Outcomes of Vascular Resection and Reconstruction in Extremity Soft Tissue Sarcoma and Bone Tumors

        Anand Chetan Shah,Vishnu Ramanujan,Krishna Muralidharan,Anand Raja 대한혈관외과학회 2021 Vascular Specialist International Vol.37 No.1

Purpose: This study aimed to evaluate and analyze the feasibility and the oncological and functional outcomes of limb salvage surgery in extremity soft tissue sarcomas (ESTS) and bone tumors invading vessels. Materials and Methods: This single-center retrospective analysis included patients with ESTS encasing or invading major blood vessels that were treated by limb salvage surgery with vascular resection and reconstruction between January 1995 and December 2019. Patients with contiguous involvement of major blood vessels and nerves and patients requiring amputation were excluded from the study. Results: A total of 24 vessels (14 arteries and 10 veins) in 14 patients were reconstructed. Ten (71.4%) patients underwent both arterial and venous reconstruction, and four (28.6%) underwent only arterial reconstruction. Reconstruction was performed with a reversed saphenous vein (RSV) graft in 12 patients and with a synthetic graft (SG) in the other 12 patients. At a median follow-up of 27 months, RSV grafts were patent in 10 of 12 (83.3%) vessels and SGs were patent in 6 of 12 (50.0%) vessels (log-rank test, P=0.083). Out of 14 arteries and 10 veins, 11 arteries and 5 veins were patent, respectively. No patient developed local recurrence, and 2 (14.3%) patients developed distant metastases. Limb salvage rate was 13/14 (92.9%). The mean Musculoskeletal Tumor Society score was 83.3%. The 5- and 10-year overall survival rates were 80% and 50%, respectively. Conclusion: Limb salvage surgery in ESTS with vascular resection and reconstruction is feasible and provides favorable oncological and functional outcomes.

Does Sarcopenia Increase the Risk for Fresh Vertebral Fragility Fractures?: A Case-Control Study

        Ashish Anand,Ajoy Prasad Shetty,K. R. Renjith,Sri Vijay Anand K. S.,Rishi Mugesh Kanna,Shanmuganathan Rajasekaran 대한척추외과학회 2020 Asian Spine Journal Vol.14 No.1

Study Design: Case-control study. Purpose: Sarcopenia is an age associated condition characterized by decrease in muscle mass, strength, and physical performance. We aimed to investigate whether sarcopenia increased the risk of vertebral fragility fractures among the elderly. Overview of Literature: Initial reports on sarcopenia suggest its contribution to the development of vertebral fragility fractures. However, recent studies showed contradictory findings. Methods: Fifty-one consecutive patients with vertebral fragility fractures and matched controls without fractures were evaluated for sarcopenia, T-score, body mass index, and presence of preexisting vertebral fractures. Sarcopenia was diagnosed as total psoas cross-sectional area (TPA) 2 standard deviations below normative value from normal young adults and decreased handgrip strength (26 kg for men and 18 kg for women). Univariate and multivariate analyses were performed using the fresh fracture occurrence as the dependent variable. Results: Sarcopenia was confirmed in 29.4% and 7.8% of cases and controls (p=0.005), respectively; 56.8% and 13.7% of cases and controls had previous vertebral fractures. Sarcopenia prevalence was greater among those with previous fractures (38% vs. 7.6%; odds ratio, 7.76; p<0.001). TPA was lower among the cases (1,278 mm2 vs. 1,569 mm2, p=0.001) and those with previous fractures (1,168 mm2 vs. 1,563 mm2, p<0.001). Handgrip strength was greater among those without previous fractures (19.6 kg vs. 16.3 kg, p=0.05). In multivariate analysis, sarcopenia was not identified as a significant predictor of fresh fractures whereas previous fractures and lower T-score were found to be significant. Conclusions: Sarcopenia is not an independent risk factor for fresh vertebral fragility fractures in the elderly.

Comparative Molecular Field Analysis of Pyrrolopyrimidines as LRRK2 Kinase Inhibitors

Anand Balupuri, Pavithra K. Balasubramanian, Seung Joo Cho 조선대학교 기초과학연구원 2016 조선자연과학논문집 Vol.9 No.1

Leucine rich repeat kinase 2 (LRRK2) is a highly promising target for Parkinson’s disease (PD) that affects millions of people worldwide. A three-dimensional quantitative structure-activity relationship (3D-QSAR) analysis was performed on a series of pyrrolopyrimidine-based selective LRRK2 kinase inhibitors. This study was performed to rationalize the structural requirements responsible for the inhibitory activity of these compounds. A reliable 3D-QSAR model was developed using comparative molecular field analysis (CoMFA) technique. The model produced statistically acceptable results with a cross-validated correlation coefficient (q2) of 0.539 and a non-cross- validated correlation coefficient (r2) of 0.871. Robustness of the model was further evaluated by bootstrapping and progressive scrambling analysis. This work could assist in designing more potent LRRK2 inhibitors.

In silico study on indole derivatives as anti HIV-1 agents: a combined docking, molecular dynamics and 3D-QSAR study

        Anand Balupuri,GADHECHANGDEV GORAKSHNATH,Pavithra K. Balasubramanian,Gugan Kothandan,조승주 대한약학회 2014 Archives of Pharmacal Research Vol.37 No.8

The HIV-1 envelope glycoprotein gp120 playsa vital role in the entry of virus into the host cells and is apotential antiviral drug target. Recently, indole derivativeshave been reported to inhibit HIV-1 through binding togp120, and this prevents gp120 and CD4 interaction toinhibit the infectivity of HIV-1. In this work, moleculardocking, molecular dynamics (MD) and three-dimensionalquantitative structure–activity relationship studies werecarried out. Molecular docking studies of the most activeand the least active compounds were performed to identifyimportant residues in the binding pocket. We refined thedocked poses by MD simulations which resulted in conformationalchanges. After equilibration, the structure ofthe ligand and receptor complex was stable. Therefore, wejust took the last snapshot as the representative bindingpose for this study. This pose for the most active inhibitorwas used as a template for receptor-based alignment whichwas subsequently used for comparative molecular fieldanalysis. Resultant 3D contour maps suggested smaller substituents are desirable at the 7-position of indole ring toavoid steric interactions with Ser375, Phe382 and Tyr384residues in the active site. These results can be exploited todevelop potential leads and for structure-based drug designof novel HIV-1 inhibitors.
