        반측성 안면경련증 환자에서 안면신경의 미세혈관감압술 후에 시행한 자기공명영상의 역할

        Objectives : The objective of this study was to investigate the role of postoperative three dimensional short-range magnetic resonance angiography(3D-TOF MRA) in predicting the clinical outcomes following microvascular decompression(MVD) for the treatment of a hemifacial spasm(HFS). Material and Method : Postoperative magnetic resonance(MR) imaging was performed on 123 patients with a HFS between March 1999 and May 2000. All patients who had postoperative MR imaging were undertaken preoperative MR imaging. Of the 123 patients, 122 patients were included in this retrospective study. The degree of the detachment of vascular contact, and change of the position of offender were determined by pre- and postoperative 3D-TOF MRA. These findings were compared with the surgical findings and clinical outcomes. Results : Of 122 patients who had successful MVD, clear decompression of offenders of the root entry zone(REZ) of facial nerve was found in 106 patients(86.9%), partial decompression in 10 patients(8.2%) and contact of offenders to the REZ of facial nerve in 6 patients(4.9%) by the postoperative 3D-TOF MRA. Our patients demonstrated that the types of offender did not influence with the degree of decompression of REZ of facial nerve and with surgical outcomes(p>0.05). Also, there was no significant relationship between the degree of decompression of the REZ of facial nerve from offenders and an improvement of symptoms(p>0.05). Futhermore, there was no significant relationship between the degree of decompression of the REZ of facial nerve from offenders and an improvement time (p>0.05). Conclusion : Our data suggests that MVD of facial nerve alone may not be sufficient to resolve the symptoms in all patients with hemifacial spasm. Therefore, another unknown factors besides vascular compression may be involved to cause symptoms in certain patients and it may be necessary to remove these factors with MVD simultaneously to obtain the resolution of symptom.

        Chordoid meningioma is a rare but increasingly recognized subtype of meningioma that has high tendency to recur and is known to be sometimes associated with Castleman syndrome. A 41-year-old woman presented with 2months history of progressive headache. Magnetic resonance image revealed a large extra-axial and well enhancing mass in the right frontal convexity that resembled a typical convexity meningioma. However, the histological and immunohistochemical examinations of the surgical specimen confirmed the diagnosis of chordoid meningioma. We describe a case of chordoid meningioma without systemic manifestations related to Castleman syndrome such as anemia and/or dysgammaglobulinemia.

        A meningioma manifesting chronic subdural hematoma(SDH) is very rare. Although several reports have been presented, the mechanisms of chronic SDH formation in the presence of meningioma are not yet understood. A 71-year-old woman presented with 5-day history of headache and vomiting. There was no history of trauma and the patient was not on any anticoagulant therapy. On admission, the patient was alert and oriented. However, the patient became drowsy suddenly, with left hemiparesis(grade III). A noncontrast computed tomographic(CT) scan of the head revealed a right chronic SDH and subsequent magnetic resonance image(MRI) showed a right chronic SDH associated with an extraaxial and well enhancing mass in the right frontal convexity. Complete resection of the tumor as well as the removal of the chronic SDH was performed. Histological examination revealed an atypical meningioma with intratumoral hemorrhage. We describe a case of meningioma associated with chronic SDH and review the previous reports.

        상지 구획 증후군 이후 발생한 치명적인 뇌출혈

        Compartment syndrome has a wide spectrum from muscle pain to a life- threatening condition, such as acute renal failure and disseminated intravascular coagulation (DIC). Intracerebral hemorrhage (ICH) due to compartment syndrome has not been reported. We report a patient who presented with ICH leading to death. A 25-year-old female with no significant past history developed extensive compartment syndrome followed by rhabdomyolysis, acute renal failure, DIC, and ICH. Although the patient underwent a fasciotomy and hemodialysis and received aggressive resuscitation with massive transfusions of blood and intravenous fluids, she died. This case stresses the importance of early diagnosis and prompt treatment of compartment syndrome to prevent devastating complications. (J Korean Soc Traumatol 2006;19:178-182)

        추간판 조직재생을 위한 지방유래 기질세포와 수핵세포의 공배양 조건

        Nucleus pulposus(NP) cells located at intervertebral disc are chondrocyte-like cells secreting cartilagespecific matrix molecules such as type 2 collagen and aggrecan. NP cells transplantation is used for recovery of damaged intervertebral disc. However, it is difficult to obtain sufficient quantity of NP cells from donor for cell therapy. Recently, adipose derived stromal cells(ASCs) as alternative cell sources have been extensively studied for cells therapy because ASCs can differentiate into several lineages including chondrogenesis. In this study, we sought to optimize the co-culture condition of NP cells and human ASCs for ASC differentiation. Media condition for NP cells and ASCs co-culture was determined by comparing proliferation and differentiation. After that, various cell ratios between NP cells and ASCs(0, 25, 50, 75, 100% ASC) were tested to find optimal cell ratio for efficient differentiation of ASCs by histochemical staining, cell proliferation, gene expression and so on. According RT-PCR and staining, differentiation of ASCs was dependent on the media condition and cell ratio between NP cells and ASCs. This demonstrated the appropriate media and the optimized cell ratio between NP cells and ASCs are able to induce efficient differentiation of ASCs that is expected to provide efficient intervertebral disc regeneration in vivo.

        A variety of genetic alterations in human glioblastomas comprises signal transduction and cell cycle arrest control of cellular processes. In an attempt to identify novel genes implicated in glioblastoma development, we performed experiments using suppression subtraction hybridization(SSH) with human glioblastoma tissue, as well as with cloned genes, that are expressed more than in normal brain tissues. After the identification of a novel gene, RT-PCR was performed to determine its mRNA expression in human glioblastoma tissues. In order to learn more about the expression profile of this gene, PCR was performed using various commercially available normal or carcinoma cell lines. The novel gene was found to be strongly expressed in glioblastoma tissues and other carcinoma cell lines. To determine whether this novel gene was associated with cell cycle regulation, a serum stimulation study with normal WI-38 fibroblast cells treated medium containing 0.1% FBS for 48hr was used for its examination. The novel clone was identified as being expressed in several glioblastoma tissues and carcinoma cell lines. The time-course expression of this novel gene in the WI-38(8PDL) normal lung cell line indicated a significant increase for G1-phase arrest. We have used suppression subtractive hybridization(SSH) to identify cDNA of a gene “Clone 15” overexpressed in human glioblastoma. When the expression of “Clone 15” was screened in numerous cell types, we could confirm that this novel gene was overexpressed in tumor compared to matched normal tissue.
