A 31-year-old female visited O.P.D. of internal medicine, Kyung Hee Medical center, with the chief complaint of generalized weakness and headache. She took a routine chest P-A and there was a 6.5 x 7.5 cm sized round mass in the right middle lung field. She admitted to the Dept. of Thoracic and Cardiovascular Surgery under the impression of malignant bronchogenic carcinoma with the remit of bronchoscopy and sputum cytology and tomogram. Middle and lower lobectomy of right lung was performed and postoperative surgical biopsy revealed out the plasma cell granuloma. The plasma cell granuloma may occur as a solitary nodule in the lung or be associated with systemic disease, plasma protein imbalance, or nonspecific local inflammatory reaction . More than two third of the reported patients were less than 30 year of age. Grossly the lesion appears reddish-brown and microscopic features include pallisade or a cartwheel distribution of plasma cells with Russel bodies and amyloid. Local excision or lobectomy has been curative in most cases.

Chest injuries due to blunt trauma often result in severe derangements that lead to death. And we have to diagnose and treat the patients who have blunt chest trauma immediately and appropriately. A clinical analysis was made on 324 cases of chest injury due to blunt trauma experienced at department of Thoracic and Cardiovascular Surgery, College of Medicine, Kyung Hee University during 8-year period from 1972 to 1979. Of 324 patients of blunt chest injuries, there were 189 cases of rib fracture, 121 of hemothorax or/and pneumothorax, 108 of soft tissue injury of the chest wall only, 41 of lung contusion, 24 of flail chest, 13 of scapular fracture, 7 of diaphragmatic rupture and others. The majority of blunt chest injury patients were traffic accident victims and falls accounted for the next largest group of accidents. Chest injuries were frequently encountered in the age group between 3rd decade and 4th decade [60%] and 238 patients were male comparing to 86 of female [Male: Female = 3:1 ]. In the patients who have the more number of fractured ribs, the more incidence of intrathoracic injury and intraabdominal organ damage were found. The principal associated injuries were head injury on 58 cases, long bone fractures on 37, skull fractures on 12, pelvic fractures on 10, renal injuries on 6 and intraabdominal organ injuries on 5 patients. The principle of early treatment of chest injury due to blunt trauma were rapid reexpansion of the lung by closed thoracotomy which was indicated on 96 cases, but open thoractomy was necessary on 14 cases because massive bleeding, intrapleural hematoma and/or fibrothorax, or diaphragmatic laceration-On 15 cases who were young and have multiple rib fracture with severe dislocation delayed elective open reduction of the fractured ribs with wire was done on the purpose of preserving normal active life. The over all mortality was 2.8% [9 of 324 cases] due to head injury on 3 cases, massive bleeding on 2,wet lung syndrome, acute renal failure on 1 and septicemia on 1 patient.

Lymphoid hamartomas are a rare benign disease which can be easily treated by complete surgical excision. They developed most often in the thorax, and can be discovered usually on routine chest X-ray.p But some of them can also be found because of pressure symptoms or the presence of a palpable mass if outside the thorax. We experienced a case of the hyaline-vascular type of lymphoid hamartoma in the left hilum of a 29 year-old Korean male in March, 1979. He was well except intermittent cough and left chest discomfort of a year duration and was treated by resection of the left upper lobe including nodular tumor masses. The histological characteristics were an aggregation of lymphoid follicles composed of concentrically arranged mature lymphocytes with centrally placed thick walled arterioles showing endothelial proliferation and some of them were hyalinized. Between the follicles, there was extensive capillary proliferation and infiltration of numerous lymphocytes, scanty plasma cells and eosinophils.
흰 쥐 적출 심장에서 비작업성 관류 회로를 이용한 인삼 성분 Ginsenoside Rg1 Mixture의 심근 보호 효과에 관한 실험적 연구

최근 심장 분야 수술의 발달로 여러 가지 고난도의 심장 수술과 심장 이식술의 시행이 증가하고 있으며, 술 후 예후에 크게 영향을 주는 심장의 심근 손상 방지에 대한 다각적인 연구가 행해지고 있는데, 수술 및 이식 전후의 허혈기와 재관류시 발생할 수 있는 심근 손상을 최소화하고, 술 후 심근 기능의 조속한 회복을 위한 목적으로 여러 약제 및 방법을 제시하고 있다. 한편 한국에서는 오래 전 부터 만병 통치의 영약으로 전해져 오고 있는 인삼을 이용한 동물 실험 및 임상 경험을 통해 성분 효과에 대한 여러 결과가 보고되고 있고, 심장 기능에 대한 효과도 약리학적 측면에서 많은 결과가 발표되었다. 그런데 여러 분획 추출물 중 ginsenoside Rg1 mixtures에 대해서는 그 결과가 다소 미비한 상태이고 ginsenoside Rg1을 이용하여 심근의 허혈 후 재관류 시행 10분 및 지속적 관류 상태에서의 심근 손상에 대한 심근 보호 정도를 혈역학적 지표 및 관상 혈류를 통한 관류액의 효소치를 측정하여 실험한 결과 심근 허혈 및 재관류 후 심근 손상 방지와 심근 기능 회복에 효과가 있다고 판단되며 향후 약제의 투여 용량에 따른 심근 보호 정도에 관한 실험이 필요할 것으로 사료되고, 인삼 성분 각 분획의 복합 투여에 의한 결과도 재차 확인하여야 할 것으로 생각된다. Panax Ginseng C.A. Meyer has been known for hundreds of years as the most valuable drug having mysterious effects among all the herbal medicines and plants in Korea. Also, many experimental studies have been performed recently that the various effects were identified and applied clinically. So we attempted an experimental study on the effect of ginsenoside Rgl mixtures in an isolated rat heart with the use of the Langendorff model. The objective of this study was to determine whether this ginsenoside Rgl mixtures would protect the myocardial injury after ischemic arrest and reperfusion. Isolated rat hearts were allowed to equilibrate for 20 minutes and were then subjected to 15 minutes of normothermic ischemia. After this ischemic period, isolated rat hearts were allowed to reperfusion for 10minutes(Ischemic Group). In other group, isolated rat hearts were perfused for 60 minutes continuously with normothermia (Normothermic Group). Hemodynamic and biochemical parameters such as heart rate, left ventricular pressure +dp/dt max, coronary blood flow and cardiac enzymes were measured during initial perfusion, ischemia, reperfusion period (Ischemic group) and 20, 40 and 60 minutes after continuous perfusion(Normothermic group). After completion of the experiment, this data was evaluated and the following results were obtained. 1. Heart rates showered an increase in both ischemic and normothermic experimental groups, but statistically significant differences were not identified. 2. LVP(Left Ventricular Pressure) showed statistically significant differences in both ischemic and normothermic experimental groups(p<0.005, p<0.01). 3. +dp/dt max showed statistically significant differences in both ischemic and normothermic experimental groups(p<0.01, p<0.01). 4. There were no statistically significant differences in coronary blood flow and cardiac cenzymes in all groups, but experimental groups seemed to have better protection and recovery. These results suggest that ginsenoside Rgl mixture has a protective effect on the myocardial injury after ischemia and reperfusion.
흉부자상에 의한 심실중격결손 및 승모판 부전증 치험 1예

A 34-year-old man was stabbed with knife in the left fourth intercostal space. He was admitted to Kyung Hee University Hospital Emergency room with Cardiac tamponade state. Through a left anterior thoracotomy, a tense hemopercardium was released. A 2㎝ wound of right ventricle was sutured with 3-0 prolein. Seven month after the injury he was readmitted for cardiac catheterization. Cardiac catheterization confirmed the presence of an interventricular septal defect in the membranous septum with left ventricle to right ventricle shunt. A laceration of the septal leflet of mitral valve was also demonstrated. The pulmonary blood flow as 1.22 times the systemic flow. A longitudinal incision was made on the right atrium on the cardiopulmonary bypass. An 1×0.5㎝ interventricular septal defect was found in the membranous septum. In order to achieve and adequate repair interventricular spetal defect and mitral insufficency. The ventricular septal defect was repaired 3-0 prolein plegeted suture with interrupted. Artifical atrial septal defect was made and the septal leflet defect(1.5㎝) was reattached with 3-0 prolein plegeted suture interrupted. The partient had and uneventful post operative recovery and was discharged the Hospital on the 24day post operative day.
