Intramural duodenal hematoma is an uncommon condition, which usually develops after blunt abdominal trauma. It is also reported as a complication of anticoagulant therapy, blood dyscrasia, pancreatic disease, and diagnostic and therapeutic endoscopy. The typical clinical pictures of intramural duodenal hematoma consist of upper abdominal pain, vomiting, fever, and hematochezia, and it is rarely accompanied by intestinal obstruction, peritonitis, and pancreatitis as its complication. We report a case of intramural duodenal hematoma extended to peritoneal cavity, and accompanied by acute pancreatitis following therapeutic endoscopy for duodenal ulcer bleeding in a 32-year-old man who was on maintenance of anti-coagulation therapy after valvular heart surgery. CAE를 이용한 파워트레인의 방사소음 저감을 위한 구조변경

One of the key elements in efforts to minimize noise radiation from a powertrain is the knowledge of the main radiating component and the relation between the surface vibration of a powertrain and the sound pressure. In this research, the powertrain model is developed based on FEM(finite element method). This model is applied to the prediction of the vibration of a powertrain by using ADAMS and the radiation noise by using BEM(boundary element method). According to this numerical analysis, the surface vibration of a powertrain is investigated as a source of radiated noise. This surface vibration is caused by the 1st order natural vibration of the cylinder block and its mode shape is the torsion mode. Therefore, this mode shape is modified to reduce the surface vibration of the powertrain. The radiation noise of the modified powertrain is also reduced to $5{\sim}12\;dB$. This modification is very successful for the noise reduction based on the CAE technology.

The blower which is installed in a FCEV(fuel cell electric vehicle) may cause noise due to misalignment and unbalance of mechanical components that rotate at high speed. One of the key points in efforts to minimize the noise radiation from a blower is the knowledge of the main radiating component and the relation between the surface vibration of a blower and the sound pressure. In this research, the blower model is developed based on FEM(finite element method). FE(finite element) model is reliable by correlation of frequencies and MAC(modal assurance criterion) values between EMA(experimental modal analysis) and FEA(finite element analysis). This model is applied to predict the vibration of a blower by using inverse force identification method and predict the radiating noise by using BEM(boundary element method). Comparing the frequencies of resonance and those mode shapes between EMA and FEA, a structural modification of the FE model is evaluated for reducing the parameters of the blower noise.

현재의 모바일 RFID 서비스는 RFID 리더가 장착된 휴대폰로 누구나 태그에 대한 접근이 가능하므로, 태그 정보가 불법적으로 수집될 수 있으며 추적이 가능하다. 특히 PAN을 구성하는 기본장치 뿐만 아니라 사용자의 소지품에 부착된 각종 RFID에 의한 정보 유출 및 사생활 침해가 큰 문제가 될 것으로 우려된다. 왜냐하면, 기존의 고정형 RFID의 경우, 리더기는 항상 고정되어 있고(예를 들면, 상점의 계산대), RFID 태그만 이동되었으나, 모바일 RFID의 경우는 “휴대폰=RFID 리더기”이므로 사생활 침해 요소가 더욱 심각해지게 된다. 따라서, 본 논문에서는 선택적인 태그 블록킹과 인가된 리더에게는 태그를 대신하여 모바일 리더가 응답하는 프락시 기능을 가진 모바일 RFID 프락시 시스템을 설계하고, 제안 시스템의 기능 및 구조적 특징을 기존 기법들과 비교 분석한다.

Nephrotic syndrome is a relatively rare complication of malignancy. A few cases of nephrotic syndrome accompanying Hodgkin`s disease, non-Hodgkin lymphoma, leukemia and other malignancies have been reported since the first case of the nephrotic syndrome associated with extrarenal malignancy was reported in 1922. Hodgkin`s disease and solid tumors are known to be the most common malignancies accompanying nephrotic syndrome. The pathologic findings of kidney in patients with Hodgkin`s disease commonly show minimal change nephropathy. Membranous glomerulonephropathy is the most common pathologic feature in patients with solid tumors. Although membranous glomerulonephropathy related to small cell lung cancer has rarely been reported in Korea, minimal change nephropathy accompanying small cell lung cancer has never been reported. We present here a case of a 70 year-old male with minimal change nephropathy that was related to small cell lung cancer. We detected small cell lung cancer during the diagnosis work-up of nephrotic syndrome. We suggest that nephrotic syndrome can be a manifestation of underlying malignancy.

Anterior ischemic optic neuropathy (AION) is rarely reported in end stage renal disease. In particular, AION combined with sudden sensorineural hearing loss (SNHL) has not been reported in domestic and international journals. Here we report the first case of this combined condition. A 58-year-old male patient in end-stage renal disease presented on painless bilateral visual and hearing deterioration. Clinical findings and imaging studies were compatible with a diagnosis of AION and SNHL. Despite of high-dose steroid therapy, vision and hearing were not significantly recovered. Uremic patients often have coexisting pathology such as hypotension during dialysis, atherosclerosis and anemia, predisposing to AION and SNHL. We describe a patient of dialysis for 10 years who presented with bilateral visual and hearing loss due to complications of long-term dialysis. Our case suggests that nephrologists have to know and treat more carefully this rare but emergent complications of dialysis.

뇌졸중이란 뇌혈류 차단으로 산소와 영양공급이 막혀 뇌조직 일부의 손상이 초래되는 질병이다. 뇌졸중에 의한 사망률은 감소하는 추세지만 후유장애, 특히 기능장애로 불편함을 호소하는 환자는 늘어나고 있다. 그러나 뇌졸중 환자를 대상으로 한 연구만 지속될 뿐, 재발 환자를 포함하지 않은 초발 뇌졸중 환자를 대상으로 한 연구는 아직 부족한 실정이다. 이에 본 연구는 한국판 수정 바델 지수(K-MBI)라 불리는 재활 분야에서 흔하게 사용하는 뇌졸중 환자의 일상생활활동 평가 도구를 기능점수의 기준으로 사용해 초발 뇌졸중 기능에 영향을 미치는 요인을 규명하고 궁극적으로 보건의료 정책에 도움을 주고자 했다. 본 연구에서 사용된 자료는 각 개체에 대한 반복적인 측정값으로 이루어진 종단자료로 상관성을 고려하는 분석 방법이 필요해 다수준 분석을 시행하였다. 일반화 추정 방정식 모형(GEE)으로는 모든 변수가 유의하게 나왔지만 다수준 모형은 배우자 유무(미혼보다 기혼), 입원 기간(짧을수록), 나이(적을수록)에 따라 기능점수 회복이 유의미했다(p-value<0.05). 또한, 초기 뇌졸중 중증도에 따른 기능점수의 차이가 있음을 확인했다. 따라서 본 연구는 초발 뇌졸중 환자의 기능향상을 위해서 초기 뇌졸중 중증도에 따라 다른 의료적 접근의 필요성을 제시한다는 점에서 의의가 있다. Stroke is a disease in which oxygen and nutrition are blocked by blocking cerebral blood flow, causing damage to some of the brain tissue. The mortality rate from stroke is decreasing, but the number of patients complaining of discomfort due to aftereffects, especially dysfunction, is increasing. However, only studies on stroke patients continue, and studies on initial stroke patients that do not include recurrence patients are still insufficient. Accordingly, this study tried to identify factors affecting initial stroke function and ultimately help health care policies by using a tool for evaluating daily life activities of stroke patients commonly used in the rehabilitation field called the Korean version of the K-MBI. The data used in this study is longitudinal data consisting of repeated measurements for each object, and a multi-level analysis was conducted because an analysis method that considers correlation was needed. All variables were significant in the generalized estimation equation model (GEE), but the majority model showed significant functional score recovery depending on the presence of a spouse (married than unmarried), hospitalization period (shorter), and age (less); p-value<0.05. In addition, it was confirmed that there was a difference in function scores according to the severity of the initial stroke. Therefore, this study is meaningful in that it presents the need for a different medical approach depending on the severity of the initial stroke to improve the function of patients with initial stroke.
