Erlotinib, an orally active epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI), is indicated for EGFR mutation-positive patients with poor performance status. Erlotinib is also used as a second-line agent in patients who show disease progression after failure of first-line therapy. Warfarin is administered in patients with advanced cancer for the prevention or treatment of venous thromboembolism. Both erlotinib and warfarin are metabolized in the human liver primarily by the CYP3A4 enzyme system. Thus, erlotinib may inhibit the metabolism of warfarin and clinicians should pay attention to the possible interaction between the two drugs when they are administered concurrently. We report the case of a 50-year-old man who showed changes in the prothrombin time-international normalized ratio (PT-INR) after coadministration of erlotinib and warfarin. 이혼 가정 아동의 탄력성에 대한 위험-보호 요인들 간의 경로 분석

The purpose of this study was to demonstrate the paths among the risk-protective factors that related to the resilience based on the Challenge Model. The subjects for this study were 209 children from divorced families in the 4th, 5th, and 6th grades of elementary schools in Seoul and Gyunggi-do. As the results, there were 28 paths which affected the resilience of children from divorced families based on the Challenge Model. The protective factors were easy temperament, problem-focused coping style, parental support, peer`s support, higher socio-economic status, experiences in therapy, presence of siblings, contact with adult caretakers. The risk factors were higher grade, emotion-focused coping style, and children`s gender, especially girls. This result demonstrated that each risk and protective factor not only affected resilience separately but interacted with one another.

Idiopathic thrombocytopenic purpura (ITP) is an autoimmune blood disorder haracterized by thrombocytopenia. Common clinical manifestations include bleeding events. Rarely, thrombotic complications may develop in ITP. A 43-year-old man was admitted with dyspnea. His platelet count at admission was 48,000/mm3. The patient had a history of ITP diagnosed 12 years earlier and had been treated with low-dose steroids. Two months before admission, he had been diagnosed with deep vein thrombosis and treated only with clopidogrel due to severe thrombocytopenia. Chest computed tomography showed filling defects in both pulmonary arteries. In the workup for precipitating factors, only lupus anticoagulant was positive. The concomitant administration of warfarin and methylprednisolone was used to treat the pulmonary thromboembolism and ITP, respectively. Six months later, the lupus anticoagulant test remained positive. The patient was confirmed to have a pulmonary thromboembolism due to antiphospholipid syndrome, which might be related to the underlying ITP. After 10 months, his symptoms and radiological findings had improved. (Korean J Med 2011;81:251-256)

This study focused on the temperament of infants and on maternal sensitivity in the investigation of the factors which affect infant-mother attachment. The 67 infant-mother pairs were divided into two groups on the basis of attachment security scores. Results showed a relationship between infant-mother attachment and temperament. There was also a relationship between maternal sensitivity and infant temperament. Factors most strongly affecting infant-mother attachment were infant`s sociability, mother`s emotion, and mother`s satisfaction with the degree of infant`s sociability.

A 68-year-old man presented with partial atelectasis of the right middle lobe on chest X-ray. Computed tomography of the chest revealed a hypodense mass in the middle lobe. Bronchoscopy showed a round mass obstructing the right middle lobe bronchus, and a biopsy specimen revealed only chronic inflammation. The tumor was removed using rigid bronchoscopy, and a diagnosis of endobronchial lipoma was made. An endobronchial lipoma is a very rare benign tumor that may cause parenchymal lung damage due to bronchial obstruction and subsequent pneumonia. We report this case to emphasize the role of endoscopic treatment in the management of endobronchial lipoma. (Korean J Med 2011;80:337-342)
청소년방과후아카데미 지도자의 직무만족, 스트레스요인 및 이에 따른 참여청소년의 방과후아카데미 만족도 및 심리사회적응의 차이

본 연구는 첫째, 청소년방과후아카데미 지도자들의 직무만족과 스트레스 등 심리적응에 영향을 미치는 요인들을 파악하고 둘째, 지도자들의 심리적응 수준에 따른 청소년들의 생활만족도 및 심리적응 수준을 파악하기 위하여 수행되었다. 연구목적을 위하여 전국의 청소년방과후아카데미 프로그램에 참여하고 있는 청소년 785명과 해당 기관 지도자 총 60명을 대상으로 설문조사를 실시하였다. 연구결과 첫째, 지도자들의 이직의도에 영향을 미치는 요인은 업무만족도와 가족 내 고정수입원이 있는지 여부인 것으로 나타났고, 업무만족도에 영향을 미치는 요인은 성취감과 정서적 고갈인 것으로 나타났다. 둘째, 지도자들의 업무만족도는 성취감과 정적 상관을 이직의도 및 정서적 고갈과는 부적 상관관계를 지니고 있으며, 업무스트레스가 높을수록 정서적 고갈의 수준도 높을 것으로 나타났다. 셋째, 지도자들의 심리적 적응수준에 따라 방과후아카데미 참여청소년의 만족도와 성취감, 자아존중감 및 효능감 등 심리적 적응수준이 달라지는 것으로 나타났다. 이러한 결과는 청소년방과후아카데미 프로그램의 효과성을 증진시키고 만족도를 높임으로써 청소년들의 건전한 발달을 도모하기 위해서는 청소년 대상 프로그램 및 운영에만 사업의 초점을 둘 것이 아니라 지도자들의 성취감과 업무만족도를 높이고 업무스트레스나 정서적 고갈을 해결할 수 있는 다양한 장치를 마련하고 전반적인 처우수준을 개선할 필요가 있음을 강하게 시사해주고 있다. This study was undertaken to examine the factors which influence the psychological adjustment of youth leaders who are working for youth?after?schoolacademy programs. It also sought to investigate the relationship with the adolescents life satisfaction and the psychological adjustment of those participating in youth?after?school?academy programs. A total of 785 adolescents and 60 youth leaders of youth?after?school?academy programs completed a questionnaire. The results of this study are as follows. Firstly, job satisfaction and additional income influenced the intention of the youth?after?school?academy leaders to remain in their positions, and a sense of achievement and emotional exhaustion influenced their job satisfaction. Secondly, the job satisfaction of the youth leaders was positively related to a sense of achievement and their intention to remain in their positions and emotional exhaustion were influenced negatively. Inaddition, the greater the job stress, the more emotional exhaustion was experienced by the subjects. Thirdly, life satisfaction, a sense of achievement, self?esteem and the self?efficacy of the adolescents who were participating in youth?after?school?academy programs were influenced by the level of the psychological adjustment of the youth leaders. These results indicate that the methods to improve youth leaders sense of achievement and job satisfaction and reduce their levels of job stress and emotional exhaustion are all very important for the improvement of the effects of youth?after?schoolacademy programs, and the life satisfaction of the adolescents.

본 연구는 보육시설의 운영과 관리에서 핵심적인 역할을 하고 있는 보육시설장의 스트레스와 소진 특성을 살펴봄으로써 보육시설장의 스트레스와 소진을 줄이고 예방하기 위한 방안을 모색하는 목적이 있다. 2009년도 충청남도 시설장 직무연수에 참가한 보육시설장 227명을 대상으로 직무스트레스와 소진 정도를 측정하여, 시설장의 배경변인별 차이와 소진에 영향을 미치는 직무스트레스 요인을 분석하였다. 그 결과 시설장이 지닌 자격유형, 연령, 근무하고 있는 어린이집 유형에 따라 하위스트레스 영역에 차이가 있었고, 시설장의 경력, 근무하고 있는 어린이집 유형에 따라 소진 정도에 차이가 있었다. 또한 교사관리, 원장직무, 시설관리, 감독기관 스트레스는 시설장의 정서고갈에, 시설관리, 부모관리 스트레스는 시설장의 성취감 저하와 비인격화에 영향을 미치는 것으로 밝혀졌다. The purpose of this study was to investigate the child care center directors' job stress and burnout related with their demographic variables and find the sub-factors of job stress which affect the burnout. Subjects were 227 directors of child care center in Chungnam province. First, the job stress of directors was significant differences in type of certificate, director's age, and type of child care center. Second, the burnout of directors differs depending on career of directors, type of child care center. Third, stress of teacher management, director's business, relationship with administrative agencies affect the emotional exhaustion of directors. The management of facilities and relationship with patents cause the depersonalization and personal accomplish of child care center directors.
