        당뇨환자에서 발생한 두개저의 침습성 아스페르길루스증 1예

        두개저의 침습성 아스페르길루스증은 매우 드물지만 치명적인 질환으로 침습성 아스페르길루스 부비동염에 합병될 수 있다. 침습성 아스페르길루스증의 치료 원칙은 병변의 외과적 적출술과 장기간의 항진균요법으로 되어 있으나, 적절한 치료 기간과 치료 반응의 경과 판정에 대한 지침은 없는 실정이다. 저자들은 부비동염 증상을 주소로 내원하여 아스페르길루스종을 진단받고 4개월 전에 수술받은 적이 있는 당뇨 환자에서 두 개저의 침습성 아스페르길루스증을 진단하였으며, 병소의 부분적인 수술적 제거와 5개월 이상의 전신적 항진균제 사용으로 환자의 임상 경과가 호전되었으며 치료전과 치료 과정에서 시행한 갈륨스캔에서 방서성동위원소의 섭취가 점차 감소하여 일치하는 소견을 관찰할 수 있었다. 이에 저자들은 당뇨를 포함한 면역저하자에서 발생하는 아스페르길루스 부비동염은 조직 침습 유무에 대한 정확한 진단이 매우 중요하며, 추후에도 침습성 아스페르길루스증으로 진행할 수 있음을 유의해야 할 것을 강조하며, 또한, 갈륨스캔이 그러한 감별과 치료 경과 판정에 유용함을 경험하여 보고하는 바이다. Invasive aspergillosis of skull base is a rare but potentially lethal disease which complicates fungal sinusitis and requires surgical debridement, and long term antifungal therapy. However, optimal duration of antifungal therapy and standard method to monitor of clinical response are not known. A 55-year-old diabetic female patient presented with headache and otatgia 4 months after surgery for non-invasive aspergilloma of sinus. Brain MRI findings revealed bone destructions at the skull base and mastoid process. Mastoidectomy and tympanoplasty were performed, and the resected specimen showed fungal hyphae invading the tissue. Amphotericin B was given for 9 weeks (a total dose of 3.3g) with oral itraconazole, followed by itraconazole (400 ㎎ daily) alone for another 12 weeks. Serial gallium scans, taken before treatment and at 8th and 16th weeks following therapy, showed gradual reduction in uptake of the lesions. We emphasize that non-invasive Aspergillus sinusitis may progress to invasive disease, and strong suspicion of invasiveness is mandatory, especially in immunocompromised patients including diabetes mellitus. Also, we suggest that gallium scan is useful for determining the presence and extent of the disease, and monitoring clinical response following therapy.

      • 고능률 소형 전자석에 의한 자왜 및 자기이방성 측정

        자왜 또는 자기이방성의 측정에 사용되는 전사석의 여자전류를 측정에 필요한 수초간만 흘려서 에너지 소모 및 측정시의 최대난점인 시료의 온도변화의 문제를 극소화하였다. 따라서 전자석의 냉각장치는 생략되고 크기와 전원의 용량도 극소화되었다. 공기간격 22㎜, 자극의 단면적 40 × 25㎟에서 0.5 T의 자장발생에는 180 W의 전원으로 족하였다. 시료의 자왜와, 자기이방성에 의한 토크를 전기용량센서에 의하여 측정하여 10^-8의 자왜분해능과 1 nJ의 토크 분해능을 얻었다. 0.02× 0.8× 10㎟의 연자성 리본의 형상이방성을 이용하여 이방성 측정시의 토크값을 교정하였다. The magnet could be miniaturized by reducing the measuring space and time. The excitation current of the electromagnet was supplied for only a few second of small. An 0.5 T magnetic field was generated with 180 W power consumption. The values of magnetostriction and magnetic anisotropy were measured with a very sensitive capacitance cell with resolution of 10^-8 and 1 no. The torque was calibrated using a soft magnetic ribbon's shape anisotropy.

      • Quantum noise reduction in intensity-sensitive surface-plasmon-resonance sensors

        <P>We investigate the use of twin-mode quantum states of light with symmetric statistical features in their photon number for improving intensity-sensitive surface plasmon resonance (SPR) sensors. For this purpose, one of the modes is sent into a prism setup where the Kretschmann configuration is employed as a sensing platform and the analyte to be measured influences the SPR excitation conditions. This influence modifies the output state of light that is subsequently analyzed by an intensity-difference measurement scheme. We show that quantum noise reduction is achieved not only as a result of the sub-Poissonian statistical nature of a single mode, but also as a result of the nonclassical correlation of the photon number between the two modes. When combined with the high sensitivity of the SPR sensor, we show that the use of twin-mode quantum states of light notably enhances the estimation precision of the refractive index of an analyte. With this we are able to identify a clear strategy to further boost the performance of SPR sensors, which are already a mature technology in biochemical and medical sensing applications.</P>

        Effects of Low and High Molecular Weight Hyaluronic Acids on Peridural Fibrosis and Inflammation in Lumbar Laminectomized Rats

        Background: Postlaminectomy peridural fibrosis is inevitable. Some studies have compared and identified the effects of high molecular weight hyaluronic acids (HMWHA) and low molecular weight hyaluronic acids (LMWHA) on peridural fibrosis in postlaminectomy animal models. However, no studies have been found that compare pain behaviors between hyaluronic acids or among hyaluronic acids and other solid materials. The purpose of this study was to examine the correlation between pain-related behaviors and histopathologic changes in laminectomized rats using various peridurally administered materials. Methods: Forty male Sprague-Dawley rats, laminectomized at the L5 and L6 levels, were divided into four groups: group C, laminectomy only; group L, laminectomy and LMWHA application; group H, laminectomy and HMWHA application; group F, laminectomy and fat interposition. Pain behaviors were checked before, 3 days, 1 week, and 3 weeks after surgery. Histopathological changes were checked at the L5 level 3 weeks after the surgery. Results: The 50% withdrawal thresholds in groups L and H were higher than that in groups C and F three days after laminectomy (P < 0.05). The paw withdrawal time did not change among the groups and in each group during the study period. Peridural fibrosis in group F was significantly lower than in the other groups (P < 0.05). Conclusions: Hyaluronic acids significantly reduced mechanical allodynia but not thermal hyperalgesia. Peridural fibrosis did not show any correlation with pain behaviors. There have been limited studies on the correlation between peridural fibrosis and pain behavioral change, which should be verified by further studies.

The magnet could be minaturized by reducing the measuring space and time. The excitation current of the electromagnet was supplied for only a few second of measuring time. Cooling system of the electromagnet could be eliminated since the dissipation energy was very small. An 0.5 T magnetic field was generated with 180 W power consumption. The values of magnetostriction and magnetic anisotropy were measured with a very sensitive capacitance cell with resolution of 10^(-8) and 1 nJ. The torque was calibrated using a soft magnetic ribbon's shape anisotropy.

        초등학교 과학동아리 운영 실태와 과학동아리 지도교사들의 인식 : 수렴적 설계를 통한 혼합연구

        본 연구의 목적은 두 가지다. 첫 번째는 초등학교 현장에서의 과학 동아리가 언제, 어디서, 어떤 방식으로 운영되고 있는지 등을 알아보는 과학동아리 운영 실태 분석이고, 두 번째는 지도교사들이 느끼는 과학동아리의 효과나 정책적 지원 등에 대한 지도교사들의 인식 분석이다. 이 연구를 위해 혼합연구 중 수렴적 설계를 활용하였으며 2가지 연구문제를 개별적으로 통합하는 과정으로 연구를 진행하였다. 연구 결과 초등 과학동아리는 지도교사의 자발적인 운영에 기초하고 있었으며 학생 성장과 같은 긍정적 경험이 지속적인 운영에 영향을 미쳤다. 하지만 형식화된 업무에 부담을 느껴 제도적으로 확대되는 것에는 중립적으로 반응하였다. 이러한 분석을 통해 과학동아리 활성화를 위해서는 과학동아리를 제도화하고 확대 운영하는 것보다 과학동아리 지도교사 간 교류 활성화 등의 간접적 지원을 확대하는 것이 효과적이라는 것을 알 수 있다. The purpose of this study is twofold. The first is the management status of the science club, which examines when, where, and how science clubs are operated at elementary school sites. The second is the perception of the teachers who check how they feel about the effects of the science club or its policy support. For this study, the convergent design was used a mixed- methods approach and the research was conducted in the process of integrating the two research problems individually. As a result of the study, the elementary science club was based on the voluntary operation of the Science Club Teachers, and positive experiences such as student growth affected the continuous operation. However, they reacted neutral to the institutional expansion because they felt burdened by formalized work. Through this analysis, we can see that it is more effective to expand indirect support such as promoting exchanges between science club teachers than to institutionalize and expand the operation of science clubs.

        요추 추간판 탈출증 환자의 한의치료 후 디스크 흡수율 비교 및 흡수에 영향을 주는 요인 분석 연구

        Objectives The purpose of this study is to analyze the effects of Korean Medicine treatment on lumbar disc herniation (LDH) and predictive factors associated with disc resorption by magnetic resonance imaging (MRI). Methods A retrospective study was carried out in 79 lumbar disc herniation patients who had visited Haeundae Jaseng Hospital of Korean Medicine. Patients' diagnosis was based on MRI. MRI was performed on two or more occasions and patients were received Korean Medicine treatment within the period. The volume of each herniated disc was measured three-dimensionally and patient characteristics, interval between MRIs, herniated disc level, disc herniation type, disc migration, intactness of posterior longitudinal ligament (PLL), initial volume of herniated disc, modic change, disc resorption rate were statistically analyzed. Results The mean volumes of herniated discs before Korean Medicine treatment and after Korean Medicine treatment were 1,547.81±598.15 mm3 and 947.06±335.28 mm3, respectively. The mean resorption rate was 35.7±16.3%. Disc extent, intactness of PLL and initial volume of herniated discs were significantly correlated with resorption rate (p=0.003, p=0.001 and p=0.024, respectively). Conclusions Korean Medicine treatment is an effective conservative treatment for lumbar disc herniation. Factors such as disc migration, intactness of PLL, initial volume of herniated disc have a significant association with disc resorption rate. (J Korean Med Rehabil 2018;28(4):33-41)

        미수정 및 저수정율의 기왕력이 있는 체외수정시술 환자에서의 난자 세포질내 정자 주입술 : 중증 남성인자 환자와의 비교

        N/A Objective: The purpose of this study was to determine whether intracytoplasmic sperm injection(ICSI) could overcome the defects of oocytes in IVF-ET patients with previous fertilization failure by conventional fertilization technique. Design: Retrospective study Materials and Methods: A total of 119 ICSI cycles in 57 IVF-ET patients performed from May, 1995 to December, 1997 was enrolled. Subjects were divided into two groups: FR group included 66 ICSI cycles in 35 patients with normal sperm who underwent ICSI due to past history of failed or poor fertilization in the previous IVF-ET cycles, and OAT group included 53 ICSI cycles in 22 patients with severe oligoasthenoterato- zoospermia(OAT) which was defined as sperm concentration < 20 million/ml, mo#dlity < 30% and normal morphology < 4% by strict morphologic criteria. The outcomes of ICSI were analyzed and compared in both groups. Results: The age of female patients, basal serum FSH level, and the numbers of oocytes retrieved and metaphase II oocytes were all comparable in both groups. The fertilization rate after ICSI was similar in both groups(68.7+-25.3% vs. 67.7+-24.5%), as were the cleavage rate of normally fertilized oocytes(93.1+-21.4% vs. 89.3+-21.6%), the number of embryos transferred(4,00+-1.98 vs. 4.64+-2.10), and cumulative embryo score(CES) indicating the quality of embryos(47.3+-33.2 vs. 54.1+-33.2). The implantation rate(4.3+-10.5% vs. 3.8+-11.0%) and the clinical pregnancy rate per cycle(15.2% vs. 13.2%) were also comparable in both groups. Conclusions: Although it has been shown that there is a higher risk of chromosomal abnormalities in oocytes from IVF-ET patients with pevious failed or poor fertilization, higher implantation and clinical pregnancy rates wer#e not observed in patients with OAT following ICSL Therefore, the functional defect of sperm such as loss of capacitation, defect of aaasome reaction, and abnormality of nucleus decondensation should be also considered in patients with previous failed or poor fertilization.
