A new approach to modeling the effects of temperature fluctuations on monthly electricity demand

<P><B>Abstract</B></P> <P>We propose a novel approach to measure and analyze the short-run effect of temperature on monthly sectoral electricity demand. This effect is specified as a function of the density of temperatures observed at a high frequency with a functional coefficient, in contrast to conventional methods using a function of monthly heating and cooling degree days. Our approach also allows non-climate variables to influence the short-run demand response to temperature changes. Our methodology is demonstrated using Korean electricity demand data for residential and commercial sectors. In the residential sector, we do not find evidence that the non-climate variables affect the demand response to temperature. In contrast, we show conclusive evidence that the non-climate variables influence the demand response in the commercial sector. In particular, commercial consumers are less responsive to cold temperatures when controlling for the electricity price relative to city gas. They are more responsive to the price when temperatures are cold. The estimated effect of the time trend suggests that seasonality of commercial demand has increased in the winter but decreased in the summer.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Estimate and identify temperature effects in a short-run electricity demand function. </LI> <LI> A new approach using temperature densities to estimate a cross-temperature response. </LI> <LI> Allow non-climate variables to influence the short-run demand response to temperature changes. </LI> <LI> Non-climate variables influence the demand response in the commercial sector of Korean electricity market. </LI> </UL> </P>

Aims: The aim of the study was to investigate the efficacy of tenofovir (TDF) monotherapy in lamivudine (LAM)-resistant chronic hepatitis B (CHB) patients who achieved a complete virological response to LAM plus adefovir (ADV). Methods: This is an investigator initiated prospective, open label, randomized controlled, non-inferiority trial. LAM-resistant CHB patients with undetectable hepatitis B virus (HBV) DNA on LAM plus ADV therapy were 1:1 randomized to LAM plus ADV or switching to TDF monotherapy. Serum biochemistry and HBV DNA were checked every 12 weeks for 96 weeks. The primary endpoint was the proportion of patients with sustained undetectable HBV DNA at week 48. Results: A total of 76 CHB patients including 26 compensated cirrhosis were enrolled in this study. Thirty-eight patients were randomized to TDF and 37 patients to LAM plus ADV arm. Baseline characteristics showed no significant differences between two groups. HBeAg was positive in three (7.9%) patients among LAM/ADV group and 7 (18.4%) patients among TDF group. Four patients (2 in LAM/ADV group and 2 in TDF group) dropped out of the study before the 48-week follow-up. HBsAg loss was achieved in two patients (one in LAM/ADV group, one in TDF group). Two patients achieved HBsAg seroconversion in LAM/ADV group (2/38, 5.3%). HBeAg loss rate at week 48 was 33.3% (1/3) in LAM/ADV group and 42.9 % (3/7) in TDF group. The proportion of patients with sustained complete virological response at week 48 was not significantly different between two groups by per protocol analysis (100% vs 100%) as well as intention-to-treat analysis (94.7% in LAM/ADV group vs 97.2% in TDF group). Among 26 patients who had been followed up for 96 weeks, sustained virological response was maintained in both groups. Conclusions: Switching to TDF monotherapy is effective and safe in LAM-resistant CHB patients with undetectable HBV DNA to LAM plus ADV combination therapy.

Aims: Transcatheter arterial chemoembolization (TACE) for hepatocellular carcinoma (HCC) in patients with aortic dissection is a potentially risky and demanding technique. Access to the true aortic lumen might not be allowed by the arterial entry route chosen. Methods: Here we report a case of repeated sessions of TACE through true and false lumens in a HCC patient with chronic aortic dissection. Results: A 56-year old male presented with infiltrative HCC with right portal vein tumor thrombosis. Abdominal computer tomography scan also revealed an incidental asymptomatic aortic dissection involving descending aorta from proximal segment to superior mesenteric artery (SMA) origin level. Celiac trunk was supplied by false lumen, and SMA was supplied by true lumen. On SMA angiography, right hepatic artery was originated from SMA. HCC was supplied by segment 7 and 8 branch of right hepatic artery. Infusion of adriamycin and lipiodol mixture and subsequent embolization was performed to superior branches of right hepatic artery. Catheterization to celiac axis was failed due to its origin from pseudolumen caused by aortic dissection. Complete response was achieved after sequential radiation therapy. After 1 year, nodular HCC recurred at segment 4. On this session of repeated TACE, catheterization to false lumen was successful. Left hepatic angiography showed tumor stain supplied by segment 4 branch, and embolization was performed. One year later, viable portion was noted in infiltrative HCC at segment 7. Repeated TACE was performed to embolize segment 7 branch supplying infiltrative tumor. Conclusions: In this case, we presented our experience with selective hepatic angiography and SMA angiography in a HCC patient with descending aortic dissection. To our knowledge, this is the first description of hepatic angiography using true and false lumen in a HCC patient with a concomitant aortic dissection.

Transcatheter arterial chemoembolization (TACE) is a useful palliative therapeutic modality for hepatocellular carcinoma (HCC). Postembolization syndromes, such as fever, abdominal pain, and elevated liver enzyme levels are commonly known complications of TACE. One post-TACE pulmonary complication, lipiodol pneumonitis, is rarely reported. Lipiodol pneumonitis after TACE appears to be associated with chemical injury due to accidental perfusion of lipiodol to the lung vasculature, promoted by arteriovenous shunts within the hypervascular HCC. Here, we report a 42-year-old man with unresectable HCC and hepatic vein thrombosis. The patient was initially treated with TACE. The following day after TACE, acute respiratory symptoms such as dyspnea and cough developed with decreased oxygen saturation. Chest X-ray and computed tomography showed multiple patches and diffuse ground-glass opacities in both lung fields, suggesting of lipiodol pneumonitis. The patient’s condition and radiologic abnormalities subsequently improved after 2 weeks of conservative treatment alone. (J Liver Cancer 2020;20:60-66)
A Case of Acute Hemoperitoneum Due to Spontaneous Rupture of the Umbilical Vein in Cirrhosis

Aims: In patients with portal hypertension, acute hemoperitoneum due to spontaneous rupture of umbilical vein was rarely reported. We report a case of spontaneous rupture of umbilical in a cirrhotic patient without extrinsic trauma history. Methods: A 50-year-old male without previous trauma was transported to the emergency department after experiencing sudden crampy whole abdominal pain. Results: He has had a regular check up for liver cirrhosis. The abdomen was distended with generalized tenderness but without peritoneal signs. Laboratory data showed a hemoglobin of 6.8 g/dl; the white blood cell count was 13260/mm3 and the platelets were 128000/mm3. Liver function tests were normal, except for aspartate aminotransferase 70 U/L, a prothrombin time of 18.5 s, with a control of 11.2, and an albumin of 2.2 mg/dl. Abdominal CT scan showed a umbilical vein with a massive contrast leakage to the peritoneum and the presence of a large amount of hemoperitoneum. An emergent angiographic embolization procedure was performed to stop bleeding from paraumbilical vein. The patient recovered well after angiographic embolization procedure and was discharged home on the sixth day after the procedure. Conclusions: In a cirrhosis patient who has rapid onset of abdominal distention and pain, as well as hypotension and tachycardia, intraperitoneal hemorrhage must be considered. Computed tomography scan typically represent a dilated umbilical vein and periumbilical contrast leakage. Early recognition and their intervention with embolization of the ruptured vessel are critical for the successful management of these patients.

본 논문에서는 전류 흐름을 확장하는 방법을 통해 PCS/IMT-2000/WLAN 단말기용 광대역 평면형 모노폴 안테나를 설계하였다. 일반적인 평면형 모노폴 안테나의 단점인 협대역 특성을 보완하기 위하여 이중 공진을 갖는 안테나에 다이아몬드 형태의 패치를 추가, 전류의 흐름을 다양하게 만들어 광대역 특성을 얻었다. 또한, 다양한 설계 파라미터들을 이용하여 주파수 특성을 최적화 하였다. 제작된 안테나는 -10 ㏈ 이하에서 1.66~3.04 ㎓(58.72 %)로 PCS/IMT-2000/WLAN 대역을 모두 만족하는 광대역 특성을 얻었다. In this paper we proposed a novel broadband printed monopole antenna for PCS/IMT-2000/WLAN terminals by widening current flow. To compensate narrow bandwidth characteristics which is one of disadvantages of general printed monopole antenna, we add the patch of diamond shape and induce the current in various directions for acquiring broadband characteristics. And frequency characteristics is optimized with various design parameters. The bandwidth of the realized antenna is 1.66~3.04 ㎓(58.72 %) below the return loss of -10 ㏈ which contain the required bandwidth of PCS/IMT-2000/WLAN band.

Although surgical resection is a curative treatment option for solitary hepatocellular carcinoma, high recurrence rate contributes to dismal long-term prognosis after curative resection. Early recurrence within 2 years after surgery is associated with intrahepatic metastasis of primary tumor. Liver regeneration after hepatic resection can accelerate tumorigenesis in remnant liver. Treatment strategies for intrahepatic recurrence after curative resection include salvage transplantation, repeated resection, local ablation, and transarterial chemoembolization (TACE). Here, we report a 51-year-old male who was presented with a single large tumor located at segment 4. The patient was initially treated with surgical resection, but intrahepatic recurrence occurred only 4 months after surgery. He achieved complete remission with repeated TACE and has survived without recurrence for 4 years so far. (J Liver Cancer 2018;18:162-167)
