        경미한 급성 두부외상환자에서 SPECT , q - EEG 및 CT의 비교

        N/A Functional cerebral impairments have been verified objectively by brain SPECT and q-EEG (quantitative electroencephalography). Microcerebral circulatory defects without anatomical changes cannot be detected by the brain CT or MRI. Brain SPECT using Tc-99m-HMPAO (Hexamethyl propyleneamine oxime) as a key radioisotope may be accepted as the useful method for identifying functional cerebral impairments. We studied 25 patients with mild head trauma to define whether the SPECT was helpful in detecting cerebral impairment. Results were as follows: The SPECT was positive in 23 patients out of 25, q-EEG positive in 16 patients and brain CT was positive in 3 cases. SPECT and q-EEG were more sensitive than CT. SPECT would be more useful method than brain CT to investigate cerebral function after head injury.

        경도 두부손상에서의 전산화 단층촬영 검사 적응증

        Purpose: Most minor head injury (MHI) patients can be discharged without complication but a small percentage of these patients have intracranial lesions which can be observed by computed tomography (CT), and in these cases, neurosurgical intervention may be necessary. Selective use of the CT-scan in cases of MHI is important in reducing the risks associated with unnecessary radiation exposure. We conducted a retrospective study with the goal of creating a set of clinical criteria for deciding when to utilize the CT scan for MHI cases. Methods: This retrospective study was conducted using 1,735 patients with MHI (GCS=15, ≥6 years old) from January 2009 to December 2010. Based on literature review results, we selected risk factors associated with the presence of intracranial lesions observable by cranial CTscan, which may have resulted from MHI. The detection of intracranial lesions by CT scan was regarded as the primary clinical outcome. Results: Of the total cases, 87(5.0%) had intracranial lesions as observed by CT scan. All patients with abnormal CT scans had at least one of the following risk factors: headache, loss of consciousness (LOC)/posttraumatic amnesia (PTA), vomiting, focal neurological deficit, coagulopathy, alcohol intoxication, skull fracture, age greater than 65 years, dangerous mechanism of injury, or facial fracture. A decision model for application of CT scanning in MHI cases was derived which consisted of 5 risk factors: headache, LOC/PTA, skull fracture, and age greater than 65 years. The decision model was 100% sensitive (95% CI, 95.8~100%) and 42.4% specific (95% CI, 40.0~44.8%) for predicting intracranial lesions, and would require only 59.8% of patients to undergo CT. Conclusion: The decision model developed for CT scanning in MHI cases was highly sensitive. Patients who meet the criteria of the model require evaluation by CT scan.

        모듈러 건축물의 내력벽 2시간 내화성능에 관한 실험적 연구

        To higher the buildings, fire resistance performance is needed following the purposes and dimensions of the buildings. Especially, two hours fire resistance performance should be satisfied for the buildings above the 4th floor and under the 12th floor. In the case of load bearing walls with steel stud, two hours fire resistance performance have never been recognized in Korea. This study was conducted to investigate two hours fire resistance performance of load bearing walls with steel stud in structural elements of modular buildings. Test specimens in this study were redesigned in the basis of typical load bearing walls used in Europe and North America. The load bearing walls ensuring two hours fire resistance performance were suggested from the test results of this study. The load bearing walls with fireproof board of 45mm thickness secured two hours fire resistance performance in this study.

        에너지 모니터링 가시화 시스템을 위한 도시 모델링 저작도구 개발

        EnerISS is a software system in which a state-of-the-art urban energy control technology is integrated. This system is ultimately aimed at developing energy-saving, ubiquitous-ecology city, usable in city planning and operating steps. As a branch module of the EnerISS, EnerISS Solution(Modeler/Viewer) was developed to visualize energy distribution, demand and consumption status of a target city. This visualization tool operated by city planers, energy-monitoring engineers, and even consumers is expected to virtually derive an energy conservation and efficient use/supply. First of all, the tool should be able to model space and buildings of the target city in terms of accurate geographic information and to express real-time energy information. One part of the packages, called EnerISS Modeler was therefore developed to build the GIS-based city models in which energy data can be input. The software package contains various user-friendly background functions such as create, open, save, view, navigate, etc. This paper is introducing the development process, theoretical background, and functional explanation for the developed modeling software. One city model was selected as an application example to examine and demonstrate the applicability and efficiency.

      • 수소연료전지를 적용한 탄소저감주택 개발사례 조사 연구

        In recent years, one of the common problems for the world has been the high price of oil. Because of that, every country is trying to cut back on emissions of greenhouse gases. According to Bali roadmap, all the nations have to bring forward the amount of cutback on emissions of greenhouse gases. Even at the meeting of G8 summit, they made this issue on the top of the list of agreements. In Korea, lots of researches have been made on this issue in building constructions using New Renewable Energy, and through improving the efficiency of energy in buildings. However we have not done much about using Hydrogen fuel cell on this issue. Even though many researches have been done on Hydrogen Fuel cell itself for some time, it actually has not been used in buildings, just in plants. In this research, by looking into the developing cases of building construction using Hydrogen fuel cell to cut back on emissions of greenhouse gases in other countries, we suggest how Carbon zerohouse should be evolved.

        OSM 공법을 적용한 저비용 주택시스템 개발 연구

        The development of a new building system which can produce inexpensive quality housing is needed for the supply of affordable housing. The increase of labor cost due to the lack of building technicians and the decrease of skilled craftsmen prevents inexpensive quality housing from being distributed without difficulty. OSM construction method that shortens the duration of field construction and maximize the factory work to improve the quality of construction is environment-friendly in that it has the high percentage of reusing materials. In this study in order for affordable housing to be diffused using module unit adapted OSM system, the standardized roof panel is suggested and system which can be put together easily with less intial cost is developed to minimizes the cost of framework. Also the system which uses module unit and panel structure to make long span and to prevent overlapping subsidiary materials as well as the system which improves execution process and maintenance due to using module unit is introduced.

        응급실실 내원한 자살기도 환자에 관한 연구

        This study was designed to investigate dinical characteristics and outcome of patient with suicidal attempt A retrospective analysis by chart review of 130 cases of patients with suicidal attempt who visited emergency department of Dankook University Hospital from January 1998 to December 1998.         외상 : 미만성 두부손상환자의 CT 분류 및 예후

        The CT classification of difluse head injury permits the early identification of patients at high risk from intracranial hypertension and allows the neurosurgeon the option of early intervention, With the CT classification it is feasible to identify the patients who would appear to be at low risk based on a clinical examination, but who are known from the CT scan diagnosis to be at high risk. The CT classification was based on the status of the mesencephalic cisterns and the degree of midline shift.. Diffuse head injury was divided into four subgoups(I, II, III, IV). We analyzed 276 pateints with diffuse head injury ftom Jannuary 1995 to september 1996 for identification of likelihood of early raised intracranial pressure in emergency room and decision about early management in patients with diffuse injury. There was close relationship between these diffuse injury type of CT scan and the prognosis. Patients suffering with no pathology(diffuse injury I) and without compression of mesencephalic cistem and midline shift (diffuse injury II) had the lowest mortality rate( 1.3%, 4.1%), while the mortality rate in patients suffering diffuse injury with a obliteration of mesencephalic cistern(diffuse injury IIIB) and midline shift((diffuse injury IV) was greater than 50%(84.2%, 50.0%). The CT diagnosis was a highly significant independent predictor of mortality in diffuse injury I and IIIB, but there was an interdependence between the degree of consciousness level and pupil change and the finding on CT scan in diffuse injury lI, IIIA and IV.

        The purpose of this study is to indicate the guideline in domestic technology development by investigating and analyzing technologies on OSM construction methods in advanced countries. The OSM construction methods were initiated from the Europe and U.S.A, and the market has been expanded gradually. Presently, the OSM construction methods are applied widely in the real construction sites. In the Germany and United Kingdom, large companies are manufacturing modular units used as the OSM system. In the case of the United Kingdom, Yorkon, Terrapin, Caledonian, Corns, Ayrframe, and Britspace are representative companies producing OSM module units. In the Germany, Alho, Caldolto, and SABU are very large companies producing the module units and Rannela Metals is a representative company producing the module units in the Finland. They are continually developing the OSM system using the modular units.
