서울시내 고등학교 위탁급식의 급식비와 투자비의 실태 및 위탁업체의 기대수준 비교 분석

The purposes of this study were to a) find out the operational characteristics of the contract-managed highschool foodservice in Seoul, b) investigate the expected level of meal-price and facilities investment cost perceived by contract-managed highschool foodservice managers c) compare the present level and expected level of meal-price and facilities investment cost. From October 12 to November 13 in 2001, the questionnaires were mailed to 249 high schools which was managed by contract foodservice company with respondent rate 40.2%. Data were analyzed using SPSS Win(10.0) for descriptive analysis and one group paired t-test. The results of this study were as follows ; 1. The student enrollment of highschools run by contract-managed foodservice was 1,243 with 72.6% participation rate of school lunch program. The average meal-price was 2,138 won. The average annual period of school foodservice operation was 156.78 days per year. The average contracting period was about 3 years. 2. The average cost concerned in the facilities investment amounts 169,578,180 won at the initial investment and 25,204,092 won at the repairs and maintenance cost in the course of operation. 3. The present level of meal-price and facilities investment cost were respectively 2,136won/meal and 171,157,336.72 won. And expected level.of meal-price and amount of facilities investment cost were 2,418.75 won and 121,353,215.19 won. Comparing the present level with expected level of the meal-price and facilities investment cost, expected level of meal-price was significantly higher than the present level of meal-price(p<.001) and expected level of facilities investment cost was significantly lower than present level of facilities investment cost(p<.001).

The Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome is characterized by congenital hypoplasia or aplasia of the uterus and agenesis of the upper part of the vagina. Here, we report the case of severe abdominal pain that was diagnosed as torsion of the right rudimentary uterine horn with multiple leiomyomas in a patient with MRKH syndrome. The twisted rudimentary uterine horn and multiple leiomyomas were successfully removed via laparoscopy. MRKH syndrome with uterine horn torsion is uncommon and ours is the first case of torsion in MRKH syndrome treated via a laparoscopic approach. Our report indicates that laparoscopy is ideal for the simultaneous diagnosis and treatment of pelvic massin MRKH syndrome.

<P>BACKGROUND: Epstein-Barr virus-associated gastric carcinoma (EBV-GC) has been characterized as a special gastric cancer subset. Lifestyle and other major factors that may contribute to EBV-GC and non-EBV-GC were investigated here. MATERIALS AND METHODS: A total of 247 patients with gastric cancer were interviewed, clinicopathological information was retrieved, and in situ hybridization was performed for EBV-encoded small RNAs. RESULTS: There were 18 EBV-GC (male:female=17:1) and 229 non-EBV-GC patients (male:female=161:68). A history of previous gastric ulcer was associated with EBV-GC, whereas frequent and heavy alcohol drinking was related to non-EBV-GC. Additionally, skipping breakfast was correlated with EBV-GC in male patients. Other factors, such as body mass index, history of gastritis, Helicobacter pylori infection, ABO blood type, family history of gastric cancer, education level, marital status, occupation, family status, and dietary factors, showed no significant differences between EBV-GC and non-EBV-GC. CONCLUSION: A history of gastric ulcer, reflecting chemical injury to the stomach mucosa, appears to contribute to development of EBV-GC. Alcohol drinking was more related to non-EBV-GC than EBV-GC.</P>

<P><B>Abstract</B></P><P>The measurement of the hepatic venous pressure gradient (HVPG) for the estimation of portal hypertension (PH) in cirrhosis has some limitations, including its invasiveness. Hepatic vein arrival time (HVAT), as assessed by microbubble contrast‐enhanced ultrasonography (CEUS), is negatively correlated with the histological grade of liver fibrosis because of the associated hemodynamic abnormalities. Anatomical and pathophysiological changes in liver microcirculation are the initial events leading to PH. However, the direct relationship between HVAT and PH has not been evaluated. The present study measured both HVPG and HVAT in 71 consecutive patients with compensated cirrhosis and analyzed the relationship between the two parameters (i.e., the derivation set). Results were validated in 35 compensated patients with cirrhosis at another medical center (i.e., the validation set). The derivation set had HVPG and HVAT values of 11.4 ± 5.0 mmHg (mean ± standard deviation; range, 2‐23) and 14.1 ± 3.4 seconds (range, 8.4‐24.2), respectively; there was a statistically significant negative correlation between HVPG and HVAT (r<SUP>2</SUP> = 0.545; <I>P</I> < 0.001). The area under the receiver operating characteristic curve (AUROC) was 0.973 for clinically significant PH (CSPH; HVPG, ≥10 mmHg), and the sensitivity, specificity, positive predictive value, negative predictive value, and positive and negative likelihood ratios for CSPH for an HVAT cut‐off value of 14 seconds were 92.7%, 86.7%, 90.5%, 89.7%, 6.95, and 0.08, respectively. In addition, a shorter HVAT was associated with worse Child‐Pugh score (<I>P</I> < 0.001) and esophageal varices (<I>P</I> = 0.018). In the validation set, there was also a significant negative correlation between HVAT and HVPG (r<SUP>2</SUP> = 0.538; <I>P</I> < 0.001), and AUROC = 0.953 for CSPH. HVAT was significantly correlated with PH. These results indicate that measuring HVAT is useful for the noninvasive prediction of CSPH in patients with compensated cirrhosis. The objective of this study was to develop a rapid, simple, and qualitative acetylcholinesterase (AChE)-detection kit, based on a modification of the Ellmanand ELISA methods, for the detection of organophosphorus (OP) and carbamate (CB) pesticide. The developed kits were used to screen a large number of agricultural samples (spiked and real) for OP and CB pesticide residues. AChE was extracted from the heads of honeybees (Apis mellifera L.) using Triton X-100, and was purified through 3 steps: diethylaminoethyl-cellulose chromatography (DEAE), affinity chromatography and membrane filtering, and Mono-Q column chromatography. Epoxy-activated Sepharose 6B affinity chromatography was used for large-scale purification. The presence of OP and CB pesticide residues in agricultural samples was assayed on the basis of AchE inhibition value. The presence (6 bands) or absence of some colored bands on the test line indicated a negative or positive result, respectively. The limits of detection for measured organophosphorus (OP) and carbamates (CB) pesticide residues in standard pesticide solutions and fortified samples were ranged from 0.50 to 2.50 ppm and 0.50 to 4.75 ppm, respectively.
