Hemostatic action of EGF-endospray on mucosectomy-induced ulcer bleeding animal models

<P>Gastric bleeding is one of the irritant problems in ulcer patients. In this study, we evaluated hemostatic action of ulcer-coating powder (EGF-endospray) on gastric ulcer animal models. EGF-endospray, containing epidermal growth factor, is designed to be applied through an endoscope. Hemostatic action of the EGF-endospray was evaluated on gastric hemorrhage models of rabbits and micro-pigs. The EGF-endospray was directly applied onto a mucosal resection (MR)-induced gastric bleeding focus in a rabbit model. In a porcine model, the EGF-endospray was applied once via an endoscopy to a bleeding lesion created by endoscopic submucosal dissection. The bleeding focus was then observed via an endoscope. In the rabbit model, EGF-endospray treatment significantly shortened mean bleeding time in comparison with other treatments (104.3 vs 548.0 vs 393.2 s for the EGF-endospray, the non-treated control and the epinephrine injection, respectively). In the micro-pig model, EGF-endospray showed immediate hemostatic action and prolonged covering of the bleeding focus for over 72 h. Histology proved mucosal thickness was more efficiently recovered in all EGF-endospray treated animals. The results of the present study suggest that the EGF-endospray is a promising hemostatic agent for GI bleeding.</P>
The Ballooning Time in Endoscopic Papillary Balloon Dilation for the Treatment of Bile Duct Stones

Background/Aims: Endoscopic papillary balloon dilation (EPBD) is a safe and effective method for the treatment of choledocholithiasis, but previous studies have rarely reported the appropriate ballooning time (BT). We prospectively evaluated the safety and efficacy of EPBD according to BT in patients undergoing bile duct stone removal. Methods: Seventy consecutive patients with bile duct stones were randomly assigned to receive EPBD with either conventional (n=35, 60 seconds) or short (n=35, 20 seconds) BT. Results: EPBD alone achieved complete bile duct clearance in 67 patients (long BT, n=33, 94.3%; short BT, n=34, 97.1%; p=0.808). We also found no significant difference in the rate of complete duct clearance, including procedures that used mechanical lithotripsy, between the long and short BT groups (97.1% vs. 100%; p=0.811). Mild pancreatitis was noted in four (11.4%) patients in the long BT group and two (5.7%) patients in the short BT group, but this incidence was not significantly different. Conclusions: The study showed that EPBD using both 20-sec and 60-sec BTs is safe and effective for the treatment of bile duct stones. Short and long BTs produced comparable outcomes. Background/Aims: Fecal microbiota transplantation (FMT) is a highly effective therapy for refractory and recurrent Clostridium difficile infection (CDI). Despite its excellent efficacy and recent widespread use, FMT has not been widely used in South Korea thus far. We describe our experience with FMT to treat refractory/recurrent CDI. Methods: We conducted a chart review of patients who underwent FMT for refractory/recurrent CDI at Inha University Hospital, between March 2014 and June 2016. The demographic information, treatment data, and adverse events were reviewed. FMT was administered via colonoscopy and/or duodenoscopy. All stool donors were rigorously screened to prevent infectious disease transmission. Results: FMT was performed in nine patients with refractory/recurrent CDI. All patients were dramatically cured. Bowel movement was normalized within one week after FMT. There were no procedure-related adverse events, except aspiration pneumonia in one patient. During the follow-up period (mean 11.4 months), recurrence of CDI was observed in one patient at one month after FMT due to antibiotics. Conclusions: FMT is a safe, well-tolerated and highly effective treatment for refractory/recurrent CDI. Although there are many barriers to using FMT, we expect that FMT will be widely used to treat refractory/recurrent CDI in South Korea. (Korean J Gastroenterol 2017;69:226-231)

간의 원발성 샘편평세포암종은 드물게 발생하며, 간의 다른 원발성 종양보다 불량한 예후를 가지고 있다. 저자들은 상행결장암으로 수술 후 항암화학요법 중 우연히 발견된 간의 원발성 샘편평세포암종을 경험하였기에 보고한다. A primary adenosquamous carcinoma of the liver is a rare subtype of a cholangiocarcinoma. Adenosquamous carcinoma is known to have an aggressive biologic behavior and is associated with a poor prognosis. Recently, we experienced a case of a primary adenosquamous carcinoma of the liver. A 69-year-old man was subjected to a right hemicolectomy five months ago due to an ascending colon cancer. At a periodic check-up, abdominal ultrasonography and magnetic resonance imaging showed a ~1.1 cm-sized hepatic mass which was highly suspicious of being a hepatic metastasis from the ascending colon cancer. A hepatic segmentectomy was performed. The tumor was microscopically composed of an adenocarcinoma and a squamous cell carcinoma, and it was different from the previous colon cancer as determined by immunohistochemistry. Three months after the hepatic segmentectomy, multiple hepatic metastasis was found and the patient has been treated as an outpatient with capecitabine. (Korean J Med 72:74-78, 2007)

Background: Eradication of Helicobacter pylori infection with triple therapy (TT) hasbeen reported to achieve unacceptable rates in Korea. Sequential therapy (ST) and concomitant therapy (CT) has been suggested as alternatives to the TT regimen. The aim of this study was to compare the effi cacy of ST and CT with that of TT in Korea. Methods: For this multicentre, randomized trial, patients (=20 years of age) with H. pylori infection from 5 centers in Korea were recruited. Patients were randomly allocated to TT (PPI, amoxicillin and clarithromycin for 10 days), ST (PPI and amoxicillin for the fi rst 5 days, followed by PPI, clarithromycin and metronidazole for the next 5 days) or CT (PPI, amoxicillin, clarithromycin and metronidazole for 10 days). The primary outcome was the eradication rate of each treatment by intention-to-treat (ITT) and per-protocol (PP) analysis. Results: From March, 2013 a total of 151 patients were enrolled in our study. Sixty one patients were allocated to the TT, 49 patients to CT group, and 29 patients to the ST group. For ITT analysis, the eradication rates of TT, ST and CT were 67.2% (41/61), 59.2% (29/49), 68.3% (28/41), respectively. For PP analysis, the eradication rates were 91.1% (41/45), 82.9% (29/35), 93.3% (28/30), respectively. We recorded no signifi cant difference in the occurrence of adverse effects or in compliance between the three groups. Conclusions: TT and CT regimens seem to achieve higher eradication rates than the ST regimen in Korea. Additional study results from more patients are expected.

A new CMOS transresistance amplifier for low-voltage analog integrated circuit design applications is presented. The proposed transresistance amplifier circuit based on common-source and negative feedback topology is compared with other recent reported transresistance amplifier. The proposed transresistance amplifier achieves high transresistance gain, gain-bandwidth with the same input/output impedance and the minimum supply voltage 2VDSAT+VT. Hspice simulation using 1.8V TSMC 0.18㎛ CMOS technology was performed and achieved 59㏈Ω transresistance gain which is above the maximum about 18㏈Ω compared to transresistance gain of the reported circuit.

ST-elevation myocardial infarction (STEMI) caused by an acute aortic dissection is relatively rare. A diagnosis of dissection can be missed and the situation can become complicated. We report a patient who presented with acute aortic dissection responsible for STEMI related to a dissecting flap into the right coronary artery. This case emphasizes the need for careful assessment of the aorta in cases of atypical coronary occlusion in patients with STEMI without evidence of atherosclerosis in non-culprit coronary segments. The patient was discharged 7 days after primary percutaneous intervention for STEMI. However, she revisited the emergency department for recurrent chest pain and aortic dissection and was diagnosed and managed successfully with surgery.. (Korean J Med 2013;85:516-520)

In this paper, a current-mode integrator for low-voltage, lew-power analog integrated circuits is presented. Using the proposed current-mode integrator, the baseband analog filter is designed for WCDMA wireless communication. To verify the proposed current-mode integrator circuit, Hspice simulation using 1.8V TSMC 0.18㎛ CMOS parameter is performed and achieved 44.9㏈ gain, 15.7㎒ unity gain frequency. The described 3rd-order current-mode baseband analog filter is composed of the proposed current-mode integrator, and SFG(Signal Flow Graph) method is used to realize the baseband filter. The simulated results show 2.12㎒ cutoff frequency which is suitable for WCDMA baseband block.
