        급성기병원에서의 간호 ∙ 간병통합서비스 운영 전후 낙상 및 욕창 발생 변화

        Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.

        이러닝 교육(인슐린 주사방법)을 통한 신규 간호사 교육 프로그램의 효과

        Purpose: The purpose of this study was to develop and evaluate a new-nurse education program utilizing both e-learning and instructor demonstration. Methods: From August to December in 2009, the e-learning education program about insulin injection was developed. The control (C) group was educated via instructor demonstration from April 15 to October 6 in 2009, and the experimental (E) group was educated via both e-learning and instructor demonstration from January 5 to October 13 in 2010. After each education, knowledge and educational effectiveness were checked. Results: Satisfaction with the education contents in the E group was significantly higher than those of the C group (Z=-3.72, p<.001), and satisfaction with the education method in the E group was higher than those of the C group (Z=-2.98, p=.003). Usefulness (Z=-3.33, p=.001), application (Z=-2.62, p=.009), and confidence (Z=-2.61, p=.009) in the E group were all higher than those of the C group. 78.9% in the E group reused the e-learning program after the experimental education. Conclusion: Combined educational program with e-learning and instructor demonstration had both merits of online efficiency and face-to-face education. It would be useful especially for new-nurses to improve their nursing skills in accomplishing their roles.

        외과계 간호사를 위한 환자안전 관점의 실무교육 프로그램 개발과 효과

        Purpose: The aim of this project was to develop a patient safety-focused inservice education program for surgical nurse and to test the effects of this program. Methods: Methodological designs for instruments development, a pretest-posttest and a posttest design were employed. After the education, nurses' satisfaction, confidence, usefulness and application of 137 nurses were evaluated at 15 surgical units of a tertiary teaching hospital in Seoul, Korea. The education contents are 6 skill areas (infusion pump use, suction, chest tube drainage, oxygen administration, nebulizer use, insulin administration) and medication knowledge. Teaching methods were lecture, instructor demonstration, and 1:1 skill test. Descriptive statistics, Wilcoxon test, Spearman's correlation and Stepwise regression were used. Results: Satisfaction scores for skills and medication education were 4.00-4.21 (out of 5), The more performance frequency in 6 skills, the higher score in confidence as well as in usefulness and application, and the higher satisfaction with the program, the higher score in usefulness, application, confidence, and medication knowledge. Medication knowledge improved after the education (Z=-7,757, p<.001). Significant predictors of skill confidence were application of skills in job performance, medication confidence, and career in present unit. Conclusion: The results of this study suggest that systematic and continuous inservice education will improve patient safety by promoting nursing quality.

        일개 대학병원 간호사의 업무수행능력에 관한 연구

        Purpose: To investigate the factors that affect nurse performance and to assess the characteristics of the high ranking group nurses based on performance appraisal in a university-affiliated hospital. Method: 49 head nurses evaluated their 830 nurses under direct control using performance appraisal tool and the nurses were grouped into 3 according to the performance score ; high ranking group(upper 10%), middle ranking group(middle 80%) and low ranking group(lower 10%). The characteristics of the nurses in high ranking group were compared with other groups on personal factors, job related factors, self concept, perceived head nurse leadership and performance self evaluations. Result: In high ranking group, the nurses were more aged. The proportion of the married nurse and nurses with over master degree were high. Total hospital career and current unit career of the nurses in the high ranking group were longer than nurses in other groups and more nurses had no career working for the other hospital in the past. In the high ranking group, more nurses had part time nurse career and preceptor experiences. The performance appraisal score has positive relationship with self-concept and perceived head nurse leadership and the self-concept and perceived head nurse leadership were higher in the high ranking group. The self evaluated performance appraisal was highest in the high ranking group but the score was lower than head nurse evaluated performance appraisal score. Conclusion: Strategic retention plan is needed because more aged and careered nurses exhibited higher performance. Educational program to improve self-concept and head nurse leadership would be helpful. Encouragement to level up nurses education status is essential.
