        증례 : 신장 ; 뇌하수체 침범 뇌수막종 환자에서 고나트륨혈증에 의한 횡문근 융해증

        횡문근 융해증의 원인에 고나트륨혈증이 속해 있지만 보고된 증례는 드물다. 본 저자들은 뇌하수체를 침범하는 거대 뇌수막종 환자에서 중추성 요붕증으로 인한 것으로 추정되는 고나트륨혈증에 의해 횡문근 융해증이 발생한 환자를 경험했다. 데스모프레신 투여 후에 고나트륨혈증 및 횡문근 융해증이 함께 호전되는 임상경과를 보인 증례를 경험했기에 문헌고찰과 함께 보고하는 바이다. Hypernatremia is a rare cause of rhabdomyolysis. Here, we report a case of hypernatremia-induced rhabdomyolysis in a with meningioma involving the pituitary gland. A 61-year-old male was admitted for decreased mentality and poor oral intake. had undergone an operation for meningioma 10 years prior. At admission, he appeared lethargic and severely dehydrated with initial sodium level of 178 mEq/L. Hypernatremia remained persistent despite massive hydration and the serum creatine phosphokinase level was 18,047 U/L after 3 days. Bone scintigraphy also showed findings consistent with rhabdomyolysis. Brain magnetic resonance imaging revealed extensive masses involving the pituitary gland and an intranasal biopsy confirmed meningioma. Polyuria, and low anti-diuretic hormone levels supported the diagnosis of central diabetes insipidus-induced hypernatremia. Desmopressin was administered intranasally and the patient`s serum sodium and muscle enzyme levels were normalized. (Korean J Med 2015;88:324-329)

        한국 가상현실 게임소설의 스토리텔링

        본 논문에서는 한국 가상현실 게임소설에 나타난 게임과 소설의 재매개 양상을 분석하고, 이를 통해 한국가상현실 게임소설의 서사적 특징을 도출했다. 나아가 웹을 기반으로 삼는 한국 게임소설이 대중문학으로서 확장될 수 있는 가능성을 모색하는 데에 그 목적을 둔다. 한국 가상현실 게임소설에서 주인공은 20대 남성으로, 신자유주의 사회에서 소외된 계층이자 결핍된 성과주체로 설정된다. 이들은 현실 세계의 결핍요소 때문에 가상세계로 진입하는데, 이 때 해외 게임소설에서 강조되는 가상세계로의 접속 방법 및 과정 등이 국내 소설의 경우 소략하게 제시된다. 또한 한국 가상현실 게임소설의 주인공이 수행하는 반복적 행동은 자기 파괴적인 노동의 범주에 속하며, 주인공은 나르시시즘적 징후를 보인다. 마지막으로 한국 가상현실 게임소설에서는 마법원이 흐릿하게 나타나면서 현실 세계와 가상세계가 연계되어 나타난다. 이 과정에서 가상세계가 현실의 문제를 해결할 수 있는 대체적 공간으로 기능한다. 이러한 서사적 특징은 북미 · 유럽 등과 차별적인 한국 게임소설만의 특이점이다. This study aims to analyze remediation of games and fictions in Korean virtual reality game fictions and to derive the storytelling of virtual reality game fictions through them. The purpose of this study is to explore the possibilities that Korean game fiction based on the web can be expanded as popular literature. The main character of the fiction is a male in his twenties. In the neoliberal society, he is set as a member of the neglected class and lacking privilege. He enters the virtual world because of the deficiency experienced in the real world. It minimizes the mediation process between the real world and the interface. On the other hand, the repetitive actions performed by the protagonist belong to the category of self – destructive labor. As a result, he shows signs of narcissism. In the Korean virtual reality game fiction, the magic circle appears blurred, and the real world and the virtual world are linked. In this process, the virtual world functions as a substitute space to solve the problem of reality. The establishment of such a space, and the resolution of conflicts are merely narrative characteristics of Korean game fictions that are different from North America and Europe.

      • 강수량 정보를 이용한 대기오염도 예측 시각화 도구

        대기 중에 비는 기체상태 혹은 입자상태로 대기오염물질을 습성침착 하는 대기정화작용을 한다. 환경과학분야에서는 이러한 자연의 대기정화 프로세스를 분석하여 강우량에 따라 대기정화 정도를 예측하기 위한 모델들을 제시하고 있고, 이러한 모델들을 이용하여 대기오염을 예보하는 응용이 확산되고 있다. 본 논문은 강수량에 따른 대기오염물질(NO2, PM10) 정화효과 모델을 기반으로, 강우량 정보를 이용하여 대기오염도 예측결과를 시각화하는 도구를 제시한다. 본 연구의 결과는 직관적인 대기오염피해를 예방하는 도구로서 활용 가능하다.

        Rheumatoid arthritis is associated with an increased risk of hematological malignancy as a result of the RA itself or its treatment. We report here on an unusual case of a 55-year-old female with long-standing rheumatoid arthritis and who was treated with low dose methotrexate and hydrochloroquine. She was diagnosed with chronic myelogenous leukemia that manifested with severe thrombocytosis and basophilia, and this was treated with imatinib mesylate. After 6 months, she achieved a complete cytogenetic response of the CML and a complete resolution of all the RA symptoms without DMARDs.

        Clinical and anatomical approach using Sihler's staining technique (whole mount nerve stain)

        Sihler’s staining allows visualization of the nerve distribution within soft tissues without extensive dissection and does not require slide preparation, unlike traditional approaches. This technique can be applied to the mucosa, muscle, and organs that contain myelinated nerve fibers. In particular, Sihler’s technique may be considered the best tool for observing nerve distribution within skeletal muscles. The intramuscular distribution pattern of nerves is difficult to observe through manual manipulation due to the gradual tapering of nerves toward the terminal end of muscles, so it should be accompanied by histological studies to establish the finer branches therein. This method provides useful information not only for anatomists but also for physiologists and clinicians. Advanced knowledge of the nerve distribution patterns will be useful for developing guidelines for clinicians who perform operations such as muscle resection, tendon transplantation, and botulinum toxin injection. Furthermore, it is a useful technique to develop neurosurgical techniques and perform electrophysiological experiments. In this review, Sihler’s staining technique is described in detail, covering its history, staining protocol, advantages, disadvantages, and possible applications. The application of this technique for determining the arterial distribution pattern is also described additionally in this study.

      • Topography of the Submental Artery That Should Be Considered in Bleeding During Dentoalveolar Surgery

        ABSTRACT: The purpose of the present study was to provide precise data regarding the branching pattern of the submental artery, which should be considered in occasions of bleeding during various dentoalveolar surgical procedures of the mandible, such as implant surgeries, tori removal, and iatrogenic injuries.Twenty-six embalmed adult hemifaces from Korean cadavers were used in this study. The vertical distance, horizontal distance, and diameter of the submental artery were measured from the site of the first premolar to the third molar. In cases where there was penetration of the mylohyoid muscle by the main branches of the submental artery, the same items were measured at that point.The vertical distance between the submental artery and the inferior border of the mandible decreased toward the premolar, whereas the horizontal distance from the lingual plate of the mandible increased gradually as it traveled in the anterior direction. The diameter of the artery narrowed slightly toward the premolar. The main branches of the submental artery perforated the mylohyoid muscle in 14 (54%) of the 26 specimens.As a result of this study, the submental artery is located higher from the inferior border and closer to the lingual plate of the mandible in the region of the molar than that of the premolar. Therefore, clinicians should be more careful of bleeding when performing surgery in the molar region compared with the premolar region. Where the mylohyoid muscle is perforated by the main branches of the submental artery, its point of insertion can be observed in diverse locations.

      • Intramuscular Nerve Distribution of the Masseter Muscle as a Basis for Botulinum Toxin Injection

        The aim of this study was to determine intramuscular distribution pattern of the masseteric nerve, thus providing information regarding the most efficient and safe site for botulinum toxin (BTX) injection for conventional BTX intervention therapy in the treatment of masseteric hypertrophy.Twelve masseter muscles were dissected, and the pattern of innervation of the masseteric nerve was observed in the superficial, middle, and deep layers. We also analyzed 10 muscles that had been stained using Sihler's staining technique.The nerve branches from the posterosuperior and posteroinferior groups innervating the deep and middle layers of the masseter muscle, respectively. Among the nerve twigs originating from the anteroinferior nerve group, 2 or 3 perforated the superficial layer of the muscle. Observation of stained specimens revealed that all perforating branches innervating the superficial layer were mainly confined to and distributed within areas V or VI.Between 2 and 4 perforating branches supply the superficial layer of the masseter muscle. In addition, the richest arborization of the perforating masseteric nerve branches is confined mostly to area V, approximately in accordance with the BTX injection point that is currently used clinically. Area V is thus strongly recommended as the most efficient and safe BTX injection area for the treatment of masseteric hypertrophy.

      • Alveolar Regions of the Mandible for the Installation of Immediate-Implant Fixtures and Bone Screws of Alveolar Distractors

        The purposes of this study were to elucidate the anatomic relationship between the dental roots and surrounding tissues and to identify the optimal sites at which to install dental surgical devices including immediate-implant fixtures and alveolar distractors. We made 5 types of measurements on horizontal cross sections obtained at 1-mm intervals from 20 Korean mandibles. The following results were obtained: (1) the mandibular facial plate was thinnest at the canine (0.5-0.7 mm) and became thicker toward the molar region; (2) the thicknesses of the facial and lingual cortical bone in the interdental region increased from anterior to posterior and from coronal to apical aspects; (3) in each section, the buccolingual root was narrower than 4 mm at depths greater than 8 and 9 mm in the central and lateral incisors, respectively, and the maximum mesiodistal root widths were 3.0 and 3.3 mm; (4) the interroot distance increased from anterior to posterior and from coronal to apical aspects; and (5) on the sections of the first and second molars, the diameter of the septal bone ranged from 4.2 to 7.9 mm buccolingually and from 1.3 to 3.3 mm mesiodistally. Achieving successful placements of implant fixtures and bone screws requires an accurate understanding of the anatomic structure at the installation site. The reported anatomic data might facilitate successful treatments and provide crucial information for use when planning and performing placements of dental surgical devices.

        분산모듈형 태양광 전력조절기의 독립운전 제어

        This paper proposes a control method under stand-alone operating mode for distributed-module type photovoltaic power conditioners. In conventional schemes, there are some problems of a controller saturation in the DC-link (or load) voltage controller when overly-heavy load, light load, and the generated PV power reduction occurs, as well as when a transition occurs from an overly-loaded to normal conditions. To overcome these problems, the proposed controller method switches the main control target from DC-link voltage to the maximum power point, which is closer to the stable operating point when it returns to normal operating conditions. For the analysis, a state-plane trajectory was given and the circuit analysis by PSIM simulation was done. For the verification, a prototype hardware with 110[W] and 50[W] dual photovoltaic modules has been implemented. From the results, it can be seen that PV power tracking is successfully done with the proposed method even under a stand-alone operation mode.
