packed within the bioreactor of the BAL system. BAL treatment was continued for up to 12 hours. Results: Six patients were given BAL treatment and were included in the safety analysis. Adverse events related to BAL treatment included coagulopathy (increased INR), pneumonia, sepsis and disease progression. One patient developed upper gastrointestinal tract bleeding and sepsis following BAL treatment and eventually died while waiting for transplant. No evidence of porcine endogenous retrovirus (PERV) were seen. Five patients completed the study per protocol and were included in the efficacy analysis. Four patients showed apparent decrease in serum ammonia levels and MELD scores during BAL treatment. Hepatic encephalopathy either decreased or remained stable throughout the BAL treatment period in 4 patients. Conclusions: The LifeLiver BAL support system showed safety and efficacy in ALF patients with hepatic encephalopathy. Two patients were successfully bridged to liver transplantation.

Objectives : The purpose of this study was as follows : 1) to compare the current status of management for dementia between city and country health center. 2) to check the amount and detailed items of budgets for dementia in dementia counseling center, referral courses for registration of dementia patient, number of dementia patient referred to welfare institutions for the elderly. 3) to understand the troubles in management for dementia in health center and to provide baseline data about future community dementia management system. Method : The questionnaire was administered to 248 public health centers. The 162 collected questionnaires were used in analysis and the whole response rate was 65.3%. Questionnaire composed by general contents about dementia management in health center and dementia counseling center. Statistical analysis was conducted to compare the difference about each item in city and country health center. Results : City and country health center showed differences in number of registered dementia patient, budgets for dementia, referral courses for registration of dementia patient, number of dementia patient referred to institution, troubles in management for dementia in health center. Conclusion : Current dementia management system does not meet the various needs of community for management of dementia. Thus, complementary strategies are needed to solve this problem.
