Purpose: An association between primary hyperparathyroidism and well differentiated thyroid carcinoma has been frequently reported. However, there have been few reports of secondary hyperparathyroidism associated with thyroid carcinoma. This study was performed to evaluate the prevalence of thyroid nodule including thyroid carcinoma in patients with secondary hyperparathyroidism and to suggest more proper diagnostic approach for such cases. Methods: A retrospective study was performed on fifty-two patients who underwent parathyroid surgery for secondary hyperparathyroidism between March, 1986 and December, 2003. Results: In 20 patients (38.5%), thyroid surgery was added because of coexistent thyroid nodules. Five (25%) out of the 22 patients with coexistent thyroid nodule had thyroid carcinoma. Among the 5 thyroid carcinoma patients, 4 had papillary carcinomas and one had a follicular carcinoma. Mean size of thyroid carcinomas was 1.4 cm (0.3∼3.0 cm). Conclusion: The prevalence of thyroid carcinoma in patients with secondary hyperparathyroidism was higher than that in the general population. An aggressive diagnostic approach (ultrasonography and FNAB) should be considered when the patients with secondary hyperparathyroidism present with thyroid nodules. Nitric oxide (NO) modulates the activities of various channels and receptors to participate in the regulation of neuronal intracellular Ca2+ levels. Ca2+ binding protein (CaBP) expression may also be altered by NO. Accordingly, we examined expression changes in calbindin-D28k, calretinin, and parvalbumin in the cerebral cortex and hippocampal region of neuronal NO synthase knockout(−/−) (nNOS−/−) mice using immunohistochemistry. For the first time, we demonstrate that the expression of CaBPs is specifically altered in the cerebral cortex and hippocampal region of nNOS−/− mice and that their expression changed according to neuronal type. As changes in CaBP expression can influence temporal and spatial intracellular Ca2+ levels, it appears that NO may be involved in various functions, such as modulating neuronal Ca2+ homeostasis, regulating synaptic transmission, and neuroprotection, by influencing the expression of CaBPs. Therefore, these results suggest another mechanism by which NO participates in the regulation of neuronal Ca2+ homeostasis. However, the exact mechanisms of this regulation and its functional significance require further investigation.

목적: The objective of the study was to analyze the prognostic role of pathological response to neoadjuvant chemotherapy (NACT) and adjuvant management in unresectable advanced ovarian cancer (AOC) patients. 방법: We retrospectively evaluated 94 unresectable AOC patients treated with NACT followed by interval debulking surgery (IDS). Pathological response was classified as follows: complete remission (CR) in the absence of residual disease, microscopic (microPR) in the presence of microscopic tumor foci, and macroscopic (macroPR) when macroscopic residual disease was detected. Adjuvant management was classified to two groups: the one was treated with 3 cycles of adjuvant chemotherapy and the other was treated with more than 3 cycles. 결과: No differences were observed in the distribution of baseline clinicopathological characteristics among 3 pathologic groups. Median progression- free survival was 28 months CR, 16 in microPR, and 13 in macroPR (p<0.05). Median overall survival was 52 months in CR, 32 in microPR, and 21 in macroPR (p<0.05). Median PFS was not different between 3ACT and 6ACT, but median OS was significantly longer in 6ACT when the analysis was conducted in platinum-respondors (28 months vs. 34 months). 결론: The pathologic response to NACT is associated with a favorable prognosis. Additionally, the sufficient adjuvant chemotherapy was important. The careful clinical setting is necessary for the patients treated with IDS following NACT
