        Malignant transformation of ovarian mature cystic teratoma into squamous cell carcinoma: a Taiwanese Gynecologic Oncology Group (TGOG) study

        Objective: The malignant transformation (MT) of ovarian mature cystic teratoma (MCT)to squamous cell carcinoma (SCC) is very rare. This study analyzed cases from multiplemedical centers in Taiwan to investigate the clinicopathologic characteristics, treatment, andprognostic factors of this disease and reviewed related literature. Methods: Pathological reports of 16,001 patients with primary ovarian cancer who weretreated at Taiwan medical centers from 1990 to 2011 were reviewed. In total, 52 patients withMT of MCT to SCC were identified. Results: Among all ovarian MCTs, the incidence of MT to SCC is 0.2%. The median age ofpatients was 52 years (range, 29–89 years), and the mean tumor size was 10.5 cm (range, 1–40cm). We analyzed the patients in our study and those in the literature and determined thatearly identification and complete surgical resection of the tumor are essential for long-termsurvival. In addition, adjuvant chemotherapy or concurrent chemoradiotherapy can be usedto treat this malignancy. Old age, large tumor size (≥15.0 cm), and solid components in MCTsare suitable indicators predicting the risk of MT of MCT to SCC. Conclusion: Similar to general epithelial ovarian cancers, the early detection of MT of MCTto SCC is critical to long-term survival. Therefore, older patients with a large tumor or those with a tumor containing a solid component in a clinically diagnosed MCT should beevaluated to exclude potential MT to SCC.

      • Changes in Markers of Liver Function in HCV 1b Patients with Compensated Cirrhosis Treated with Ombitasvir/Paritaprevir/ Ritonavir plus Dasabuvir with Ribavirin

        Aims: Patients chronically infected with HCV are at risk of developing extrahepatic manifestations of HCV as well as progressing to compensated or decompensated cirrhosis and HCC. Although current treatments have high rates of SVR, relatively little is known about possible regression of liver fibrosis after achieving an SVR. The ONYX-II trial examined the efficacy and safety of ombitasvir/paritaprevir/ritonavir plus dasabuvir + ribavirin (RBV) in Asian patients with HCV genotype 1b infection and compensated cirrhosis. Here we report changes in key markers of liver fibrosis and function. Methods: Patients with chronic HCV GT1b infection and compensated cirrhosis were enrolled in China, South Korea and Taiwan and received 12 weeks of OBV/PTV/r (25 mg/150 mg/100 mg once daily) and DSV (250 mg twice daily) with weight-based RBV. The primary objective of ONYX-II was to assess efficacy (SVR12) and safety of the regimen. Changes in markers of liver fibrosis and function between baseline (BL) and post-treatment week (PTW) 12 are presented. Results: Overall, 104 patients were enrolled and treated in ONYX-II. All patients (104/104, 100%) achieved SVR12. BL and PTW12 data for FibroTest score, alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin, albumin, platelet count and alpha fetoprotein (AFP) are shown in Table 1. All selected parameters showed numerical improvements between BL and PTW12. Mean ALT and AST levels returned to within normal range and FibroTest scores demonstrated a numerical improvement, suggesting improvement in liver status. The complete set of data between BL and PTW12 will be presented for these parameters and other liver composite parameters at the conference. Conclusions: Measurement of key liver function markers during the ONYX-II trial showed a numerical improvement within 12 weeks of completion of treatment in HCV GT1b-infected patients with compensated cirrhosis. Further follow-up of these patients will determine the long-term durability of these changes.

        Comparative Study on the Supramolecular Nanocarriers Derived from the Assembly of Gemini and Conventional Surfactants onto Hyperbranched Polyethylenimine

        Conventional surfactant dodecylbenzenesulfonic acid (DBSA) and its corresponding Gemini surfactant acid (GSA) were simply mixed with a solution of hyperbranched polyethylenimine (HPEI) in chloroform. This resulted in the novel supramolecular complexes, HPEI-DBSA and HPEI-GSA, through a neutralization reaction between the amino groups of HPEI and the sulfonic-acid groups of DBSA and GSA. The formed supramolecular complexes and their precursors were characterized by FTIR, 1H NMR, and dynamic light scattering. HPEI-DBSA and HPEI-GSA exhibited inverted-micelle properties that could accommodate the water-soluble guest molecules in an apolar solvent. With the anionic dye methyl orange (MO) as the model guest, both HPEI-DBSA and HPEI-GSA showed superior guest-encapsulation performance than the supramolecular nanocarrier derived from the assembly of aliphatic acid onto HPEI. For example, HPEI-DBSA and HPEI-GSA nanocarriers could encapsulate more MO molecules, and a much smaller amount of the surfactant molecules was required to achieve the maximum MO encapsulation. These results highlight the importance of strong-acid groups of the surfactants in raising the guest-encapsulation efficiency of this type of supramolecular nanocarrier. HPEI-DBSA and HPEI-GSA had different guestencapsulation mechanisms. This indicates that HPEI-GSA can encapsulate basic guests more than HPEI-DBSA,such as MO and fluorescein sodium (FS), but fewer relatively acidic guests, such as Alizarin Yellow R sodium salt and bromophenol blue.

        Demand Response Based on Kalman-Filtering for the Frequency Control

        DR control can be incorporated with traditional frequency control methods to achieve better performance. However, in the frequency measurement process, disturbances may infl uence the precision of measured frequency, and further infl uence the control performance. In this paper, a hierarchical DR control strategy is considered for the frequency control. In order to reduce the infl uence of detection error, a Kalman-fi ltering based method is proposed. At last, the eff ectiveness of the proposed method is verifi ed by the testing results.

        miR-23a Regulates Cardiomyocyte Apoptosis by Targeting Manganese Superoxide Dismutase

        Cardiomyocyte apoptosis is initiated by various cellular insults and accumulated cardiomyocyte apoptosis leads to the pathogenesis of heart failure. Excessive reactive oxygen species (ROS) provoke apoptotic cascades. Manganese superoxide dismutase (MnSOD) is an important antioxidant enzyme that converts cellular ROS into harmless products. In this study, we demonstrate that MnSOD is down-regulated upon hydrogen peroxide treatment or ischemia/reperfusion (I/R) injury. Enhanced expression of MnSOD attenuates cardiomyocyte apoptosis and myocardial infarction induced by I/R injury. Further, we show that miR-23a directly regulates the expression of MnSOD. miR-23a regulates cardiomyocyte apoptosis by suppressing the expression of MnSOD. Our study reveals a novel model regulating cardiomyocyte apoptosis which is composed of miR-23a and MnSOD. Our study provides a new method to tackling apoptosis related cardiac diseases.

      • Impact of Hepatoprotective Medications on the Safety and Efficacy of OBV/PTV/r plus DSV ± Ribavirin in HCV GT1b-infected Asian Patients

        Aims: Hepatoprotective medications (HPMs) are commonly used in patients with chronic liver disease, especially across Asia. The phase 3 ONYX-I and ONYX-II studies evaluated the safety and efficacy of the 3-DAA regimen of ombitasvir and paritaprevir/ritonavir (OBV/PTV/r) plus dasabuvir (DSV) ± ribavirin (RBV) in an exclusively HCV GT1b-infected Asian population. This post-hoc analysis evaluated the impact of HPM use in patients treated with OBV/PTV/r + DSV ± RBV in these studies. Methods: ONYX-I and ONYX-II enrolled patients in China, South Korea and Taiwan. SVR12, treatment-emergent adverse events (AEs), and alanine transaminase (ALT) normalization, as well as mean changes in ALT over time were assessed in patients using vs not using HPMs. HPM use defined as all medications administered during any treatment period. Results: Overall, 11% (36/325) of non-cirrhotic and 57% (59/104) of cirrhotic patients were receiving HPMs, with ursodeoxycholic acid being the most commonly used in both non-cirrhotic (5.2% [17/325]) and cirrhotic (14.4% [15/104]) patients. SVR12 rates were high (99.7- 100%) in both non-cirrhotic and cirrhotic patients irrespective of HPM use. The regimen was generally well tolerated, with low rates of SAEs and AEs leading to treatment discontinuation (Table). Of patients with ALT above normal at baseline (BL), 100% vs 95% of non-cirrhotic and 98% vs 89% of cirrhotic patients using or not using HPMs, respectively, had normal ALT values at end of treatment (EOT). Mean ALT levels during treatment declined rapidly and similarly with and without HPM use; mean changes from BL to EOT were -38.8 and -37.0 U/L, respectively, in non-cirrhotic and -54.2 and -66.6 U/L, respectively, in cirrhotic patients. Conclusions: OBV/PTV/r + DSV ± RBV achieved high SVR12 and was generally well tolerated regardless of HPM use. HPM use had no impact on the safety profile of OBV/PTV/r + DSV therapy in Asian HCV infected subjects.

        Model Parameters Online Identifi cation and SOC Joint Estimation for Lithium-Ion Battery Based on a Composite Algorithm

        The purpose of this paper is to discuss how to solve the problem of on-line identifi cation of model parameters of Li-ion battery and on-line estimation of SOC. Based on the matlab/simulink platform, a fi rst-order RC equivalent circuit model of the battery is built, and a joint estimation algorithm of the model parameters and SOC of the lithium ion battery is designed based on the dynamic model, which is compared with the single adaptive Kalman fi lter algorithm (AEKF). The simulation results show that the proposed joint estimation algorithm can make eff ective online estimation and update of the battery model parameters and SOC. The average estimation error of SOC is less than 2.8%, the estimation accuracy is higher than that of adaptive Kalman fi lter, and its robustness level is relatively high.

      • Superior Hybrid Cathode Material Containing Lithium-Excess Layered Material and Graphene for Lithium-Ion Batteries

        <P>Graphene-wrapped lithium-excess layered hybrid materials (Li<SUB>2</SUB>MnO<SUB>3</SUB>·LiMO<SUB>2</SUB>, M = Mn, Ni, Co, hereafter abbreviated as LMNCO) have been synthesized and investigated as cathode materials for lithium-ion batteries. Cyclic voltammetry measurement shows a significant reduction of the reaction overpotential in benefit of the graphene conducting framework. The electrochemical impedance spectroscopy results reveal that the graphene can greatly reduce the cell resistance, especially the charge transfer resistance. Our investigation demonstrates that the graphene conducting framework can efficiently alleviate the polarization of pristine LMNCO material leading to an outstanding enhancement in cell performance and cycling stability.       • Which is the Best Chinese Herb Injection Based on the FOLFOX Regimen for Gastric Cancer? A Network Meta-analysis of Randomized Controlled Trials

        Background: Few studies have directly compared clinical efficacy and safety among Chinese herb injections (CHIs) for gastric cancer (GC). The present study aimed to compare CHIs combined with FOLFOX regimens for GC to show which provides the best CHIs results. Materials and Methods: 9 electronic databases and 6 gray literature databases were comprehensive searched in April 20, 2013. According to inclusion and exclusion criteria, two reviewers independently selected and assessed the included trials. The risk of bias tool described in the Cochrane Handbook version 5.1.0 and CONSORT statement were used to assess the quality of the trials. All calculations and graphs were performed and produced using ADDIS 1.16.5 software. Results: A total of 541 records were searched and 38 RCTs met the inclusion criteria (2,761 participants), involving 10 CHIs. The results of network meta-analysis showed that compared with FOLFOX alone, combinations with Kanglaite, Astragalus polysaccharides, Cinobufacini, or Yadanziyouru injections could furthest strengthen ORR, improve the quality of life, reduce nausea and vomiting, and reduce the incidence of leukopenia (III-IV). Conclusions: Kanglaite injection, Astragalus polysaccharides injection, Yadanziyouru injection were superior to other CHIs in clinical efficacy and safety for GC. The conclusions now need to be confirmed by large sample size direct head-to-head studies.
