        직난시 및 도난시에 따른 동축 소절개 백내장 수술의 난시와 고위수차의 비교분석

        Purpose: To assess the changes of corneal astigmatism and higher order aberrations (HOAs) of the anterior and posterior corneal surface after cataract surgery with on-axis clear corneal incision in eyes with-the-rule (WTR) astigmatism and against-the-rule (ATR) astigmatism. Methods: This study included 50 patients who underwent phacoemulsification and IOL insertion through a 2.8-mm on-axis clear corneal incision. The eyes were divided into two groups: (1) 26 eyes with WTR astigmatism with a superior incision and (2) 24 eyes with ATR astigmatism with a temporal incision. During the follow-up period, visual acuity was measured, and the surgically induced astigmatism (SIA) and HOAs of the anterior and posterior corneal surface were measured with Pentacam? (Occlus, Wetzlar, Germany) preoperatively and 1 week, 1 month, and 2 months postoperatively. Results: There were no significant differences in UCVA and BCVA between the two groups. HOAs increased in both groups 1 week after surgery, but no significant differences were found between the groups (p > 0.05). Surgically induced astigmatism was larger in the WTR group than in the ATR group (p < 0.05). At postoperative 2 months, there were significant differences in HOAs between the two groups, and there were statistically significant differences in HOAs, oblique trefoil at front side, and in HOAs, horizontal coma at rear side (p < 0.05). Conclusions: In conclusion, superior incision in eyes with WTR astigmatism resulted in higher SIA compared to temporal incision in eyes with ATR astigmatism. Moreover, HOAs was significantly decreased in eyes with WTR astigmatism with superior incision. Thus, superior incision could be more effective in reducing corneal astigmatism in eyes with WTR astigmatism. J Korean Ophthalmol Soc 2014;55(2):182-189

        Purpose: This study is to identify the effect of communication intervention of holistic arts on conversation ability of children with language developmental delay. Methods: According to result from Receptive & Expressive Vocabulary Test or REVT, 3 children with a language developmental delay had a language level equivalent with 5 years old child and understanding and expression of story and vocabulary change were investigated through a prior- and post-test design for intervention effect. Results: This study was confirmed to influence on an improvement of story understanding ability and vocabulary. But there was a significant difference of the result of story expressing ability on each subject. Conclusions: To summarize the results, it can be concluded that holistic arts communication intervention program used in this study is an effective method to improve understanding of story and vocabulary of children with a language developmental delay and further profound study needs to be conducted for story expression.

        국민건강보험공단 제2형 당뇨 건강검진사업의 비용-효과분석

        While health screening service of the National Health Insurance Corporation (NHIC) is the largest health screening program in South Korea, its cost-effectiveness has not been examined. The present study, therefore, conducted cost-effective analysis of Type 2 Diabetes Mellitus(DM) screening that is a part of NHIC health screening program. Of simulated cohort selected among all health screening participants in 1996, a Markov model that reflects natural course of Type 2 DM was used to predict the occurrence of DM and its complications until death or 100 years of age. The distribution of DM and its Complications that was initial input data for Markov model was extracted from individual 10-year cohort data from NHIC database. The results show that the Type 2 DM screening group compared to non- DM screening group increased 0.76 life year gained (LYG) and 0.23 quality-adjusted life years (QALY) per person and reduced 370,000 won per person from the insurer`s perspective and 7,200,000 won per person from the societal perspective for asymptomatic diabetic patient. This total medical cost reduction is attributable to the reduced direct and indirect medical treatment costs for less complications of DM by early detection. The incremental cost effectiveness ratio was -3,132,215won/QALY and -943,316won/LYG. The results of the present study proved that NHIC DM Screening is effective in extending the quality of life as well as reducing medical cost.

        약용식물의 추출방법에 따른 항산화 및 항당뇨 활성

        본 연구에서는 예로부터 전해 내려오는 당뇨처방전에 사용되고 있는 문헌고찰을 토대로 6종의 약용식물(당귀, 백복령, 상백피, 상엽, 총목피 및 홍삼)의 항산화 및 항당뇨 활성을 비교 검토하고 이들의 상관관계를 검토하여 가장 효과적인 추출방법을 조사하였다. 당귀의 열수와 에탄올 추출물에서 각각 39.43와 37.75%로 다른 시료에 비하여 추출수율이 높았다. 폴리페놀 함량은 상백피 에탄올추출물에서 가장 높았고, 홍삼 초음파추출물에서 가장 낮았다. 플라보노이드 함량과 DPPH RC50 값은 상엽 에탄올 추출물에서 가장 우수하였다. α-Glucosidase저해활성은 상엽과 상백피 초음파추출물에서 높았고 추출방법 따른 저해율은 차이를 보였으나 세가지 추출물 모두 acarbose보다 높은 활성을 보였다. α-Amylase저해활성은 상엽 에탄올추출물에서 가장 높은 저해활성을 보였고, 상백피와 백복령 에탄올추출물에서도 acarbose와 비슷한 결과를 보였다. 항산화 활성과 항당뇨 저해활성의 상관성은 에탄올 추출물과 초음파 추출물에서 유의적인 상관관계를 보였다. 에탄올 추출물에서 α-glucosidase저해활성에 대한 폴리페놀 함량 또는 플라보노이드 함량과는 양의 상관관계(r<SUP>2</SUP>=0.792, 0.588)를 보였고, α-amylase저해활성에 대한 플라보노이드 함량 또는 DPPH 소거능의 RC<SUB>50</SUB> 값에서도 음의 상관관계(r<SUP>2</SUP>=0.918, ?0.594)를 나타내었다. 또한 α-amylase 저해활성과 α-glucosidase저해활성에서는 양의 상관관계(r<SUP>2</SUP>=0.746)를 보였다. 초음파 추출물에서는 α-glucosidase저해활성에 대해 폴리페놀 함량과 플라보노이드 함량에서만 양의 상관관계(r<SUP>2</SUP>=0.763, 0.618)를 보였다. 항산화능에서의 상관관계를 살펴 본 결과 열수, 에탄올 및 초음파추출물에서 모두 폴리페놀 함량과 플라보노이드 함량, DPPH RC<SUB>50</SUB> 값과 유의적으로 높은 양의 상관관계가 있는 것으로 나타났다. 결론적으로 약용식물이 추출방법에 따른 항산화, 항당뇨 활성과 상관성에 차이를 보이고 있음을 알 수 있었다. Korean traditional medicinal herbs have been reported to possess antioxidant and anti-hyperglycemic activities. We tested the antioxidant and anti-hyperglycemic activities of 6 kinds of medicinal herbs: Angelica gigas N., Poria cocos, Mori radicis Cortex, Mori folium, Aralia elata Cortex, and Panax ginseng, prepared as hot water, ethanol, and sonication extracts. The antioxidant activities of the extracts were examined by performing total polyphenol, total flavonoid, and α,α-diphenyl-β-picrylhydrazyl (DPPH) assays. For M. folium, the ethanol extract showed the strongest effects in DPPH radical scavenging activity among the three extraction methods. In addition, sonication extracts of M. radicis Cortex and M. folium showed the highest inhibitory activities for α-glucosidase among the different extracts. The ethanol extracts of M. folium had the highest inhibition effects against α-amylase. A direct correlation between antioxidant and anti-hyperglycemic inhibition activity was found in the ethanol and sonication extracts. From the results, it is considered that these six medicinal herbal extracts have antioxidative, anti-hyperglycemic, and correlation effects based on different extraction methods.

        마이크로웨이브로 추출한 용매별 치자(Gardeniae Fructus) 추출물의 생리활성

        추출용매를 달리하여 마이크로웨이브로 추출한 치자의 생리활성을 측정하였다. 치자를 물, 50% 및 100% 에탄올을 용매로 하여 마이크로웨이브로 추출하였고, 이의 생리활성을 측정하였다. 세 종류의 추출용매 중 50% 에탄올 추출구의 생리활성이 다른 추출구에 비해 높은 경향이었으며 100% 에탄올 추출구의 활성은 낮은 편이었다. 전자공여능은 0.6 ㎎/mL 농도일 때 50% 에탄올 추출물의 활성이 96.43±0.25%로 0.1, 1% L-ascorbic acid와 유사한 활성이 있었고, 아질산염 소거능은 71.99±3.71%로 0.1, 1% L-ascorbic acid에는 못 미쳤지만 0.01% L-ascorbic acid보다 우수한 활성이 있었다. 티로시나아제 저해활성은 11~19%로 모든 추출물에서 20% 미만의 낮은 활성을 보였고 물과 50% 에탄올 추출구는 농도에 반비례하여 활성이 증가하는 양상이었다. SOD 유사활성도 티로시나아제 저해활성과 유사한 결과로 모든 추출구가 농도 증가에 따라 활성이 감소하였으며 전체적으로 40% 미만의 활성이 있었다. 결론적으로 치자 추출물은 SOD 유사활성 및 티로시나아제 저해 효과는 높지 않지만 우수한 전자공여능과 아질산염 소거능이 있는 것으로 판단되므로 천연 항산화제로서 이용될 수 있는 가능성이 있었다. Gardeniae Fructus (GF) is a fruit of Gardenia jasminoides Ellis (Rubiaceae) which has been used in traditional medicine. It contains not only geniposide but also resveratrol, a kind of stilbene as a natural antioxidant. In this study, we investigated physiological activities of GF extracts by measuring electron donating ability (EDA), nitrite scavenging ability (NSA), super oxide dismutase (SOD)-like activity and inhibitory effect of tyrosinase activity (ITA). As a solvent, it was extracted with water, 50 and 100% ethanol using microwave. At the concentration of 6 ㎎/mL, EDA was the highest at 50% ethanol extract but the lowest at 100% ethanol extract, 96.43±0.25% and 77.06±0.22% respectively. NSA showed the strongest activity at 50% ethanol extract like EDA (6 ㎎/mL: 75.26±0.28%). ITAs were below 20% in all samples and their activities reduced according to increasing concentration. Likewise, SOD-like activities decreased with increasing concentration of extracts in all samples. In conclusion, GF extracts showed low SOD-like activities and ITAs but EDAs and NSAs were fairly high; especially EDA of 50% ethanol extract (6 ㎎/mL) was high as those of 0.1 and 1% L-ascorbic acid. Therefore the results suggest that GF extracts may be useful as potential sources of natural antioxidants.

        에탄올 농도에 따른 머루(Wild Grape, Vitis coignetiae) 추출물의 폴리페놀 함량 및 항산화 활성

        추출용매를 달리하여 마이크로웨이브로 추출한 머루의 생리활성을 측정하였다. 전자공여능, 아질산염 소거능은 모든 추출구에서 농도 의존적으로 활성이 증가하였고 폴리페놀도 농도 증가에 따라 함량이 높게 나타났다. 세 가지 추출 용매 중 50% 에탄올 추출 시 항산화 활성이 높았는데 이는 총 폴리페놀 함량 측정 결과와도 일치하여 머루의 항산화활성이 폴리페놀과 밀접한 관련이 있다고 사료된다. 전반적인 활성은 전자공여능이 약 13~101%로 특히 1.6 g/dL 농도의 50% 에탄올 추출물의 활성의 경우 1% L-ascorbic acid보다 우수했다. 아질산염 소거능은 pH가 낮을수록 활성이 높은 경향으로 pH 1.2에서 활성이 34~99%로 가장 높게 측정 되었다. 총 폴리페놀 함량은 농도에 따른 폴리페놀 함량의 차이가 컸으며 50% 에탄올 추출, 1.6 g/dL 농도에서 약 38.76 mM gallic acid equivalent로 가장 높았다. SOD 유사활성은 30~50%로 전자공여능이나 아질산염 소거능만큼 우수하진 않았지만 0.01% L-ascorbic acid보다 50%, 100% 에탄올 추출구에서 높은 활성을 보였다. 머루의 항산화 활성과 폴리페놀 함량은 밀접한 관련이 있는 것으로 보이며 폴리페놀 함량이 풍부한 머루를 이용한 항산화 건강기능성 식품 개발의 기초 자료로 제공하고자 하였다. As an oriental medicinal plant, wild grape (WG, Vitis coignetiae) has been known to contain abundant nutrients compared to grape (Vitis vinifera L.) and as a source of stilbenes, a kind of polyphenol. This study was performed to investigate the antioxidative activity of WG extracts by measuring electron donating ability (EDA), nitrite scavenging ability (NSA), super oxide dismutase (SOD)-like activity and total polyphenol content (TPC). The extracts were obtained using microwave-assisted extraction (microwave power: 90 W, extraction time: 5 min, extractant: water, 50 and 100% ethanol). EDAs, SOD-like activities and TPCs were the highest at 50% ethanol extracts while conversely lowest at 100% ethanol extracts. EDAs of 50% and 100% ethanol extracts at 1.6 g/dL concentration, 97.70±0.55 and 98.05±0.36%, were higher than those of 0.1 and 1% L-ascorbic acids, 101.44±0.98% and 99.43±0.78%, respectively. At the concentration of 1.6 g/dL, 50% ethanol extract showed lower NSA (pH 1.2) than water extract unlike EDA, TPC and SOD-like activity. Regarding TPCs of WG extracts, the activities were the highest at 50% ethanol extracts (1.6 g/dL: 38.76±0.23 mM gallic acid equiv.) followed by water and 100% ethanol extracts. The results suggest the usefulness of developing functional foods using antioxidative active compounds of WG with high polyphenol contents.
