Chloride current has been known as a part of backgroud current. Recently, its roles in the maintenance of action potential have much attention. The purposes of the present study are clarifying the chloride current component which might play an important role in generation and maintenance of the action potential in the pyloric circular smooth musele cells. We used the whold cell voltage clamp techniques to assess the current and voltage responses induced by caffeine superfusion. The results were as follow: 1. Caffeine superfusion induced the increase of inward currents in the hyperpolarized potential ranges, and the increase of outward currents in the depolarized ranges. 2. Sodium ion removal from the external medium and the addition of cadmium ion rarely affected these current response. 3. Duration of the action potential was elongated by application of the caffeine and modified by the change of the intracellular chloride ion concentraion. 4. The effect of caffeine superfusion on the action potential disappeared by the addition of the anion channel blocker. The results suggest that caffeine superfusion may prolong the duration of repolarized phase of the action potential probably by the chloride current activation in the gastric antral smooth muscle.

In this study we consider a flow line CONWIP system in which two types of product are produced. The processing times of each product type at each station follow an independent exponential distribution and the demands for the finished products of each type arrive according to a Poisson process. The demands that are not satisfied instantaneously are either backordered or lost according to the number of unsatisfied demands that exist at their arrival instants. For this system we develop an approximation method to obtain the performance measures such as steady state probabilities of the number of parts of each product type at each station, mean waiting times of backordered demands and the proportion of backordered demands. For the analysis of the proposed CONWIP system, we model the CONWIP system as a two class closed queueing network with a synchronization station and analyze the closed queueing network using a product-form approximation method for multiple classes developed by Baynat and Dallery. In the approximation method, each subsystem is analyzed using a matrix geometric method. Comparisons with simulation show that the approximation method provides fairly good results for all performance measures.

Infliximab is a chimeric mouse/human monoclonal antibody against tumor necrosis factor-alpha (TNF-α) that is used worldwide to treat rheumatic disease. However, the reactivation of tuberculosis, including extrapulmonary menifestation, is a severe side effect of anti-TNF-α treatment. Mycobacterium tubercluosis causes two forms of joint involvement, TB arthritis and Poncet`s disease. Poncet`s disease is a rare aseptic form of arthritis, known as reactive arthritis in tuberculosis. We encountered a case of tuberculous peritonitis with Poncet`s disease in a 38-yearold man with a history of ankylosing spondylitis that was treated with infliximab. We report this case with a review of the literature.
배급수계통에서 잔류염소 및 THMs 분포 예측에 관한 연구

본 연구에서는 배급수계통에서 잔류염소와 THMs의 분포를 예측하는 방안을 제시하였다. 잔류염소와 THMs의 등의 수질변화에 대해 현장조사를 실시하였고, 수체와 관벽에 대한 잔류염소 감소계수와 THMs의 생성계수를 산출하여 관망해석에 의한 수질모델링에 적용하였다. 병실험을 통해 잔류염소 병렬 1차 수체 반응계수를 구하고, 5개 관벽 감소 모델을 비교 평가하여 1차 관벽 감소계수 산출하고 적용했을 때 잔류염소 현장 측정값과 관망 수질모델링에 의한 예측값의 평균절대오차 및 평균제곱근오차가 각각 0.03, 0.037 mg/L로 가장 작았다. 또한 병실험에 의한 THMs생성량을 비선형 회귀분석으로 1차 생성계수를 구하고 현장 측정값과 비교하였다. 그 결과, 9월의 현장 측정값과 예측값의 결정계수 R2는 0.98, 11월에는 0.82로 예측이 가능하였다. This study suggested a method for prediction of residual chlorine and THMs in water distribution system by measurement of residual chlorine, THMs, and other parameters, estimation of chlorine decay coefficients and THM formation coefficients, and simulation of water qualities using pipe network analysis. Bulk decay coefficients of parallel first-order were obtained by bottle tests, and pipe wall decay coefficients of first-order were estimated through evaluation of 5 models, which showed the lowest values of 0.03 for MAE(mean absolute error) and 0.037 MAE in comparison with the observed in field. And bottle tests were conducted to model first-order reaction of THM formation by nonlinear least square regression and the resultant coefficients were compared with the observed in field. As a result, the coefficients of determination(R2) for the observed and the predicted values were 0.98 in September and 0.82 in November, and the formation of THMs was predicted by modeling.

Background/Aims: Although acid suppressants are widely used for the prevention or treatment of drug-induced upper gastrointestinal bleeding (GIB), evidence regarding the prevention of anticoagulant-related GIB is scarce. The aim of this study was to evaluate the protective effect of acid suppressants against anticoagulant-related GIB. Methods: A systematic review was conducted of studies that evaluated the protective effect of acid suppressants against anticoagulant-related GIB found in PubMed, the Cochrane library, Embase, and KoreaMed from the date of database inception to April 2018. Random effect model meta-analyses with sensitivity analyses were conducted. The methodological quality of each included publication was evaluated using the Risk of Bias Assessment Tool for Nonrandomized Studies. Publication bias was assessed. Results: In total, six nested case-control or cohort studies were identified and analyzed. Proton-pump inhibitors (PPI) had a protective effect against upper GIB in patients on dicumarinics (risk ratio [RR], 0.56; 95% confidence interval [CI], 0.38 to 0.83; I2, 0%); however, the histamine-2 receptor antagonist did not have the same effect (RR, 0.97; 95% CI, 0.52 to 1.81; I2, 0%). Acid suppressants did not have a protective effect against GIB in patients on dabigatran (hazard ratio, 0.78; 95% CI, 0.44 to 1.37; I2, 81.8%). Conclusions: The protective effect of PPIs against dicumarinics-related upper GIB was clear, while there was no evidence supporting the protective effect of acid suppressants against dabigatran-related GIB. However, in the absence of randomized trials demonstrating a lack of bias, solid conclusions cannot be drawn.
