        유방암 재발 환자의 장궐 및 현훈이 오수유부자이중탕으로 호전된 치험 1례

        A 63-year-old female patient was admitted to the hospital with complaints of coldness in the head and right femur, persistent vertigo, dry mouth, vexation, general weakness, anorexia, and hot flashes. After being diagnosed with left-sided ductal carcinoma in 2017 and undergoing partial resection, she was recommended radiotherapy, chemotherapy, and hormone therapy, but she refused and did not receive any treatment. At a checkup in November 2022, the left-sided invasive ductal carcinoma recurred. She underwent partial resection and was scheduled to receive radiotherapy two months later. During hospitalization, the patient received traditional Korean medicine treatment, including Korean herbal medicine (Osuyubujaijung-tang), acupuncture, moxibustion, and cupping. Subjective symptom changes were evaluated daily. At discharge, the patient's symptoms improved, and her condition varied with the presence of Aconitum carmichaelii. This study suggests that "Osuyubujaijung-tang" cared the coldness and vertigo caused by "Han-dam (寒痰)" in patients with Soeumin visceral syncope pattern (少陰人 臟厥症) and that A. carmichaelii played a major role in this outcome.

        자가 근막 이완요법(self-myofascial release)이 근피로가 유도된 대퇴직근의 탄성과 경직도에 미치는 영향

        The study was designed to evaluate the effect of self-myofascial release(SMR) technique on elasticity and stiffness of rectus femoris muscle which experienced the exhaustive fatigue induced by isokinetic endurance exercise. Total 25 college students(male: 14, female: 11) conducted two isokinetic endurance exercise(Biodex System 4, USA) with a week interval. In this cross-over study, each subject were asked to perform 6 sets of muscular endurance exercise(60°/sec, 10 times of maximal knee flexion and extension/each set) with 10seconds of resting interval. Each subject was asked to conduct SMR treatment on rectus femoris(experimental group) or take a complete resting(control group) for 5 minutes after 6 sets of isokinetic endurance exercise at each separated day with a week interval. In order to measure eth mechanical properties like muscle stiffness and elasticity, MyotonPro(Myoton AS, Tallinn, Estonia) was applied to muscle belly of rectus femoris muscle at pre-exercise, right after exercise, 5min. and 20min. after exercise. From the Myoton measurement, Five indexes(Frequency, Logarithmic Decrement, Dynamic Stiffness, Creep, Mechanical Stress Relaxation) were obtained. Electromyograph(EMG) on rectus fermoris m. was also measured during first set and 6th set of isokinetic exercise in order to obtained the muscle activity index and compare it with Myoton measurement indexes. The results were as follows: 1. The muscle tension and stiffness were significantly increased while the elasticity was decreased immediately and at 5 minutes after exercise. All mechanical properties of fatigued muscle were recovered to pre-exercise level at 20 minutes after exercise. 2. No significant effect of SMR on mechanical properties indexes of fatigued muscle was found at every time point after endurance exercise. 3. The changes of mechanical properties(tension, stiffness and elasticity) between pre-and post-exercise which were measured by Myoton-measurement had significant corelation with the changed muscle activities estimated by EMG.

        목과 견갑골 부위의 교정운동이 정적자세균형과 근경직도에 미치는 영향

        The purpose of this study is to investigate the effects of neck · scapula corrective exercise on static posture balance and muscle stiffness. Sixty-one university students (22±4.39 year) participated in this study. Corrective exercise included myofascial release and isometric exercise was applied on neck, shoulder and scapular region for 17 min. To evaluate the static posture, forward head angle · length, Scapula flexion angle were measured using a posture analysis device(exbody 550 series, seoul, Korea) at rest and right after corrective exercise. In order to measure the mechanical properties like muscle stiffness and elasticity, MyotonPro(Myoton AS, Tallinn, Estonia) was applied to muscle belly of trapezius · sternocleidomastoid muscle at rest and right after corrective exercise. From the Myoton measurement, Five indexes (Frequency, Logarithmic Decrement, Dynamic Stiffness, Creep, Mechanical Stress Relaxation) were obtained. The results were as follows. Firstly, Forward head angle and length decreased significantly after corrective exercise compared to pretreatment(p<.05~.001). Secondly, Scapula flexion angle during overhead squat posture increased significantly after corrective exercise compared to pre-treatment(p<.05). Thirdly, in the mechanical muscle properties, tension of upper trapezius · stermocleidomastoid muscle increased significantly while the elasticity of upper trapezius muscle deceased significantly after corrective exercise(p<.05~.001). These results suggest that corrective exercise applied on neck and shoulder region might be effective in improving static posture balance and mechanical muscle properties.

        Objectives: This study was performed to evaluate the quality of case reports from the Maternal and Child Health Care & Functional Medicine and Nutrition Association in accordance with CARE (CAse REport) guidelines. Methods: Researchers selected case reports of Maternal and Child Health Care & Functional Medicine and Nutrition Association in the database of Google Scholar. The quality of selected reports was assessed based on CARE guidelines. Results: Total of 12 case reports were included in this study. Reporting rate of each sub-items was 92.59% at the maximum, 62.96% at the minimum, 77.42% at the median. ‘7 Time line’, ‘8b Diagnostic challenges’, ‘10c Intervention adherence and tolerability’, ‘10d Adverse and unanticipated events’, ‘12 Patient perspective or experience’, ‘13 Informed consent’ were missed in more than 50% of the reports. Conclusion: For improvement of the quality of case reports, it is necessary to evaluate case reports from the Maternal and Child Health Care & Functional Medicine and Nutrition Association in adherence to CARE guidelines.

        4650m 상당의 평압·저산소 조건에서 근지구성 운동이 심박변이도에 미치는 영향

        The purpose of this study was to investigate the effect of extreme normobaric hypoxia exposure corresponding to 4650m altitude on heart rate variability during muscular endurance exercise. In a cross-over design, 19 young (21.7±2.4years) healthy males were asked to perform the isokinetic leg endurance exercise twice in hypoxia and control(sea level) condition with a week interval. Subjects assigned to the hypoxia condition group was breathed in hypoxia air corresponding to 4,650m altitude condition for 30minutes through the hypoxic control device(Altrainer, SMTEC Sports & Medical Technologies SA, Nyon, Switzerland). Each Subject conducted iso-kinetic knee flexion and extension exercise at 180°/sec, 10bouts, and 10sets using isokinetic dynamometer(Biodex system 4, Medical System Inc, USA). The parameters of heart rate variability were obtained from both time-and frequency domain analysis of 5min EC G recording at rest, post exercise, and post 10min(3 time) after exercise. Mean RR interval and its standard deviation(SDNN), the mean square successive differences(RMSSD), the low and high frequency components were calculated as normalized units. Arterial oxygen saturation was also measured by pulse oximetry at rest, post exercise, and post 10minute. The results were as follows. First, following the hypoxia exposure a significant decrease in arterial oxygen saturation was found(p<.05). Second, analysis of HRV revealed a significant increase in sympathetic activity and a significant decrease in parasympathetic activity following muscular endurance exercise both group(p<0.1~0.05). Third, no significantly different in parameters of HRV was not found between the hypoxia group and the control group. These findings suggest that the dropped level of arterial oxygen saturation induced by hypoxia exposure may not affect heart rate variability at resting status, during exercise and recovery period. The results also showed that exercise itself rather than status of blood might have more powerful effect on autonomic nerve activities.

        전기식 온침기의 온도 변화 특성에 관한 연구

        Objectives : The purpose of this study is to understand the temperature characteristics depending on the thickness and material of the needle used with the Electronic Warm Acupuncture Device (EWAD). Methods : We controlled experimental environment and measured temperature changes of a silicon phantom in which K-type thermocouples were inserted at depths of 2, 7 mm. EWAD perfomed with acupuncture needles of various thicknesses (0.25×60 mm, 0.40×60 mm, and 0.50×60 mm) and materials (Gold 0.40×50 mm). We set non-needle (only heated with EWAD skin heater) group as a control group. Results : The maximum temperature and temperature changes of 0.40 mm, 0.50 mm needle group were significantly higher than the non-needle group. The highest temperature range in all needle groups was 0.50 mm needle group (41.44±0.31℃). However, the 0.25 mm needle group was not significantly different from the non-needle group. Maximum temperature of gold needle group was significantly higher than stainless steel needle group. Temperature changes of gold needle group were higher than stainless steel group at the depth of 7 mm. Conclusions : It was found that needle thickness and material of acupuncture had an effect on the temperature of the EWAD. When performing EWAD treatment, consideration of thickness and material of acupuncture is needed. Future research is needed using phantoms that can reflect actual clinical situations and better mimic the human body.
