Slowly Progressing Thrombotic Microangiopathy during Two Years of Treatment with Sunitinib

A 75-year-old man with mild renal impairment was started on sunitinib for a metastatic gastrointestinal stromal tumor. After 7 months of this therapy, proteinuria became aggravated. Serum creatinine concentration was increased from 1.34 to 2.57 mg/dL 24 months after sunitinib administration. Hematologic features of thrombotic microangiopathy (TMA) were absent. Renal histology revealed endothelial swelling and plasmatic insudation of the glomeruli. Proteinuria and renal function improved after discontinuation of sunitinib. Our experience suggests that TMA associated with sunitinib can be diverse in onset and severity, and that the hematologic features of TMA may be absent.

본 연구는 만성질환 환자들의 건강상태나 건강행위에 영향을 미치는 원격진료의 수용행위를 체계적으로 규명하고자 하였다. 이를 위해 2013년 강원도지역에서 원격진료를 받고 있는 210명의 고령층 만성질환자들을 대상으로 설문조사를 실시하고 구조방정식 모델을 통해서 수용성에 미치는 영향요인을 검증하였다. 연구결과 첫째, 원격진료에 대한 지각된 유용성은 지속적인 원격진료 이용의도에 유의미한 영향을 미쳤으며, 지각된 사용용이성 역시 원격진료 이용의도에 유의미한 영향을 미치는 것으로 나타났다. 둘째로 원격진료에 대해 주변사람들의 주관적 인식이 관대하고 긍정적일수록 원격진료에 대한 사용용이성과 유용성에 더 긍정적인 것으로 나타났다. 셋째로 원격진료에 대한 신뢰 역시 정보수용과정에서 정보의 질적 수준이 정보의 활용경험과 유용성에 영향을 미친다는 기존의 연구와 일치하는 결과를 보여주었다. 마지막으로 건강에 대한 외적 통제소재는 지각된 유용성에 긍정적으로 영향을 미치는 것으로 확인되었다. 이 연구는 지역사회 원격진료의 수용을 구조적으로 검증함으로써 모델의 정교화에 기여하고 있다. This study is to propose an acceptance model that explains the use of telemedicine effects for chronic disease. For this purpose, the effect factors and variables for Technology Acceptance Model (TAM) were investigated through a structural equation model by performing a sample survey for 210 senior patients who are under treatment with telemedicine in Kangwon area in 2013. The findings from the results are as follows: 1) perceived usefulness and perceived easy of use influence the intent to use telemedicine; 2) open-minded patients group is more positive to the easy of use and usefulness for the telemedicine; 3) it matches with the previous research that shows the trust in telemedicine system is affected by the experience for practical use of information; 4) it is known that the external control factors for health affect perceived usefulness positively. This study contributes to optimize the TAM by verifying the acceptance of telemedicine system in a rural community.

Background: Both neuroendocrine tumors (NETs) and neuroendocrine carcinomas (NECs) exhibit neuroendocrine differentiation and are classified as neuroendocrine neoplasms (NENs). NECs and nonneuroendocrine neoplasms (non-NENs), such as adenocarcinoma, have similar mutational profiles. The purpose of this study was to identify differences in metastatic patterns and to identify the key factor causing these differences by simultaneously comparing the metastatic patterns of NETs, NECs and non-NENs from various primary organs. Methods: We retrieved data for 4,223 patients with NENs and 41,637 patients with non-NENs arising at various primary sites from an institutional database and then compared NET, NEC, and non-NEN metastatic patterns. Results: NETs and NECs showed generally similar metastatic patterns. Most NEN patients had a higher liver organotrophic metastasis rate, lower lung organotrophic metastasis rate, and lower pleural/peritoneal organotrophic metastasis rate than non-NEN patients. Some differences were characteristics of specific organs. Some of these site-specific differences were not caused by NENs but by non-NENs, including a higher bone organotrophic metastasis rate for medullary thyroid carcinoma and a lower bone organotrophic metastasis rate for pulmonary NEN. Other differences were probably caused by NENs, including a higher bone organotrophic metastasis rate for colorectal NETs. Uterine cervical NEC showed unique patterns of metastasis compared to NEN from other sites. Conclusion: Significant differences between the metastatic patterns of NENs and non-NENs were detected. The multigene program that causes neuroendocrine differentiation might be associated with organotropic metastasis.
