Background Intervertebral disk degeneration (IDD) is a common orthopedic disorder, and nucleus pulposus cells (NPCs) serve to stabilize the intervertebral disk. Objective This study was implemented to explore the protective eff ect of endplate stem cells (EPSCs) on IDD. NPCs and EPSCs were isolated from rats and tert -Butyl peroxide (TBHP) was used to treat NPCs and simulate IDD. EPSC-derived conditioned medium (CM) was collected and mixed with a culture medium at diff erent proportions, which was then used to treat NPCs. SOD assay was employed to examine the oxidative stress level in the NPCs. To assess apoptosis and caspase-3 activity, fl ow cytometry and western blots for cleaved PARP1 and BAX were carried out. The mRNA expression levels of diff erent genes were detected by quantitative real-time polymerase chain reaction. Furthermore, the concentrations of amphiregulin (Areg) and infl ammatory factors were measured with ELISA kits and cell signaling pathways were evaluated using western blotting. Results Following TBHP treatment, caspase-3 activity in the NPCs increased. However, exposing TBHP-induced NPCs to the EPSC-derived CM reduced oxidative stress, caspase-3 activity, cell apoptosis, fi brotic response, and cytokine production. Additionally, the EPSC-derived CM contained a higher concentration of Areg compared to that in the NPCs. Furthermore, Areg decreased oxidative stress, cell apoptosis, fi brosis, and infl ammation via both PI3K/AKT and ERK1/2 signaling pathways. Conclusion ECSCs reduce NPC apoptosis, oxidative stress, production of infl ammatory factors, and fi brotic response via the release of Areg. Moreover, Areg can exert these protective eff ects on NPCs via PI3K/AKT and ERK1/2 signaling pathways.

Subchondral insufficiency fracture (SIF) of the femoral head is one of the predominant etiologies of rapidly progressive osteoarthritis of the hip (RPOH). SIF is a rare disease that causes acute pain in the hip joint. It is most frequently found in elderly women with osteoporosis. It is often underdiagnosed or misdiagnosed as osteonecrosis of the femoral head. SIF is currently a well-established cause of RPOH; however, the deeper etiology of SIF is not clear. Good clinical outcomes have been reported for hip preservation therapy and hip replacement. SIF is not obvious radiologically in the early stage, and a T1-weighted magnetic resonance imaging shows a discontinuous low-intensity band under the articular cartilage convex to the articular surface as its characteristic manifestation. Some patients will lose the opportunity to preserve the hip joint due to symptoms such as progressive joint space narrowing and subchondral collapse within a very short period. Patients with progressive hip space narrowing and subchondral collapse on Xray should be converted to total hip arthroplasty. Based on the characteristics of the disease, surgeons need to master the clinical and radiological characteristics of SIF and strive for early diagnosis and treatment.

Objective: To investigate the predictive value of radiomics features based on cardiac magnetic resonance (CMR) cine images for left ventricular adverse remodeling (LVAR) after acute ST-segment elevation myocardial infarction (STEMI). Materials and Methods: We conducted a retrospective, single-center, cohort study involving 244 patients (random-split into 170 and 74 for training and testing, respectively) having an acute STEMI (88.5% males, 57.0 ± 10.3 years of age) who underwent CMR examination at one week and six months after percutaneous coronary intervention. LVAR was defined as a 20% increase in left ventricular end-diastolic volume 6 months after acute STEMI. Radiomics features were extracted from the oneweek CMR cine images using the least absolute shrinkage and selection operator regression (LASSO) analysis. The predictive performance of the selected features was evaluated using receiver operating characteristic curve analysis and the area under the curve (AUC). Results: Nine radiomics features with non-zero coefficients were included in the LASSO regression of the radiomics score (RAD score). Infarct size (odds ratio [OR]: 1.04 (1.00–1.07); P = 0.031) and RAD score (OR: 3.43 (2.34–5.28); P < 0.001) were independent predictors of LVAR. The RAD score predicted LVAR, with an AUC (95% confidence interval [CI]) of 0.82 (0.75–0.89) in the training set and 0.75 (0.62–0.89) in the testing set. Combining the RAD score with infarct size yielded favorable performance in predicting LVAR, with an AUC of 0.84 (0.72–0.95). Moreover, the addition of the RAD score to the left ventricular ejection fraction (LVEF) significantly increased the AUC from 0.68 (0.52–0.84) to 0.82 (0.70–0.93) (P = 0.018), which was also comparable to the prediction provided by the combined microvascular obstruction, infarct size, and LVEF with an AUC of 0.79 (0.65–0.94) (P = 0.727). Conclusion: Radiomics analysis using non-contrast cine CMR can predict LVAR after STEMI independently and incrementally to LVEF and may provide an alternative to traditional CMR parameters.

Artesunate (ART) has been indicated as a candidate drug for hepatocellular carcinoma (HCC). Glucosylceramidase (GBA) is required for autophagic degradation. Whether ART regulates autophagic flux by targeting GBA in HCC remains to be defined. Herein, our data demonstrated that the dramatic overexpression of GBA was significantly associated with aggressive progression and short overall survival times in HCC. Subsequent experiments revealed an association between autophagic activity and GBA expression in clinical HCC samples, tumor tissues from a rat model of inflammation-induced HCC and an orthotopic mouse model, and human HCC cell lines. Interestingly, probe labeling identified GBA as an ART target, which was further verified by both a glutathione-S-transferase pulldown assay and surface plasmon resonance analysis. The elevated protein expression of LC3B, the increased numbers of GFP-LC3B puncta and double-membrane vacuoles, and the enhanced expression of SQSTM1/p62 indicated that the degradation of autophagosomes in HCC cells was inhibited by ART treatment. Both the in vitro and in vivo data revealed that autophagosome accumulation through targeting of GBA was responsible for the anti-HCC effects of ART. In summary, this preclinical study identified GBA as one of the direct targets of ART, which may have promising potential to inhibit lysosomal autophagy for HCC therapy.

Hierarchical porous NiO/Al2O3 composites were successfully prepared by two-steps. First, the core-shell structured Al2O3 microspheres were prepared via a template-free hydrothermal route using KAl(SO4)2·12H2O and Al2(SO4)3·18H2O as aluminum source. Then, the NiO/Al2O3 composites with micro- and nano-hierarchical structures were prepared by a hydrothermal method combining the subsequent calcination process. The obtained characterization result presented that the morphology of hierarchical Al2O3 microsphere tuned to irregular platelets by simply varying Ni/Al ratios. The BET analysis showed that the special surface area from 52.12m2 g−1 to 214.8m2 g−1 after two hydrothermal complex process. Effects of Ni/Al ratio, adsorbent dosage, Congo red (CR) concentration, coexisting ions, adsorption time and temperature were investigated. The obtained results indicated that NiO/Al2O3 composite had the high adsorption efficiency (99.6%) and great adsorption capacity (186.9mg g−1) under the optimum conditions. The adsorption isotherm and kinetics data were found to be well fitted and in good agreement with the Langmuir isotherm model and pseudo-second order model, respectively. The hierarchical porous NiO/Al2O3 composites presented remarkably higher adsorption efficiency during five recycling, which showed their potential as the highly efficient adsorbent for removal of CR in wastewater.

Background and Purpose Hyperekplexia (HPX), a rare neurogenetic disorder, is classically characterized by neonatal hypertonia, exaggerated startle response provoked by the sudden external stimuli and followed by a shortly general stiffness. Glycine receptor alpha 1 (GLRA1) is the major pathogenic gene of the disease. We described the clinical manifestations of genetically confirmed HPX patients and made a literature review of GLRA1-related HPX to improve the early recognition and prompt the management of the disorder. Methods Extensive clinical evaluations were analyzed in 4 Chinese HPX patients from two unrelated families. Next generation sequencing was conducted in the probands. Sanger sequence and segregation analysis were applied to confirm the findings. Results All four patients including 3 males and 1 female presented with excessive startle reflex, a cautious gait and recurrent falls. Moreover, startle episodes were dramatically improved with the treatment of clonazepam in all cases. Exome sequencing revealed 2 homozygous GLRA1 mutations in the patients. The mutation c.1286T>A p.I429N has been previously reported, while c.754delC p.L252* is novel. Conclusions HPX is a treatable disease, and clonazepam is the drug of choice. By studying and reviewing the disorder, we summarized the phenotype, expanded the genotype spectrum, and discussed the possible pathogenic mechanisms to enhance the understanding and recognition of the disease. Early awareness of the disease is crucial to the prompt and proper administration, as well as the genetic counseling.
