        고정형 슬관절 단일 구획 치환술의 중기 추시 결과

        목적: 고정형 슬관절 단일 구획 치환술의 5년 추시 결과로 임상적 및 방사선적 결과, 합병증 발생 빈도를 알아보고자 하였다. 대상 및 방법: 2003년 5월부터 2011년 8월까지 고정형 슬관절 단일 구획 치환술을 시행한 25명, 26예를 대상으로 하였다. 평균 연령은 63.5세였으며 남자가 3명(3예), 여자가 22명(23예)이었으며 술 전 진단은 골관절염 23예, 골괴사 3예였다. 평균 추시 기간은 평균 67개월(60-149개월)이었다. 임상적으로 술 전과 술 후의 미국슬관절학회 슬관절 점수 및 기능점수, 관절 운동 범위를 조사하였고, 방사선적으로 슬관절 기립 전후방 사진과 측면 사진, 투시기 촬영을 통하여 술 후의 정렬상태와 골용해 여부를 조사하였다. 결과: 임상적 결과로 슬관절 점수는 평균 42.0점에서 87.9점으로, 기능점수는 57.5점에서 85.0점으로 향상되었다(p<0.001). 술 전 관절 운동 범위는 평균 132.9°에서 술 후 132.5°로 큰 차이를 보이지 않았다. 방사선적 결과로 대퇴경골각은 술 전 평균 내반 0.5°에서 술 후 외반 2.2°로 측정되었다. 방사선 투과성 선(radiolucent line)이 관찰되는 경우가 2예 발견되었으며 그 중 1예는 삽입물이 안정적이었으나 다른 1예는 단일 구획 치환술 시 대퇴슬개관절의 관절염을 확인하였고, 추시상 슬관절 내측 관절선의 동통 및 슬관절의 미만성 통증이 있어 슬관절 전치환술을 권유하였다. 그 외 골용해, 감염, 술 후 강직, 탈구 등의 합병증은 없었다. 결론: 고정형 슬관절 단일 구획 치환술의 중기 추시 결과는 임상적 및 방사선적으로 만족스러운 결과를 보였다. Purpose: To evaluate the clinical and radiological outcomes, and the complications of unicompartmental knee arthroplasty (UKA) using a fixed bearing prosthesis after 5-year follow-up. Materials and Methods: Twenty-six knees (25 patients) that underwent fixed bearing UKA between May 2003 and August 2011 were included. The subjects were 3 males (3 knees) and 22 females (23 knees), and the average age was 63.5 years. The preoperative diagnosis was osteoarthritis (23 knees) and osteonecrosis (3 knees). The mean follow-up duration was 67 months (from 60 to 149 months). The clinical evaluation included pre- and postoperative American knee society knee and function score, and range of motion. The radiology evaluation included standing antero-posterior, lateral view, and fluoroscopic film to analyze the postoperative alignment and osteolysis. Results: The mean American Knee Society knee score and function score were improved from 42.0 and 57.5 to 87.9 and 85.0, respectively (p<0.001). The mean preoperative and postoperative range of motion was 132.9° and 132.5°, respectively. The mean femorotibial angle were varus 0.5° preoperatively and valgus 2.2° postoperatively. A radiolucent line was observed in 2 knees; one knee had a stable implant, while in the other knee, patellofemoral arthritis was identified during UKA. Diffuse pain of the knee joint with tenderness of the medial joint line was identified at the follow-up, so conversion to total knee arthroplasty was recommended. No other complications, such as osteolysis, infections, postoperative stiffness, and dislocation, were encountered. Conclusion: The midterm results of fixed bearing UKA were clinically and radiologically satisfactory.

        회전근 개 파열 봉합술 후 재파열 환자에서 불량한 임상 결과의 원인 인자

        목적: 관절경적 회전근 개 봉합술 후 재파열이 확인된 환자를 분석하여 불량한 임상 결과에 영향을 미치는 원인 인자를 평가하고자 하였다. 대상 및 방법: 2013년 1월부터 2018년 12월까지 회전근 개 파열로 관절경적 회전근 개 봉합술을 시행 받은 631명의 환자 중 1년 이상 외래 추시를 하고 추시 자기공명영상(magnetic resonance imaging, MRI) 검사상 재파열이 확인된 42예의 환자 데이터를 후향적으로 수집하였다. 수술 전후 운동 범위, 관절경하 건의 파열 정도 및 양상, MRI 검사상 파열 및 재파열의 크기, 파열의 진행 여부, 지방 변성 정도 등 환자의 기능적 결과에 영향을 줄 수 있는 요인을 측정하고 기능적 결과는 American Shoulder and Elbow Surgeons (ASES) 점수로 평가하였다. 불량한 임상 결과에 영향을 주는 인자를 찾기 위해 ASES 점수가 80점 미만인 군을 불량한 임상 결과의 군으로 정의한 뒤 80점 이상인 군과 비교하여 불량한 임상 결과의 위험 인자를 평가하였다. 결과: 관절경적 회전근 개 봉합술 후 재파열이 확인된 군(n=42)의 수술 후 임상 결과는 수술 전에 비하여 의미 있게 호전되었다. 단변량 분석상 수술 전 visual analogue scale (VAS) 점수, 파열의 진행은 불량한 임상 결과와 연관이 있었고 동반된 견갑하건의 봉합은 양호한 임상 결과와 연관이 있었다. 하지만 다변량 로지스틱 분석에서 견갑하건의 봉합을 제외한 수술 전 VAS 점수 및 파열의 진행만이 불량한 임상 결과에 대해 통계적 유의성을 보였다. 결론: 수술 전 VAS 점수와 추시 MRI 검사에서 회전근 개 파열의 진행은 관절경적 회전근 개 봉합술 후 재파열을 진단받은 환자의 불량한 임상 결과와 연관 있는 위험 인자이다. Purpose: To evaluate the prognostic factors affecting poor functional outcomes in patients with retear after rotator cuff repair. Materials and Methods: From January 2013 to December 2018, among 631 patients who underwent arthroscopic repair of a rotator cuff tear, 42 patients, who could be followed-up for more than one year and showed a retear of the repaired cuff on magnetic resonance imaging (MRI), were collected retrospectively. The preoperative demographic data, range of motion, American Shoulder and Elbow Surgeons (ASES) score, fatty degeneration, and tear progression on postoperative MRI, as well as other factors that could affect the clinical outcomes, were analyzed. Patients who scored <80 points on the ASES score were allocated to the poor function group. The risk factors for poor clinical outcomes were compared with the group with ASES scores of 80 or above. Results: The postoperative functional results in the group with retear (n=42) after arthroscopic rotator cuff repair showed significant improvement. Univariate analysis revealed the preoperative visual analogue scale (VAS) score and tear progression to have associations with a poor shoulder function. In addition, subscapularis repair was found to be associated with a good shoulder function. The preoperative VAS score and tear progression except for subscapularis repair were independent factors associated with poor clinical outcomes according to multivariate logistic regression analysis. Conclusion: In patients with retear after rotator cuff repair, the preoperative VAS and tear progression in postoperative MRI are factors predicting a poor functional outcome.

        도선에 커플링 되는 고출력 전자파에 의한 CMOS IC의 피해 효과 및 회복 시간

        본 논문은 고출력 전자파에 따른 CMOS IC 소자의 피해 효과와 회복 시간을 알아보았다. 고출력 전자파 발생장치는 마그네트론을 사용하였고, CMOS 인버터의 오동작/부동작 판별법은 유관 식별이 가능한 LED 회로로 구성하였다. 그리고 고출력 전자파에 의해 오동작된 CMOS 인버터의 전원 전류와 회복 시간을 관찰하였다. 그 결과, 전계 강도가 약 9.9 ㎸/m에서의 전원 전류는 정상 전류의 2.14배가 증가하였다. 이는 래치업에 의한 CMOS 인버터가 오작동된 것을 확인할 수 있었다. 또한, COMS 인버터의 파괴는 컴포넌트, 온칩와이어, 그리고 본딩와이어에서 다른 형태로 관찰하였다. 위 실험 결과로, 전자 장비의 고출력 전자파 장해에 대한 이해를 돕는데 기초 자료로 활용될 것으로 예측된다. This paper examines the damage effect and delay time of CMOS integrated circuits device with coupling caused by high power microwaves. The waveguide and magnetron was employed to study the influence of high power microwaves on CMOS inverters. The CMOS inverters were composed of a LED circuit for visual discernment. Also CMOS inverters broken by high power microwave is observed with supply current and delay time. When the power supply current was increased 2.14 times for normal current at 9.9 ㎸/m, the CMOS inverter was broken by latch-up. Three different types of damage were observed by microscopic analysis: component, onchipwire, and bondwire destruction. Based on the results, CMOS inverters can be applied to database to elucidate the effects of microwaves on electronic equipment.

        Reverberation chamber have become a common tool in radiated susceptibility and emission testing of electronic equipment. In a reverberation chamber, internal electromagnetic fields are mixed by metallic stirrer. Due to elimination of Polarization characteristic in reverberation chamber, electromagnetic wave reached to EUT all aspect angle. Hence, Field distribution in overmode reverberation chamber can be presented by chi-square distribution. Field uniformity in reverberation chamber have to satisfy standard deviation below 3㏈ and is influenced by mechanical stirrer. This paper investigates relationship between change of stirrer parameter and field uniformity in reverberation chamber.

        Aspergilllus fumigatus 에 의한 CAPD 복막염후 도관내강의 초형태학적 변화

        Recently, dialysis-related fungal peritonitis have been increasingly reported, accounting for 5% of all cases of peritonitis. Most of these infections were caused by Candida species, Aspergillus species are an extremely rare cause of peritonitis in CAPD patients. Aspergillus peritonitis in CAPD patients have been fatal. We examined luminal surface of removed Tenckhoff peritaneal catheter in a patient on CAPD with Aspergillus peritonitis by scanning electron microscopy to study the morphology of fungal adherence. A 53-year-old female with an end-stage renal disease and hypertension had been on CAPD for 14 months. The patient experienced sudden onset of fever, diffuse abdominal pain, and cloudy dialysate effluent. She was initially treated as an outpatient with intraperitoneal cefazoline and netilmicin, Three days later, there was no resolution of her symptoms and the patient was admitted. The dialysate effluent was cloudy with 468 leukocytes per mm3 and 70% segmented neutrophils. On the surface of inner lumen of the Tenckhoff peritoneal catheter numerous tiny small visible colonies were found. Dialysate samples were obtained for microbiological study, and found evidences of a positive fungal mycelium and spores in the KOH mount. The Tenckhoff peritoneal catheter was removed immediately and intravenous fluconazole was begun. She received continuous antifungal therapy with intravenous fluconazole over 2 weeks. All sample of dialysate grew Aspergillus fumigatus. The other symptoms improved gradually over the next week. We studied the intraperitoneal portion of the removed Tenckhoff peritoneal catheter with scanning electron microscope. Mycelium of the Aspergillus fumigata with several spores grew along the irregular patterns of shallow fissure on the luminal surface of removed Tenckhoff peritoneal catheter. Similar patterns of these picture were observed on the several place of luminal surface of the catheter. Growth of several microcolonies were found on the surface of the catheter. Cross section of fungal mass which occluded catheter lumen revealed numerous mixture of fungal mycelium and spores. In conclusion, early removal of the Tenckhoff peritoneal catheter combined with antifungal agents is important steps to cure fungal peritonitis in a patients on CAPD because of fungus adhered to on silastic catheter materials in peritoneal dialysis fluid with formation of fungal colonization and fungal mass.

        식도에 발생한 점막교 4예

        Mucosal bridge, endoscopically observed, is a cord-like mucosal connection across the lumen. The bridge is very elastic and stretches easily, unlike granulation tissue. Mucosal bridges of the esophagus have been occasionally described in various circumstances, particularly in congenital or acquired origin as the inflammatory diseases. The occurrence of mucosal bridges due to inflammatory process may arise anywhere from the esophagus to the colon. It has been more frequently reported in the colon than in the esophagus, stomach, and duodenum. We experienced four cases of esophageal mucosal bridges and three of them were accompanied by esophageal diverticulum. We report these cases with a review of relevant literatures.

      • Application of Spray Pyrolysis Process for Production of Ultra Pure and Fine Powder

        Newly modified spray pyrolysis system was developed to produce ultra pure and fine powder by spray pyrolysis process. In this system, raw material solution was effectively atomized and sprayed into the reaction furnace. Also, thermal decomposition process fully completed in the three zone reaction furnace, and produced powder was effectively collected. A technology to reduce impurities in complex acid solution below 20ppm was also developed. The characteristics of produced powder were studied by changing the reaction conditions such as reaction temperature, the injection velocity of the solution and air, nozzle tip size and concentration of solution. The morphology of powder had spherical shape under the most experimental conditions, and the composition and the particle size distribution were almost uniform. Under the most experimental conditions average particle size of most produced powder was below 100nm.
