      • Assessing the "Renewable Energy 2040" target: Roadblocks and recommendations for the Philippines

        In line with the goal of achieving a low carbon future, the Philippines made an aspirational target which aims to increase the total installed capacity coming from renewable energy (RE) to at least 20,000 MW by 2040. The country's RE sector has progressively advanced over the years following the enactment of its comprehensive RE Law in 2008. Among other RE technologies, solar achieved the highest installation growth from 2008 to 2018. The paper seeks to assess whether the Philippines' RE target by 2040 is achievable in terms of the current status and future outlook for RE. Major roadblocks that hamper RE development are identified and discussed, including some recommendations for policy-making and energy planning.

        성인,중년,노인 태권도수련의 세계화를 위한 SEKOWNDO연구의 건강-과학적 논의

        본 연구의 목적은 장기간의 태권도 수련이 성인과 중년, 그리고 노년층 수련생들의 균형성,인지적 능력, 그리고 신체 협응에 미치는 영향을 규명하여, 이들의 삶의 질을 향상시키기 위한운동중재 요법의 일환으로 태권도 수련효과의 기초자료를 제공하는 것이다. 40세부터 71세에 사이의 남성 12명과, 여성 12명을 대상으로 일주일에 한 시간씩 일 년 동안 태권도 수련을 실시하였다. 연구 결과, 균형을 조절하는데 있어서 유연한 움직임을 나타내었고, 젊은 성인들과 비슷하게 신체의 방향을 앞쪽으로 하여 균형을 조절하는 전략을 사용한 경향이 관찰되었다. 또한 인지적 능력에서도 유의하게 증가하였으며, 품새 숙련도가 향상됨에 따라 신체 협응 능력도 증가하였다. 본 연구를 통해 태권도 수련이 중년층과 노년층의 수련생들의 생리적인 변인뿐만아니라 인지적, 신체 협응 등과 같은 다양한 변인에서도 효과가 있음을 확인하였다. 이러한 결과를 토대로 체계적인 태권도 수련프로그램을 개발하는데 기초자료로 활용할 것을 제안한다. Purpose of research: Taekwondo is a Korean martial art which addresses a large variety of physical fitness components in its execution, thereby stimulating various health- and skill- related physical fitness components. Potentially, therefore, Taekwondo training qualifies as an ideal and integral method of improving general physical fitness, thereby possibly improving markers of health. We aimed to investigate whether Taekwondo as a movement art would both be “do-able”, safe and fun in the older age category, and whether it improves some physical fitness components. Methods: We performed the SEKWONDO Study, in which 24 volunteers, 12 men and 12 women, varying in age between 40 and 71years, participated in weekly one-hour training sessions for approximately one year. Results: Our results indicate that Taekwondo training in middle-aged beginners is possible, safe and fun, whereas it improves various aspects of subjective physical and mental wellbeing. The Taekwondo curriculum also improved balance, in the sense that Taekwondo trainees had learned to adjust a balance shift more favorably, like younger people do, which is correction forward rather than backward. Several measured cognitive functions also improved. From progress in Poomsae performance we inferred improvement of motor coordination. Conclusions: Because of the potential of substantial health gain on a societal level, we conclude that Taekwondo practice amongst adult, middle-aged and elderly beginners should be stimulated worldwide. The worldwide existing infra-structure as currently endorsed by Kukkiwon may well serve such activity.

        Novel Syntheses of 5-Aminothieno[2,3-c]pyridazine, Pyrimido[4',5':4,5]thieno[2,3-c]pyridazine, Pyridazino[4',3':4,5]thieno-[3,2-d][1,2,3]triazine and Phthalazine Derivatives

        Condensation of 4-cyano-5,6-dimethyl-3-pyridazinone 1 with aromatic aldehydes gave the novel styryl derivatives 2a-c. Refluxing of compound 2a with phosphorus oxychloride furnished 3-chloropyridazine derivative 3. Compound 3 was reacted with thiourea and produced pyridazine-3(2H)thione 4. Thieno[2,3-c]- pyridazines 5a-e were achieved by cycloalkylation of compound 4 with halocompounds in methanol under reflux and in the presence of sodium methoxide. Also, refluxing of compound 4 with N-substituted chloroacetamide in the presence of potassium carbonate afforded thienopyridazines 6a-e. Cyclization of compound 6 with some electrophilic reagents as carbon disulfide and triethyl orthoformate furnished the novel pyrimido[4',5':4,5]thieno[2,3-c]pyridazines 12 and 13a-c, respectively. Diazotisation of compound 6 with sodium nitrite in acetic acid produced the pyridazino[4',3':4,5]thieno[3,2-d][1,2,3]triazines 14a-c. Ternary condensation of compound 1, aromatic aldehydes and malononitrile in ethanol containing piperidine under reflux afforded the novel phthalazines 16a-c. Compound 3 was subjected to some nucleophilic substitution reactions with amines and sodium azide and formed the aminopyridazines 17a, b and tetrazolo[1,5-b]-pyridazine 19, respectively. The structures of the synthesized compounds were established by elemental and spectral analyses.

        Deep sternal wound infections: Evidence for prevention, treatment, and reconstructive surgery

        Median sternotomy is the most popular approach in cardiac surgery. Post-sternotomy wound complications are rare, but the occurrence of a deep sternal wound infection (DSWI) is a catastrophic event associated with higher morbidity and mortality, longer hospital stays, and increased costs. A literature review was performed by searching PubMed from January 1996 to August 2017 according to the guidelines in the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. The following keywords were used in various combinations: DSWI, post-sternotomy complication, and sternal reconstruction. Thirty-nine papers were included in our qualitative analysis, in which each aspect of the DSWI-related care process was analyzed and compared to the actual standard of care. Plastic surgeons are often involved too late in such clinical scenarios, when previous empirical treatments have failed and a definitive reconstruction is needed. The aim of this comprehensive review was to create an up-to-date operative flowchart to prevent and properly treat sternal wound infection complications after median sternotomy.

        NF-κB Activation in T Helper 17 Cell Differentiation

        CD28/T cell receptor ligation activates the NF-κB signaling cascade during CD4 T cell activation. NF-κB activation is required for cytokine gene expression and activated T cell survival and proliferation. Recently, many reports showed that NF-κB activation is also involved in T helper (Th) cell differentiation including Th17 cell differentiation. In this review, we discuss the current literature on NF-κB activation pathway and its effect on Th17 cell differentiation.

        Etiology, Evaluation, and Treatment of Failed Back Surgery Syndrome

        The study aimed to review the etiology of failed back surgery syndrome (FBSS) and to propose a treatment algorithm based on a systematic review of the current literature and individual experience. FBSS is a term that groups the conditions with recurring low back pain after spine surgery with or without a radicular component. Since the information on FBSS incidence is limited, data needs to be retrieved from old studies. It is generally accepted that its incidence ranges between 10% and 40% after lumbar laminectomy with or without fusion. Although the etiology of FBSS is not completely understood, it is possibly multifactorial, and the causative factors may be categorized into preoperative, operative, and postoperative factors. The evaluation of patients with FBSS symptoms should ideally initiate with reviewing the patients’ clinical history (observing “red flags”), followed by a detailed clinical examination and imaging (whole-body X-ray, magnetic resonance imaging, and computed tomography). FBSS is a complex and difficult pathology, and its accurate diagnosis is of utmost importance. Its management should be multidisciplinary, and special attention should be provided to cases of recurrent disc herniation and postoperative spinal imbalance.
