        Comparison of the Hospital-Acquired Clostridium difficile Infection Risk of Using Proton Pump Inhibitors versus Histamine-2 Receptor Antagonists for Prophylaxis and Treatment of Stress Ulcers: A Systematic Review and Meta-Analysis

        Background/Aims: Although proton pump inhibitors (PPIs) have been widely used for the prevention and treatment of stress gastric ulcers in hospital settings, there are concerns that PPIs increase the risk of Clostridium difficile infection (CDI). However, little is known about the risk of CDI following PPI and histamine-2 receptor antagonist (H2RA) use. We evaluated the comparative hospital-acquired CDI occurrence risk associated with the concurrent use of PPIs versus H2RAs. Methods: A systematic search of PubMed, MEDLINE/Ovid, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Web of Science, and Google Scholar through August 19, 2016, identified 12 studies that reported the hospital-acquired CDI occurrence following H2RA and PPI use for the prevention and treatment of stress gastric ulcers. Random-effects pooled odds ratios and 95% confidence intervals were estimated. Heterogeneity was measured using I2, and a meta-regression analysis was conducted. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system was used to assess the overall quality of the evidence. Results: A total of 74,132 patients from 12 observational studies were analyzed. Compared to H2RAs, PPIs increased the risk of CDI by 38.6% (pooled odds ratio, 1.386; 95% confidence interval, 1.152 to 1.668; p=0.001; I2=42.81%). Subgroup analyses of the purpose of study medication use, study site, and study design confirmed the consistency of a greater CDI risk with PPIs than with H2RAs. The overall quality of evidence was rated as low. Conclusions: The use of PPIs for both the prevention and treatment of stress ulcers was associated with a 38.6% increased risk of hospital-acquired CDI occurrence compared to H2RA use.         Harmonic Elimination in Three-Phase Voltage Source Inverters by Particle Swarm Optimization

        This paper presents accurate solutions for nonlinear transcendental equations of the selective harmonic elimination technique used in three-phase PWM inverters feeding the induction motor by particle swarm optimization (PSO). With the proposed approach, the required switching angles are computed efficiently to eliminate low order harmonics up to the 23<SUP>rd</SUP> from the inverter voltage waveform, whereas the magnitude of the fundamental component is controlled to the desired value. A set of solutions and the evaluation of the proposed method are presented. The obtained results prove that the algorithm converges to a precise solution after several iterations. The salient contribution of the paper is the application of the particle swarm algorithm to attenuate successfully any undesired loworder harmonics from the inverter output voltage. The current paper demonstrates that the PSO is a promising approach to control the operation of a three-phase voltage source inverter with a selective harmonic elimination strategy to be applied in induction motor drives.

      • Data-driven prediction of compressive strength of FRP-confined concrete members: An application of machine learning models

        The strength models for fiber-reinforced polymer (FRP)-confined normal strength concrete (NC) cylinders available in the literature have been suggested based on small databases using limited variables of such structural members portraying less accuracy. The artificial neural network (ANN) is an advanced technique for precisely predicting the response of composite structures by considering a large number of parameters. The main objective of the present investigation is to develop an ANN model for the axial strength of FRP-confined NC cylinders using various parameters to give the highest accuracy of the predictions. To secure this aim, a large experimental database of 313 FRP-confined NC cylinders has been constructed from previous research investigations. An evaluation of 33 different empirical strength models has been performed using various statistical parameters (root mean squared error RMSE, mean absolute error MAE, and coefficient of determination R2) over the developed database. Then, a new ANN model using the Group Method of Data Handling (GMDH) has been proposed based on the experimental database that portrayed the highest performance as compared with the previous models with R2=0.92, RMSE=0.27, and MAE=0.33. Therefore, the suggested ANN model can accurately capture the axial strength of FRP-confined NC cylinders that can be used for the further analysis and design of such members in the construction industry.

        Urodynamic Detrusor Overactivity in Patients with Overactive Bladder Symptoms

        Purpose: To evaluate the relationship between urodynamic detrusor overactivity (DO) and overactive bladder (OAB) symptoms in men and women. Methods: We reviewed the records of adult males and females who attended a tertiary referral center for urodynamic evaluation of OAB syndrome symptoms with the presence or absence of DO. DO was calculated for symptoms alone or in combination. Results: The overall incidence of DO was 76.1% and 58.7% in male and female OAB patients, respectively. Of men 63% and 61% of women with urgency (OAB dry) had DO, while 93% of men and 69.8% of women with urgency and urgency urinary incontinence (OAB wet) had DO. Of women, 58% who were OAB wet had stress urinary incontinence symptoms with 26.4% having urodynamic stress incontinence. 6% of men and 6.5% of women with OAB symptoms had urodynamic diagnosis of voiding difficulties with postvoid residual greater than 100 mL. Combination of symptoms is more accurate in predicting DO in OAB patients. The multivariate disease model for males included urge urinary incontinence (UUI) and urgency while for females it included UUI and nocturia. Conclusions: There was a better correlation in results between OAB symptoms and the urodynamic diagnosis of DO in men than in women, more so in OAB wet than in OAB dry. Combination of symptoms of the OAB syndrome seems to have a better correlation with objective parameters from the bladder diary, filling cystometry, and with the occurrence of DO.

        The effect of immediate neoadjuvant electromotive instillation of mitomycin C with Bacillus Calmette–Guérin versus BCG alone in non-muscle-invasive bladder cancer: A randomized controlled trial

        Purpose: The clinical effect of neoadjuvant intravesical instillation of chemotherapy immediately before transurethral resection of bladder tumors (TURBT) has been a subject of recent research. The aim of this study was to assess the effect of immediate neoadjuvant electromotive instillation of mitomycin C before transurethral resection for patients with non-muscle-invasive urothelial bladder cancer. Materials and Methods: Our study was a randomized clinical trial carried out on 50 patients diagnosed with non-muscle-invasive urothelial bladder cancer. Patients were classified into two groups: Group I consisted of 25 patients who received neoadjuvant electromotive drug administration of mitomycin C before TURBT and intravesical bacille Calmette–Guerin (BCG) per week for 6 weeks; Group II consisted of 25 patients who were treated with TURBT followed by intravesical BCG per week for 6 weeks alone (standard of care). Patients were followed up at 3, 6, 12, and 18 months by cystoscopy. Results: Patients who received neoadjuvant electromotive drug administration of mitomycin C before TURBT in combination with BCG had a low recurrence rate compared with those who received BCG alone (12.0% vs. 48.0%, respectively; p=0.012) and a longer disease-free interval (88.0% vs. 52.0%, respectively; p=0.012). Four patients developed progression to muscle-invasive disease (16.0%) in the BCG alone group. However, this difference was not statistically significant (p=0.516). Regarding adverse effects, there were no statistically significant differences between the groups. Conclusions: Neoadjuvant intravesical electromotive drug administration of mitomycin C before TURBT is safe; reduces recurrence rates and enhances the disease-free interval compared with TURBT followed by BCG alone.

      • A new quasi-3D plate theory for free vibration analysis of advanced composite nanoplates

        This paper presents an analytical solution to study the combined effect of non-local and stretching effect on the vibration of advanced functionally graded (FG) nanoplates. A new quasi-3D plate theory is presented; there are only five unknowns and any shear correction factor is used. A new displacement field with a new shear warping function is proposed. The equilibrium equations of the FG nanoplates are obtained using the Hamilton principle and solved numerically using the Navier technique. The material properties of functionally graded nanoplates are presumed to change according to the power-law distribution of ceramic and metal constituents. The numerical results of this work are compared with those of other published results to indicate the accuracy and convergence of this theory. Hence, a profound parameterstudy is also performed to show the influence of many parameters of the functionally graded nanoplates on the free vibration responses is investigated.

        Impacts of PAH accumulation on reproductive hormones, indices of oxidative stress and BPDE-albumin adduct in women with recurrent pregnancy loss

        Chronic exposure to Poly aromatic hydrocarbons (PAHs) may be associated with adverse pregnancy outcomes. Disruption of hormonal and redox balance by toxic PAH metabolites may interfere with successful pregnancy leading to miscarriage. The association of exposure to PAH contaminated mussel via the dietary route with perturbations in reproductive hormones, biomarkers of oxidative stress, and PAH metabolites were assessed in women with recurrent pregnancy loss (RPL). Furthermore, an analysis of the concentration of PAHs in environmentally relevant bivalve animals was performed to preliminary get insights into the levels of these pollutants in the environment. Seventy-six women (20–35 years) were categorized into 18 fertile women without RPL (control), and Groups I, II, and III comprising 24, 18, and 16 women with RPL (2, 3, and > 3 abortions respectively) were studied. Whole blood samples were collected for the estimation of malondialdehyde (MDA), catalase, reduced glutathione (GSH), glutathione-S-transferase (GST), progesterone (P4), follicle-stimulating hormone (FSH), benzo[a]pyren-7,8-dihydrodiol-9,10-epoxide-albumin adduct (BPDE-albumin) and urine for α-naphthol and β-naphthol. Two species of mussel Donax trunculus and Andar aduloii samples were collected for the estimation of 16 priority PAHs. The concentration of PAHs exceeding the maximum limits was observed in the two species of mussels studied. Higher levels of BPDE-albumin, MDA, GST, α and β-naphthol and lower GSH, catalase, FSH, and P4 were observed in women with RPL (Groups I-III) compared to controls (p = < 0.001). Negative associations were observed between BPDE-albumin and catalase (r = − 0.276, p = 0.036), and GSH (r = − 0.331, p = − 0.011) only in women with RPL. Collectively, our findings indicate a possible association of chronic PAH accumulation with recurrent pregnancy loss in women.
