        청각장애 영유아의 청각언어인지재활(KARI)을 이용한 조기 중재 효과 연구

        Purpose: The present study was aimed to identify the effect of early intervention using Korean Auditory, language, and cognitive Rehabilitation for Infants (KARI) program for the hearing-impaired infants. Methods: The KARI program composed with the evaluation materials, parent education and counseling materials, professional guidelines were applied to 12 infants from 1 to 24 months old once a week. The evaluation materials were Korean version of LittlEARS¢ç Auditory Questionnaire (LEAQ-K), Categories of Auditory Performance (CAP), Infant-Toddler Auditory & Communicative Behavioral Checklist (IT-ACBC), Sequenced Language Scale for Infants (SELSI), Korean MacArthur-Bates Communicative Development Inventories (K MB CDI), and Communication and Symbolic Behavior Scales: Developmental Profile (CSBS DP). Results: The scores of LEAQ-K, CAP, and IT-ACBC were enhanced from 23.27 to 27.55, 4.14 to 4.36, and 48.05 to 61.53. SELSI scores in receptive and expressive languages increased from 24.76 and 19.59 to 31.12 and 24.41. K MB CDI scores increased in expressive and receptive vocabularies and gesture and play from 10.00, 62.60 and 23.60 to 59.00, 111.00 and 47.80. CSBS DP scores increased from 37.50 to 46.71. After applying KARI, the early intervention effects were shown in scores of the evaluation materials with the statistical significances (p < 0.05) in LEAQ-K, IT-ACBC, SELSI, receptive vocabulary in K MB CDI, and CSBS DP checklist. Conclusion: The current results support that KARI is an effective rehabilitation tool for the early intervention to the hearing-impaired infants. When given the suitable rehabilitation at the early stage, the hearing-impaired infants can develop the early communication skills near to normal range.

        애착육아와 청각언어인지 재활

        This study was aimed to review the history and definition of attachment parenting and to identify the types and merits of it and attachment plays based on attachment parenting. The early rehabilitation is essential to infants and toddlers with a congenital hearing loss for auditory and language development. Attachment parenting refers to close ties between parent and child based on skinship and emotional exchange. There are four different types of attachment: secure, insecure-avoidant, insecure-resistant, and insecure-disorganized. Among them, secure attachment is the optimal type through which an infant could learn to trust mother and maintain emotional stability even when the mother was absent. In Korea, the use ondol and podaegi, which can create attachment naturally had been used traditionally. Further Dandongsibhoon, a traditional play, is in accordance with the definition of attachment parenting. Therefore, Korean method of child rearing is proper for attachment parenting. It was known that attachment parenting enhanced self-esteem, increased independence, promoted social skills, and facilitated brain and language development, so that it could be used as a method of rehabilitation related to the development of auditory, language, and cognitive abilities. Recently developed the Korean Aural Rehabilitation for Infants integrated many attachment parenting means and sought to raise the efficacy of early rehabilitation. Conclusively, we hope that early rehabilitation applying attachment parenting services in Korea could be expanded so that even infants and toddlers with congenital hearing loss would grow up without having to go through many hearing-impairment problems.

        Early Communicative Skills of Children with Normal Hearing and Hearing Loss Depending on Chronological and Hearing Ages

        배경 및 목적: 말, 언어 평가는 건청인과 난청인의 발달 과정을 확인하는데 중요한 역할을 한다. 본 연구는 건청과 난청 영유아의 초기 의사소통 기술 특성을 분석하고 이를 생활연령과 듣기연령에 따라 비교하고자 하였다. 방법: 4-56개월 사이의 건청 영유아 26명과 난청 영유아 24명으로 총 50명의 영유아와 부모를 대상으로 연구를 실시하였다. 검사도구로 한국판 맥아더-베이츠 의사소통 발달평가, 영유아 언어발달검사, 의사소통 및 상징행동 척도: 발달 프로파일의 체크리스트와 행동 샘플을 사용하여 초기 의사소통 기술을 2-3개월마다 정기적으로 평가하고 생활연령과 듣기연령으로 구분하여 분석하였다. 건청 영유아와 난청 영유아의 평균 생활연령은 각각 17과 19개월이었으며 난청 영유아의 평균 듣기연령은 13개월이었다. 난청 영유아는 평균적으로 생후 9개월부터 청각 보조기기의 사용을 포함한 적절한 중재를 받기 시작하였다. 결과: 듣기연령으로 분석했을 때 난청 영유아 집단은 건청 영유아 집단보다 일반적으로 높은 점수를 보였다. 난청 영유아 집단은 생활연령을 기준으로 한 경우보다 듣기연령을 기준으로 하였을 때 모든 평가 결과에서 유의하게 높은 점수를 나타냈다(p<.05). 논의 및 결론: 결론적으로 난청 영유아의 초기 의사소통 기술 향상을 위한 청각 증폭기기 착용과 적절한 조기 중재의 효과를 평가하고 분석할 때 난청 영유아의 우수한 수행능력 결과는 듣기연령 사용의 효율성을 시사하였다. Objectives: The evaluation of speech and language is crucial for children with normal hearing (NH) and hearing loss (HL). The aim of this study was to investigate the early communicative skills of children with NH and HL and to compare the performance depending on chronological age (CA) and hearing age (HA). Methods: Four tools, Korean MacArthur-Bates Communicative Development Inventories (K M-B CDI), Sequenced Language Scale for Infants (SELSI), Communication and Symbolic Behavior Scales Developmental Profile (CSBS DP) checklist and behavior sample were used. The data was collected from 50 children, 26 NH and 24 HL from 4 to 56 months of age. The average age of NH and HL groups was between 17- and 19-months-old, respectively. The HA of the HL group was 13-months-old. They were tested every two to three months. The developmental progress was analyzed by CA and HA. Results: With K M-B CDI, when CA and HA were compared for the HL group, the production of expressive language did not show any difference, but the receptive language and action-gesture scores were significantly better in HA at 7-9 months and 19-24 months. With SELSI, both receptive and expressive languages showed significantly better scores in HA (F(1,73)=0.580, p<.05, F(1,73)=11.036, p<.05). With CSBS DP, the HA showed significantly better social scores for checklist (F(1,63)=6.248, p<.05). Conclusion: As the HA of the HL group showed superior performance compared to the corresponding age of the NH group even in the early stage, the effectiveness of HA following fitting of amplification device and the appropriate intervention has been implicated.
