Amyloidosis is a disease that characterized by accumulation of an amorphous, proteinaceous materials in the various tissues and organs, but its origin is unknown. Recent clinical study showed that incidence of amyloidosis in rheumatoid arthritis was near 10% to 21%. Secondary amyloidosis is caused by accumulation of serum amyloid. A which is doing the acute phase behavior faster than C reactive protein. Symptoms of amyloidosis are various according to the involved organ. Gastrointestinal symptoms are obstruction, ulcer, malabsorption, and bleeding. Diarrhea is very intractable with the conventional antidiarrheal agent, the mechanism of that is infiltration of amyloid material in the intestinal myenteric plexus, sympathetic nerve and ganglion. We report a case of 65 years old woman with rheumatoid arthritis presented with abdominal pain, intractable diarrhea which was not controlled by antidiarrheal agents. Duration of arthritis is only 2 years. Immunohistochemical stain showed AA type which meant secondary form. We treated with Octreotide analogue and total parenteral nutrition for chronic diarrhea.
콤팩트 안테나 테스트 레인지용 경면수정 오프셋 복반사경 시스템의 설계

경면수정 오프셋 그레고리안 반사경 안테나는 고이득, 저 사이드로브 특성 때문에 위성 통신 시스템에 널리 사용되고 있다. 그러나 본 논문에서는 CATR(Compact Antenna Test Region)을 위한 반사경 시스템으로 설계하고 그 근접 전계 특성을 조사하였다. CATR은 시험 영역에 진폭과 위상 리플이 작고 교차 편파가 낮은 균일 평면파를 제공하여야 한다. 따라서 근접 전계를 개구면의 전력 분포, 급전 혼의 복사 패턴, 개구면과 시험 영역까지의 거리 변화에 따라 계산하여 나타냈다. 또한, 30 ㎓에서 동작하는 경면수정 오프셋 복반사경을 제작하고 근접전계를 측정하였다. 그 결과, 측정치와 계산치가 비슷하게 나타났다. 이로서 본 논문에서 설계된 경면수정 오프셋 복반사경은 콤팩트 안테나 테스트 레인지로 사용될 수 있을 것으로 생각된다. Gregorian offset dual-shaped reflector antennas have been widely used in the satellite communication systems for their high gain and low sidelobe characteristics. However, in this paper, it is designed as the CATR(Compact Antenna Test Range) reflector system, and its near-field characteristics are investigated. The CATR facility needs to provide an uniform plane wave with the minimum amplitude and phase ripple and the low cross polarization to the test region. Therefore, the reflector near-field patterns are calculated and presented with the variations of the aperture power distribution, the feed horn pattern, and the distance from the aperture to the test zone. Also, the offset dual-shaped reflector is fabricated at 30 ㎓, and its near-field patterns are measured. The measured results are in good agreement with the calculated results. From theses results, we confirm that the designed offset dual-shaped reflector can be used as the reflector system for the compact antenna test range.

Actinomycosis is chronic suppurative and granulomatous disease caused by anaerobic, gram positive filamentous bacterium, The most common sites of involvement are the cervicofascial area, thorax, abdomen and pelvis. Primary hepatic actinomycosis is rare and presents a diagnostic challenge. A 57-year old man was admitted because of right upper quadrant abdominal pain and fever for 1 month. abdominal ultrasonography, computed tomography examination showed mass lesion in liver. Primary hepatic actinomycosis was confirmed by identification of sulfur granule, ultrasonography-guided percutaneous needle aspiration. We report a case of primary hepatic actinomycosis, emphasizing the role of imaging study followed by percutaneous liver biopsy in a condition which frequently requires laparotomy for diagnosis.
Chloroquine 내성 유입 말라리아 14 예에 대한 Halofantrine 의 치료효과

Objectives: The prevalence of malaria is increasing in recent years and also multidrug resistant malaria is increasing around the world and there is an increasing concern about imported malaria in nonendemic areas. Now many drugs are tried to find out effect on multidrug resistant malaria. We performed this study to investigate the thrapeutic effect of halofantrine in the treatment of chloroquine resistant imported malaria. Methods: From Feb. 1992 to May 1995, we experienced 35 patients infected with malaria and treated 14 patients among 35 patients with halofantrine. Results: 1) All 14 patients were sailor with a mean age of 39.4 years and infected with malaria. 2) The majority of patients were infected with malaria at Africa. 3) 10 patients were infected with Plasmodium falciparum and the remainder were undetermined. 4) In the 11 cases of chloroquine resistant malaria treated with quinine plus tetracycline combination therapy or Fansidar, 4 cases could not be tolerable due to side effects and resistance to the therapy, we substituted halofantrine for above regimens. 5) In the 10 cases, treated after May 1994, halofantrine was the first choice of treatment because they were the cases of malaria infected in the mid-Africa where the prevalence of chloroquine resistant malaria is high. 6) With halofantrine, all 14 cases were treated with minimal side effects suc4 as nausea, vomiting, anorexia, abdominal pain and fatigue. Conclusion: We think halofantrine is a simple and effective regimen against chloroquine resistant malaria and consider this agent as an alternative therapeutic regimen on chloroquine resistant malaria.

Quinine-induced hypoglycemia may be the fatal complication during the treatment of P. falciparum, especially in the severe form of malaria and pregnant patient. A 32 year old male was admitted for high fever. Abdominal CT scan showed marked hepatosplenomegaly with normal pancreatic structure and the trophozoites and gamatocytes of P. falciparum were detected on peripheral blood smear. He was improved with oral medication of quinine and tetracycline. But confused mentality and cold sweating were suddenly developed on the 3rd day of quinine medication, so we examined the blood sugar and insulin level. In correlation between hypoglycemia and clinical course of patient, this attack was considered as quinine-induced hypoglycemia. He was improved thereafter, and treated with halofantrine completely. We report one case of quinine-induced hypoglycemia that is complicated with oral medication in P. falciparum patient.

항공우주분야의 원격측정시스템은 비행체의 내부 상태 및 비행정보를 지상으로 전송하는 역할을 하는 탑재부분과 지상에서 비행 데이터를 수신하여 처리하는 지상시스템으로 구성된다. 원격측정시스템을 통해 취득된 데이터는 모든 비행시험 및 임무 수행의 성공여부를 결정하는데 매우 중요한 역할을 한다. 본 논문에서는 항공우주분야 원격측정 기술의 기술동향에 관하여 기술하였는데 고전적 1세대 기술에서부터 최근의 3세대 기술에 이르기까지 기술경향을 기술하였다. 특히 최근 무인항공기 분야를 비롯한 항공분야 원격측정 데이터 전송율의 요구가 급격히 증대됨에 따라 고속 네트워크를 활용한 4세대 항공우주 원격측정기술의 표준화가 진행되고 있으며 본 논문에서는 이에 관한 기술 동향도 함께 기술하였다. Telemetry system aerospace industry consists of the on board section which gathers and transmits status of the aerospace vehicles and the ground section which receives and processes the telemetry data. Telemetry data acquired through the flight tests of all the mission can be used to determine the success or failure of the mission. In this paper, aerospace telemetry technology trend is described from the conventional first generation telemetry technology to the latest technology trends, the third generation technology. Since the aeronautical telemetry industry including the unmanned aircraft telemetry data rate increases rapidly, new telemetry system, the fourth generation telemetry technology, is underway to cope with the future telemetry mission using high speed Ethernet network. The trend of the fourth generation telemetry system is also described in this paper.

방대한 웹 페이지의 홍수 속에서, 의미상 서로 연관되었지만 산재되어 있는 정보들을 사용자에게 효과적으로 제공하기란 그리 쉽지 않다. 웹 페이지에서 질적으로 향상된 정보를 얻기 위해서는, 이질적이지만 서로 연관된 의미를 갖는 데이터들을 하나로 융합하는 방법이 필요하다. 본 논문에서는 이질적인 형태로 이루어진 다수의 웹 페이지들을 XML 기반의 메타데이터(metadata)로 융합하여 사용자에게 제공하는 방법을 제시한다. 본 논문에서 제안한 시스템에서 메타데이터는 온톨로지와 OWL을 이용하여 융합된다. 또한 제시한 방법을 검증하기 위해 지역정보 중 부동산을 사례를 들어 시스템을 구현한다. 구현된 시스템은 각기 다른 데이터를 가지고 있는 다수의 웹 페이지를 하나의 웹 페이지로 통합하는 과정을 거쳐 XML 문서 형태로 사용자에게 제공한다.

De novo B-cell prolymphocytic leukemia (B-PLL) is a distinct clinicopathologic entity that was first described in 1974 by Galton et al. B-PLL is characterized by marked lymphocytosis with predominance of prolymphocytes, often massive splenomegaly, minimal lymphadenopathy, often aggressive clinical course and frequently poor prognosis.We experienced a case of B-PLL associated with false-positive direct antiglobulin test. The patient was 52 year-old man who presented with marked leukocytosis (160.2x103/μL) and 60% of characteristic prolymphocytes in the peripheral blood. The bone marrow aspirate showed dry tap and the PAS stain of peripheral blood smear showed positivity of prolymphocytes. The immunophenotyping of the leukemic prolymphocytes revealed the positivity of surface immunoglobulin (IgM, lambda type), HLA-DR, CD19 and CD5.(Korean J Med 59:428-432, 2000)
