Background: Microneedle fractional radiofrequency (RF) induces deep dermal heating and leaves the epidermis less affected, which showed therapeutic effects for wrinkle, pore and various scars. Sublative fractional RF induces not only superficial fractional ablation of upper epidermis but also upper dermal remodeling, which had been reported to show improvement of skin aging. Objectives: We evaluated efficacy and safety of combined microneedle fractional RF and sublative fractional RF for acne scars in Asian skin. Methods: 20 patients (mean age 23.65±2.94, skin phototype III-IV) received three consecutive combined microneedle fractional RF and sublative fractional RF at 4-week intervals over 12 weeks. Investigator`s global assessment and complications were assessed up to 2 months after last treatment by standardized photographs. Subjective improvement and pain score were also assessed. Results: Follow-up results revealed that 4 patients had clinical improvement of 76% to 100%, 10 had improvements of 51% to 75%, 6 had improvements of 26% to 50%. The mean duration of post-therapy crusting or scaling was 5.21 days and post-therapy erythema lasted 2.52 days. Conclusion: We suggest that combined microneedle fractional RF and sublative fractional RF may provide a new treatment algorithm for acne scars in Asians. The limitations are the small number of patients.

Background: Postherpetic neuralgia (PHN) is the most common complication of herpes zoster (HZ). Recently, there was an open label pilot study which showed that gabapentin administered in acute herpes zoster was effective in reducing the incidence of PHN. However, high-dose gabapentin may cause adverse effects in older patients. Objectives: A prospective study was performed to evaluate the efficacy of treatment with low-dose gabapentin plus valacyclovir for the management of acute pain and the prevention of PHN in patients with acute HZ. Methods: From March, 2012 to July, 2013, 64 patients (age, ≥50 years) who presented with HZ within 3 days of rash onset with moderate to severe pain (≥4 on the 10-point Likert scale) were recruited. The patients received 1000 mg of valacyclovir 3 times a day for 7 days plus gabapentin at an initial dose of 300mg/d, titrated up to a maximum of 900 mg/d. The severity of pain, quality-of-life measures (determined by the Dermatology Life Quality Index, DLQI), and adverse events were assessed at recruitment and 1, 3, 5, 8, and 12 weeks later. Results: Among the total of 64 patients (mean age, 63.4 years), the overall incidence of zoster pain (≥4 on the Likert scale) at 3 months was 3.13%. The DLQI improved significantly at 3 months and patients experienced minimal adverse events from the medication. Conclusion: The combination of low-dose gabapentin and valacyclovir administered acutely in patients with HZ is well-tolerated and reduces the incidence of PHN.

Background: Presently, only few tests have been published for evaluating the efficacy of skin cleansers, and they are neither standardized nor widely-accepted. Objectives: We developed a new in-vivo test method for the semi-quantitative evaluation of the efficacy of cleansing products and evaluated its validity and reliability. Methods: Five different skin cleansers were tested on the forearms of 25 healthy volunteers according to the washing procedure using standard dirt. We evaluated the residual amount of standard dirt on the cyanoacrylate skin surface stripping sample using a dermoscopy and an image analysis program. We also graded the cleansing power through a pattern analysis of dermoscopic images. For the comparison regarding the validity, we obtained data according to other preexisting methods using a Chromameter, Mexameter and Glossymeter. Results: The cleansing efficacy of five skin cleansers which was assessed by our new method showed significant correlations with the results from other preexisting methods using a Chromameter and Mexameter (Pearson correlation coefficient 0.826 and 0.738, respectively, P<0.001). This new method also showed a high internal consistency reliability (Cronbach`s alpha=0.997). Conclusion: We presented a new in-vivo method for assessing the cleansing efficacy of skin cleansers which was shown to be reliable and valid. Further studies are required to establish a standardized method to objectify cleansing effectiveness of skin cleansers.

Purpose: The number of the deceased from free-fall is increasing nowadays. Free-fall comes to a great social problem in that even the survivor will be suffering for cord injury or brain injury, and so on. We analyzed the cases of free-fall patients to find out whether the injury severity is mainly correlated with the height of fall. Methods: We retrospectively investigated the characteristics of patients, who fall from the height above 2m from January 2000 to August 2004. We excluded the patients who transferred to other hospital, transferred from other hospital, and not known the height of fall. 145 patients were evaluated. Variables included in data analysis were age, height of fall, injury severity score (ISS), the being of barrier, and the survival or not. To find out the correlation between height of fall and death, we used receive operating characteristics (ROC) curve analysis. Results: The mean age of patients was 36.5±19.4 years old. 110 were male and 35 were female. Mean height of fall was 11.1±8.5m. 51 patients (35.2%) were died and 30 patients of them (58.9%) got emergency room on dead body. The mean height of fall is 8.9±5.8m for 94 survivors and 15.2±11.0m for the 51 deceased (p<0.001). The area under the ROC curve was 0.646, which means the height of fall was not adequate factor for predicting for death. At 13.5m, as cut?off value, sensitivity is 52.9%, specificity is 86.2%, positive predictive value is 67.5% and negative predictive value is 77.1%. There were statistical differences in mortality rate and ISS between ``below 13.5m group`` and ``above 13.5m group``, but there was not statistical difference in head and neck AIS. Conclusion: The height of fall is not adequate factor for prediction of death. So other factors like intoxication or not, the being of barrier or protection device need to be evaluated for predicting of free-fall patient`s death.

Purpose: The primary goal of wound management is to avoid infection. Wounds in all patients presenting to the Emergency Department are contaminated with bacteria. Despite this, there is a low incidence of infection. Unfortunately, physicians continue to use antimicrobial agents indiscriminately. The authors intended to determine the effect of selective antimicrobial agents and the indications for appropriate antimicrobial agent use in traumatic simple wounds. Methods: This prospective study was performed from Jul. 2005 to Aug. 2005. A pilot study had been performed from Nov. 2003 to Jul. 2004 at the Ewha Woman`s University Mokdong Hospital. Structured data sheets were completed at the times of the patient`s visits to the Emergency Department and to the Outpatient Department for follow-up. Infection was determined at the time of follow-up. The indications of antimicrobial agent use are immunocompromised patients, wounds contaminated for 3 hours or longer, devitalized tissue, and extremity wounds except hand wounds caused by sharp objects. Results: The study enrolled 216 injured patients. The general characteristics of patients and wounds between the two groups were not significantly different. The antimicrobial agent use and infection rate of the pilot study were 227 cases (90.4%) and 10 cases (4.0%), and those of this study were 100 cases (46.3%) and 9 cases (4.1%). In this study, antibiotic use was reduced to almost half compared with the previous study, but the infection rate was similar (p<0.001). Conclusion: Rational use of antimicrobial agents in simple wounds reduced the use of antimicrobial agents in the Emergency Department without increasing the infection rate. 숙성홍어에 3, 5, 7% 초산 및 구연산을 처리한 후 4℃에서 15일 동안 저장하면서 이화학적 특성을 조사하였다. 유기산 처리에 의한 홍어육의 색소 침출 또는 미오글로빈의 산화에 의해 육색의 변화가 발생하였고, 홍어육 단백질의 응고를 초래하여 조직의 경도가 증가하였다. 유기산의 농도가 진할수록 TMA 함량은 감소하였고, 홍어 특유의 향미를 나타내는 암모니아태 질소 함량은 저장 3일 째 가장 낮은 값을 보였다. 이는 숙성홍어에 유기산을 처리함으로써, 어류 특유의 비린내 뿐만 아니라, 홍어 특유의 이취를 감소시킬 수 있을 것으로 판단되었다. 하지만 저장 6일 째 부터 암모니아태 질소 함량은 다시 증가하였는데, 이는 숙성홍어가 저장 중에도 지속적으로 요소를 분해하여 암모니아를 생성시키기 때문이라고 생각된다. 결론적으로, 숙성홍어에 7% 구연산을 처리하는 것이 홍어의 이취를 감소시켜 현대인의 기호에 부합할 것이라고 생각되며, 관능적 특성에 대한 연구가 필요하다고 생각된다. Fermented skate has a unique ammonia-like flavor. The flavor is preferred by a few lovers of skate muscle, while women and young people may be sensitive to the odor. Organic acids were used to reduce the ammonia-like odor in fermented skate and to investigate the physicochemical properties. Fermented skate muscles were sprayed with 20 ㎖ of acetic acid or citric acid (3, 5, and 7%) for 30 seconds and stored at 4℃ for 15 days. The physicochemical properties of organic acid-treated fermented skate were investigated during storage. The control, which was treated with distilled water, showed a higher pH value than the samples treated with organic acids. The L<SUP>*</SUP> value increased with increasing organic acid concentration, while the a<SUP>*</SUP> and b<SUP>*</SUP> values were not significantly different among the samples. The trimethylamine (TMA) decreased with increasing in the organic acid concentration, but it was not significantly different after 9 days of storage. Ammonia-type nitrogen and ammonia-like flavoring, decreased with increasing in the organic acid concentration, whereas ammonia-type nitrogen increased with a storage period more than 6 days. In conclusion, fermented skate treated with 7% citric acid was the best treatment to reduce the ammonia-like odor.
