      • Elucidation of the expression and clinical significance of tripartite motif-containing proteins in Behcet``s disease

        Background: Behcet’s disease (BD) is a chronic, multisystem vasculitis and autoinflammatory disorder, characterized by oral ulcers, genital ulcers, various inflammatory skin lesions, and uveitis. The exact cause of BD is still not known, many study suggest that this disease triggered by environmental factor including infectious agents such as herpes simplex virus (HSV) and streptococcus sanguinis. Tripartite motif containing 21 (TRIM21), an E3 ligase protein, regulates the production of cytokine by ubiquitination of transcriptional factors such as interferon regulatory factor (IRF) family and NF-kB. Moreover TRIM21 can present antiviral properties, mediating antiviral defense mechanisms to DNA/RNA viruses. Objective: The purpose of this study was to elucidate TRIM21 proteins expression in monocyte of BD patient, and to identify the role of TRIM21 on immune dysregulation in BD. Methods: Monocytes and T cells were isolated from peripheral blood using Ficoll-Hypaque density-gradient centrifugation and relevant immunomagnetic negative selection method. Protein levels were measured using western blot, ELISA, and flow cytometry. Co-cultures between monocyte and responder T cells (1:1 to 1:2 ratio) were maintained in media for 7~14 days. Knock-down of TRIM21 was performed using siRNA technique on THP-1 or freshly isolated monocytes. Results: TRIM21 expression was increased in peripheral blood monocytes from BD patients compared to healthy control. The expression of IRF8, a representative ubiquitination target of TRIM21, was also decreased in BD monocyte. BD monocyte promoted secretion of pro- inflammatory cytokines including Th17 promoting cytokine such as IL-6, IL-1β and IL-23 following NF-kB activation. BD monocytes promoted Th17 polarization after co-culture with responder T cells from healthy donors, and IL-17A production from these co-cultured T cells was also increased. Known-down of TRIM21 in monocyte using siRNA prevents NF-kB activation and decreased Th1 and Th17 polarization of responder T cell in co-culture system. Conclusion: From our results suggest that BD monocytes facilitate Th/1Th17 differentiation of naive T cells and TRIM21 may regulate the secretion of pro-inflammatory cytokine from monocytes.

        Treatment results of breast cancer patients with locoregional recurrence after mastectomy

        Purpose: To analyze the results of locoregional and systemic therapy in the breast cancer patients with locoregional recurrence (LRR) after mastectomy. Materials and Methods: Seventy-one patients who received radiotherapy for isolated LRR after mastectomy between January 1999 and December 2009 were retrospectively reviewed. Among the 71 patients, 59 (83.1%) underwent wide excision and radiotherapy and 12 (16.9%) received radiotherapy alone. Adjuvant hormonal therapy was given to 45 patients (63.4%). Oncologic outcomes including locoregional recurrence-free survival, disease-free survival (DFS), and overall survival (OS) and prognostic factors were analyzed. Results: Median follow-up time was 49.2 months. Of the 71 patients, 5 (7%) experienced second isolated LRR, and 40 (56%) underwent distant metastasis (DM). The median DFS was 35.6 months, and the 3- and 5-year DFS were 49.1% and 28.6%, respectively. The median OS was 86.7 months, and the 5-year OS was 62.3%. Patients who received hormone therapy together showed better 5-year DFS and OS than the patients treated with locoregional therapy only (31.6% vs. 22.1%, p = 0.036; 66.5% vs. 55.2%, p = 0.022). In multivariate analysis, higher N stage at recurrence was a significant prognostic factor for DFS and OS. Disease free interval (≤30 months vs. >30 months) from mastectomy to LRR was also significant for OS. The patients who received hormone therapy showed superior DFS and showed trend to better OS. Conclusion: DM was a major pattern of failure after the treatment of LRR after mastectomy. The role of systemic treatment for LRR after mastectomy should be investigated at prospective trials.

        Background: Emerging evidence demonstrates a critical role for inflammasome activation in vitiligo pathogenesis. Extracellular adenosine triphosphate (ATP) is a versatile mediator of inflammatory processes, which may be involved in vitiligo pathogenesis. Objectives: To investigate the role of ATP-P2X7 axis on pathogenesis of vitiligo Methods: Various experimental desing were used including mTT assay, confocal microscopy, western blotting, ELISA etc. Results: This study presents how extracellular ATP, released from keratinocytes by oxidative stress, affects melanocyte survival in vitiligo skins. H2O2-induced oxidative injury increased ATP release from keratinocytes and skin tissues. High concentration of extracellular ATP induced both ROS production and cell death in melanocytes. Treatment with ATP in melanocytes led to caspase-1 activation as well as production of active forms of IL-1β and IL-18 via P2X7 receptor (P2X7R). Lesional and perilesional skins of vitiligo showed higher levels of ATP as well as upregulation of active caspase-1 compared to non-lesional skins, suggesting its possible role in inflammasome activation in vitiligo. Moreover, the expression of CXCL9 in keratinocytes, mediated through ATP-P2X7R-dependent inflammasome activation, was responsible for CLA+ CD8+ T-cell chemotaxis into the skin. Conclusion: In conclusion, extracellular ATP as a danger signal not only activates inflammasome pathway as well as increases cutaneous chemotaxis of CD8+ T cells via CXCL9 in vitiligo.

        원삼국시대 전기 유개대부토기 편년

        본고에서는 기원후 1-2세기에 영남과 호서지역에서 출토되고 있는 유개대부토기의 편년에 대하여 연구하였다. 대상유물인 유개대부토기는 다양한 기형을 가지고 있으며 영남과 호서지역의 분묘 유적에서 주로 출토된다. 유개대부토기는 형태와 제작 방식을 통하여 분류할 수 있는데, 개와 대부토기는 별도로 분류하였다. 우선 개의 경우는 삿갓형과배형으로 분류하고, 삿갓형개는 다시 접합 방식에 따라 삿갓형Ⅰ류와 삿갓형Ⅱ류로 세분된다. 대부토기는 대각 위에얹어진 토기의 형태로 유개대부배, 유개대부호, 유개대부옹으로 분류된다. 분류된 유개대부토기는 형식학적속성분석법을 통하여 편년할 수 있는데, 이때 유개대부배의 상반부고 속성과 삿갓형개의 드림고 속성, 그리고 대부호의 전체 비율 속성이 시간성 반영에 유효한 속성임을 확인하였다. 분석 결과 유개대부배와 삿갓형Ⅱ류개는 7개의 단계, 삿갓형Ⅰ류와 대부호는 6개의 단계로 형식 조열되었다. 이를 검증하기 위하여공반되는 와질옹과 주머니호, 조합식우각형파수부호를 중심으로, 철기 유물를 보조적 검증 수단으로 활용하였고, 목관묘 단계와 교차 검토하였다. 유개대부토기의 공반 유물과 기종 간의 공반상을 확인하기 위하여 순서배열보충법을 사용하였고, 이를 통해 유개대부토기의 공반상과 군집, 단계를 확인하였다. 순서배열보충법을 통한 양식편년 결과 유개대부배와 유개대부호는 각각 5기로 확인되었고, 기존 연구의 목관묘 단계를 적용·비교하여 유개대부토기는 총 5기로 구분 지을 수 있다. 유개대부토기를 통하여 설정한 1·2기는 Ⅳ·Ⅴ단계로, 1세기 중엽에 해당한다. 이 단계가 되면 영남지역에서 유개대부배가 등장한다. 3기는 Ⅴ·Ⅵ단계로, 1세기 후엽에 해당한다. 호서지역에서 점차 유개대부호가 등장하는 시기이다. 4기는 Ⅵ·Ⅶ단계로, 1세기 후엽 ~ 2세기 전엽에 해당한다. 영남에서 유개대부호와 호서에서 대부배, 전기와질토기가 출토되는 단계이다. 5기는 Ⅶ·Ⅷ단계로, 2세기 전~중엽에 해당한다. 유개대부토기가 점차 소멸하는 단계이자 전기와질토기의 마지막 단계이다. 유개대부토기의 4기에서 언급하였던 양 지역에서 상대지역 유물이 확인되는 현상은 단순한 유물의 이동이 아닌 전파 혹은 교류를 통하여 토기와 철기의 영향을 받은 것으로 생각해 볼 수 있다. Lidded leg jars are brick-clay pottery from the early proto-Three Kingdome of Korea. These artifacts are mainly discovered in the Yeongnam and Midwest regions in Korea. As they are excavated from both regions, these artifacts could be crucial to ascertain the relationship between Youngnam and Midwest regions. Through this essay, three points could be considered. Firstly, the Youngnam and Midwest regions produced totally different types of pottery in the early time. Next, when it goes to VII level, Both regions affected each other and share the pottery form. Lastly, VII and VIII levels are very significant. In early time, both regions had their unique and different pottery types. However, they started to share the shape of the pottery in VII and VIII levels. The early types of each regions originally thought to be exclusive, now they are found in each other area. Normally, it is believed that the early brick-clay pottery does not have leg and lid while the typical type of brick -clay pottery in late proto-Three Kingdome has both of them. Potteries from after VIII level are the late brick-clay pottery. However, these lidded leg jars are from the early time and they already have a lid and leg. According to these, it could assume that the early lidded leg jars has some connection with the late pottery. Hence, the further study will be conducted later.

        Age-Related Effects of Sodium Arsenite on Splenocyte Proliferation and Th1/Th2 Cytokine Production

        Aging is associated with immune dysfunction and conditions such as inflamm-aging and immunosuppression. Arsenic, an environmental contaminant distributed worldwide, affects the immune system. This study tested the hypothesis that arsenic has distinct effects on T cell proliferation and the production of cytokines by activated T cells. Murine splenocytes from young (2 months) and aged (24-26 months) C57BL/6 mice were exposed to arsenite (As3+), the most toxic form of inorganic arsenic, and stimulated with concanavalin A (Con A) or anti-CD3 antibody. T cell proliferation decreased significantly in response to Con A and anti-CD3 at subtoxic doses of arsenite in splenocytes from both young and aged mice. Arsenite, added concurrently with Con A or anti-CD3, significantly inhibited the production of interleukin-2 (IL-2), interferon-γ (IFN-γ), and interleukin-4 (IL-4) by splenocytes from young mice and significantly reduced the production of IL-10 by splenocytes from aged mice. In contrast, the production of IL-2 and IL-4 by splenocytes from aged mice was only slightly affected by arsenite. The results show that arsenic exposure reduces the immune response in splenocytes. Moreover,this effect may be influenced by aging.

        Endoscopic resection of gastric gastrointestinal stromal tumor using clip-and-cut endoscopic full-thickness resection: a single-center, retrospective cohort in Korea

        Background/Aims: To overcome the technical limitations of classic endoscopic resection for gastric gastrointestinal stromal tumors (GISTs), various methods have been developed. In this study, we examined the role and feasibility of clip-and-cut procedures (clip-and-cut endoscopic full-thickness resection [cc-EFTR]) for gastric GISTs. Methods: Medical records of 83 patients diagnosed with GISTs after endoscopic resection between 2005 and 2021 were retrospectively reviewed. Moreover, clinical characteristics and outcomes were analyzed. Results: Endoscopic submucosal dissection (ESD) and cc-EFTR were performed in 51 and 32 patients, respectively. The GISTs were detected in the upper third of the stomach for ESD (52.9%) and cc-EFTR (90.6%). Within the cc-EFTR group, a majority of GISTs were located in the deep muscularis propria or serosal layer, accounting for 96.9%, as opposed to those in the ESD group (45.1%). The R0 resection rates were 51.0% and 84.4% in the ESD and cc-EFTR groups, respectively. Seven (8.4%) patients required surgical treatment (six patients underwent ESD and one underwent cc-EFTR,) due to residual tumor (n=5) and post-procedure adverse events (n=2). Patients undergoing R0 or R1 resection did not experience recurrence during a median 14-month follow-up period, except for one patient in the ESD group. Conclusions: cc-EFTR displayed a high R0 resection rate; therefore, it is a safe and effective therapeutic option for small gastric GISTs.
