        내측 원판형 연골 전방부의 비정상적 부착 - 1례 보고 -

        Lateral discoid meniscus is a common cause of internal derangement of the knee joint. The discoid medial meniscus is much less common than the discoid lateral meniscus. Anomalous insertion of the anterior horn of the medial meniscus is extremely rare. The authors performed arthroscopy on the symptomatic knee joint and incidentally found an anomalous insertion of the anterior horn of the medial meniscus into the anterior cruciate ligament combined with incomplete discoid medial meniscus. We treated discoid medial meniscus by reshaping but anomalous insertion of the medial meniscus into anterior cruiate ligament was left alone. On the 13 months follow-up after surgery, the result was satisfactory.

      • 연속성이 유지된 불안정 전방 십자 인대 손상의 수술적 치료

        Purposes: The purpose of this study was to analyze the result of ACL reconstruction preserving the intact ACL fiber for the ACL injury with continuity. Materials and Methods: We evaluated 14 cases of 82 cases which had been received ACL reconstruction between Jan. 1997 and March 1999. Their mean age at surgery was 32.5 years and average length of follow-up was 18.4 months. We evaluated the results of reconstructive surgery by patient's symptom, Lysholm knee score, physical examination especially presence of pivot shift test. Results: Symptom was improved in every cases postoperatively. There was improvement of average Lysholm knee score from 55.3 to 91.2 and the anterior translation was less than 5mm in 10 cases(83.3%) and between 6 to 10mm in 2 cases(16.7%) at follow up period. Conclusions: We obtained good clinical results of ACL reconstruction for the unstable ACL injury with continuity. We thought preserving the injured ACL fiber would be important for the proprioceptive function of the knee joint but more comparative study would be needed.

      • 수술적 방법으로 치료한 흉요추부 골절의 기능적 결과에 관한 연구

        흉요추부 손상의 수술적 적응증 및 최적의 수술적 치료방법의 선택에는 아직까지 논란의 여지가 있다. 저자들은 급성기 수술적 치료를 시행한 흉요추부 손상 환자에서 신경학적 상태, 술후후만변형, 동통, 작업능력 상태 및 합병증 등을 후향적 조사를 통해 조사하였으며 전방도달법에 의한 수술적 치료의 잇점을 비교분석, 문헌고찰과 함께 보고하고자 한다. The functional outcome of the surgically treated thoracolumbar spine fractures is focused mainly on the neurologic status but there are few reports about the pain and work capacity scale evaluation on the neurologically intact patients. We evaluated 90 cases of the thoracolumbar injuries which were treated surgicaly and followed more than 12 months between the periods of January 1987 to December 1992. These 90 cases were divided into the three group according to the method of surgical treatment and fixation and measured pain and work scale of the neurologically intact patients according to the scale of Denis, et, al. Through this retrospective study, we could summerize as follows ; 1.Posterior approach and instrumentation can effectively treat the major thoracolumbar injuries by decompression and correction of the deformity but may increase anterior instability. 2.Anterior surgery result in a more complete and reliable decompression and adequate stability can be obtained by tricortical strut bone graft and application of the kaneda deice. 3.Finally, the pain and work scale in neurologically intact patients was bettor in anteriorly created group than in posteriorly operated group.

      • 골 결손이 심한 인공 고관절 재수술에 있어서 골유도 능력을 가진 이식골(Osteoilductive allo-implant)의 조기 골 융합에 관하여

        인공 고관절 재대치술에 있어서 해리가 합병된 인공 고관절은 광범위한 골결손을 동반하고 있으며, 자가 골이식은 골량이 한정되어 있어 치료에 난점이 있다. 저자는 두개골 결손 치료에 사용되는 골 유도 능력을 가진 동종골(autolysed, antigen extracted, allogenic bone matrix: 이하 AAA bone)을 광범위한 대퇴부 골결손을 가진 고관절 재 대치술에 적용하여 단기 추시이지만 우수한 결과를 얻었기에 문헌 고찰과 함께 보고하는 바이다. Repair work of human skull defects using AAA bone (allogenic, autolysed, antigen free) is challenging. We empolyed clinical study wish several struts and cortical powder composed of AAA bone which was obtained from Germany for reconstruction of Type III femoral deficiency as well as simplistic classification system of the femoral deficiency before revision total hip arthroplasty. During the reconstruction, so called Bicontact revision stem is empolyed in all 4 patient in 5 hips for comparative study. The design of this prosthesis is based on diaphyseal primary stabilizations are provided two holes for locking the implant and metaphyseal secondary fixation by bone growth on to the proximal part of the prosthesis. Although porous-surfaced orthopedic implants are designed for fixation by bone ingrowth, there is clinical evidence that this does not always occur. Nevertheless, we observed 2 cases of clinical success that AAA bone facillitate the early incorporation and reproduction of the defect in cortical tube within 3 months time. In another case, we proved efficacy of AAA cortical powder which is promising when we apply osteotomy site in order to correct the mal-alignment in revision total hip arthroplasty. On the other hand, 2 cases of non-osteoinductive and inadequate use of osteoinductive at loimplatnts underwent delayed incorporations and one of these resulted as bending of the interlocking screw due to loss of primary fixation of the implants as well as secondary metaphysial secondary fixation. We believe osteogenic potential seems to be more pronounced when combined with bone marrow extracts during preparation of grafting material.

        생태계 교란 어종 활용 자원화 특성

        Recently, various studies have been actively conducted for the protection of indigenous freshwater fish species and reduction of ecological disturbances that caused by foreign species. The Ministry of Environment has designated and managing foreign species such as Micropterus salmoides (Largemouth bass), Lepomis macrochirus (Blue gill) and Salmo trutta (Brown trout) as species that disrupt the ecosystem. In particular, the Micropterus salmoides, the top-rated predator without natural enemies which is a carnivorous fish, spreads quickly through our water system. This predaceous and fish net-eluding major risk factor for the ecosystem is continuously extending its habitat nationwide by predating on smaller indigenous fishes. Controlling the population of largemouth bass is inevitable to protect our native fishes and maintain the balance of the freshwater ecosystem. The largemouth bass is a great source of protein and minerals. If the repulsion against foreign, invasive, ecosystem-disturbing fish gets in the way of direct consumption, we should find indirect way to solve this problem by making the fish as a part of preferring food such as fruits and vegetables. In this study, the high-grade amino acids and other nutrients extracted from largemouth bass, an invasive species that disturbed the ecosystem, applied in organic and general agriculture to find out the effectiveness on the growth of crops and plants was evaluated. Furthermore we try to introduce the educational facility for specializing in freshwater ecological environment improvement.

      • Ilizarov 술식을 이용한 골절 치료

        골절의 고정방법에는 여러 가지가 있으며 이중 Ilizarov 외고정 기구는 강력한 골절 부위 고정력과 압박 신연 운동이 가능하며 사지의 단축이나 각변형 및 회전변형의 고정이 용이하다는 장점이 있어 그 사용이 증가되는 추세이다. 타 기구를 이용한 고정이 어려운 조건에서 적절한 적응증을 선별하여 시행한 Ilizarov 외고정 방법은 여러 형태의 난치성 골절에서 만족할만한 방사선학적, 임상적 유합을 기대할 수 있다. There are some complicated fractures of the long bones that cannot be treated by conventional methods. Nineteen patients with dificult long bone fractures were treated by Ilizarov ring external fixators between the periods of January 1993 to January 1997. 1.There were 4 cases of severe comminuted fracture which had difficulty in reduction or reduction maintainance. 3 cases of segmental and comminuted fracture, 3 cases of short periarticular fracture, 2 cases of open fracture with soft tissue defect, 2 cases of open fracture with bone loss, 3 cases of Jehovah's witness and 1 case of physeal injury. 2.The bone union was obtained in all cases and the duration of Ilizarov ext. fixator application was minimum 6 weeks unto 22 weeks. With an average of 14 weeks and full weight bearing was possible in all cases after removal of the Ilizarov ext. fixator. So, we conclude that the Ilizarov external fixation technique is one of effective methods for the treatment of difficult fracture of the long bone.

        대기 플라즈마 CYSZ 단열코팅 후처리 공정에 따른 마모거동

        Plasma sprayed zirconia-base coatings are of general interest as thermal barrier coatings in diesel and gas turbine engines. However, residual pores and cracks in these coatings decrease the mechanical properties. Therefore the improvements to ZrO_2-25CeO_2-2.5Y_2O_3(CYSZ) coatings by SiC impregnation were studied. The effect of the nano-SiC sealing on the wear properties was also studied. The coatings were produced by an atmospheric plasma spraying process. The CYSZ coatings were sealed by repeated polycarbosilane (PCS) solution infiltration and pyrolysis. Through thermogravimetry analysis, the PCS precursor solution started to decompose at 100℃ and finished at 800℃. After 800℃ pyrolysis process, the SiC phase was amorphous. DSC analysis revealed that the crystallization of the amorphous silicon carbide started at about 1030℃ and finished at 1100℃. The mean grain size determined using the Sherrer and Wilson formula from the breath of the SiC(220) line was not larger than 5 nm. Under dry condition sliding wear against sintered ZrO_2-8Y_2O_3 (YSZ), weight losses were markedly decreased by SiC sealing. The sealing resulted in the wear transition, which corresponded to the change in the mechanism from a brittle fracture mode causing severe wear to a plastic deformation mode causing mild wear. The friction coefficient was also decreased by both the surface smoothing effect of the SiC sealing and the possible excess carbon of the nonstoichiometric SiC. During sliding wear of unsealed and sealed coatings, a stress induced tetragonal to monoclinic martensitic phase transformation, which deteriorates wear resistance, was not observed.
