Objective: A prolonged length of stay in the emergency department (EDLOS) is known to be associated with poorer outcomes in critically ill patients. However, this has not been proven in patients who visit the emergency department (ED) due to acute heart failure (AHF). We aimed to find out whether prolonged EDLOS is associated with major in-hospital complications in patients with AHF. Methods: This is a retrospective cohort study of AHF patients who were admitted to intensive care units (ICU) via the ED of a single academic hospital from January 2015 to December 2019. We divided the patients into two groups: EDLOS <24 hours and EDLOS≥24 hours. The primary outcome was major in-hospital complications, which included in-hospital death, application of continuous renal replacement therapy, or extracorporeal membrane oxygenation treatment. Secondary outcomes included in-hospital death, prolonged ICU stay (≥10 days), and prolonged hospital stay (≥14 days) excluding ED stay. Results: A total of 265 patients were enrolled. Of these 163 patients stayed in the ED for over 24 hours. The multivariable logistic analysis demonstrated that EDLOS ≥24 hours was independently associated with major in-hospital complications (odds ratio [OR], 3.296; 95% confidence interval [CI], 1.291-8.413; P=0.013). Analysis of the secondary outcomes showed that EDLOS ≥24 hours was associated with in-hospital death (OR, 2.607; 95% CI, 1.005-6.759; P=0.049) and prolonged hospital stay ≥14 days (OR, 2.458; 95% CI, 1.303-4.636; P=0.006). Conclusion: Our study showed that in patients with AHF who visited ED and were admitted to the ICU, prolonged EDLOS was associated with major in-hospital complications.

Since the global positioning system receivers on the surface of the Earth use satellite signals sent from a remote distance and the intensity of received signals is weak, they are vulnerable to jamming. This paper implements a vector-tracking loop (VTL)-based global navigation satellite system (GNSS) receiver algorithm as an anti-jamming technique and compares the performance of VTL-based receivers with that of scalar-tracking loop (STL) that is used in general GNSS receivers at various jamming environments and a vehicle’s dynamics. The simulation results shows that VTL is more robust against jamming than STL in all operating environments.

Introduction Prolonged length of stay in the emergency department (EDLOS) is known to be associated with poorer outcomes and prolonged inpatient stays in critically ill patients with various disease entities. However, this has not been proven in patients who visited the emergency department (ED) due to acute decompensated heart failure (ADHF). Our study is to find out whether prolonged EDLOS is associated with major in-hospital complications and prolonged in-hospital stays in patients with ADHF. Material & Method This is a retrospective cohort study of ADHF patients who were admitted to intensive care units (ICU) via ED of a single academic hospital from January 2015 to December 2019. We divided patients into two groups; EDLOS < 24 hours and EDLOS > 24 hours. Primary outcome was major in-hospital complications, which included in-hospital death and application of continuous renal replacement therapy or extracorporeal membrane oxygenation. Secondary outcomes included in-hospital death, prolonged ICU stay (≥ 10 days) and prolonged hospital stay (≥ 14 days) without ED stay. Result Out of 368 patients, 94 patients were excluded, and a total of 274 patients were enrolled. Median age was 78 years (interquartile range (IQR) 72-83), and median EDLOS was 27.6 hours (IQR 20.3-39.7). A total of 170 patients (62.0%) stayed in ED for more than 24 hours. Multivariate logistic analysis including covariates with a p value of < 0.1, which were history of hypertension and diabetes mellitus, and cardiopulmonary resuscitation and vasopressor use in ED, demonstrated that EDLOS > 24 hours was independently associated with major in-hospital complications (odds ratio (OR) 2.51, 95% confidence interval (CI) 1.07-5.92, p=0.035). Analysis on secondary outcome variables showed that although EDLOS > 24 hours was not associated with in-hospital death or prolonged ICU stay ≥ 10 days, it was independently associated with prolonged hospital stay ≥ 14 days. (OR 2.49, 95% CI 1.33-4.66, p=0.005). Conclusion Our study showed that in patients with ADHF who visited ED and admitted to ICU, prolonged EDLOS is independently associated with major in-hospital complications and prolonged hospital stay.

객체지향 분석에 널리 사용되어 온 상태 전이도(transition diagram)는 객체 각각의 동적인 행태에 대해서만 기술할 뿐, 객체간의 상호작용을 나타내는 데는 미비하다. 객체지향 시스템은 다수의 객체들과 그들간의 상호작용으로 이루어지기 때문에 전체 시스템의 동적인 행태를 표현하기 위해서는 객체간의 상호작용을 기술하는 것이 매우 중요하다. 본 연구에서는 평면적으로 나열된 객체간의 상호작용들을 추상화(abstraction)하여 관리 및 유지, 보수에 필요한 높은 수준의 시스템 이해가 가능하도록 하면서, 각 객체가 제공하는 method에서 출발하여 상위 상호작용을 구성해 가는 추상화 방법을 취하여 OMT 모델과 연계될 수 있는 방법을 제시한다. 이 방법을 통해 시스템의 이해와 상호작용의 관리 및 유지, 보수가 용이해지는 효과를 볼 수 있다.

비대칭 암호 알고리즘을 설계하는 데 있어서 매우 큰 소수를 구하는 것은 필수적이다. 그러나 지금까지는 결정론적인(deterministic) 큰 소수를 발견하기는 매우 어려웠기 때문에, 일반적으로 확률적으로 소수일 가능성이 높은 의사소수(psedoprime)를 비대칭 암호 알고리즘에서 사용하였다. 이 논문에서 결정론적인 소수 생성 방법을 제안하며, 제안된 방법에 의해 생성된 소수는 증명이 가능한 100% 정확한 소수이다. 또한 이 방법에 의해 생성된 소수는 신뢰성, 비도, 원시원소(primitive element)생성 능력 등을 보장한다. It is essential to get large prime numbers in the design of asymmetric encryption algorithm. However, the pseudoprime numbers with high possibility to be primes have been generally used in the asymmetric encryption algorithms, because it is very difficult to find large deterministic prime numbers. In this paper, we propose a new method of deterministic prime number generation. The prime numbers generated by the proposed method have a 100% precise prime characteristic. They are also guaranteed reliability, security strength, and an ability of primitive element generation.
