        피부밑이마당김술

        Purpose: The purpose is to present an useful and simple surgical method to improve the aging of upper third face in patients with high frontal hairline as well as low frontal hairline. Methods: Forty eight female patients were treated with subcutaneous forehead lift using an anterior hairline incision over 14 years. This surgical technique is performed under direct vision utilizing a beveled incision made 4 to 5 mm into the anterior hairline with subcutaneous dissection, which is continued near to eyebrow, sometimes extended to supraorbital rim to remove corrugator and procerus muscles. In patients with high frontal hairline, excess forehead skin anterior to incision line is removed. On the contrary in the patients with low frontal hairline, scalp posterior to incision line is removed. Results: This technique provided constant and good results with the forty six patients, who were satisfied with eyebrow elevation and removal of wrinkles in forehead and glabellar region. However two patients were undercorrected, and focal alopecia developed in another two patients. One patient complained of pruritus over one year, but subsided spontaneously without any treatment. Temporary paresthesia developed in the forehead and frontal scalp of all cases after operation but permanent sensory loss never occurred in all the patients. Conclusion: Subcutaneous forehead lift using an anterior hairline incision is suggested to be one of the effective surgical methods to improve the aging of upper third face in the patients with high frontal hairline as well as low frontal hairline.

      • Cytotoxic Effect of Adriamycin in Cultured Skin Cells of Fetal Rat

        Adriamycin (Doxorubicin HCl)의 혈관밖 유출에 따른 조직의 손상, 특히 피부괴양 및 괴사 기전을 규명하기 위한 연구의 일환으로 흰쥐 피부세포를 이용한 in vitro 실험에서 adriamycin에 의한 산소라디칼 생성 및 그와 관련된 세포독성 기전으로 지질과산화를 검토하였다. Adriamycin은 흰쥐 태자 피부의 배양세포에서 lactic dehydrogenase(LDH) 유리를 용량 및 시간 의존적으로 증가 시켰으며, NADPH 및 NADH 첨가 조건에서 $superoxide\;anion(O^-\;_2{\cdot})$ 생성을 현저히 증가시켰다. Adriamycin은 지질과산화 반응의 척도인 malondialdehyde(MDA) 생성을 역시 NADPH, NADH 존재하에서 용량의존적으로 증가시켰고, 산소라디칼 제거물질들인 superoxide dismutase (SOD), catalase 및 thiourea와 항산화물질인 butylated hydroxytoluene(BHT), ${\alpha}-tocopherol$은 MDA 생성증가를 현저히 억제하였다. 1, 3,-bis(2-chloroethyl)-1-nitrosourea(BCNU)를 처리하여 산화성 공격에 대한 방어기전의 하나인 glutahione 체계를 억제할 경우 adriamycin에 의한 MDA 생성은 더욱 현저히 증가하였고, 이는 역시 항산화 물질들에 의하여 억제되었다. 이상의 연구성적에서 adriamycin은 산소라디칼 생성의 증가와 그에 따른 지질과산화를 촉진하므로서 피부세포에 손상을 줄 것으로 사료되었다. Local extravasation during intravenous administration of adriamycin (doxorubicin HCl) can cause severe skin ulceration and necrosis. To investigate the mechanism of adriamycin-induced skin toxicity, effects of adriamycin on reactive oxygen radical metabolism using cultured skin cells of fetal rat. Adriamycin produced significant release of lactic dehydrogenase from cultured skin cell preparations dose- and time-dependently. The production of superoxide anion in sonicated suspensions of cultured skin cells was significantly increased by adriamycin under the presence of NADPH and NADH. The drug also stimulated malondialdehyde (MDA) production, an index of lipid peroxidation, in NADPH- and NADH-supported cell preparations. The increased production of MDA was significantly inhibited by oxygen radical scavengers (superoxide dismutase, catalase, thiourea) and antioxidants (butylated hydroxytoluene, ${\alpha}-tocopherol$). Treatment of cultured skin cells with 1, 3,-bis (2-chloroethyl)-1-nitrosourea (BCNU), an inhibitor of glutathione reductase, enhanced the lipid peroxidation induced by adriamycin. The present study suggests that lipid peroxidation which is resulted from the stimulated production of reactive oxygen radical causes cellular damage in adriamycin-treated skin cells of rat.

        비특이성안와염에서 고용량 부신피질호르몬 주사요법의 효과

        Purpose: To evaluate the efficacy of intravenous methylprednisolone pulse therapy in patients with idiopathic orbital inflammation. Methods: Fifty-two patients who received three-day pulse methylprednisolone for acute severe swelling, loss of visual acuity, limitation of eye movement, diplopia, or pain were included in the present study. The lesions were divided into five subtypes using computed tomography or magnetic resonance imaging; anterior, lacrimal, myositic, diffuse and apical. A case diagnosed as sclerotic type was excluded. The response rate, recurrence rate and cure rate were assessed. Results: The total response rate to intravenous methylprednisolone from all groups was 84.6%, with 100% in the anterior subtype and 71.4% in the diffuse subtype. During the mean follow-up period of 10.5 months, 15.9% of patients experienced a recurrence after a mean of 5.7 months. Overall, the cure rate was 71.2%. Recurrence was more common in the younger patients, and three of the seven patients with recurrence had myositic inflammation. Conclusions: Intravenous methylprednisolone was effective in patients with severe idiopathic orbital inflammation. However, combining other treatment modalities in patients with diffuse or myositic subtypes and in younger patients who show relatively low response or high recurrence is recommended.         아래눈꺼풀 미용성형술 후 발생한 눈꺼풀속말림에 대한 연구

        목적: 아래눈꺼풀 미용성형술 후 눈꺼풀속말림이 발생한 환자 34명이 관찰되어 이를 수술적으로 치료하였기에 보고하고자 한다. 대상과 방법: 2015년부터 2021년까지 우리연세안과의원에서 눈꺼풀속말림을 진단받은 환자 중 아래눈꺼풀의 미용성형술을 받은 과거력이 있는 환자 34명을 대상으로 하였다. 환자는 모두 여성이었으며, 이들의 평균 연령은 30.6세였다. 우리연세안과의원 방문 전 받은아래눈꺼풀 미용성형술은 필러주사, 밑트임, 밑트임 및 뒤트임, 혹은 아래눈꺼풀성형술이었다. 결과: 대상 환자들은 눈꺼풀속말림 외에 눈꺼풀뒤당김, 아래눈꺼풀 상방 이동제한이 나타났으며, 정도에 따라 grade I부터 grade IV까지 분류하였다. 모든 환자에서 Hotz법 수술을 시행하였으며, 심한 정도에 따라 필러제거술, 아래눈꺼풀당김기후전술, 경구개점막이식술 등을 추가적으로 시행하였다. 결론: 우리나라 성형외과계에서는 눈을 크게 보이고 싶은 환자들에게 밑트임 혹은 뒤트임이라는 수술로 아래눈꺼풀 미용성형술을시행하고 있다. 이번 보고에서는 아래눈꺼풀 미용성형술 후 합병증으로 많은 수의 환자에서 눈꺼풀속말림이 발생하여 수술이 필요하였다. 이러한 부작용이 발생하는 것을 방지하기 위해서는 수술 전 아래눈꺼풀의 이완 정도, 눈꺼풀틈새의 크기 그리고 형태학적 문제등 눈 상태에 대한 충분한 사전검사가 필요할 것으로 보이며, 환자들에게도 수술의 필요성과 수술 후의 부작용에 대한 충분한 설명이 필요할 것으로 보인다. Purpose: Cosmetic procedures for lower eyelids are popular in the field of plastic surgery for patients who want to make their eyes look bigger in Korea. We have experienced patients who developed entropion after the lower eyelid cosmetic procedure and treated it surgically, and therefore we would like to report on these cases. Methods: From 2015 to 2021, among all patients diagnosed as having entropion at Woori Yonsei eye clinic, 34 patients with a history of lower eyelid plastic procedure were included in this study. Results: All patients were female and their mean age was 30.6 years. We classified subjects from grade I to grade IV according to the degree of resistance to the lower eyelid retraction and lower eyelid upward movement restriction. The Hotz operation was performed in all patients. Filler removal, lower eyelid retraction recession, or palate mucosal transplantation were additionally performed depending on the severity of entropion. Conclusions: After the cosmetic procedure of lower eyelid, some patients had entropion as a complication and required surgery. To prevent these complications, sufficient pre-examination of eye conditions such as the degree of relaxation of the lower eyelid, size of eyelid fissure, morphological problems, and reevaluation of surgical technique are required before the procedure starts. In addition, it seems the patients need to provide with sufficient explanation about the necessity of the procedure and its side effects.
