        CASE REPORT : A Case of Posterior Mediastinal Plasmacytoma Confounded by Community-Acquired Pneumonia

        Plasmacytomas are extramedullary accumulations of plasma cells originating from soft tissue. Mediastinal plasmacytoma is a rare presentation. A 67-year-old man recovered after antibiotic treatment for community-acquired pneumonia. However, on convalescent chest radiography after 3 months, mass like lesion at the right lower lung field was newly detected. Follow-up chest computed tomography (CT) revealed an increase in the extent of the right posterior mediastinal mass that we had considered to be pneumonic consolidations on previous CT scans. Through percutaneous needle biopsy, we diagnosed IgG kappa type extramedullary plasmacytoma of the posterior mediastinum.

      • S-77 : Recurrent Duodenal Stricture and Duodenal Phytobezoar After Billoth I Operation: A Case Report

        Introduction: Phytobezoars are conglomerates of non-digestible vegetable matter in the gastrointestinal (GI) tract that may be insidious or symptomatic with general GI manifestations. There have been only a few case reports of phytobezoars in the literature. We report a rare case of recurrent duodenal stricture and duodenal bezoar which was detected following Billoth I operation for early gastric cancer (EGC). Case Report: A 74-year-old female patient presented at our clinic with a 2-day history of intense epigastric pain. Vomiting developed after water intake, and there was no bowel movement or gas passing. The patient had been diagnosed with EGC through a routine health workup 7 months prior to this presentation. Histopathologic examination of the specimen taken by endoscopic ultrasonography revealed a well-differentiated adenocarcinoma, type I, mucosal cancer (T1aN0Mx). Endoscopic submucosal dissection was indicated for the patient; however, she decided to undergo a laparoscopic distal gastrectomy (Billoth I). One month after the operation, she was diagnosed with anastomosis site stricture and received endoscopic balloon dilatation twice. Plain abdominal X-rays showed that there were dilatation of the stomach and gaseous patterns throughout the bowel. Computed tomography of the abdomen and pelvis showed an abruptly narrowed transition zone at the junction of the third and the fourth duodenum. At endoscopy, a 3.5-cm phytobezoar was found in the third portion of the duodenum. Endoscopic removal along with coke intake was tried. However, endoscopic removal was incomplete due to the large size of phytobezoar and a narrow endoscopic field. On the seventh hospital day, a third endoscopic trial was performed, but no phytobezoars were detected. She complained of recurrent abdominal pain thereafter. Computed tomography of the abdomen and pelvis showed a partially destroyed phytobezoar was obstructing the proximal ileum. The phytobezoar was removed through exploratory laparotomy.

        국내 아토피피부염 관련 모바일 애플리케이션 평가 분석

        Purpose: As the mobile internet spread, patients can access to health information via mobile applications. But this information must be a reliable for treatment. This study evaluated the quality of information of mobile applications that with atopic dermatitis. Methods: On April 2014, the 98 mobile applications that had been searched on three application markets using the keywords, “atopic dermatitis” etc. were enrolled. Thirty-one applications with information about atopic dermatitis written in Korean were surveyed. The informational quality of each application was examined using DISCERN tool and we also examined authorship, price, number of downloads. And we analyzed relations between these factors and informational quality. Results: The mean score (±standard deviation) of overall rating of the publications of the 31 applications were 1.55 (±0.888) points. The overall quality of the publication was “low” in 80.6% of applications, “moderate” in 16.1%, “high” in 3.2% of the applications we evaluated. There were no significant differences between the score of price of applications in all questions. However the mean score of group of downloads 1,000 and more were 2.25 (±1.035), significantly higher than group of downloads less than 1,000, 1.30 (±0.703) (P=0.043).Conclusion: The quality of mobile applications providing health information on atopic dermatitis needs significant improvement. Some guidelines and certifications of the mobile application are needed to provide health information about atopic dermatitis, association with experts.(Allergy Asthma Respir Dis 2014;2:370-376)

      • Pd/graphene gate HEMT 기반 이산화질소(NO₂) 가스 센서

        The catalytic metal-based nitrogen dioxide (NO₂) gas sensors have good adsorption of gas, but their desorption occurs at high temperatures. Thus they show an extremely slow recovery at room temperatures and high temperature operation is preferred. In this study, a unique gate structure of a graphene and catalytic metal (Pd) was employed to demonstrate the HEMT-based nitrogen dioxide gas sensor with a high NO₂ response and fast recovery. The Pd nanoisland structure was formed on graphene to maximize the surface through which nitrogen dioxide can react with the catalytic metal. Above all, when electron hole pairs are generated in graphene by irradiation with ultraviolet or visible light, oxygen ions adsorbed on the gate are oxidized to oxygen, and desorption is possible with a small amount of thermal energy. Thus, a nitrogen dioxide sensor having a high response of 2,430 and a fast recovery time of <100 s at room temperature was experimentally demonstrated.

        Evidence for Protective Effects of Coffees on Oxidative Stress-induced Apoptosis through Antioxidant Capacity of Phenolics

        This study evaluated total phenolics, total flavonoids, and antioxidant capacity of randomly selected regular and decaffeinated coffees commercially available in Korea and their protective effects in human hepatic epithelial HepG2 cell line against oxidative stress. All coffees tested exhibited potent antioxidant capacity in chemical systems and, consequently, significant protection of cells from oxidative stress in vitro in a dose-dependent manner. In particular, $H_2O_2$-induced apoptosis as evaluated by annexin V staining and flow cytometry was prevented by coffee extracts, resulting in the enhanced cell viability. Of interest, the content of total phenolics and flavonoids in coffees demonstrated a positive correlation with antioxidant capacity, indicating that the antioxidant capacity of coffees may be attributed to those phytochemicals. In accordance with previous studies, caffeoylquinic acid (CQA) and its derivatives including 3-CQA, 4-CQA, 5-CQA, 3,4-diCQA, 3,5-diCQA, and 4,5-diCQA were identified as phenolic phytochemicals by a reversed-phase HPLC, with 5-CQA being a major component. Taken together, the present study demonstrated protective effects of regular and decaffeinated coffees on cells in vitro against overwhelming oxidative stress due to richness in phenolics, especially CQA and its derivatives. Coffees, regular or decaffeinated, may serve as a good source of health-beneficial phytochemicals in diet.

        Analysis of clinical information and reverse transcriptase-polymerase chain reaction for early diagnosis of enteroviral meningitis

        Purpose: Meningitis is among the most common infections affecting the central nervous system. It can be difficult to determine the exact pathogen responsible for the infection and patients are often treated with empiric antibiotics. This study was conducted to identify the most common clinical characteristics of enteroviral meningitis in children and evaluate the diagnostic efficacy of reverse transcriptase-polymerase chain reaction (RT-PCR) for early detection of an enterovirus. Methods: We analyzed the medical records of children admitted to Korea University Medical Center and diagnosed with meningitis on the basis of cerebrospinal fluid (CSF) analysis and RT-PCR from CSF and other samples from January 2010 to August 2013. Results: A total of 333 patients were enrolled and classified into four groups based on diagnosis: enteroviral meningitis (n=110), bacterial meningitis (n=23), other viral meningitis (n=36), and unknown etiology (n=164). Patients with bacterial meningitis were younger than those in the other groups (P<0.001). Pleocytosis in CSF was similar across all groups. Of patients in the enteroviral meningitis group, 92.7% were diagnosed based on RT-PCR findings. Mean length of hospital stay for patients with enteroviral meningitis was 6.08 days, which was significantly shorter than that for patients with meningitis of bacterial etiology (19.73 days, P<0.001). Conclusion: Diagnosis of enteroviral meningitis before viral culture results are available is possible using RT-PCR. Accurate diagnosis reduces the length of hospital stay and helps to avoid unnecessary empiric antibiotic treatment.

        Degradation of All-Solid-State Lithium-Sulfur Batteries with PEO-Based Composite Electrolyte

        Lithium-sulfur batteries (LSBs) have emerged as a promising alternative to lithium-ion batteries (LIBs) owing to their high energy density and economic viability. In addition, all-solid-state LSBs, which use solid-state electrolytes, have been proposed to overcome the polysulfide shuttle effect while improving safety. However, the high interfacial resistance and poor ionic conductivity exhibited by the electrode and solid-state electrolytes, respectively, are significant challenges in the development of these LSBs. Herein, we apply a poly (ethylene oxide) (PEO)-based composite solid-state electrolyte with oxide Li<sub>7</sub>La<sub>3</sub>Zr<sub>2</sub>O<sub>12</sub> (LLZO) solid-state electrolyte in an all-solid-state LSB to overcome these challenges. We use an electrochemical method to evaluate the degradation of the all-solid-state LSB in accordance with the carbon content and loading weight within the cathode. The all-solid-state LSB, with sulfur-carbon content in a ratio of 3:3, exhibited a high initial discharge capacity (1386 mAh g<sup>-1</sup>), poor C-rate performance, and capacity retention of less than 50%. The all-solid-state LSB with a high loading weight exhibited a poor overall electrochemical performance. The factors influencing the electrochemical performance degradation were revealed through systematic analysis.

      • 3차원 카본클로스를 사용한 전고체 리튬-황 전지의 성능 개선

        전고체 리튬-황 전지는 리튬-황 전지의 단점인 고차 폴리설파이드의 셔틀현상을 억제 할 수 있는 유망한 해결책이다. 또한, 3차원적으로 넓은 표면적과 높은 전도도를 가지는 양극은 리튬-황 전지의 낮은 전도도를 향상시키는데 효과적이다. 본 연구에서 빠른 넓은 표면적과 높은 이온전도도를 바탕으로 빠른 전하전달이 가능한 3차원 카본클로스를 사용하여 양극을 제작하였다. 그 결과, 전고체 리튬-황 전지는 0.1C에서 약 25% 증가한 초기용량과 0.2C에서 10% 증가한 고율 특성을 구현하였다. 이는 carbon cloth를 사용하여 제작한 양극이 전고체 리튬-황 전지의 전기화학적 성능을 개선할 수 있음을 보여주었다. All-solid-state lithium-sulfur batteries is a promising solution to restrict the shuttle effect of longchain polysulfides. In addition, three-dimensionally large-surface area and high conductivity cathode is effective method to improve low conductivity of lithium-sulfur batteries. Herein, we constructed three-dimensional carbon cloth cathode with fast charge transfer through large surface area and high conductivity. As a result, the all-solid-state lithium-sulfur batteries with as-constructed cathode achieved about 25% improved initial capacity at 0.1C and about 10% increased C rate performance at 0.2C, respectively. It showed possible to improve the electrochemical performance of all-solid-state lithium-sulfur batteries with as-constructed cathode.
