        요부 경막외 차단후 발생한 두통 및 기뇌증 - 증례보고 -

        Headache after epidural block is the most frequent result of an unintentional dural puncture. This form of headache is usually caused by cerebrospinal fluid leakage through the dural puncture site. Another proposed cause of postdural puncture headache is the un- intentional injection of air into the subarachnoid space. We experienced a case of severe headache with a patient after lumbar epidural block and discovered air in the intracranial subarachnoid space(pneumocephalus) with the aid of brain computerized tomography.

        경막외 Morphine 부작용 치료를 위한 Nalbuphine의 적정 정주 용량

        N/A Background: Epidural morphine provides excellent postoperative analgesia but is often associated with side effects such as nausea, vomiting and pruritus. It has been reported that mixed agonist-antagonist, nalbuphine can reverse side effects of epidural morphine without compromising analgesia. This study was designed to compare the efficacy of each intravenous dose of nalbuphine for treatment of side effects following epidural morphine. Methods: All patients received continuous infusion(2 ml/hr) of epidural morphine-local anesthetics mixture(morphine 4 mg, 1% mepivacaine 50 ml and 0.25% bupivacaine 50 ml) following a loading dose (morphine 2 mg with 1% mepivacaine 7 ml). Patients requesting treatment for nausea, vomiting and pruritus randomly received intravenous nalbuphine 0.05 mg/kg(Group 1; n=20), 0.1 mg/kg(Group 2; n=20) or 0.15 mg/kg(Group 3; n 20). The severity of nausea, vomiting, pruritus, degree of pain, sedation and vital sign were assessed prior to and 30 min after each dose. Results: The severity of nausea, vomiting and pruritus decreased significantly in all groups(p<0.01). Pain and sedation scores were unchanged in all groups. One patient received nalbuphine 0.15 mg/kg, complained of dizziness, agitation and palpitation. His blood pressure who had increased to 170/100 after first dose. Conclusions: This study suggests that intravenous nalbuphine is good for treatment of side effects following epidural morphine, and the dose of Group 1, 0.05 mg/kg, may be recommended as an optimal dose.

        Chronic pain is a frequent complication after spinal cord injury. Various medical and surgical approaches have been applied for management of spinal cord injury pain but none of them are definitive. The N-methyl-D-Aspartate (NMDA) receptor antagonist, ketamine has been reported to have a significant effect in the management of neuropathic pain. We used small divided doses of intravenous ketamine (30 mg divided by 6 equals 5 mg, 5 min interval) in spinal cord injury patients suffering from chronic pain, and accomplished significant pain relief without side effects

        N/A Background : This study was undertaken to evaluate the effect of plasma concentration of vitamin C is on the plasma total antioxidant status(TAS) in patients with paraquat intoxication. Methods : As a preliminary research for this study, we measured plasma vitamin C levels, uric acid, TAS, and bilirubin from 100 healthy Koreans aged between 25-55 years. Various concentrations of vitamin C were constructed in vitro with normal pooled plasma between 1mg/dl and 100 mg/dL and TAS was measured. Both vitamin C concentrations and TAS were measured from the blood samples, taken at 0.5, 1, 2, 3, 5, 7, 9 hours after a bolus injection of vitamin C(50 mg/kg) from 7 volunteers who had been recruited from medical students at our hospital. Various amounts of vitamin C were given to 10 patients with paraquat intoxication for 5 consecutive days in the morning : first day ; 100 mg single dose, second day; 500 mg single dose, third day ; 1,000 mg single dose, fourth day : 3,000 mg single dose, and fifth day ; 3,000 mg three times every 8 hours. Results : The mean value of TAS and vitamin C measured in 100 healthy Korean adults was 2.22 ±0.16 mmol/L and 0.48±0.10 mg/dL. Age and sex do not influence these levels(p>0.05, data are not presented). A direct correlation was observed between TAS and vitamin C concentrations of 1 mg/dL and 100 mg/dL both in vivo and vitro. As the vitamin C concentration was increased gradually 5 consecutive days, the TAS values increased in the same way as follows : first day(vitamin C 100 mg) 2.26±0.98 mmol/L, second day(vitamin C 500 mg) 2.76±0.78 mmol/L, third day(vitamin C 1.000 mg) 2.81±0.68 mmol/L, fourth day(vitamin C 3.000 mg) 3.18 mmol/L. On the fifth day when 3.000 mg of vitamin C was given by bolus intravenous injection 3 times(every 8 hours for a day), the TAS values measured one hour after the injection was 3.58±0.37 mmol/L. Conclusion : When vitamin C is used as a free radical scavenger, the loading dose and maintenance dose should be around 2278 mg intravenous bolus and 146 mg/hour. High doses of vitamin C are effective and safe for patients with acute paraquat intoxication.(Korean J Med 58:666-674, 2000)

        백서의 피부 및 뇌혈류에 미치는 고려홍삼 사포닌 및 비사포닌의 영향

        본 연구에서는 혈류량의 변화를 실시간으로 측정할 수 있는 레이져 도플러 혈류량측정장치를 이용하여 피부혈류 및 뇌혈류량에 미치는 고려홍삼의 사포닌과 비사포닌의 효능을 연구하고자 고려홍삼의 정맥내 복강내 및 구강내로 주입하고 혈류의 변화를 관찰하여 다음과 같은 결론을 얻었다. 고려 홍삼<br/> 의 사포닌 성분은 피부혈류량에는 영향을 주지 않으나 뇌혈류량을 증가시키는 효능이 있다. 고려 홍삼의 비사포닌 성분은 피부혈류량 및 뇌혈류량에 거의 영향을 주지 않았다. 고려홍삼 사포닌에 의한 뇌혈류량의 증가는 복강내 및 경구로 투여할 경우는 관찰할 수 있으나 정맥내로 직접 투여할 경우는<br/> 관찰되지 않았다.이상의결과를종합해볼때고려홍삼의 사포닌 성분은 뇌혈류량을 증가시키는 혈류개선작용이 있는 것으로 사료된다. To study the effect of Korea red ginseng (KRG) on the skin and cerebral blood flow, we evaluated the change of skin perfusion rate and cerebral perfusion rate after the intravenous, intraperitoneal, and oral administration of crude saponin (CS) and saponin-free fraction (SFF) of KRG in the rats. The change of skin perfusion rate and cerebral perfusion rate was measured laser doppler flowmetry. The intravenous injections of CS or SFF of KRG and intraperitoneal injection of SFF of KRG did not change the relative skin and cerebral blood flow in the rats. When the rats were treated by the<br/> intraperitoneal injection of CS of KRG, relative cerebral blood flow was significantly increased with a time-dependent manner, however, relative skin blood flow was not influenced by the them. Oral administration of CS of KRG slightly increased skin blood flow in the rats. Also, the change of cerebral blood flow by transient bilateral carotid arterial clamp in the CS-treated rats was significantly decreased, compared with control groups. From the above results, it was suggested that Korea red ginseng have a increasing property of cerebral blood flow in the rats.<br/> Key words : Korea Red Ginseng, Cerebral blood flow, Skin blood flow, Laser Doppler flowmeter.

      • 유전인자의 해석을 위한 하부구조의 물리적 상호작용 연구

        유전인자의 해석을 위한 하부구조의 온도변화에 의한 Base pairing transition의 분자구조적 상호작용을 규명하기 위해 RNA 계통의 Polyinosinic-Polycytidylic, Polyguanylic-Polycytidylic, Polyadenylic-Polyuridylic을 이용하였다. 주요연구는 Superconductor에 의한 Ultra-high resolution NMR을 이용하였고, Data는 Computer의 처리에 의해 분석되었다. NMR 분석결과는 UV 스펙트럼에 의한 melting point 결정방법과 비교하여 이론치와 잘 일치됨을 얻었다. In order to interpret the physical interaction of hyper-fine structure for the molecular analysis of gene, the transition of binding structure based on a temperature dependence is applied to the polyribonucleotides which is prepared by polyadenylic-polyuridylic, polyinosinic-polycytidylic, polyguanulic-polycytidylic, respectively. Ultra-high-resolution proton nuclear magnetic resonance spectra at 400MHz which is generated by superconductor have been used to investigate the properties of base-pairing transition. And also UV spectra contributed to the melting point in RNA are compared with the dynamic properties in NMR singnals, which is fitted by the computed spectra showing that there is no need for any additional phyical mechanism to explain NMR positions.

        The ESR parameters of the spin Hamiltonian for the so-called S1-center in cultured Quartz were measured at 40 K by the least squares fitting method. The temperature dependence of the parameters of the S1-center in quartz was also analized over a range of 20 - 295 K, using experimental results. Absolute values of the principal components of the D-tensor are decreased with increasing temperature, and the z-direction of the principal axes is also changed slightly over this temperature range.

      • 자연산 수정의 압흔경도와 불순물의 관계

        The effect of impurities on the hardness of crystalline SiO₂ samples has been investigated. Seven samples of differently colored natural quartz are adopted for this investigation. In these samples, major impurities. Al, Fe and Na, constitute more than 90 weight % of total impurity contents respectively. The hardness is measured by the Vickers microindentation method on the plane perpendicular to the c-axis of these crystals. The hardness values are found to be decreased by the amount of 20% as the contents of impurity are increased in the range of 0.09-0.22 wt%, whereas the hardness is insensitive to the impurity in the range of 0.22-0.44 wt%.
