National Institute of Mental Health Diagnostic Interview Schedule for Children (NIMH-DISC) was translated into Korean version through several times of back-translation and expert-review. We studied the validity of NIMH-DISC Korean Version by examining agreements between DISC diagnoses and clinical diagnoses. 52 subjects and their parents were concurrently interviewed by lay-interviewers using the DISC, who had been trained for 10 weeks. Of the all subjects, 27 were outpatients or inpatients at child psychiatric clinic of Yonsei Hospital and 25 were the psychiatric outpatients at School Health Center. The results were as follow; 1) It was necessary to several times of back-translation and expert-review in developing the Korean Version of DISC. 2) Kappa values of agreement between DISC diagnoses and clinical diagnoses were low except schizophrenic disorder, so DISC does not seem to be useful to diagnose the child psychiatric disorders.

Background: We compared the incidence and degree of post-operative nausea and vomiting (PONV) in patients who received general anesthesia with propofol or sevoflurane using the Rhodes index of nausea, vomiting, and retching (RINVR) to assess the degree of PONV quantitatively and objectively during the post-anesthetic period. Methods: We performed a prospective study involving 38 patients who underwent gynecologic laparoscopic surgery in our hospital between September 2008 and August 2009. Nineteen patients were anesthetized with propofol during the entire anesthetic period and the other 19 patients received 2.0 mg/kg of propofol intravenously, followed by sevoflurane inhalation. Three patients who were anesthetized with sevoflurane were excluded from the analysisbecause they were omitted during the survey. We studied the patients who had PONV and RINVR scores 1, 6, and 24 hours post-operatively. Results: The propofol group had a statistically lower incidence of PONV and lower RINVR scores in the following subclasses within 1 hour of surgery: symptom occurrence; symptom distress; and symptom experience. Conclusions: Propofol at induction and during maintenance of anesthesia can be used to prevent PONV within 1 hour post-operatively in patients undergoing gynecologic laparoscopic surgery. Background: Combination of propofol and remifentanil is an ideal regimen for total intravenous anesthesia. The purpose of this study is to determine the effect-site concentration of remifentanil for prevention of hemodynamic responses to tracheal intubation during fixed propofol infusion (4μg/ml) and to find any sexual differences. Methods: Thirty ASA physical status I-II patients undergoing general anesthesia were assigned to male (n = 15), and female (n = 15) group. All patients received a target controlled infusion (TCI) of propofol with a fixed effect-site concentration of 4μg/ml. After target effect-site concentration of propofol and remifentanil was reached, tracheal intubation was performed. The hemodynamic changes (systolic/diastolic blood pressure, mean arterial pressure, and heart rate) were measured at 1 and 2 min before tracheal intubation (baseline), immediately after, 1, 2, 3, 4 and 5 min following tracheal intubation. In both groups, effect-site concentration of remifentanil was initiated with 3 ng/ml. Subsequent concentration of remifentanil was determined by hemodynamic responses of the previous patient to tracheal intubation based on up and down sequential allocation. Results: The mean EC50 of remifentanil for prevention of hemodynamic responses to tracheal intubation were 1.37 ng/ml (95% CI, 0.95-1.81 ng/ml) in male group and 1.05 ng/ml (95% CI, 0.68-1.40 ng/ml) in female group, respectively. In addition, there were no statistical significant differences between two groups. Conclusions: Relatively small dosages of remifentanil (0.68-1.81 ng/ml) for attenuation of hemodynamic responses to tracheal intubation was needed in Korean population in propofol TCI and there were no sexual differences. Background: The effect of a single nucleotide polymorphism (SNP) of the μ-opioid receptor gene at nucleotide position 118 (OPRM1:118A > G) and the MDR1 gene (exon 26: C3435T) have an influence on the interindividual variability of clinical opioid pain therapy. This study aims to evaluate the correlation among pain control and side effects of epidural morphine and these pharmacogenetic modulators. Methods: 194 patients who were undergoing abdominal surgery were included in the study. Patients received a morphine 2 mg bolus and 2 mg/day via epidural route. The VAS score and opioid side effects were checked at postoperative 6, 24 and 48 hr. Patients were genotyped for the known SNPs of the OPRM1 and MDR1. Results: For the SNP of OPRM1, the mutated genotype frequency (homo-wild, heterozygous, and homo-mutants) were 36.8, 47.9 and 15.3%, respectively, and the mutated genotype frequencies for the MDR1 SNP were 46.7, 40.2 and 13.1%, respectively. There were no significant differences in the VAS scores and side effects among the three groups of OPRM1 and MDR1. Yet carriers of the mutated allele 3435 TT, CT of the MDR1 gene showed marginally greater significant sedation effects than did non-carriers (CC) (P = 0.065, the OR was 1.78, 95% CI 0.98-3.24, P = 0.059) and also a lower incidence of analgesic usage (P =0.058). Conclusions: In our data there was a large difference in OPRM1 SNP allele frequency for the Korean population compared to other populations. The SNP of OPRM1 and MDR1 genes did not have significant altered clinical morphine analgesia and side effects via the epidural route. But the SNP of MDR1 gene is more sensitive genetic predictor of the clinical side effects (especially for sedation) and analgesic effects by opioid. (Korean J Anesthesiol 2007; 52: 16~22)

A cross-sectional survey was conducted with a nationwide sample of 26 judges and 25 family court probation officers(FCPO) to find out their perception on the duties and roles of FCPO in juvenile protection justice. Study results revealed that almost all judges and FCPOs thought that the duties and roles that FCPOs play in juvenile protection justice were helpful in resolving the juvenile cases. Of the duties and roles that FCPOs play, judges especially appreciated the counseling and educational services provided to the juvenile offenders, reports explaining the reasons behind delinquent behaviors and activities linking services from outside agencies to juvenile offenders and their families FCPOs in juvenile protection justice allot most of their working time to conducting social studies as well as finding and linking outside resources to juvenile offenders and their families. They mainly use the method of investigative interviewing with juvenile offenders and their families but also do home visiting and psychological testing. Both judges and FCPOs thought that of all duties and roles, the activities of finding and linking outside resources to juvenile offenders and their families as well as counseling and educational services for juvenile offenders and their families should be increased and reinforced. Judges, however, expressed a statistically higher need than FCPOs for the reinforcement of investigative activities and supervision of agencies that execute juvenile protective actions. Implications for ways to further improve and reinforce the duties and roles of FCPOs in juvenile protection justice were discussed.
증례보고 : 전신마취 유도 중 발생한 Vecuronium에 의한 아나필락시스

Anaphylaxis is one of major causes of morbidity and mortality during anesthesia. Muscle relaxants are the most common cause of anaphylaxis during anesthesia. A 54-year-old woman was scheduled for thyroidectomy. She had no history of allergy and had never previously undergone general anesthesia, Lidocaine, propofol and vecuronium were injected sequentially to induce general anesthesia. Two minutes after the vecuronium injection, severe hypotension, tachycardia and bronchospasm developed, and delayed skin rashes appeared. The patient recovered without any significant complications after immediate proper intensive care. The operation was delayed and a skin test was performed on the 7th day after discharge. She revealed a positive skin test for vecuronium. The anesthesia was re-induced without muscle relaxant and maintained with propofol and remifentanil infusion. Surgery was completed uneventfully, and the patient recovered without any adverse reaction. (Korean J Anesthesiol 2008;55:613~7)
